RESUMEN

Objective To study the influence of patient perceived medical service quality on the satisfaction and behavioral intention.Methods The AMOS21.0 software was adopted to construct a structural equation model with satisfaction as a medium for conducting the path analysis among the perceived medical service quality,satisfaction and behavioral intention.Results The perceived medical service quality produced the direct impact on behavioral intention(y=0.50) and satisfaction(γ=0.87),the satisfaction produced the direct impact on behavioral intention(γ=0.38);with the satisfaction as a medium,the perceived medical service quality had the indirect impact(0.33) on behavioral intention and the intermediary rate was 39.76%.Conclusion The perceived medical service quality has active influence on the patient satisfaction and behavior intention,and indirectly acts on the behavioral intention with the satisfaction as intermediary agent.

RESUMEN

Objective To introduce a new appointment registration way in the outpatient department.Methods A WeChat appointment registration platform based on HIS was established with smart phone and WeChat public platform,which was gifted with the functions of online registration and payment and etc.The processes including preliminary diagnosis and return visit,cancelling appointment,treatment of appointment breaking,and the limitations during the appointment could be accomplished by the appointment registration.Results The platform realized appointment registration,information feedback and etc.Conclusion The appointment registration based on WeChat public platform contributes to satisfying the patient,enhancing medical service and distributing medical resources rationally.

<b>Objective: </b>As a “family pharmacy and pharmacist,” high-quality medical service is demanded.  Many patient satisfaction investigations are performed to evaluate community pharmacies, but it has been impossible until now to do so using an improvement index of the medical quality.  Therefore, I investigated the influence and evaluation structure of two evaluation concepts regarding quality of medical service and patient satisfaction and intention to use again, which becomes the important key as a “family” pharmacy.<br><b>Methods: </b>I used anonymous patient survey data gathered for the purpose of duties improvement in July 2009.  I subjected the responses to factor and covariance structure analyses.<br><b>Results: </b>Dates for four dimensions were obtained for factor analysis.  As a whole, covariance structure analysis showed that, in terms of privacy and only for service quality, entertainment primarily influenced satisfaction.  The ingestion instruction had a big effect on both sides.  General satisfaction was also substantially influenced by service quality.  The difference in influence degree was confirmed at each parameter.<br><b>Conclusion: </b>It was found that not only satisfaction but also quality of service were equally demanded in community pharmacies.  It is necessary to increase these because most are influenced by ingestion instruction as a factor in service quality.  However, future investigation is necessary to clarify the detailed ingestion instruction contents.

Tipo de Estudio: Prospectivo, cohorte, cuali-cuantitativo que investiga la prevalencia del síndrome de Burnout en relación directa con el régimen de los médicos residentes del área de emergencia y su impacto en la relación médico – paciente incidiendo en la calidad del servicio que presta. Objetivo: Reflexionar sobre la importancia de la relación médico paciente y su incidencia en la salud individual y la calidad de los procesos de atención hospitalaria. Materiales y métodos: Estudio realizado en 36 médicos residentes que rotan en el área de emergencia, como parte de su posgrado, y 60 pacientes que requirieron atención durante el tiempo de la investigación. El método específico que se utilizó fue la observación y investigación multimodal a través de la aplicación de entrevistas, encuestas y el test con la finalidad de estudiar cuantitativa y cualitativamente la relación del síndrome burnout con la calidad en la atención de los servicios de emergencia. Resultados y conclusión: Al medir los tres ejes del síndrome de burnout tenemos que se manifiesta en el 52.2 de los residentes. Se distribuyó casi igual para los dos géneros; la edad media de los residentes fue de 31.4 años y siendo los traumatólogos quienes presentaban con un mayor porcentaje (69). Las manifestaciones clínicas que presentaron fueron 40 cefalea, 27 trastornos del sueño, 23 irritabilidad, 20 epigastralgia, 17 ansiedad y un 10 trastornos del ritmo evacuatorio. En cuanto a los indicadores de calidad tenemos que el 75 de los pacientes no recibieron la explicación adecuada para el entendimiento de su enfermedad y el 80 perciben al médico como cansado, con sueño, irritable o con mal humor. Se demostro que la existencia del síndrome esta relacionado con la disminución de la calidad en la atención.


Type of study: prospective cohort, quali-quantitatve study that investigates the prevalence of the Burnout Syndrome, the relationship that exists with the workload of residents in the emergency room and the impact it has on the doctor - patient relationship. Objective: Reflect on the importance of doctor-patient relationship and how it affects the quality in medical attention. Materials and Methods: A study of 36 residents on their emergency room rotation and 60 patients that needed medical attention. The method used was observation and investigation using interviews, polls and a test to study the quantitative and qualitative relationship between burnout syndrome and medical attention in the emergency room. Results and Conclusions: This study shows that 52.2 of the residents had burnout syndrome. It was the same for both sex and mean age was 31.4 years. It was more common in trauma residents with 69. The symptoms were 40 cephalea, 27 sleep disorder, 23 irrtablility, 20 epigastric pain, 17 anxiety, and 10 bowel disorder. I found that 75 of patients did not get a good explanation of there illness and 80 said they felt that doctor was tired and irritable. In conclusion the existence of this syndrome is related with the quality of medical attention.
