RESUMEN

@#This paper summarized the clinical experience of AI Rudi in the treatment facial hormone-dependent dermatitis with the method of clearing heat and protecting yin. It is believed that the key pathogenesis is the heat toxin accumulation, yin depletion and collaterals obstruction. The clinical treatment should focus on “heat exuberance” and “yin depletion”. It is advocated that “half treatment is from heat and half from yin” is the general principle, and the treatment is staged. In the acute phase, the treatment is half from cooling blood and dispersing wind to dispel heat pathogen, and half from protecting fluid and moisturizing skin to strengthen yin; and the modified Liangxue Xiaofeng Powder (凉血消风散) could be used. In the chronic phase, half treatment is from clearing residual toxin to eliminate heat pathogen, and half from nourishing yin and unblocking collaterals to strengthen yin, for which Xuanmai Ganju Decoction and Erzhi Pills (玄麦甘桔汤合二至丸) can be used and modified according to the symptoms. At the same time, we should pay attention to the simultaneous internal and external treatment, and emphasize the importance of daily protection in the treatment of the disease.

RESUMEN

Objective:To observe the changes of skin lesion, anxiety and depression in patients with facial hormone-dependent dermatitis before and after treatment.Methods:Ninty-two cases of hormone-dependent dermatitis were collected from October 2014 to December 2020 in the Department of Dermatology, the First People's Hospital of Changshu, Jiangsu province, including 24 males and 68 females, aged 18 to 52 (31±10) years. The skin lesions and subjective symptoms changes were compared 2 weeks and 4 weeks after treatment. Depression and anxiety changes were compared before and after treatment with Hamilton depression scale and Hamilton anxiety scale. Telephone follow-up was conducted 4 weeks and 8 weeks after treatment. Results:The skin lesions and subjective symptoms of the 92 cases were improved in different degrees after 2 weeks and 4 weeks treatment, and the effective rates were 52.17% (48 cases) and 88.04% (81 cases) respectively, and the difference was statistically significant ( Z=-9.24, P<0.01). The depression scores (16.89±4.53) before treatment were significantly higher than those after treatment (4.38±2.10), and the difference was statistically significant ( t=35.79, P<0.01). 28 cases (30.43%) had clinically significant depression symptoms before treatment, and none after treatment. The difference was statistically significant ( Z=-5.39, P<0.01). The scores of anxiety (14.39±3.97) before treatment were significantly higher than those after treatment (4.46±1.67). and the difference was statistically significant ( t= 28.87, P<0.01). 50 cases (54.35%) had clinically significant anxiety symptoms before treatment, and none after treatment. The difference was statistically significant ( Z=-7.07, P=<0.01). The total scores of Hamilton depression scale were positively correlated with course of disease before and after treatment ( r=0.27, P<0.05; r=0.22, P<0.05). The total scores of Hamilton anxiety scale were positively correlated with course of disease before and after treatment ( r=0.29, P<0.01; r=0.35, P<0.01). They were followed up by telephone 4 weeks and 8 weeks after the end of treatment, and there was no relapse. Conclusions:Facial hormone-dependent dermatitis can affect appearance, leading to depression and anxiety, and early and full treatment can achieve satisfactory results.

RESUMEN

Aggressive angiomyxoma (AA) is an extremely rare locally invasive mesenchymal tumor with a high risk of recurrence. Till date, only about 350 cases reported worldwide. Because of the rarity it should be considered as differential diagnosis whenever patient present with vulvovaginal growth. The diagnosis is clinched on histopathology. These are hormone-dependent and have estrogen and progesterone receptors. Hence sometimes GnRH agonists are used for ovarian estrogen secretion suppression but long-term use is not advocated due to side effects. A 45-year-old P4 L4 perimenopausal female presented to the GOPD with a 4×4×3 cms pedunculated painless globular mass on right labia majora. On palpation, the globular mass was firm, non-tender and with a smooth surface. Mass was excised and on gross histopathology, cut sections showed white myxoid areas. On microscopy epidermal lined tissue with stellate and spindle-shaped mesenchymal cells was found, embedded in a loose myxoid stroma with few collagen fibers. The cells were small and bland and lacked nuclear atypia. Small to medium-sized blood vessels were present with the thickened wall. Entrapped nerves and adipocytes were also present. No necrosis or mitosis was identified. All these features were suggestive of an aggressive angiomyxoma. Immunohistochemistry markers ER, PR, CD34, desmin, SMA were all positive. Imaging was done to rule out metastatic lesions and wide local excision was done around the stump with laparoscopic bilateral oophorectomy. Aggressive angiomyxoma is a rare disease. In women with asymptomatic growth in the vulvovaginal region, perineum or pelvis, aggressive angiomyxoma should be considered as a differential diagnosis. Ideal treatment is a wide local excision to prevent local recurrences, which are common and a hypoestrogenic milieu is created by either GnRH Agonists or by bilateral oophorectomy due to their hormone-sensitive nature.

RESUMEN

Objective: To investigate the in vitro antiproliferative action of essential oil from Salvia officinalis L. (S. officinalis) grown in Sicily (Italy), and its main components on hormone-dependent cancer cell lines. Methods: S. officinalis essential oil was prepared by hydrodistillation. The actions of the S. officinalis essential oil and its three principal components ( -thujone, 1,8-cineole and camphor) were evaluated in LNCaP cells (prostate carcinoma), MCF7 cells (breast carcinoma) and HeLa cells (cervical carcinoma) at various dosages and diverse time points. Cell viability and proliferation were estimated using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay. Results: S. officinalis essential oil at doses of 100 μg/mL and 200 μg/mL induced a significant reduction of cell viability in MCF7, LNCaP and HeLa cell lines after a 48-hour incubation. The same cell lines also showed decreased cell viability when they were treated with a mixture of three major components of the essential oil, at doses of 100 μg/mL and 200 μg/mL, after a 48-hour incubation. Conclusions: These preliminary results could shed light on the formulation of new therapeutic agents with antiproliferative activity. Thus supplementary investigations are fundamental to examine the molecular mechanisms of the anticancer effects of this species of Salvia in cancer cells and to achieve confirmation of its in vivo anticancer activity.

RESUMEN

This study was aimed to observe the mechanism of Jia-Jian Xiao-Yao (JJXY) Powder improving facial skin barrier function of the hormone-dependent dermatitis patients. A total of 100 patients were randomly divided into two groups. In the treatment group, 50 cases were orally administered with JJXY Powder. In the control group, loratadine and vitamin EC particles were orally administered to the 50 cases. Thirty days is one treatment course. Changes of transepidermal water loss (TEWL), skin erythema amount and IL-4 were detected in patients of both groups before and after the treatment. The results showed that the clinical efficiency of treatment group and control group was 82.0% and 58.0%, respectively. In the treatment group, the TEWL and skin erythema amount were obviously decreased after the treatment (P < 0.05). In the treatment group, the decreasing range of serum IL-4 was significant compared to control group (P < 0.05). It was concluded that JJXY Powder is one of the effective methods for the treatment of facial hormone-dependent dermatitis. It is able to repair facial hormone-dependent dermatitis in patients with impaired skin barrier function and reduce the amount of skin erythema. The therapeutic mechanism may be related to the regulation of serum IL-4.

RESUMEN

Chemotherapy can induce bone loss. It is especially severe and the incidence of estimated bone loss may be as high as almost 100% for cancer patients treated with anti-sex hormone agents because of inhibition and deficiency of estrogens and/or androgens with the inappropriate ratio of two hormones during and "after therapy. Anti-sex hormone-dependent neoplasm agents-induced bone loss, characterized by an increase of bone turnover rate, should be treated in the early stage in order to prevent it from deterioration and bone fractures. The etiology, mechanism and clinical management of anti-sex hormone-dependent neoplasm agents-induced bone loss are briefly discussed in this review.

RESUMEN

Objective:To investigate the effect of intermittent hormonal therapy(IHT) for patients with different stage/grade prostate cancer(PCa).Methods:The number of cycles and the duration of ON/OFF therapy for 45 PCa patients receiving IHT were observed.Maximal androgen blockade(MAB) the-rapies were used for six to nine months, and then stopped until the serum prostate specific antigen(PSA) was decreased below 0.2 ?g/L,which lasted for three months. It was decided whether MAB went on according to the level of PSA.Results:The average follow-up time was 40.7?13.4 months.Forty-one patients started the second cycle of treatment,of whom,8 became androgen-independent and 7 were at T3-4M0 or M1 stages and the Gleason scores were above 8.Sixteen patients entered the third cycle,of whom,14 were at lower than stage Ⅲ and 13 had the Gleason scores below 7.From the first to the fourth courses of treatment, the average intervals were 8.7?5.4(47.1%),8.4?4.9(49.3%),7.0?3.4(43.7%),and 3.7?0.6(42.5%)months respectively.Five patients developed bone metastasis. No one died up to now.According to the evaluation criteria, patients were divided into tolerance(n=16) and intolerance groups(n=29).Compared with the intolerance group, the patients who tolerated the treatment well had lower Gleason scores(P=0.002),lower PSA levels(P=0.053) and lower tumor stages(P=0.001).There was no evidence that age, lymph node metastasis, bone metastasis and the state of recurrence were associated with an increased risk of the outcome.Non-conditional Logistic regression analysis showed that the proportion of patients at stage Ⅳ was the only independent risk factor for the tolerance of the treatment(OR=12.113,95%CI 1.330-110.312,P=0.027).Conclusion:Intermittent hormonal therapy is more effective and proper for the patient with highly differentiated tumor and at lower stages(≤Ⅲ).The patients who progressed to hormone-independence are mostly at stage Ⅳ with poorly differentiated tumor.Intermittent hormone therapy could be more adaptive for the patients at lower than stage Ⅲ.
