RESUMEN

Periarthritis is a chronic, inflammatory disorder of the shoulder and surrounding soft tissues, caused by injury, leading to pain and lack of use. Various therapeutic methods and surgical treatments have been introduced for shoulder periarthritis. This observational study was carried out on 16 patients clinically and radiologically diagnosed with periarthritis of shoulder to assess the efficacy of SSNB in the treatment of periarthritis of the shoulder. In this study the mean abduction increased and reached from 72.3° ± 3.10° to 116.3°± 4.8° at the follow-up of 12-weeks. Mean flexion, mean internal rotation and mean external rotation shows an improvement of 41.50, 19.8° and 23.50 respectively at 12-weeks follow-up. The mean VAS score was 7.25 ± 0.9, which shows a significant reduction at different time intervals. In this study we conclude that supra-scapular nerve block (SSNB) is an effective method for controlling pain and accelerating recovery of shoulder motion in patients with periarthritis of shoulder.

ObjectiveTo investigate the clinical efficacy of Guizhi Shaoyao Zhimutang (GZSYZM) combined with fire needling in the treatment of periarthritis of shoulder with wind-cold-dampness impediment syndrome by stimulating pain points and "shoulder three acupoints". MethodA total of 120 patients with periarthritis of shoulder with wind-cold-dampness impediment syndrome, admitted to Hainan General Hospital from August 2020 to August 2022, were randomly divided into two groups using a random number table. The control group (60 cases) received treatment with GZSYZM for two weeks, while the observation group (60 cases) received treatment with GZSYZM combined with fire needling at pain points and "shoulder three acupoints" for two weeks. The clinical efficacy, adverse reactions, shoulder pain intensity, shoulder joint function, and levels of calcitonin gene-related peptide (CGRP), cyclooxygenase-2 (COX-2), interleukin (IL)-2, and IL-17 before and after treatment were compared between the two groups. ResultThe total effective rate in the observation group was 88.33% (53/60), significantly higher than 68.33% (41/60) of the control group (χ²=7.070, P<0.01). Compared with the results before treatment, both groups showed significant reductions in pain rating index (PRI), visual analog scale (VAS) scores, present pain intensity (PPI), and Simplified McGill Pain Questionnaire (SF-MPQ) total scores, as well as serum levels of CGRP, COX-2, IL-2, and IL-17 after treatment (P<0.01), and improved pain intensity, daily life abilities, joint mobility, muscle strength, and Constant-Murley score (P<0.01). Compared with the control group after treatment, the observation group showed significantly reduced PRI, VAS score, PPI, SF-MPQ total score, as well as serum levels of CGRP, COX-2, IL-2, and IL-17 (P<0.01), and increased pain intensity, daily life abilities, joint mobility, muscle strength, and Constant-Murley score (P<0.01). There was no significant difference in the incidence of adverse reactions between the two groups. ConclusionGZSYZM combined with fire needling at pain points and "shoulder three acupoints" can effectively reduce the levels of serum inflammatory factors and pain mediators, alleviate pain, and improve shoulder joint function in patients with periarthritis of shoulder with wind-cold-dampness impediment syndrome.

Objective:To investigate the clinical value of the modified lateral approach to the shoulder for the treatment of proximal humeral fractures.Methods:A total of 64 patients with proximal humeral fractures who received treatment in Zhoushan Branch, Ruijin Hospital, Shanghai Jiaotong University School of Medicine from May 2018 to May 2022 were included in this study. They were randomly divided into observation and control groups ( n = 32/group). The observation group was treated using a modified lateral approach to the shoulder. The control group was treated using the anteromedial approach to the shoulder. Perioperative indexes (operation time, intraoperative blood loss, postoperative drainage volume, hospital stay, and incision length) were compared between the two groups. Before surgery, 1 week and 3 months after surgery, the visual analogue scale score and the Constant-Murley shoulder assessment score were compared between the two groups. Before surgery and 3 months after surgery, the shoulder range of motion was compared between the two groups. The incidence of complications was also compared between the two groups. Results:The operation time, intraoperative blood loss, postoperative drainage volume, hospital stay, and incision length in the observation group were shorter or lower than those in the control group ( t = 7.42, 26.85, 10.90, 2.73, 10.59, all P < 0.05). At 1 week and 3 months after surgery, the visual analogue scale score in the observation group was significantly lower than that in the control group ( t = 5.80, 6.06, both P < 0.001). At 1 week and 3 months after surgery, the Constant-Murley shoulder assessment score in the observation group was (62.96 ± 12.05) points and (74.96 ± 14.52) points, respectively, which were significantly higher than (56.74 ± 9.62) points and (67.88 ± 12.25) points in the control group ( t = 2.28, 2.10, both P < 0.05). After surgery, the range of motion of the shoulder joint in the observation group was greater than that in the control group, including forward flexion, backward extension, external rotation, and internal rotation ( t = 2.54, 3.19, 2.40, 4.00, all P < 0.05). The incidence of complications in the observation group was 6.25% (2/32), which was significantly lower than 28.13% (9/32) in the control group ( χ2 = 5.39, P < 0.05). Conclusion:The modified lateral approach to the shoulder has a marked effect on proximal humeral fractures. The approach can improve shoulder function, shorten operation time, and decrease the incidence of complications.

Objective::To explore the clinical efficacy of Magui Wenjingtang combined with joint mobilization in the treatment of periarthritis of the shoulder and the effect on hemodynamics and serum pain media. Method::A total of 120 patients with scapulohumeral periarthritis were enrolled in Tianjin Ninghe Hospital from July 2016 to July 2018.All cases were divided into control group and observation group, with 60 patients in each group. The control group received shoulder joint loosening operation (qd), and celecoxib capsules for oral administration. The observation group was treated with Magui Wenjingtang in addition to the shoulder joint loosening operation, 1 dose/d, 2 times/d. After 4 weeks of continuous treatment, the subjects were compared for the subjective pain level McGill Pain Questionnaire (SF-MPQ) score, shoulder mobility hemodynamic changes, and serum prostaglandin E2 (PGE2), substance P (SP), serotonin (5-HT) level and clinical efficacy. Result::Compared with before treatment, visual pain score (VAS), current pain status (PPI), and pain rating index (PRI) scores of the two groups were significantly lower after 4 weeks of treatment, and those of the observation group was significantly lower than the control group (P<0.01). The range of shoulder joint activity was significantly higher than that of the control group (P<0.01), erythrocyte sedimentation rate (ESR), fibrinogen (Fb), whole blood viscosity and serum were compared between the two groups after 4 weeks of treatment and before treatment. The levels of PGE2, SP and 5-HT were significantly lower (P<0.01), and the observation group was significantly better than the control group (P<0.01). The total effective rate of the observation group was 96.61%(57/59), which was significantly higher than 82.76% (48/58) of the control group(χ2=4.685, P<0.05). Conclusion::Magui Wenjingtang combined with shoulder joint loosening operation in treating periarthritis of shoulder with syndrome of blood deficiency and cold coagulation can effectively improve the pain and shoulder joint mobility, increase the curative effect, and improve the hemodynamics, and down-regulatie pain mediator of PGE2, SP, 5-HT, which may be related to the curative effect.

Objectives: The objective of this study is to identify and categorize the variations in origin of tendon of long headof biceps brachii (LHBB).Methods: The study was carried out on 60 upper limbs of cadaver in the Anatomy department, SMBT IMS & RC,Nashik, India. After exposing the glenoid fossa, we find out the origin of tendon of LHBB from the supraglenoidtubercle and the adjoining glenoid labrum. Labrum is divided into anterior and posterior part.Results: In 11 specimens origin of LHBB was seen from supraglenoid tubercle only, Other specimens has dualorigin of LHBB along with Glenoid labrum. We classified it according to Vangsness et al. Type I: The labralattachment is entirely posterior, with no contribution to anterior labrum, seen in 31 specimens. Type II: Thetendon attached mainly to the posterior part of the Glenoid labrum with also extended up to the anterior labrum,seen in 16 specimens. Type III: There are equal contribution to both the anterior and posterior parts of thelabrum, found in only 2 specimens. Type IV: Most of the labral contribution is anterior, with a small contributionto the posterior labrum. No specimen found.Conclusion: Anatomical variation in origin of LHBB help us to explain the correlation of recurrent shoulderdislocation and labral detachment.This knowledge is necessary to avoid errors in shoulder arthroscopy,radiological investigations and surgical repair

Objective To explore the clinical effect of the semi-shoulder arthroplasty in the treatment of humeral head necrosis.Methods Twenty patients with head necrosis of the humerus in first hospital affiliated to army medical university from February 2008 to January2018 were collected, including 8 cases of males, 12 cases of females, 7 cases of left shoulder and 13 cases of right shoulder.The patients were aged from 45 to 83 years old, mean (67.40±5.06) years old.All patients were followed up for at least 6 months, the anterior flexion angle, abduction angle, external rotation angle and internal rotation angle of shoulder joint were measured, the function of shoulder joint was evaluated by ASES, UCLA, SST, and VAS, and the imaging examination was conducted.Results All patients were followed up for 6 to 37 months after surgery, with average (18.50±5.31) months, 2 patients presented mild pain during shoulder joint activity, 1 patient presented brachial plexus nerve damage, but returned to normal 3 months after surgery.No complication happened.X-ray reexamination showed good position and angle of the prosthesis during the follow-up period.The preoperative anteflexion angle, angle of outreach, swing angle and swing angle of the shoulder joint were respectively (55.24±8.21) °, (42.58±6.21) °, (12.95±2.74) °, (17.79±3.65) °, the last follow-up were respectively (120.76±13.15) °, (103.08±10.54) °, (33.51±3.14) °, (50.10±7.25) °, the differences were significant (P<0.01);The preoperative ASES score, UCLA score, SST score, VAS score of the shoulder joint were respectively (38.24±5.21), (12.58±3.93), (3.25±1.42), (6.79±1.65), the last follow-up were respectively (75.74±9.69), (33.08±4.5), (9.11±1.85), (1.45±0.24), the differences were significant (P<0.01).Conclusion Artificial semi-shoulder replacement for the treatment of humeral head necrosis can significantly improve the range of limb function, relieve the pain symptoms of patients and improve patients'quality of life, which has excellent and good shoulder function rate and fewer complications.

SUMMARY: The critical shoulder angle and acromion index are conventional radiological tools employed as predictors of shoulder degeneration. As they represent the static components of glenohumeral stability, the scapulo-humeral geometry and underlying subacromial tissue appear as the resultant cause-effect factors. Consequently, the purpose of this study was to investigate the critical shoulder angle and acromion index as interrelated parameters within the South African population. The measurement of both biomechanical parameters was conducted on two-hundred and sixty (n = 260) true AP radiographs. This was a cross-sectional study that also incorporated the demographic representation of the population group which was analysed accordingly. The mean values recorded for both the critical shoulder angle (36.31±5.84º) and acromion index (0.74±0.13) suggested rotator cuff arthropathy. The results confirmed the theories of Nyffeler et al. (2006) and Moor et al. (2012) who alluded to glenoid inclination and the acromial coverage over the humeral head. A significant proportionality correlation, verified by a P value of 0.000, was established between the acromion index and critical shoulder angle which may assist to differentiate between normal asymptomatic shoulders and those with cuff disease. Furthermore, these predictors of shoulder degeneration may present as a preventative tool against tear progression.


RESUMEN: El ángulo crítico del hombro y el índice acromial son herramientas radiológicas convencionales empleadas como indicadores de la degeneración del hombro. Debido a que representan los componentes estáticos de la estabilidad glenohumeral, la geometría escápulo-humeral y el tejido subacromial subyacente aparecen como los factores causa-efecto resultantes. En consecuencia, el propósito de este estudio fue investigar el ángulo crítico del hombro y el índice acromial como parámetros interrelacionados dentro de la población sudafricana. La medición de ambos parámetros biomecánicos se realizó en 260 radiografías antero-posteriores (AP). Se realizó un estudio transversal que también incorporó la representación demográfica del grupo de la población que fue analizada. Los valores medios registrados tanto para el ángulo crítico del hombro (36,31 ± 5,84º) como para el índice del acromión (0,74 ± 0,13) sugirieron una artropatía del manguito rotador. Los resultados confirmaron las teorías de Nyffeler et al. (2006) y Moor et al. (2012) que aludían a la inclinación glenoide y a la cobertura acromial sobre la cabeza humeral. Se estableció una correlación de proporcionalidad significativa, verificada por un valor de P de 0,000, entre el índice acromial y el ángulo crítico del hombro, lo que puede ayudar a diferenciar entre los hombros asintomáticos normales y aquellos con enfermedad del manguito rotador. Además, estos predictores de degeneración del hombro pueden ser útiles como una herramienta preventiva contra la progresión del desgarro.

Objective To investigate the method and effect of arthroscopic repair of recurrent anterior shoulder dislocation caused by anterior and inferior labrum complete defect.Methods Retrospective analyzed the clinical data of 23 patients with anterior and inferior la-brum complete defect who were admitted into hospital from June 2014 to October 2016 and recieved arthroscopic repair opreation with gleno-humeral ligament combined with articular capsule.Intraoperative arthroscopy was used to suture the lower glenohumeral ligament and articular capsule at the missing part of the glenoid labrum.The Rowe score,ASES score and UCLA score were compared before and after operation. Results There was no postoperative complications such as dislocation or infection occurred.All the 23 cases were followed up for 12~42 months,with an average of(19.6 ±3.4)months.The average lost angle of shoulder external rotation was(17.3 ±3.9)°.And the postopera-tive Rowe score,ASES score and UCLA score were significantly better than those before operation.Conclusion Arthroscopic repair opreation with glenohumeral ligament combined with articular capsule can effectively restore the stability of front shoulder joint,and it can be a good method for the treatment of recurrent anterior shoulder dislocation caused by anterior and inferior labrum complete defect.

Objective:To explore the modification of the evaluation scale for periarthritis of shoulder and to assess the clinical use of it.This would be helpful to provide reference for the diagnosis and treatment of periarthritis of shoulder.Method:The periarthritis of shoulder assessment scale was modified by resetting the scoring logic and adjusting the contents and structures.The efficiency of the modified scale was then judged by the analysis of clinical assessment data.Result:The modified evaluation scale for periarthritis of shoulder showed good reliability and validity.The Cronbach's α coefficient was 0.876,and the KMO value was 0.900.Conclusion:The modified evaluation scale for periarthritis of shoulder is applicable for the evaluation of clinical symptoms and treatment efficacy.This scale still needs to be optimized and improved in future clinical practice.

Objective To observe the clinical efficacy of triple needling at myofascial trigger points plus warm needling in treating periarthritis of shoulder. Method Eighty patients with periarthritis of shoulder were randomized into a treatment group of 44 cases and a control group of 40 cases. The treatment group was intervened by triple needling at myofascial trigger points plus warm needling, while the control group was by warm needling alone. The two groups were treated once a day, 10 times as a treatment course. The Pain Rating Index (PRI), Visual Analogue Scale (VAS), Present Pain Intensity (PPI), and motion of shoulder joint were observed before the treatment and after 2 treatment courses, and the clinical efficacies were compared between the two groups.Result In the treatment group, the PRI, VAS, and PPI scores respectively after 1 and 2 treatment courses were significantly different from those before the treatment (P<0.05,P<0.01). In the control group, the scores after 2 treatment courses were significantly different from those before the treatment (P<0.05). After 2 treatment courses, the scores in the treatment group were significantly different from those in the control group (P<0.01). In both groups, the motions of shoulder joint respectively after 1 and 2 treatment courses were significantly changed compared with those before the treatment (P<0.01). The motions of shoulder joint in the treatment group were significantly different from those in the control group respectively after 1 and 2 treatment courses (P<0.05,P<0.01). The recovery plus markedly-effective rate and total effective rate were respectively 70.5% and 95.5% in the treatment group, versus 52.5% and 90.0% in the control group, and the between-group differences were statistically significant (P<0.01,P<0.05).Conclusion Triple needling at myofascial trigger points plus warm needling is an effective approach in treating periarthritis of shoulder.

Objective To observe the clinical efficacy of extracorporeal shock wave (ESW) plus electroacupuncture in treating periarthritis of shoulder.Method Ninety patients with periarthritis of shoulder were randomized into an electroacupuncture (EA) group, an ESW group, and an ESW plus EA group, 30 cases in each group. The EA group was intervened by EA, the ESW group was treated with ESW, and the ESW plus EA group by ESW and EA. The three groups were treated once every 2 d, with successive 10 sessions as a treatment course. The Visual Analogue Scale (VAS) score and shoulder range of motion (ROM) score were evaluated before and after the treatment in the three groups. Result The VAS scores dropped significantly in the three groups after the intervention (P0.05); the VAS score in the ESW plus EA group was significantly different from that in the EA group and ESW group after the treatment (P0.05); the ROM score in the ESW plus EA group was significant different from that in the other two groups (P<0.05).Conclusion ESW plus EA can more significantly ease the pain and improve the shoulder ROM in treating periarthritis of shoulder compared with the two methods used separately.

Objective To investigate the clinical effect of sodium hyaluronate combined with glucocorticoid on the treatment of periarthritis of shoulder.Methods106 patients in department of orthopedics, the second hospital of Ningbo with periarthritis of shoulder were randomly divided into 2 groups (n=53).The control group were treated with anti-inflammatory and analgesic treatment, at this basis, the study group were treated with sodium hyaluronate combined with Glucocorticoid.5 weeks for a course of treatment.Levels of serum inflammation, laboratory-related indicators were compared.ResultsCompared with before treatment, shoulder joint activity in the two groups increased, VAS score decreased, the scores of strength of shoulder joint, range of motion and daily activities increased, levels of serum IL-6, IL-10, TNF-α decreased(P<0.05).Compared with the control group, scores of activity in the study group were higher, VAS scores were lower, scores of strength of shoulder joint, range of motion and daily activities were higher, levels of serum IL-6, IL-10, TNF-α were lower(P<0.05).The effective rate in the control group(73.59%) was lower than the study group(90.57%), but there was no statistical difference.ConclusionThe effect is accurate which sodium hyaluronate combined with glucocorticoid was used in the treatment of periarthritis of shoulder, and it can reduce the inflammation index and improve the joint activity.

Objective To observe the effect and adverse effect of ultrashort wave combined with Etroricox-ib in treatment of periarthritis of shoulder. Methods 80 cases with periarthritis of shoulder from March 2015 to March 2016 were randomly assigned into treatment group and control group. Etroricoxib therapy was provided in control group while ultrashort wave plus Etroricoxib therapy in treatment group. VAS ,ROM and MBI were applied for the evaluation before and after the treatment. The differences of clinical cure rate ,recurrence rate and adverse reactions were observed between 2 groups. Results There were higher cure rates ,lower recurrence rates and less adverse reactions in 2 groups after the treatment. The shoulder joint pain ,rang of should motion and BI of both groups were improved with significant differences after the treatment ,but treatment group witnessed more improve-ment(P<0.05). Conclusions Ultrashort wave combined with Etroricoxib therapy can relieve shoulder pain ,and further improve the function of shoulder joint activity in the treatment of periarthritis of shoulder. It is recommended for clinical application.

Objective To study and analyze the clinical efficacy of massage combined with Astragalus and eucommia Decoction in the treatment of periarthritis of shoulder.Methods100 patients with periarthritis of shoulder treated in our hospital from July 2015 to July 2016 were selected.They were randomly divided into the control group and the experimental group, with 50 patients in each group.Patients in the control group were treated with massage, and the experimental group was treated with massage combined with astragalus root and Eucommia ulmoides decoction.The 2 groups were treated with 2 or two courses, the 1 course of treatment for 15 days.The treatment effect and relative shoulder index score of the experimental group and the control group were compared and analyzed.ResultsAfter the corresponding treatment, the effective number of patients in the experimental group was 48 cases.The effective rate of treatment was 96%.The number of effective cases in the control group was 44, and the effective rate was 88%.The effective rate of the experimental group was significantly higher than that of the controlgroup, with statistical difference (P<0.05).After treatment, the shoulder function, activity and pain in the experimental group and the control group were significantly better than those before the treatment,The scores of shoulder functional performance index and shoulder pain index in the experimental group were significantly lower than those in the control group, with statistical difference (P<0.05).ConclusionMassage combined with Huangqi Eucommia Decoction in the treatment of periarthritis of shoulder can improve the therapeutic effect to a great extent, reduce the pain of patients with shoulder joint, to help with the recovery of shoulder joint function, with further clinical promotion and application significance.

Objective To observe the clinical effect of brachial plexus anesthesia shoulder manipulation solution combined with individualized treatment in treatment of periarthritis of shoulder.Methods 80 patients with periarthritis of shoulder were randomly divided into control group and observation group,40 cases in each group.The control group was given brachial plexus anesthesia shoulder manipulation solution,and the observation group was given brachial plexus anesthesia shoulder manipulation solution combined with individualized treatment.Joint function score and cure rate were compared before and after treatment in the two groups.Results The joint function score [(93.68 ±3.74)] and cure rate (62.50%) of the observation group were significantly higher than those in the control group[(79.35 ±3.21),35.00%] after 2 months treatment (t =1.57,3.86,χ2 =6.05,all P <0.05). Conclusion Brachial plexus anesthesia shoulder manipulation solution with individualized treatment has obvious effect in treatment of periarthritis of shoulder,and can effectively restore joint function in patients.

This study was aimed to observe curative effect of periarthritis of shoulder with activating brain and re-gaining consciousness acupuncture method combined with surrounding needling acupuncture on shoulder joint. A to-tal of 120 patients were randomly divided into the treatment group (60 cases) and the control group (60 cases) on the basis of routine internal medicine treatment and rehabilitation exercises. Cases in the control group received conven-tional acupuncture treatment. And cases in the treatment group received activating brain and regaining conscious-ness acupuncture method combined with surrounding needling acupuncture on shoulder joint. The treatment course was 28 days. And then, the curative effect evaluation was conducted. The results showed that the effective rate was 73.33% in the control group and 93.34% in the treatment group. There was statistical significance on effective rate between both groups(P< 0.05). It was concluded that the activating brain and regaining consciousness acupuncture method combined with surrounding needling acupuncture on shoulder joint had significant curative effects for peri-arthritis of shoulder. This treatment method is worthy of further popularization and application.

RESUMEN

Objective To observe the treatment effect of joint manipulation under brachial plexus anesthesia combined with comprehensive physical therapy in treatment of periarthritis of shoulder. Mehods From 2010.3 to 2010.9, 60 patients with periarthritis of shoulder in the Department of Pain, The First People's Hospital of Jingzhou,were divided into 3 groups:group A,B and C.Twenty patients in Group A were given Joint manipulation under brachial plexus anesthesia for treatment of the periarthritis of shoulder. Twenty patients in Group B were given comprehensive physical therapy fro treatment of periarthritis of shoulder. Twenty patients in Group C were given Joint manipulation under brachial plexus anesthesia combined with comprehensive physical therapy for treatment of periarthritis of shoulder.Results All 60 patients were followed up for 1 months. At the 1st month after surgery, vas score was evaluated for patients. Postoperative and preoperative vas score difference was compared between 3 groups by t test. There was statistically significant difference in the postoperative and preoperative vas score between Group C and Group A,and between Group B and Group A (P<0.05) . Conclusion The treatment effect of Joint manipulation under brachial plexus anesthesia combined with physical therapy in treatment of periarthritis of shoulder is good, but has no obvious difference compared with comprehensive physical therapy.

@#ObjectiveTo observe the effects of the Partner Rehabilitation Gymnastics on periarthritis of shoulder in later stage. MethodsPatients were randomly divided into 2 groups: 60 in treatment group accepted the Partner Rehabilitation Gymnastics, and the other 60 accepted the forcing active and passive physiotherapy.Results76.66% of the treatment group recovered, 96.66% improved; while 45% of the control group recovered, 73.33% improved. The scores of visual analogous scale and activity of daily living improved after treatment in both groups, and the differences pre- and post-treatment were more in treatment group than in control group. ConclusionThe Partner Rehabilitation Gymnastics is more effective than the routine forcing active and passive physiotherapy on periarthritis of shoulder.

RESUMEN

Objective To observation the effect of treating wind-cold type periarthritis of shoulder with acupuncture with warmed needle. Methods Study the influence of acupuncture with warmed needle on clinical indexes as total therapeutic effect, pain, and range of activity of 60 patients with wind-cold type periarthritis of shoulder. Result The total effective rate was over 80%. Conclusion acupuncture with wanned needle is a good method in treating wind-cold type periarthritis of shoulder clinically.
