RESUMEN

Objective:To explore the effect of Z type lateral decubitus position in patients with stroke-related pneumonia.Methods:Totally 52 patients with stroke-related pneumonia admitted to the neurology care unit in Qilu Hospital of Shandong University (Qingdao) were randomly divided into the control group and the observation group with 26 cases in each group from February to December 2021.The control group adopted routine position management, and the observation group adopted the Z type lateral decubitus position to observe. The differences in sputum volume, clinical pulmonary infection score (CPIS), blood oxygen saturation change, and hospital stay between the two groups were compared.Results:The SpO 2 was 0.966 ± 0.210 in the observation group and 0.948 ± 0.223 in the control group on the 7th day after the implementation of the measures. There was a significant difference between the two groups ( t=3.07, P<0.01). The sputum aspiration volume of the observation group was (30.65 ± 10.01), (39.27 ± 11.30), (36.92 ± 12.94) ml on the 1st, 3rd, 7th day after the implementation of the measures, which were significantly higher than those of the control group (24.81 ± 9.55), (28.92 ± 9.40), (27.65 ± 7.85) ml. The differences were statistically significant ( t=2.16, 3.59, 3.12, P<0.05). On the 7th day after implementation, the CPIS score of the observation group was 4.54 ± 1.63, which was better than that of the control group 5.46±1.61 ( t=-2.06, P<0.05). The average length of stay in the observation group was (13.35 ± 2.74) d, which was significantly shorter than (15.46 ± 4.42) d in the control group ( t=-2.07, P<0.05). Conclusions:The use of Z type lateral decubitus position has a positive significance for the rehabilitation and recovery of patients with stroke-related pneumonia. This position can promote sputum drainage and improve patients pulmonary function, accelerate the dissipation of patient inflammation, and reduce the length of hospital stay.

RESUMEN

Using data mining technology, the rules of acupoint selection of acupuncture and moxibustion were explored in treatment of stroke-related pneumonia. The clinical articles of acupuncture and moxibustion in treatment of stroke-related pneumonia were retrieved from CNKI, SinoMed, Wanfang and VIP databases from their inception through to January l 2021, and then, the acupuncture-moxibustion prescription database was set up for stroke-related pneumonia. SPSS Modeler 18.0 Apriori algorithm was adopted to analyze the association rules of acupoints and draw complex network diagrams. SPSS26.0 was used in clustering analysis of acupoints. Finally, a total of 44 articles were included, with 51 acupoint prescriptions and 82 acupoints extracted. The total frequency of acupoints was 340 times. The high-frequency acupoints in treatment with acupuncture and moxibustion for stroke-related pneumonia were Feishu (BL 13), Fenglong (ST 40), Hegu (LI 4), etc. These acupoints were mainly distributed on the limbs and back and mostly from yang meridians. Of these extracted acupoints, the five-shu points, convergent points and back-shu points were selected specially. Regarding acupoint combination, the association of Quchi (LI 11) had the highest support with Hegu (LI 4) and Zusanli (ST 36). The core prescription of acupuncture-moxibustion treatment for stroke-related pneumonia could be composed of Quchi (LI 11), Hegu (LI 4), Zusanli (ST 36), Fenglong (ST 40) and Taichong (LR 3).

RESUMEN

Objective::To observe the clinical efficacy of modified Shenling Baizhusan on stroke-associated pneumonia (SAP) with lung-Qi deficiency syndrome and its regulatory effect on inflammatory factors and immune function. Method::One hundred and ten patients were randomly divided into control group (55 cases) and observation group (55 cases) by random number table. Patients in control group got piperacillin sodium and tazobactam sodium for slow intravenous injection, 4.5 g/time, 3 times/days, for 7-10 days, and new antibiotic was chose by drug outcomes, ambroxol hydrochloride injection for low intravenous infusion, 30 mg/time, 2 times/days, for 10 days, and nutritional support and symptomatic comprehensive treatment. In addition to the therapy in control group, patients in observation group were added with modified Shenling Baizhusan, 1 dose/day, for 10 days. And scores of pulmonary infection (CPIS), time of CPIS<6 time of disappearance of cough, time of recovery of temperature, time of recovery of leukocyte, and time of disappearance of lung rales were recorded. And before and after treatment, lung-Qi deficiency syndromes were scored, and levels of procalcitonin (PCT), γ-interferon (IFN-γ), tumor necrosis factor-α (TNF-α), high-sensitivity C-reactive protein (hs-CRP), immunoglobulin A (IgA), immunoglobulin G (IgG), immunoglobulin M (IgM), T Lymphocyte subsets (CD4+, CD8+ and CD4+ /CD8+) were all detected. Result::By rank sum test, the clinical effect in control group was better than that in control group (Z=2.106, P<0.05). Scores of CPIS and lung-Qi deficiency syndromes were lower than those in control group (P<0.01). And time of CPIS<6, time of disappearance of cough, time of recovery of temperature, time of recovery of leukocyte, and time of disappearance of lung moist rales were all shorter than those in control group (P<0.01). And levels of PCT, TNF-α, hs-CRP and CD8+ were lower than those in control group (P<0.01, P<0.05), while levels of IFN-γ, IgA, IgM, CD4+ and CD4+ /CD8+ were all higher than those in control group (P<0.01, P<0.05). Conclusion::In addition to the comprehensive anti-infection therapy, modified Shenling Baizhusan can control the degree of illness, alleviate symptoms, regulate the expressions of inflammatory factors, increase the immune function of the body, and improve the comprehensive efficacy.
