Efficacy of Shensong Yangxin Capsules in treating bradycardia combined with premature beat: a systematic review and Meta-analysis of randomized clinical trials / 中国中药杂志

To analyze the efficacy and safety of Shensong Yangxin Capsules in treatment of bradycardia combined with premature beat. Databases, such as CNKI, VIP, WanFang, SinoMed, PubMed, Cochrane Library, ClinicalTrials were retrieved by computers for relevant randomized controlled trials of Shensong Yangxin Capsules in treatment of bradycardia combined with premature beat. Two researchers independently screened out the literatures, extracted data according to the inclusion criteria, and applied the Risk of Bias assessment tool in assessing the methodological quality. The Cochrane systematic evaluation software RevMan 5.3 was used for data analysis. Totally 9 randomized controlled trials including 706 subjects were included. The intervention measure was the single administration with Shensong Yangxin Capsules, and the control measure was the blank control. The results showed that Shensong Yangxin Capsules had an obvious effect on average heart rate(MD=6.59, 95%CI[3.87, 9.31], I~2=90%), premature beat efficacy(RR=1.72, 95%CI[1.53, 1.93], I~2=0%), heart rate efficacy(RR=1.74, 95%CI[1.40, 2.17], I~2=47%), and objective efficacy(RR=1.50, 95%CI[1.31, 1.70], I~2=31%). Eight studies reported safety events, with no significant adverse reaction. In conclusion, the single administration with Shensong Yangxin Capsules may have a certain effect in improving heart rate, controlling premature beats and alleviating clinical symptoms in patients with bradycardia combined with premature beat, with no obvious adverse reaction. Shensong Yangxin Capsules can be used in clinic. This potential conclusion needs to be confirmed in future trials using rigorous methodology.

Clinical Observation of Metoprolol Combined with Shensong Yangxin Capsule in the Treatment of Ventricular Premature Beat / 中国药房

OBJECTIVE:To observe therapeutic efficacy and safety of metoprolol combined with Shensong yangxin capsule in the treatment of ventricular premature beat. METHODS:A total of 104 patients with ventricular premature beat during Jan. 2016-Jan. 2017 were analyzed retrospectively,and they were divided into control group(52 cases)and observation group(52 cases)according to medication plan. Control group was given Metoprolol tartrate tablet 25 mg orally,3 times a day. Observation group was given Shensong yangxin capsule 1.2 g orally,3 times a day,on the basis of control group. Both groups were treated for consecutive 28 d. Clinical efficacies of 2 groups were observed. The electrocardiogram P-R interval and Q-T interval,24 h premature beat times,heart rate,the occurrence of palpitation before and after treatment and ADR were observed. RESULTS:Total response rate of observation group(92.30%)was significantly higher than that of control group(76.92%),with statistical significance(P<0.05).Before treatment, there was no statistical significance in electrocardiogram P-R interval and Q-T interval,24 h premature beat times,heart rate or palpitation times between 2 groups(P>0.05). After treatment,electrocardiogram P-R interval and Q-T interval of 2 groups were significantly higher than before treatment,and the observation group was significantly higher than the control group. 24 h premature beat times,palpitation times of 2 groups and heart rate of control group were significantly lower than before treatment;and 24 h premature beat times,palpitation times of the observation group was significantly lower than the control group,with statistical significance(P<0.05);there was no statistical significance in heart rate of observation group before and after treatment(P>0.05). There was no statistical significance in the total incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:For ventricular premature beat,metoprolol combined with Shensong yangxin capsule show significant therapeutic efficacy,can reduce the times of premature beat and improve patients'electrocardiogram without increasing the occurrence of ADR.

Indentation in the Right Ventricle by an Incomplete Pericardium on 3-Dimensional Reconstructed Computed Tomography

We report the case of a 17-year-old girl who presented with an indentation in the right ventricle caused by an incomplete pericardium on preoperative 3-dimensional reconstructed computed tomography. She was to undergo surgery for a partial atrioventricular septal defect and secundum atrial septal defect. Preoperative electrocardiography revealed occasional premature ventricular beats. We found the absence of the left side of the pericardium intraoperatively, and this absence caused strangulation of the diaphragmatic surface of the right ventricle. After correcting the lesion, the patient’s rhythm disturbances improved.

The drug therapy of cardiac arrhythmias / 天津医药

Cardiac arrhythmias are varied and very common. They are usually classified as bradyarrhythmias and tachyarrhythmias, which are treated according to their own common principle separately. There is no long term effective oral drugs that can be used to treat bradyarrhythmias effectively up to now. Pacemaker is still the only effective treatment for it. Now catheter ablation has already applied to almost every kind of tachyarrhythmias, and which is the first choice for treating supraventricular tachycardia and atrial flutter because of the satisfactory result. Most patients with other types of tachyarrhythmias, especially atrial fibrillation and all kinds of premature beats are still given drug therapy. Amiodarone, propafenone, sotalol and mexiletine are common used antiarrhythmic drugs in our country. This article overviewed the principle and the matters need to attention in the drug therapy of cardiac arrhythmias.

Efficacy Observation of Potassium Magnesium Aspartate in the Auxiliary Treatment of Ventricular Prema-ture Beat / 中国药师

Objective:To observe the curative effect of potassium magnesium aspartate in the auxiliary treatment of ventricular pre -mature beat.Methods:Totally 70 patients with ventricular premature beat were randomly divided into the observation group (36 ca-ses) and the control group (34 cases).The patients were treated with metoprolol , and the patients in the observation group was treated with potassium magnesium aspartate as the auxiliary therapy , and the patients in the control group were treated with potassium chloride sustained release tablets as the adjuvant treatment .After 3-week continuous treatment , the changes of serum potassium , serum creatine kinase, creatine kinase, lactate dehydrogenase and ECG were observed in the two groups .The therapeutic effects and adverse drug re-actions were compared between the two groups .Results: After the treatment , the levels of serum potassium , serum creatine kinase isoenzyme, creatine kinase and lactate dehydrogenase were significantly improved when compared with those before the treatment (P0.05).The total effective rate of the observation group was 91.67%, which was higher than that of the control group (82.35%, P<0.05).The incidence of adverse drug reactions in the observation group was significantly lower than that in the control group (P<0.05).Conclusion:Potassium magnesium aspartate in the adjuvant treatment of ventricular premature beat can significantly improve the levels of serum potassium and serum myocardial enzyme and heart rate and ECG of the patients , and the efficacy and the incidence of adverse drug reactions are better than those of potassium chloride sustained -release tablets in the adju-vant treatment .

Effect of wenxin granule combined with metoprolol on blood rheology and clinlical efficacy of patients with hypertension and ventricular premature beat / 中国生化药物杂志

Objective To investigate effect of wenxin granule combined with metoprolol on blood rheology and clinlical efficacy of patients with premature hypertension heart disease.Methods 71 cases of hypertensive patients with hypertension and ventricular premature beat were selected, and randomly divided into a control group of 34 cases and observation group of 37 cases.The control group received metoprolol, the observation group on the base of the control group with wenxin granule.Blood rheology level, clinical efficacy and adverse reaction rate were compared before and after the treatment.Results Before treatment, high blood viscosity, whole blood low shear viscosity, plasma viscosity and fibrinogen levels had no significant difference in two groups of patients.After treatment, these indexs reduced compared with the same group pre-treatment(P<0.05);compared with the control group, the observation group had lower blood rheology(P<0.05), the total effective rate was higher(χ2 =8.60,P<0.05), and the incidence rate of adverse reactions was no difference.Conclusion Wenxin Granule combined with metoprolol in treatment of hypertension and ventricular premature beat patients, can improve effectively blood rheology of patients, and has low adverse reactions.

Treatment of refractory frequent ventricular premature beat by radiofrequency catheter ablation / 中国基层医药

Objective To study the curative effect on refractory frequent ventricular premature beat(VPB) by radiofrequency catheter ablation.Methods The refractory frequent ventricular extrasystoles of 18 patients were treated by radiofrequency catheter ablation(RFCA).The way of ventricular excited sequence and pacemaker-mapping was used to map and locate the site of VPB origin.When the original site of eliciting premature beat,where the excitation point of VPB appeared 20ms earlier than QRS waves of surface EKG or QRS waves of ablation electrode pacing electrogram were completely coincident with QRS waves of VPB,was localized,the frequency current should be delivered in situ.Results 18 patients were all successful,the success rate was 100%.No intraoperative or postoperative complications happened.Conclusion RFCA is an effective and safe surgical approach for frequent ventricular ectopic beats.

Clinical Efficacy of Intravenous Administration of Amiodarone in the Treatment of Postoperative Heart Failure Combined with Ventricular Tachycardia / 中国药房

OBJECTIVE:To observe the effect of intravenous administration of amiodarone on postoperative heart failure and ventricular arrhythmia.METHODS:A total of 42 postoperative heart failure patients with ventricular tachycardia and multi-source ventricular premature beat were treated with lidocaine,but clinical efficacy was not obtained.Those patients received intravenous administration of diluted amiodarome 150 mg.After 30 minutes the dose of amiodarone was kept at 0.5～1.0 mg?min-1,and the total dosage ranged from 1 000 mg to 1 200 mg within 24 h.Clinical symptoms were recorded in every hour,such as cardiac rhythm,heart rate,blood pressure,heart failure.RESULTS:After 24 h of treatment,17 cases of ventricular arrhythmia had apparent effect(40%),20 cases of partial effect(47%),5 cases of no effect(12%),there were statistical significance in difference before and after treatment(P

Clinical Study on Xianglian Mixture in Treatment of Premature Beat Accompanied Anxiety Disorder / 中医杂志

Objective To study the clinical effect of Xianglian in Mixture (Mixture of Nutgrass Galingale Rhizome and Golden Thread) in treatment of premature beat accompanied anxiety disorder.Methods Totally 100 patients of premature beat accompanied anxiety disorder were randomly assigned into two groups.The 50 patients in the treatment group were treated with Xianglian Mixture and the 50 in the control group were treated with Rytmonorm and Alprazolam for 4 weeks.The TCM symptom scores,premature beats in Holter,scores of Hamilton anxiety scale (HAMA) and the therapeutic effect on all these indices of both groups were observed.Results After treatment,the TCM symptom scores,premature beats in Holter,scores of HAMA of both groups were decreased significantly (P0.05).Conclusion Xianglian Mixture can reduce premature beats and relieve anxiety as well.
