RESUMEN

Objective To study the efficiency changes of private hospitals in Shenzhen and explore influencing factors for the utilization efficiency of health resources. Methods DEA-Tobit two-stage model was used to analyze the operating efficiency and influencing factors of Shenzhen′s private hospitals. Results The range of technical efficiency varied between 0.690 to 0.742 in six years, and the index of total factor production was consistent with the technical change index. The input and output indicators showed non-linear growth. The internal and external factors of sample hospitals jointly influence the efficiency of operation. Conclusions The two-stage method effectively reveals the dynamic efficiency change and the influencing factors of these hospitals in Shenzhen. The technical efficiency of sample hospitals fluctuates in waves in six years,with investment redundancy and insufficient output found. Sample hospitals need to strengthen their innovative application of diagnostic and therapeutic technology,and to strengthen their internal management and improve operating efficiency.

RESUMEN

Objective To evaluate operating efficiency of 68 Zhejiang Provincial tertiary public hospitals in 2015 and analyze the reason of the efficiency differences among different types of hospitals so as to put forward correspondent policy suggestions.Methods Adopting the three-stage DEA model analysis upon filtering the external influencing factors and random error to the hospital and quantitatively evaluating the comprehensive technical efficiency,pure technical efficiency and scale efficiency of the hospitals.Results The result shows that it is necessary to carry out three-stage DEA model analysis.The mean value of hospital efficiency increases by 0.1,which is higher than before adjustment.Provincial hospitals efficiencies are higher than those of the non-provincial hospitals.Efficiencies of general hospitals are higher than those of the traditional Chinese Medicine(TCM) hospitals and the specialized hospitals.Conclusion Specialized hospitals utilize more resources than the TCM hospitals.The efficiency of provincial hospitals are relatively high,but they are facing the risk of excessive expansion.There is room for efficiency improvement.Prospective policies need to be initiated to make the hospital administrators change their management concept,control the scale of hospital,promote the fine management so as to achieve sustainable development.Furthermore,policies are needed to be adopted to support the development of TCM hospitals.

RESUMEN

PURPOSE: To improve the management of a medical linear accelerator, the records of operational failures of a Varian CL2100C over a ten year period were retrospectively analyzed. MATERIALS AND METHODS: The failures were classified according to the involved functional subunits, with each class rated into one of three levels depending on the operational conditions. The relationships between the failure rate and working ratio and between the failure rate and outside temperature were investigated. In addition, the average life time of the main part and the operating efficiency over the last 4 years were analyzed. RESULTS: Among the recorded failures (total 587 failures), the most frequent failure was observed in the parts related with the collimation system, including the monitor chamber, which accounted for 20% of all failures. With regard to the operational conditions, 2nd level of failures, which temporally interrupted treatments, were the most frequent. Third level of failures, which interrupted treatment for more than several hours, were mostly caused by the accelerating subunit. The number of failures was increased with number of treatments and operating time. The average life-times of the Klystron and Thyratron became shorter as the working ratio increased, and were 42 and 83% of the expected values, respectively. The operating efficiency was maintained at 95% or higher, but this value slightly decreased. There was no significant correlation between the number of failures and the outside temperature. CONCLUSION: The maintenance of detailed equipment problems and failures records over a long period of time can provide good knowledge of equipment function as well as the capability of predicting future failure. More rigorous equipment maintenance is required for old medical linear accelerators for the advanced avoidance of serious failure and to improve the quality of patient treatment.

RESUMEN

Objective To find an efficient plan for the arrangement of the beds in hospital so that the beds-operating efficiency can be raised and the requirement of patients can be perfectly met. Methods According to the data analyzed via the method of mathematical statistics, optimizing model was built and automatic stimulation program was generated as well as conducted by matlab software. Results The characteristics of bed-occupation condition was acquired by data analysis, the result of optimizing model was stimulated by program and the most efficient plan emerges which was obviously better than the original FCFS (First Come First Serve) Plan. Conclusion The mathematical model built up is of great maneuverability in the practical situation to ameliorate the management efficiency of hospital beds considerably.
