RESUMEN

Objective To make a systematic review of therapeutic effect of electroacupuncture(EA) for idiopathic facial palsy at acute stage. Methods With reference to the included criteria and excluded criteria, clinical controlled trials of EA for idiopathic facial palsy at acute stage were screened from the databases of CBM, CNKI, Wanfang, VIP by computer retrieval and from the primary domestic academic journals of acupuncture by manual retrieval. Systematic review was performed following the Cochrane Reviewers' Handbook, and RevMan 5.3 was used for Meta-analysis. Results Sixteen clinical trials were included. The Meta-analysis results showed that the cure rate evaluated with 3 kinds of therapeutic effect criteria in the observation group was superior to that of the control group, the difference being significant (P<0.05 or P<0.01); the effects on improving days for cure, cure case number within one month, blink reflex, and case number with complications in the observation group were also superior to that of the control group, the difference being significant (P < 0.01). Conclusion EA exerts certain therapeutic effect for idiopathic facial palsy at acute stage. But for the quantity and quality of the included trials are not satisfactory, the conclusion still needs more high-quality, large-size and long-term follow-up trials to verify.

RESUMEN

Objective To observe the clinical effect of acupuncture combined with ganglioside and mecobalamine injections on idiopathic facial palsy (IFP).Methods A total of 120 IFP patients were divided into a ganglioside group,a mecobalamine group and a consociation group,each of 40.All were given routine drug and acupuncture treatment,while the ganglioside and mecoblamine groups were additionally provided with the corresponding injections of ganglioside and mecobalamine,respectively.The treatment was administered once daily for a total of 14 days.All the groups were evaluated using the House-Brackmann facial nerve grading system before and after the interventions of 14 days ended.The compound muscle action potential (CMAP) wave amplitude and R1 incubation,the motor nerve conduction velocity (MCV) and the sensory nerve conduction velocity (SCV) of the affected facial nerves were observed and compared before and after the 14 days of treatment.Results The total effectiveness rate of the consociation group (95.0%) was significantly higher than that of the ganglioside (90.0%) or the mecobalamine group (87.5%).After the treatment the CMAP wave amplitude had improved significantly and the R1 latency had shortened significantly in the consociation group compared to the other 2 groups.The average MCV and SCV of the consociation group had also improved significantly compared with the other two groups.Conclusion Acupuncture combined with ganglioside and mecobalamine injections is an effective therapy for idiopathic facial palsy.It can improve the patient's clinical symptoms,shorten the CMAP wave amplitude and R1 latency on the affected side and improve MCV and SCV.

RESUMEN

Objective To explore the factors related to the outcome of idiopathic facial palsy. Methods 308 patients with idiopathic fa-cial palsy were included. The data of clinic and follow-up were collected from January, 2010 to December, 2014. The data were analyzed by Logistic regression analysis. Results 210 cases (68.2%) were cured (good group) and 98 cases (31.8%) appeared sequelae of different de-grees (poor group). The age, onset of disease, type of disease, damaged section, impaired glucose control, high neutrophil-to-lymphocyte ra-tio (NLR), high blood glucose, high blood triglyceride, time and methods of invention were significant factors related to the outcome (P<0.05). Conclusion Old, servious facial nerve injury, Hunt's palsy, high damaged section, poor glucose control in the patients with diabetes, high NLR, high blood triglyceride, delay and simple invention are independent risk factors for the poor outcome of idiopathic facial palsy.

Objective To investigate the reliability and validity of Simple Facial Grading System (SFGS) in patients with idiopathic fa-cial palsy. Methods 80 patients with idiopathic facial palsy were evaluated with the House-Brackmann (H-B) scale and SFGS before and ev-ery 2 weeks after treatment until 12 weeks or recovery. They were assessed by 2 testers before and 2 weeks after treatment. The results were analyzed and compared. Results The Cronbach'sαcoefficient was 0.93 of SFGS, and 0.74 of H-B scale. The intraclass correlation coeffi-cient (ICC) between testers was 0.84 or higher of SFGS, and 0.77 or higher of H-B scale. The Spearman's rank correlation coefficient be-tween SFGS and H-B scale was 0.73, Kappa coefficients was 0.81 for grade II, 0.74 for grade III, 0.49 for grade IV, 0.66 for grade V, and 0.92 for grade VI. The scores of SFGS and H-B scale positively correlated with the recovery time. Conclusion SFGS is a kind of evaluation for peripheral facial paralysis with satisified reliability and validity, which can be applied to evaluate the severity and outcome during acute stage.

@#: Idiopathic facial palsy,also called the Bell's paralysis,refers to around the single side with unclear reason and acute onset.It was one common disease caused by acute non-purulent inflammation of the stylomastoid foramen and the main characteristic was the motor function disorder of facial mimetic muscles.When the facial nerve or stylomastoid foramen had the inflammatory edema,it will oppress the facial nerve inducing mimetic muscles paralysis on sick side.In recent years domestic and foreign experts treat the idiopathic facial palsy with medicine,physical therapy,acupuncture and moxibustion,and etc.,and obtain exciting effect..

RESUMEN

OBJECTIVE: The purpose of this study was to observe clinical characteristics and electrodiagnostic findings of idiopathic facial palsy and to follow up beyond 1 year after onset. METHOD: From February 2002 to July 2003 the authors analyzed 103 cases that could be followed up after 1 year since diagnosed as idiopathic facial palsy by electrodiagnostic study which was performed at approximately 2 weeks after the onset time. The patients were classified by House- Brackmann (H-B) facial nerve grading system on their first visits and followed up by telephone interview using H-B system. Treatment method, age, sex, medical history and symptoms were noted. In addition, the blink reflex, nerve conduction study and needle electromyography (EMG) were done. RESULTS: When degree of degeneration (% degeneration) was greater than 90% at approximately 2 weeks after the onset or motor unit action potentials were not detected in at least one among the four tested muscles, patients did not gain satisfactory facial function after 1 year. CONCLUSION: Methods related to prognosis of idiopathic facial palsy were compared with side to side evoked potential amplitude and needle EMG. This methods would be helpful to explain its prognosis.
