RESUMEN

Precocious puberty is defined as children attaining puberty more than 2.5 to 3 standard deviations (SD) earlier than the median age, or before the age of eight years in girls and nine years in boys. Hypothalamic hamartoma (HH) are rare, non progressive tumor like malformation. Precocious puberty due to HH occurs particularly at early ages, even 2 or 3 years. Treatment options for isolated CPP due to HH include GnRH analogs agonists continuously stimulates pituitary gonadotrophs, which further help in decreasing and desensitizing the release of LH, and to a lesser extent, FSH till the time puberty naturally set in. We present a case of precocious puberty due to hypothalamic hamartoma in 3 years old girl. Treated with GnRH analog lupirode and responded well to treatment with cessation of menstruation and reduction in breast size.

An indigenous contraceptive herbal formulation consisting of a mixture of Lepidagathis longifolia, Palaquium sp and Phyllagathis rotundifolia is being used by the Temuan Aborigins of Malaysia. Although the previous studies demonstrated that this contraceptive herbal formulation causes anovulatory estrous cycle, altered circulating hormone levels and fetal resorption in rats, but the effects of this formulation on the gonadotrphs of the pituitary gland are yet to be evaluated. The present study was designed to observe the morphometric changes of the gonadotrophs and the plasma concentrations of follicle stimulating hormone and leutinizing hormone. Thirty five Sprague-Dawley adult female rats were randomly divided into 5 groups. Experimental animals were given a combined herbal extract or individual herbal extract at a dose of 540 mg/kg/day subcutaneously for 7 days. Immunostained gonadotrophs were studied by using image analyzer. FSH and LH serum concentrations were determined using RIA. The FSH and LH concentrations were low in animals that received combined herbal extract (p<0.01). FSH concentration was noted to be significantly low in animals that received P. rotundifolia (p<0.05). The mean cell area and cell density of gonadotrophs of animals that received combined herbal extract were significantly low compared to control group (p<0.05). It was concluded that the herbal extracts do suppress the production of gonaotrophins along with the demonstrable suppresive effect on the FSH cells.

In the subadult Rana tigrina administration of 2 μg luteinizing hormone releasing hormone-acetate/frog six days a week for 4 weeks in April resulted in the formation of medium (in all 8 frogs) and large sized (in 4 out of 8 frogs) yolky oocytes and, concomitant increases in the oviductal mass. The ovarian and oviductal masses showed a 10-fold increase over the control frogs. In untreated frogs the ovaries were transparent and contained first growth phase oocytes only. The oviducts were also infantile. The pituitary sections were stained using antisera raised in rabbit against the β-subunit of human luteinizing hormone and human follicle stimulating hormone. Immunoreactivity, staining intensity, cytoplasmic granulation and, cell, nuclear and cytoplasmic areas of gonadotrophs (B2 cells) increased significantly in luteinizing hormone releasing hormone treated frogs. The above findings suggest that pituitary-ovarian axis in the subadult Rana tigrina is responsive to luteinizing hormone releasing hormone and that long-term treatment with the hormone induces cytomorphological changes in the gonadotrophs which result in the conversion of inactive cells into secretory cells. This is accompanied by precocious vitellogenic growth of oocytes in the subadult frogs.

This paper presents the ultrastructrual changes of male rats gonadotrophs at 7, 15, 30, 50 days after cadmium chloride (Cd) injection (2mg/kg). The differences of morphometric parameters of gonadotrophs and plasma value of FSH were revealed by statistics.The results showed that no injured alterations were found in gonadotrophs but appeared castration like features. These changes feature were most prominent after 15 days and then recovered to normal gradually. The main alterations of gonadotrophs were dilatation, and proliferation and decrease in the specific surface of RER as well as there were low density materials in the dilated cisterna. The numerical and volume density of large and small secretion granules were decreased greatly in all of the treatment groups. The variance of diameter of the large and small secretion granules were not conspicuous and the differences of volume and numerical density of the mitochondria among the control and treatment groups were also no significance. The hypertrophy of Golgi complex of the gonadotrophs were very obvious and the number of the gonadotrophs were increased in the Cd treated rats. The plasma value of FSH measurement indicated that the FSH level increased after 7days and the highest value appeared after 15 days. However, it did not return to normal after 50 days. These results suggested that gonadotrophs were not susceptible to Cd. The hypertraphy, hyperplasia and increase in synthesis and secretion of the gonadotrophs resulted from feedback by the effect of Cd induced injury on testes.
