RESUMEN

ObjectiveTo construct and validate a clinical prediction model for diabetic kidney disease (DKD) based on optical coherence tomography angiography (OCTA). MethodsThis study enrolled 567 diabetes patients. The random forest algorithm as well as logistic regression analysis were applied to construct the prediction model. The model discrimination and clinical usefulness were evaluated by receiver operating characteristic curve (ROC) and decision curve analysis (DCA), respectively. ResultsThe clinical prediction model for DKD based on OCTA was constructed with area under the curve (AUC) of 0.878 and Brier score of 0.11. ConclusionsThrough multidimensional verification, the clinical prediction nomogram model based on OCTA allowed for early warning and advanced intervention of DKD.

【Objective】 To establish deferral criterion of HIV ELISA (enzyme-linked immunosorbent assay) and electrochemiluminescence immunoassay(ECLIA) by using receiver operating characteristic curve(ROC) method to screen HIV reactive blood donors suitable for entering the re-entry process and improve the management efficiency of reactive blood donors. 【Methods】 The test results of 92 001 blood donors from February to September 2019 were analyzed, and 177 reactive samples were screened by conventional screening mode (twice ELISA and once nucleic acid), supplemented with electrochemiluminescence immunoassay assay (ECLIA), and confirmed by Western blotting (WB). Screening reactive samples were divided into three groups: group A was both serological and nucleic acid reactivity, group B was only serological reactive, and group C was only nucleic acid reactivity. Its efficacy in blood donor classification was assessed by drawing ROC curves with 99% specific corresponding S/CO low values as the deferral criterion of the corresponding serological method. 【Results】 1) A total of 177 HIV reactive samples were detected in conventional mode, including 34 in group A, 142 in Group B and 1 in Group C. The positive predictive value (PPV) was 100%, 0.75% and 100%, respectively. ECLIA detection mode (once ECLIA and once NAT), a total of 67 HIV reactive samples including 34 in group A, 32 in group B and 1 in group C, with positive predictive values of 100%, 3.7% and 100%, respectively.2) The HIV test results showed diversity, with 36 true positive samples including 1 HIV elite controller and 3 early HIV infections (1 HIV ELISA antigen/antibody window and 2 ELISA HIV antibody window), and 32 serological and NAT cases were reactive infections.3) The deferral limit of ELISA 1 and ELISA 2 in conventional screening mode were 20.25 and 9.85, respectively, can screen 97.14% (34/35) of all true positive samples in group A and B, except for one ELISA HIV antibody window (ELISA 2 reactivity). The positive predictive values were 93.94% and 92.85%, respectively. The ECLIA deferral limit of 7.83 can screens all true positive samples in Groups A and B (35/35)in ECLIA mode. The positive predictive value was 94.59%. 【Conclusion】 The establishment of deferral limits in this study can effectively screen HIV-positive blood donors, and the number of screened blood donors is greatly reduced, which is helpful to fine and scientific management of HIV-reactive blood donors. The deferral limit values of different testing reagents are quite different, so each laboratory should choose appropriate testing methods to establish the deferral limit values suitable for the laboratory according to its own testing ability, so as to provide technical support for optimizing the process of returning blood donors to the team.

<p><b>PURPOSE </b>: To create a receiver operating characteristic curve (ROC curve) to screen for excessive sensitivity to cold (ESC).<br><b>PARTICIPANTS AND METHODS </b>: Subjects were 99 women who visited a specialty outpatient clinic for ESC. The controls were 371 female nurses. A numerical rating scale (NRS ; from 0 indicating someone is asymptomatic to 10 indicating profound discomfort) was used to determine the extent of chills in both groups. ROC curves were created to distinguish ESC based on their score on the NRS. An ROC curve was created for each premenopausal and postmenopausal woman.<br><b>RESULTS </b>: The ESC group had a mean score on the NRS of 7.3 (95% confidence interval [CI] : 6.9 to 7.6) while normal subjects had a mean score of 4.0 (CI : 3.7 to 4.3). The ESC group had a significantly higher mean score on the NRS. When the cut-off point for the NRS was set at ≥5, it had 98% sensitivity and 54% specificity for distinguishing premenopausal ESC. Similarly, that cut-off point had 96% sensitivity and 67% specificity for distinguishing postmenopausal ESC.<br><b>CONCLUSION </b>: If an individual with chills has an NRS score of ≥5, that individual may be experiencing discomfort sufficient to warrant being seen by a medical facility. Using an NRS to assess chills should help to screen for ESC.</p>

Objective To assessment short-term prognosis in patients with acute on chronic liver failure , several scoring systems were compared. Methods Two hundred and sixteen patients with acute on chronic liver failure were divided into survival group and death group according to the results of 90 days after admission.CTP , MELD,APACHEⅡ, SOFA and SMSVH score were calculated.After ROC curves were performed ,the areas under the curves of these scoring systems were compared. Results The areas under the ROC curves of MELD, APACHEⅡ, SOFA, CTP and SMSVH were 0.88, 0.76, 0.89,0.79and 0.69,respectively. The areas under the curves of SOFA and MELD were larger than the APACHEⅡ, CTP and SMSVH (P0.05). The area under the curve of CTP was larger than the APACHEⅡ, but there was no statistically significant difference (P > 0.05). The area under the curve of SMSVH were less than 0.7. Conclusions The SOFA, MELD,CTP and APACHEⅡcan predict the short-term prognosis of acute on chronic liver failure. The SOFA and MELD are the best scoring systems.CTP,APACHEⅡ are better than SMSVH. SMSVH fail to predict the prognosis of acute on chronic liver failure.

BackgroundDiabetic retinopathy (DR) is the result of the cytokine network disorders,the imbalance of angiogenic factor and vascular inhibitory factor is the start factor.ObjectiveTo analyze the levels of CXC chemotatic factors of type 2 diabetes mellitus patients,evaluate the clinical application value of them in different clinical types of DR using receiver operating characteristic (ROC)analysis and to approach the new way of individualized treatment.Methods This was a prospective research.The gold standard was ophthalmolscope and fundus fluorescein angiography.The levels of CXC chemotatic factors and multiplicaiton factors were measured in 96 cases with type 2 diabetes mellitus (66 cases with retinopathy and 30 cases without retinopathy as control).The assessment tasks were performed for these index and courses of DR with ROC curve.Results The expression of age,course of disease has significant difference in different courses of DR ( F =8.507,P =0.001 ; F =28.143,P =0.000).Compared with the control group,the expression of growth-related oncogene-α ( GROα ) ( t =- 2.172,P =0.035,AUC =0.625 ),whole blood viscosity 200 ( t =- 3.724,P =0.001,AUC =0.904 ) and neutrophilic leukocyte (t=-2.562,P =0.013,AUC =0.577 ) has significant difference in the group of mild NPDR.Compared with the control group,the expression of interferon-γ-inducible protein 10 ( IP-10 ) ( t =-3.591,P =0.001,AUC =0.592 ),platelet derivation growth factor-BB ( PDGF-BB ) ( t =- 3.233,P =0.003,AUC =0.735 ),vascular endothelial growth factor(VEGF) ( t =- 3.617,P =0.001,AUC =0.776 ),C peptide ( t =- 3.366,P =0.002,AUC =0.962 ),leukocyte ( t=-3.201,P =0.003,AUC =0.852) and neutrophilic leukocyte(t =-4.201,P=0.000,AUC =0.852) has significant difference in the group of moderate and severe NPDR.ConclusionsCXC chemotatic factors may act as reactivator in the pathogenesis of DR,GROα and IP-10 may be useful for clinical monitoring of the severity of DR,and evaluating the imbalance state of chemotatic factors maybe a new approach to clinical monitoring and prognosis of DR.

RESUMEN

PURPOSE: To assess the utility of shoulder MRI by using ROC (receiver operating characteristic) curve in theevaluation of rotator cuff tear, anterior labral tear, superior labral tear and Hill-Sachs Lesion. MATERIALS AND METHODS: We evaluated 38 arthroscopically confirmed patients who had undergone shoulder MRI. According to thesignal intensity of the rotator cuff, as seen on T2 and proton density imaging, a five-stage grading system wasdevised. Labral tears were graded according to the separation of the labium; this was based on gleaned,morphologic and signal intensity changes of the labrum : six grades of anterior labral tear and three grades ofsuperior labral tear. Hill-sachs lesion was also classified into four grades according to morphologic and signalchanges of the humeral head. These findings were reviewed by two musculoskeletal radiologists and ROC curves andareas under the curve (Az) was abtained. RESULTS: The ratio of area under the curve of rotator cuff tear,anterior labral tear, superior labral tear and Hill-Sachs lesion were 82.4%, 88.%%, 62.4% and 91.3%, respectively. CONCLUSION: The Accuracy of shoulder MRI using ROC curve was relatively high in rotator cuff tear, anteriorlabral tear and Hill-Sachs lesion, but low in superior labral tear.
