RESUMEN

During the pathogensis of rheumatoid arthritis (RA), activated RA fibroblast-like synoviocytes (RA-FLSs) combines similar proliferative features as tumor and inflammatory features as osteoarthritis, which eventually leads to joint erosion. Therefore, it is imperative to research and develop new compounds, which can effectively inhibit abnormal activation of RA-FLSs and retard RA progression. Neohesperidin (Neo) is a major active component of flavonoid compounds with anti-inflammation and anti-oxidant properties. In this study, the anti-inflammation, anti-migration, anti-invasion, anti-oxidant and apoptosis-induced effects of Neo on RA-FLSs were explored to investigate the underlying mechanism. The results suggested that Neo decreased the levels of interleukin IL-1β, IL-6, IL-8, TNF-α, MMP-3, MMP-9 and MMP-13 in FLSs. Moreover, Neo blocked the activation of the MAPK signaling pathway. Furthermore, treatment with Neo induced the apoptosis of FLSs, and inhibited the migration of FLSs. It was also found that Neo reduced the accumulation of reactive oxygen species (ROS) induced by TNF-α. Taken together, our results highlighted that Neo may act as a potential and promising therapeutic drug for the management of RA.

@#The incidence of insufficiency fractures is approximately 1% in rheumatoid arthritis patients. The predisposing factors are chronic inflammation, skeletal deformities, biomechanical stresses and osteoporosis. The medications used in the treatment of rheumatoid arthritis such as Glucocorticosteroids and Methotrexate also contribute to the development of osteoporosis and insufficiency fractures. A 68-year old lady who was suffering from rheumatoid arthritis and on long term Methotrexate was seen in the outpatient clinic with recurrent episodes of heel pain. Examination revealed diffuse tenderness around the heel with full range of ankle movements but painful limitation of subtalar joint movements. Radiographic examination of the ankle showed a highly suspicious fracture of the calcaneus and confirmed on MRI as an insufficiency fracture. She was treated successfully with oral bisphosphonates and moon boot brace. She presented after two years with recurrent episodes of heel pain. The plain radiograph and MRI scan confirmed an insufficiency fracture of the talus. She was treated nonoperatively with intravenous Zolendronic acid and bracing. In chronic rheumatoid arthritis patients especially on Methotrexate and Glucocorticoids a high index of suspicion of insufficiency fractures should be considered if they present with bone pain. MRI scan is the investigation of choice and is conclusive.

Objective:To explore the efficacy and safety of Certolizumab pegol in patients with rheumatoid arthritis. Methods:Such databases as Pubmed, Medline, Embase, The Cochrane Library, WANFANG, CNKI, Science, CBM and VIP were searched from their establishment to March 2015 for collecting the randomized controlled trials comparing Certolizumab pegol and placebo in the treatment of RA. The meta-analysis was undertaken using RevMan5. 3 for Windows. Results:Ten publications and eight researches met the inclusion criteria with high quality. The results revealed CZP significantly improved the ACR20,ACR50,ACR70 response rates,and physical function. CZP was associated with a statistically significant reduction in Disease Activity Score in 28 joints-Erythrocyte sedimentation rate, arthritis pain, and fatigue. There were no significant differences of incidence of treatment-related adverse events between CZP group and placebo group. Conclusion:CZP significantly reduced the RA signs and symptoms,improved physical function and life quality as compared with the placebo in the treatment of RA. More large-scale RCTs are needed to evaluate the long-term efficacy and safety of CZP in the treatment of RA.

RESUMEN

Objective To observe the changes of serum 25‐hydroxy vitamin D[25‐(OH)D] level of patients with rheumatoid ar‐thritis(RA) and study its clinical significance .Methods The method of chemiluminescence was adopted to detect peripheral blood serum 25‐(OH) D of 60 cases of RA patients(RA group) and 60 cases of healthy people(control group) .Results The serum 25‐(OH) D level of patients in RA group was(34 .37 ± 8 .87) nmol/L ,while that of the control group was(61 .33 ± 7 .74) nmol/L ;after comparison ,the differences were statistically significant (t= -6 .892 ,P 0 .05) . Conclusion The serum 25‐(OH) D level of RA patients is significantly low ,which might be associated with the RA disease activi‐ty .

La Artritis Reumatoidea (AR) es una enfermedad inflamatoria crónica que se asocia a disminución de la capacidad funcional, de la calidad de vida y aumento de la mortalidad. El objetivo de este estudio fue evaluar la mortalidad en una cohorte de pacientes con AR, y la influencia de factores sociodemográficos, clínicos y de laboratorio. Pacientes y métodos: Se analizó una cohorte de pacientes con diagnóstico de AR, (criterios ACR’ 87), evaluados entre los años 2004 y 2010. Se recolectaron datos sociodemográficos, características de la enfermedad, clínicas, analíticas, tratamientos recibidos y comorbilidades. Radiografías de manos y pies fueron leídas por medio del índice de Sharp van der Heijde por 3 observadores independientes ciegos (CCI ≥0,8). En caso de discontinuación del seguimiento, se realizó contacto telefónico. Se identificaron los pacientes fallecidos durante el período estudiado y se determinó la causa de muerte. Análisis estadístico: Estadística descriptiva, para la comparación de datos categóricos se utilizó test de Chi2 o test exacto de Fisher y para las variables continuas test t de Student o Mann Whitney según la distribución de la muestra. Los factores asociados a mortalidad se analizaron por regresión logística múltiple. Se consideró significativa una p <0,05. Resultados: Se evaluaron 307 pacientes, de los cuales 45 perdieron seguimiento y 262 pudieron ser reevaluados a los 5 años de seguimiento. De los 262 pacientes, 86,3% eran de sexo femenino, la edad mediana fue de 58 años (RIC 50-67), la edad mediana al inicio de la AR fue de 40 años (RIC 31-50) con un tiempo mediano de evolución de 15 años (RIC 10-21). 17 pacientes (6,5%) fallecieron. En el análisis univariado, mayor edad, menor escolaridad, presencia de comorbilidades gastrointestinales y uso de tratamiento biológico, estuvieron significativamente asociados con mayor mortalidad...


Rheumatoid arthritis (RA) is a chronic inflammatory disease that isassociated with decreased functional capacity, quality of life andincreased mortality. If discontinuation of follow-up was present,telephone contact was made. Deceased patients were identifiedduring the study period and the cause of death was determined.Statistical analysis: Chi2 test or Fisher exact test were used forcategorical variables and Student t test or Mann Whitney forcontinues variables. Factors associated with mortality wereanalyzed by multiple logistic regression. P value <0.05 wasconsidered significative.Results: 307 patients were evaluated, of whom 45 were lost tofollow-up and 262 were reassessed after 5 years of follow-up. Of the262 patients, 86.3% were female, median age was 58 years (IQR 50-67), the median age at onset of RA was 40 years (IQR 31-50) with amedian disease duration of 15 years (IQR 10-21).
