RESUMEN

Background: Prediabetes is the preceding stage of diabetes which puts an individual to induce complications same as that of diabetes; hence, it should be treated to prevent its progression to diabetes and other consequences. However, there is very less literature about impact of home-based physical therapy on glycemic control and quality of life in Indian prediabetic population. Aims and Objectives: The aims of this study were to evaluate whether the home-based physical therapy shows effect on glycemic control and individual’s quality of life after a 3-month intervention. Materials and Methods: The study was conducted on 55 individuals who were diagnosed with prediabetes (36 males and 19 females) on basis of HbA1c level ranging from 5.7% to 6.4%. A 12-week exercise protocol was made which includes warm up exercises, main exercise program (aerobic and strengthening training), and cool down exercises along with dietary changes. SF-36 and HbA1c level is taken at baseline and at the end of 12 weeks for quality of life and glycemic control measures in participants, respectively. Results: The difference in HbA1c levels before and after treatment was found to be statistically significant. The HbA1c levels after the 3-month intervention showed significant reduction (P < 0.008) along with significant difference in all domain of SF-36 except in domain 3 (role limitation due to emotional problem) and domain 6 (social functioning). Conclusion: The home-based physical therapy program is effective in glycemic control and quality of life in adults with prediabetes.

RESUMEN

BackgroundHome-based intervention occupies a prominent place in the treatment of autism spectrum disorder (ASD) in children, whereas previous studies on the effect of home-based early intensive behavioral intervention on verbal ability of children with ASD are somewhat inadequate. ObjectiveTo study the effects of intensive family behavioral of intervention based on Verbal Behavior Milestones Assessment and Placement Program (VB-MAPP) on the language ability of children with ASD, so as to provide references for the development of family intervention strategies for children with ASD. MethodsChildren aged 2 to 3 years old who attended the Children's Rehabilitation Department of the Second Affiliated Hospital of Shantou University Medical College from September 2020 to December 2021 and met the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) diagnostic criteria for ASD were selected as the study objects. A non-randomized concurrent control trial was conducted to compare a study group (n=24) receiving home-based early intensive behavioral intervention using VB-MAPP with a control group (n=14) receiving training from other special institutions or early childhood education institutions. The intervention lasted for 6 months in both groups. Before and after the intervention, the VB-MAPP milestone assessment was performed in the two groups, and the VB-MAPP milestone score, mand, trac and listener responds were selected as the study indicators. Then the intervention effect was compared between two groups, and the multiple linear regression was performed to screen the related influencing factors. ResultsAfter intervention, the total milestone assessment score, mand, tact and listener responds scores of study group were higher than those of control group, with statistical difference (Z=-4.339~-2.195, P<0.05 or 0.01). Multiple linear regression analysis denoted that the average weekly hours of home-based intervention in the first three months had certain effect on listener responds (β=1.029, P<0.05). ConclusionApplication of home-based early intensive behavior intervention using VB-MAPP may contribute to the improvement of verbal abilities such as mand, tact and listener responds in children with ASD. RESUMEN

At present, China’s response to the aging population situation is unprecedentedly urgent, and the construction and planning of the pension system face great challenges. Home-based elderly care services stand out in institutional-based elderly care and family-based elderly care due to their unique advantages in meeting the individual needs of the elderly. By sorting out the main types, policy lines, and supply and demand status of home-based elderly care service, this paper indicated that there are five aspects of problems in home-based elderly care service, including weak professional talent team construction, relatively insufficient capital investment, weak supply-demand matching, imperfect operating mechanism, and imperfect supporting facilities. It was proposed that in the future home-based elderly care services should strive to strengthen the construction of professional talent teams, widen financing channels, fully play the leading role of the government, perfect the operational mechanism and the construction of supporting facilities of home-based elderly care services, so as to provide reference for the optimization of the home-based elderly care service system in China.

RESUMEN

With aging population becoming increasingly prominent, more intelligent, integrated, and anthropomorphic nursing robots are becoming an important option for future home-based elderly care. However, a series of ethical dilemmas may arise when nursing robots that provide long-term services participate in the social life of the elderly as social entities. For example, the encroachment of low-level interaction between humans and nursing robots on the high-level interaction between humans, as well as the dispute over the definition of responsibility caused by unclear moral subjects. The value sensitive design theory advocates incorporating value into technical design, and based on its conceptual research, empirical research, and iterative approach, it can be found that the fundamental reason for the above-mentioned ethical dilemma lies in the confusion and dislocation of the ternary nursing relationship among human caregivers, the elderly, and nursing robots. Accordingly, this paper proposed to clarify the user status of the elderly and human caregivers, while nursing robots serve as auxiliary tools to meet the nursing needs of the elderly, protect and enhance the well-being of the elderly.

RESUMEN

Introduction: Congenital heart diseases are related to heart defects that develop during pregnancy and are present at birth. Children must regularly attend specialized clinical centers for treatment, which includes medical check-ups, hemodynamic procedures, and surgery. Nevertheless, the success of interventions largely depends on the education of parents and caregivers about the symptoms and warning signs that babies with complex congenital heart disease may present during the period between stages. Likewise, it is necessary for caregivers to have tools to face the barriers of the health system, such as, obstacles or delays in obtaining healthcare services. This facilitates timely health care for patients and avoids fatal outcomes or deterioration in quality of life. Materials and Methods: This document presents a system that provides support to patients with complex congenital heart disease in follow-up by a home monitoring program. The system consists of a mobile application that allows parents/caregivers to monitor physiological variables of the patients (weight, intake, and oximetry). The information registered in the application is sent for real-time evaluation by the medical team. Results:The application provides an effective means for sending data and communicating with the medical control center, in addition to offering educational and informative material. In this way, parents/caregivers get constant help and feedback about their baby's condition. Conclusions: The mobile application could help mitigate the costs of medical care and overcome the limitations of follow-up at home, giving doctors the possibility to see the patient's evolution and give timely recommendations.

RESUMEN

Introduction: The number of events, either natural or manmade, that can potentially cause disasters, has increased sharply in recent years. Life losses, along with the displacement of millions of people, and the corresponding economic cost, should mobilize international organizations to effectively address the situation, with part of this effort referring to the mobilization and cooperation of all stakeholders involved. An investigation, through literature review, ofObjective: the role of Primary Health Care in disaster management. This assignment is based on a descriptiveMaterial – Method: review. A literature search was performed on the PubMed, Sciencedirect, and Scopus databases focusing on the years 2015-2022. The following keywords were used: Primary health, community health, Home-Based Primary Care, disaster management. No reference was included to the provision of health care outside of primary health care structures. Results: The results can be grouped in two categories. In the first category, an analysis takes place regarding PHC actions in each stage of the disaster management cycle. In the second category, the conditions are listed that should be paid attention to, especially by the political leaders of each country. The World Health Organization,Conclusions: especially in the aftermath of the pandemic of the last two years, has reinstated the critical role of PHC as a cornerstone of solid health systems. At the same time, international organizations that specialize in disaster management focus on reducing risks, as a responsibility that is mostly assigned to communities working together with all organizations involved. Combinedly, it naturally follows that primary health care structures play a leading role in the management of mass events.

RESUMEN

Under the background that the family doctor teams undertake the evaluation function of long-term care insurance for the elderlies living at home, it is imperative to further improve the efficiency of its integrated aging-medical care by strengthening the management. The Huacao community health service center and Hongqiao community health service center in Minhang District of Shanghai explored the construction of a family doctor′s home-based aging care management path, a practice launched since February 2021. This path was mainly designed with the entry criteria, management requirements for various services and evaluation indicators, with a service path form formulated on such basis. The form covered three stages: that before the on-site service, that on the day of the on-site service and that after the on-site service. Each stage was subdivided into three dimensions: the work content of the main aging-medical care, the key works of family doctors and the key works of assistant nurses. The implementation of this management path can enable such teams to provide further refined, standardized and procedural aging-medical care services, avoid service delays, improve service quality and management efficiency, and improve the utilization efficiency of health resources.

RESUMEN

Home-based rehabilitation can effectively reduce the clinical complications of patients with spinal cord injury and accelerate the recovery of neurological function, as well as reduce the heavy economic burden on healthcare system. Home-based rehabilitation has shown effectiveness in multiple studies. Neurofeedback could relieve the neuropathic pain of 40% home patients with spinal cord injury. Wearable technology could monitor and promote the recovery of hand motor function. Household portable stimulators and surface electrodes could inhibit unnecessary bladder activity and improve urinary incontinence. Strengthening stretching training at home could reduce shoulder pain in patients with chronic spinal cord injury. Using functional electrical stimulation at home could improve the muscle function of patients with spinal cord injury due to loss of neurotrophic muscle atrophy. There are still some disadvantages of home-based rehabilitation, such as complicated operation, expensiveness and not convenient to carry for training equipment, which need to be further developed.
