RESUMEN

Objetivo: Determinar la validez y confiabilidad de la versión colombiana de la Escala de cuidado individualizado (ICS-paciente) diseñada por la Doctora Riitta Suhonen y colaboradores, en pacientes hospitalizados en una institución hospitalaria de la ciudad de Bogotá. Diseño del estudio: Este es un estudio de tipo metodológico, planteado en cuatro etapas que corresponden a la validez facial, validez de contenido, estimación de fiabilidad y validez de constructo. La muestra de pacientes (n=170) para la estimación de confiabilidad y validez de constructo fue recolectada en los servicios de hospitalización de un hospital universitario privado sin ánimo de lucro en la ciudad de Bogotá. Resultados: Esta escala presentó un índice de validez facial y de contenido superior a 0,80 en cada una de las variables evaluadas. En cuanto a la validez de constructo, el instrumento en general evalúa el concepto de cuidado individualizado planteado por la autora; sin embargo, la estructura factorial obtenida difiere de la resultante en la validación de la escala original, y de las obtenidas en otras investigaciones. La confiabilidad, en términos de consistencia interna, fue estimada mediante el coeficiente de alfa de Cronbach, que reportó un valor de 0,935 para la parte A y de 0,919 para la parte B. Conclusión: La versión colombiana de la Escala de cuidado individualizado ICS-paciente resulta válida y confiable para la medición de la percepción del cuidado individualizado de enfermería recibido por el paciente adulto, enmarcado en el modelo de atención centrado en el paciente. Se exponen las recomendaciones para el uso de este instrumento en futuros estudios.


Objective: Determine the validity and reliability of the Colombian version of the Individualized Care Scale (ICS-patient) designed by Dr. Riitta Suhonen and collaborators, in patients hospitalized in a hospital institution of the city of Bogotá. Study design: This is a methodological study, raised in four stages that correspond to facial validity, content validity, reliability estimation and construct validity. The sample of patients (n = 170) for estimating reliability and construct validity was collected in the hospitalization services of a private non-profit university hospital in the city of Bogotá. Results: This scale presented an index of face and content validity greater than 0.80 in each of the variables evaluated. Regarding construct validity, the instrument generally assesses the concept of individualized care proposed by the author; However, the factorial structure obtained differs from the one resulting from the validation of the original scale, and from those obtained in other investigations. Reliability, in terms of internal consistency, was estimated using Cronbach's alpha coefficient, which reported a value of 0.935 for part A and 0.919 for part B. Conclusion: The Colombian version of the ICS-Patient Individualized Care Scale resulting valid and reliable for measuring the perception of individualized nursing care received by adult patients, framed in the patient-centered care model. Recommendations for the use of this instrument in future studies are presented.

RESUMEN

We experienced 8 cases of children who presented with somatic complaints and found it difficult to attend school successfully treated with Kampo medicine. We evaluated them objectively by using the evaluation scale in the guideline for medical care of school refusal. Four cases were orthostatic dysregulation, 2 cases were migraine, 1 case was involuntary movement in upper abdomen and the other case was autonomic dysfunction. Kampo medicines were selected according to conventional method, so-called sho. Among 8 cases, 5 cases had a major response, 2 cases had a minor response and one case had no beneficial response. These results suggest that Kampo therapy can be effective therapeutic means. Kampo therapy is quite personalized and beneficial especially for children who find it difficult to attend school and need individualized care. This is the first report evaluating many cases objectively by using the evaluation scale and there have been no reports similar to ours. We have to continue to enlighten general pediatricians about the effectiveness of Kampo therapy.

Objective To translate and revise the Individualized Care Scale-Patient Version(ICS-P) into Chinese,then to assess the reliability and validity of the Chinese version of the Individualized Care Scale-Patient Version (C-ICS-P).Methods Standard forward-back translation techniques were used in the translation of the ICS-P according to the Brislin translation model.Cross-cultural revision of the translated ICS-P was carried out through group discussion and pretesting.Totally 223 patients were recruited through convenience sampling method from a tertiary hospital in Changsha and investigated using general information questionnaire and the C-ICS-P,and its reliability and validity were assessed.Results The C-ICS-P contained two subscales,and both C-ICS-P-A and C-ICS-P-B contained 3 factors explaining 61.330% and 65.263% of the total variance.The dimensions of C-ICS-P-A were clinical characteristics (6 items),personal life characteristics (4 items) and participation willingness (5 items);the dimensions of C-ICS-P-B were clinical care (6 items),personal life care (4 items) and decisional control over care (5 items).The Cronbach's α coefficients of C-ICS-P-A and its dimensions were 0.897,and 0.730~0.774;the Cronbach's α coefficients of C-ICS-P-B and its dimensions were 0.909,and 0.688~0.754.Split-half reliability was 0.856 for C-ICS-P-A and 0.688~0.754 for its dimensions;split-half reliability was 0.889 for C-ICS-P-B and 0.750~0.758 for its dimensions.Analysis of content validity of the C-ICS-P indicated that I-CVI was at least 0.83,S-CVI was 0.943.Conclusion The reliability and validity of C-ICS-P are satisfactory and well meet the requirements of psychological measurement,indicating C-ICS-P is a reliable and valid instrument in the context of Chinese culture.

RESUMEN

Objective To analyze the effect of individualized care on prevention of postoperative urinary retention of malignancy patients undergoing hysterectomy and pelvic lymph node dissection.Methods 74 cases of patients with malignancies undergoing total hysterectomy and pelvic lymph node dissection in our hospital from June 2009 to December 2011 were chosen as the research object.They were divided into the control group and the observation group with 37 cases in each group.The control group was given traditional routine care,while the observation group was given individualized nursing assessment and intervention.The grasping level of knowledge about prevention of urinary retention,the cooperation degree with nursing,incidence of urinary retention,the residual urine volume and satisfaction degree of patients were analyzed and compared between two groups.Results The grasping level of knowledge about prevention of urinary retention,the cooperation degree with nursing,incidence of urinary retention,the residual urine volume and satisfaction degree of patients in the observation group were better than those of the control group,there were significant differences.Conclusions The effect of individualized care to prevent postoperative urinary retention of malignancies patients after hysterectomy and pelvic lymph node dissection is better.It plays an active role in improving mastering of knowledge about prevention of urinary retention and cooperative degree with nursing.
