RESUMEN

One of the symptoms of COVID-19 is olfactory dysfunction. Most of them resolve spontaneously at an early stage, but there are some cases that remain as sequelae, and there is no established drug treatment at present. This time, we report two cases of olfactory dysfunction after COVID-19 successfully treated with Kampo medicine centered on kososan. Case 1 was a 32-year-old woman. She was diagnosed with COVID-19 eight months ago, and olfactory dysfunction remained after that. She was yin pattern and slight deficiency pattern, and tenderness was found in the groin. We prescribed her tokishigyakukagoshuyushokyoto. After 8 weeks of administration, she felt warm and well, but her olfaction did not change. Depression due to olfactory dysfunction was also conspicuous, so we administered her kososan in combination. Since then, her olfaction has gradually improved. Case 2 was a 17-year-old man. He was diagnosed with COVID-19 four months ago, and olfactory dysfunction remained after that. He was yang pattern and medium or slight deficiency pattern, and with noticeable signs of qi stagnation. We administered him kososan, and his olfaction has gradually improved.

Abdominal distension is one of the common symptoms and can cause abdominal pain and anorexia, which may raise adverse effect on general well-being and quality of life. In Western medicine, there are specialized treatments for structural lesions associated with abdominal distension, but functional abdominal distension is often difficult to treat. There have been some reports of Kampo medicine for treatment of abdominal distension, but no case reports on tokishigyakukagoshuyushokyoto. We experienced two cases of abdominal distension successfully treated with tokishigyakukagoshuyushokyoto. Case 1 was 86-year-old woman who developed abdominal distension with frequent belching. Case 2 was 59-year-old man who developed abdominal distension associated with opioid. Regulating qi medicine was not effective for abdominal distension in both cases. Both cases had reversal coldness of the extremities, tympanic sounds of abdomen, coldness of abdomen, and inguinal tenderness and resistance. Therefore, we diagnosed them as tokishigyakukagoshuyushokyoto sho. After administration of tokishigyakukagoshuyushokyoto, in Case 1, the frequency of belching decreased and the appetite increased with the improvement of abdominal distension. In Case 2, increasing the dose of goshuyu in tokishigyakukagoshuyushokyoto relieved abdominal distension. Tokishigyakukagoshuyushokyoto can be effective for abdominal distension with interior cold pattern.

Pruritus of external auditory canal is often treated with blood-enriching formulation and heat-clearing formula. Here, we report a case of intractable pruritus of external auditory canal successfully treated with tokishigyakukagoshuyushokyoto, which did not improve with blood-enriching formulation or heat-clearing formula. Our patient was a 52-year-old woman. She had itching in her left ear from the age of 45, and was treated by 5 otolaryngologists, but her symptom did not improve. I treated her with tokiinshi, shofusan, yokukansankachimpihange, unseiin, jumihaidokuto and keishibukuryogankayokuinin, but they had no effect. After reconsideration of sho, she was diagnosed as interior cold pattern, and she had tenderness in groin area. So I treated her with tokishigyakukagoshuyushokyoto, and her itching disappeared. We should consider interior cold pattern as one of the differential diagnoses of pruritus of external auditory canal.

The authors experienced two cases of trigeminal neuralgia successfully treated by a Kampo formulation tokishigyakukagoshuyushokyoto (TSG). Case 1 is 70-year-old woman who complained of severe pain on the right side of her face. She was administered carbamazepine. It was effective for the facial pain but she could not take it continuously because of side effect, namely ataxia. Therefore, we tried to administer TSG. This Kampo formulation proved effective, and her pain was controlled without carbamazepine. Case 2 is 69-year-old woman who complained of localized pain on the right side of her face. In this case TSG alone was effective.

We report two cases of patients with intractable upper abdominal pain and histories of injury or surgery. The patients' pains were successfully treated with tokishigyakukagoshuyushokyoto (TSG). Case 1 involved a 59-year-old woman with left upper abdominal and thoracodorsal pain lasting 5 years. Kampo drugs such as tokito or prescriptions containing saiko were ineffective. Two years after her first visit, we noticed an injury scar from childhood that passed near the left hikon (pi-gen, ExB4) and induration and tenderness of the left hikon. TSG was administered, and the pain promptly improved. Case 2 involved a 66-year-old woman. She complained of epigastric, right­-upper-­quadrant abdominal and back pain lasting 7 years. Two years before presentation, the pain had worsened, but the cause was unclear. Although keishibukuryogan or Kampo drugs containing saiko were ineffective, her pain improved when TSG plus bushi was administered. A wound from a cholecystectomy was observed on her right abdomen. Induration and tenderness of the right hikon were demonstrated. The combination of past history of injury or surgery of the upper abdomen or back and induration of hikon can be associated with development of indications for TSG.

<p>Chilblains is one indication for the use of an ethical Kampo formulation, i.e. <i>tokishigyakukagoshuyushokyoto</i>. This disease is a very common disorder which occurs in cold circumstances. It has been noted that this disorder correlates with diminished peripheral blood flow. However, the inflammatory mechanism associated with this disorder are still unknown. The author's intention is to speculate on the mechanism behind this disorder from the view point of homeostatic inflammation, as proposed by recent immunology. To wit, chilblains may occur via cytokines which are released from ischemic vessels of the microcirculation and/or cells associated with micro-coagulation in the post-capillary venula.</p>

<p>Although “coldness” exaggerates chronic pain in the lower back and extremities in patients who experience it, there is no western-style medicine focusing on “coldness” Therefore, we investigated the efficacy and safety of a Kampo medicine that improves “coldness”. In the present study, we retrospectively reviewed medical chart records on the prescription of <i>tokishigyakukagoshuyushokyoto </i>7.5 g/day (dry weight) to patients who had subjective coldness in their lower extremities. We performed pre- to 1 month post <i>tokishigyakukagoshuyushokyoto </i>treatment comparisons with regard to Visual Analogue Scale (VAS), axillary temperature, dorsal foot temperatures, Ankle-Brachial pressure Index (ABI), Pulse Wave Velocity (PWV) and patient satisfaction. The records of 21 patients with lower back pain and “coldness” were evaluated. VAS, and both axillary and dorsal foot temperatures were significantly improved, and satisfaction score was increased. Subjective coldness was alleviated in 7 patients ; however, objective temperatures were increased in all patients. In summary, we noted that <i>tokishigyakukagoshuyushokyoto </i>increased axillary and the dorsal foot temperatures, and objectively contributed to pain relief as well as patient satisfaction.</p>

<p>It is well known that an abdominal sign of painful groin indicates an appropriate condition for choosing the Kampo formulation tokishigyakukagoshuyushokyoto (TSG). This evidence was found by Keisetsu Otsuka in 1963. He thought that this sign is correlated with the liver meridian.<br>However, the pathophysiological background for this sign has been unknown. Recently, the author made clear that this sign disappears by acupunctural procedure at the Hikon (ExB4) meridian point. According to this clinical evidence, the author proposes a new hypothesis for this abdominal sign in that it closely relates to a homeostatic mechanism. In the cold environment, sympathetic drive to the femoral arteries occurs in order to guard from radiation heat loss in the lower extremities. This drive also causes a reduction of blood supply to the pelvic viscera. The acupunctural procedure at the Hikon (ExB4) meridian point may release both the induration of the iliocostal muscle and the contraction of IOM simultaneously. TGS disconnects the vicious cycle which exist between the pelvic viscera and sympathetic ganglion, and improves various signs that correlates with ischemia of the pelvic viscera.</p>

<p>In 1963, Keisetsu Otsuka reported painful groin as a typical abdominal sign for tokishigyakukagoshuyushokyoto (TSG) <i>sho </i>(indication). In 1974, he also reported Senki syndrome type A. However, this useful abdominal sign was not described as a ‘painful groin' medical term by him. By investigation his pupil's descriptions, the author revealed that this ‘painful groin' term was commonly used in their articles. The author has recently found that inguinal pain accompanies muscle contraction and hyperalgesia in this region clinically. Thus, these sign in the groin should be considered as noted in the diagnosis of TSG <i>sho</i>.</p>

<p>We present two cases of aged males with cold-induced colic successfully treated with tokishigyakukagoshuyushokyoto. The first case is a 74-year-old male with bilateral sciatic neuralgia, who had a history of right leg amputation following a traffic accident. The second case is an 80-year-old male with chills of the lower body, who has undergone excision of biliary duct cancer. Their physical status appeared normal to excessive by traditional medical examination, whereas the pulse and abdominal patterns are usually deficient in female cases in whom tokishigyakukagoshuyushokyoto is effective. The common features in our cases are age and history of trauma by injury or surgery. Although males are more tolerant of cold than females, advancing age and history of injury or surgery tend to cause cold-induced colic in males. The number of male cases that can be effectively treated with tokishigyakukagoshuyushokyoto is likely to increase in our aging society, and we conclude that it is important to consider the presence of cold when we examine male cases.</p>

<b>Objectives</b> : Persistent coldness is a common complaint for which Kampo, traditional Japanese herbal medicine, is often tried, because there is no definition or objective measure of subjective coldness in Western medicine. The aim of this study was to evaluate the efficacy of a common Kampo agent, tokishigyakukagoshuyushokyoto, and to identify key symptoms that would predict positive treatment outcomes for coldness.<br><b>Design</b> : A retrospective cohort study.<br><b>Setting</b> : 181 patients who complained of coldness without any organic abnormalities self-surveyed their health using a Kampo questionnaire database system on their first visit. Another 28 patients were assigned to verify the discriminatory predictability of treatment efficacy.<br><b>Interventions</b> : Patients were treated with extract product, 7.5 grams a day for a month.<br><b>Outcome measures</b> : Treatment efficacy as a binary response.<br><b>Results</b> : Frequency and severity scales (0 to 4) for coldness decreased from 3.2 ± 0.7 to 2.1 ± 0.1 (<i>p </i>< 0.01)and from 3.1 ± 0.7 to 2.2 ± 0.9 (<i>p </i>< 0.01), respectively. Improvements in both frequency and severity of coldness were observed in 74.0% of patients. The status of tenderness in the iliac region, without upset stom ach or depression, was obtained as an appropriate model for the prediction of treatment effect, and its discrimi natory predictability was calculated as 84.4%. The predictive accuracy of the model was validated with 82.1% for the 28 new patients.<br><b>Conclusions</b> : Coldness susceptible to tokishigyakukagoshuyushokyoto was very frequently accompanied by tenderness in iliac region, without upset stomach or depression.

RESUMEN

Livedo vasculitis is one of the most difficult dermatological diseases to cope with in routine clinical practice. We report the case of a 23-year-old woman, who was diagnosed with livedo vasculitis and was responsive to Kampo treatment. She had a rash-like bruise from the spring of X-2, being diagnosed with livedo vasculitis at another hospital. In April, X, painful multiple ulcers on both lower limbs were beginning to appear and were resistant to the western standard therapy. In May 6,X, we started Kampo therapy for her at our hospital. After 4 months, her painful ulcers disappeared. We used tokishigyakukagoshuyushokyoto as the main agent, along with kakobushimatsu, hainoto, senkinnaitakusan, hakushusan and tokishakuyakusan. Although her livedo symptoms still remain and may need long-term treatment to heal completely, she obtained relief from painful ulcers, returning to her normal daily life.

For two infertile couples with normal tubal function and spermatogenesis, who were diagnosed with infertility attributed to lack of competent ovum with poor grade embryo in spite of having received timing therapy and step-up therapy (artificial fertilization from husband ; AIH and in vitro fertilization and embryo transfer ; IVF-ET), Kampo-only therapies diagnosed according to their “Zheng” syndrome-type consisting of one cycle of tokishigyakukagoshuyushokyoto, and two cycles of kamishoyosan and anchusan was performed. As a result, both women became pregnant and gave live births. The causes of infertility in these two cases were analyzed, and problems with step-up therapy, commonly used in Western medicine and efficacy of Kampo-therapy diagnosed according to their Zheng were evaluated from the standpoint of Western medicine.Kampo-therapy diagnosed by Zheng was found to improve the follicular and luteal functions as well as related functions. The two cases presented were diagnosed as infertile due to ovarian dysfunction on the basis of therapeutic diagnosis. The indications of IVF-ET are tubal and male infertilities, not to be ovarian dysfunctional infertility.For infertility due to ovarian dysfunction, Kampo-therapy diagnosed by Zheng with Kuoketsu and Risui effects are drugs of first choice, rather than clomiphene, which has an anti-estrogen activity.

RESUMEN

A case of atypical facial pain treated with Toki-shigyaku-ka-goshuyu-shokyo-to was reported. The patient was a 30-year-old female with oversensitivity to cold. She had suffered from intractable atypical facial pain in her left cheek for eight years. Diagnostic images showed no organic lesion and microvascular compression along the left trigeminal nerve. Although conventional medical therapy had not been successful in relieving the pain, Toki-shigyaku-ka-goshuyu-shokyo-to, in combination with several analgetic agents, was therapeutically effective. Later, Toki-shigyaku-ka-goshuyu-shokyo-to alone was effective in relieving painful episode.
