Objectives: To ascertain if aural syringing is a one size ?ts all solution for aural foreign body removal in a rural setting A Record based descriptive study was looked at paediatric and adult patients Methods: with Aural FB (foreign bodies) who visited ER (Emergency) and ENT-OPD (Out Patient Department) of a single institution between January 2022 and December 2022.The Diagnosis of Aural foreign bodies was based on personal history and Otoscopic ?ndings. Patient characteristics, foreign body type, removal attempts and complications were evaluated with respect to clinical setting and patient outcome. The Data was obtained from the records. 86 patients were brought to Results: the emergency department and ENT-OPD over a 12-month period with foreign bodies of the EAC (External Auditory Canal). Otolaryngologists used otoscope and standard metallic aural syringe as their mainstay of management. Analysis of ER and OPD cases revealed Inorganic aural foreign bodies were 65.11% and 34.88% were Organic foreign bodies. Unilateral Purulent Aural Discharge was in 46.51% (40) patients, pain in 17.44% (15), Ear Bleeding in 8.13% (7), conductive hearing loss and tinnitus was 10.46% (9) and itching in 4.65% (4). Duration of Symptoms was <1 week in 65.11% (56) cases ,1-2 weeks in 22.09% (19) and 2-4 weeks in 12.79% (11) cases. Successful removal of foreign bodies from ear by only applying aural syringing was achieved in 91.86% of cases, and usage of other methods of aural foreign body removal Forceps, Hook and Suction in 5.8% and foreign body removal under GA was 2.32% were recorded Aural foreign bodies were commonly seen in paediatric Conclusion population. Patients commonly present to the ER and OPD for removal of EAC foreign bodies. The common inorganic aural foreign bodies were cotton tip, stone and eraser whereas ?y, lice and bee were the organic FB. The most common symptoms and complications of aural FBs, were unilateral purulent discharge followed by pain, ear Bleeding, Tympanic membrane perforation, external meatus laceration, chronic otitis media and facial Nerve paralysis. The most common procedures used were Syringing followed by Forceps, hook and suction. It is observed that aural syringing performed by the trained hands of an otolaryngologist is a very effective method and can almost be described as a one size ?ts all solution for managing foreign bodies in EAC with some notable exceptions.

Purpose: To determine the diagnostic accuracy of manual regurgitation on pressure over the lacrimal sac (ROPLAS) versus lacrimal irrigation for screening nasolacrimal duct obstruction (NLDO) in adults prior to intraocular surgeries. Methods: This cross-sectional study took place in a tertiary eye care hospital in South Tamil Nadu, India. From January to December 2017 and included consecutive patients who presented for routine cataract surgery. Prospective data collection occurred in 8369 eyes of patients who underwent cataract surgery. All patients underwent ROPLAS testing by an ophthalmologist followed by lacrimal irrigation by trained ophthalmic assistants, rechecked or confirmed in equivocal cases by ophthalmologists who were masked to the ROPLAS status. The primary outcome, the sensitivity, specificity, positive, and negative predictive values to detect lacrimal occlusion by ROPLAS compared with lacrimal irrigation with 95% confidence intervals was estimated. Results: A total of 8369 eyes underwent cataract surgery during the time periods of the study. ROPLAS and lacrimal irrigation were performed in all eyes. The sensitivity of ROPLAS to diagnose NLDO correctly was 54.5% (95% CI, 44.8%�.9%) and its specificity was 100% (95% CI, 100%�0%). The positive and negative predictive values were 75.3% (95% CI, 65.6%-83.0%) and 99.4% (95% CI, 99.2%�.5%), respectively. Conclusion: We found that ROPLAS when used alone had very low sensitivity and low positive predictive value in detecting NLDO prior to cataract surgery as compared with lacrimal irrigation. Hence, we recommend performing ROPLAS and lacrimal irrigation in every patient as part of the routine preoperative workup prior to cataract surgery

Objective To establish a method to simultaneously determine syringing and isofraxidin by HPLC;To investigate the features of percutaneous absorption in vitro of Jiegugao blended and pasted by white vinegar, honey and vaseline; To discuss the mechanism of commonly used ointment matrices in Tujia Minority. Methods Rat abdomen skin in vitro was as transdermal barrier;the modified Franz diffusion pool was used to simulate human skin medication; the content of syringin and isoprofen was determined by HPLC; the percutaneous absorption equation was established and the related parameters, such as cumulative permeation rate and permeation rate, were calculated. Results When using Syncronis C18 (250 mm × 4.6 mm, 5 μm) as chromatographic column, acetonitrile-0.1%phosphoric acid as mobile phase, 1.0 mL/min as perfusion speed and 265 nm as determine wavelength, regression equation of syringingwas A=10 686.454 6C+1565.778 8 (r=1.000 0), regression equation of isofraxidin was A=12 297.305 4C-5913.729 9 (r=0.999 9). Cumulative permeation quantity of syringing in Jiegugao blended and pasted by white vinegar, honey, vaseline and blank were 7.549 2, 4.580 3, 3.890 8 and 5.378 4 μg?cm-2?h-1 respectively and permeation rate were 25.66%, 16.11%, 13.73% and 18.78%. Meanwhile, cumulative permeation quantity of isofraxidin were 2.536 9, 1.941 8, 1.178 2 and 2.293 6 μg?cm-2?h-1 respectively and permeation rate were 47.04%, 35.06%, 22.11%and 41.11%. Conclusion Using white vinegar as the ointment matrix can promote the percutaneous absorption of effective composition in Jiegugao blended. However, it will retard the percutaneous absorption of effective composition in Jiegugao when using honey and vaseline as the ointment matrices, but honey and vaseline can be used as a slow-release matrix.

PURPOSE: To evaluate prognostic factors affecting silicone tube intubation outcomes in Asian patients with lacrimal drainage system stenosis. METHODS: A retrospective review was conducted on the medical records of 822 patients (1,118 eyes) who had undergone silicone tube intubation to treat lacrimal drainage system stenosis between January 2011 and December 2012. Patients were divided into two groups: a success group and a failure group. Success was defined as the disappearance of epiphora symptoms, normalization of tear meniscus height, and the easy passage of fluid without resistance on the postoperative syringing test. Patient and ocular parameters were compared between the success and failure groups. RESULTS: A total of 994 eyes of 727 patients were included in analyses. Patients had a mean follow-up period of 34.11 ± 18.70 weeks. Silicone tube intubation was successful in 67.2% of participants. Significant differences between the success and failure groups were found for age (p < 0.001), history of ipsilateral facial palsy (p = 0.028), follow-up period (p < 0.001), and degree of passage on the preoperative syringing test (p = 0.001). Only age (p < 0.001) and degree of passage on the preoperative syringing test (p = 0.002) remained significantly associated with silicone tube intubation success in multivariate analysis. CONCLUSIONS: Age was negatively associated with silicone tube intubation success in patients with lacrimal drainage system stenosis. The success rate was higher in patients who showed easy passage of fluid without resistance on the preoperative syringing test. These factors should be considered by surgeons planning silicone tube intubation in patients with lacrimal drainage system stenosis.

RESUMEN

PURPOSE: To evaluate clinical outcomes of silicone tube intubation according to the site of resistance to lacrimal duct probing in complete or partial nasolacrimal duct obstruction patients. METHODS: This study included 102 eyes of 72 patients who were diagnosed with complete or partial nasolacrimal duct obstruction and who underwent silicone tube intubation. According to the site of resistant to nasolacrimal duct probing, eyes were divided into proximal resistance (Group I), distal resistance (Group II) and both side resistance (Group III). The success rate was estimated based on functional (symptom relief) and anatomical (normalization of tear meniscus) success. RESULTS: The success rates in Group I, Group II, and Group III were 53.1%, 78.8%, and 27.0%, respectively, showing that Group II attained the highest success rate (Pearson chi-square test, p = 0.001). CONCLUSIONS: In cases of only distal resistance to lacrimal probing without dacryocystography, silicone tube intubation should be performed with expectation of good clinical outcomes, even if complete nasolacrimal obstruction was suspected on syringing.

RESUMEN

PURPOSE: To compare the surgical success rate of silicone tube intubation according to the preoperative lacrimal syringing test and dacryocystography (DCG) in adult patients with epiphora. METHODS: Medical records of 142 patients who underwent silicone tube intubation were retrospectively reviewed. The surgical results were divided into 3 groups based on postoperative symptoms and the syringing test results: Good, Fair, and Fail. Good and Fair were classified into the surgical success groups. We compared the surgical success rate according to the preoperative syringing test and stenosis site observed on dacryocystography. RESULTS: The success rates of silicone tube intubation were 88.4% and 87.4% in eyes with preoperative syringing results of totally-passed and partially-passed syringing tests, respectively, revealing no statistically significant differences (p = 0.838, chi-square test). However, in eyes with the preoperative syringing results of partially-passed syringing tests, the success rates were 66.7% in patients with canalicular stenosis, 83.3% in patients with nasolacrimal duct stenosis, and 94.5% in those with diffuse stenosis (p = 0.018, chi-square test). CONCLUSIONS: No significant differences were found in the success rates of silicone tube intubation according to preoperative syringing test results. However, when considered together with DCG findings, the lowest success rate was found in eyes with the partially-passed syringing test and simultaneously showed canalicular stenosis on DCG. The syringing test results considered with DCG findings may help predict the success rates of silicone tube intubation.

RESUMEN

Objective: To study the chemical constituents from the radix and rhizome of Myrsine stolonifera. Methods: The chemical constituents were isolated and purified by D101 macroporous adsorption resin, silica gel, and Sephadex LH-20 column chromatography. The structures of the isolated compounds were identified on the basis of physicochemical properties and spectroscopic methods including 1H-NMR, 13C-NMR, and MS spectra. Results: Eleven compounds were identified as askaempferol (1), dihydrokaempferol (2), quercetin (3), 5, 7, 4'-trihydroxyisoflavone (4), chrysoeriol (5), quercetin-7-O-α-D-glucoside (6), (-)- epicatechin (7), 3, 5-dimethoxy-benzylalcohol-4-O-β-D-glucopyranoside (8), 2, 6-dimethoxy-4-hydroxyphenol-1-O-β-D- glucopyranoside (9), syringing (10), and (+)-lyoniresinol3α-O-β-D-glucopyranoside (11). Conclusion: Compounds 2, 4-11 are all isolated from the plants of Myrsine Linn. for the first time, and compounds 1-11 are all isolated from this plant for the first time.
