RESUMEN

Objective To evaluate the efficacy and safety of modified technique of ureteral stent placement during transurethral resection of bladder tumor.Methods Seventeen patients with muscle invasive bladder cancer invading the ureteral orifice from March 2014 to June 2016 in Beijing Friendship Hospital were randomly divided into two groups.In the modified technique group (group A,n =10),tumor was resected until the ureteral orifice was exposed.Then,a guide wire was placed through the tunnel of Electrode loop and the ureteral stent was placed along the guide wire.In control group (group B,7 cases),after ureteral orifice exposed,the guide wire and ureteral stent was placed with cystoscopy.The demographic,oncological baseline and operative factors were evaluated between two groups.Results There was no difference in tumor grade and maximum diameter between two groups (P > 0.05).The operation time in group A was significantly shorter than that in group B [(39.5 ± 14.8)min vs (59.3 ± 16.2) min,P =0.020],and the intraoperative vision clarity score was better in group A as well [(7.7 ± 1.3) vs (5.9± 1.2),P =0.010].There was no statistical difference between the two groups in the complication.Conclusions The modified technique can improve the efficiency of ureteral stent placement during transurethral resection of bladder tumor without any risk enhancement.

RESUMEN

Objective To evaluate the clinical efficacy of transurethral resection of ureteral orifice invaded by advanced prostate cancer caused hydronephrosis. Methods A retrospective study was done in 15 patients who were diagnosed by advanced prostate cancer with invasion of ureteral orifice and treated by transurethral resection of ureteral orifice and maximal androgen blockade. 24 kidneys were diagnosed as hydronephrosis by ultrasound. Before the procedure, the average serum BUN was 13.2 mmol/L (8.9~28.5), the average serum Cr was 243.3 μmol/L (126.7~369.2), the average GFR evaluated by renal radionuclide imaging was 48.6 mL/min (31.1~66.2), and the upper urinary tract was obstructed in kidneys with hydronephrosis. Results All 15 patients underwent successfully transurethral resection of ureteral orifice and discharged after 4 days stay. The average procedure time was 65 min (50~100 min) and mean blood loss was 45 mL (30~65 mL) . The patients were followed up for 2~4 weeks. Hydronephrosis examined by ultrasound was ameliorated in 18 kidneys (75%) and not obviously changed in 6 kidneys (25%) in one week after procedure. Hydronephrosis examined by ultrasound was ameliorated in 20 kidneys (83.3%) and not obviously changed in 4 kidneys (16.7%) in two weeks after procedure. Within two weeks after procedure,the average serum BUN was 10.75 mmol/L (6.6~21.30 mmol/L), the average serum Cr was 187.3μmol/L (97.5~286.6 μmol/L), the average GFR evaluated by renal radionuclide imaging was 58.1 mL/min (37.8~79.2 mL/min),and upper urinary tract was unobstructed. Conclusion Upper urinary tract obstruction and renal function were ameliorated and improved in a short time by transurethral resection of ureteral orifice invaded by advanced prostate cancer which caused hydronephrosis.

Objective Discussion transurethral holmium laser treatment of ureteral orifice cyst and stones efficacy and safety . Methods A retrospective analysis of 23 cases of transurethral holmium laser treatment in patients with ureteral orifice cysts and stones (minimally invasive group) and 11 cases of open surgical excision of the cyst and stone -treated patients (open group), two groups were compared operative time, blood loss , catheterization time, hospital stay, postoperative complication rate .Results Minimally invasive surgery patients time (20.0 ±5.6)min, blood loss (15.0 ±2.3)ml, ambulation time (10.0 ±2.5)h, catheterization time (2.3 ± 0.2)d, length of hospital stay (5.0 ±0.6)d; open group of patients were (90.0 ±10.3)min, (80.0 ±12.7)ml, (48.0 ±5.3)h, (7.0 ±1.1)d, (11.0 ±1.7)d, two groups of indicators differences were statistically significant ( P <0.05).Postoperative complica-tions occurred in patients with minimally invasive group was 8.6%, down from 45.4% in the control group, the difference was statisti-cally significant ( P <0.05).Conclusions Transurethral holmium laser treatment of ureteral orifice cysts and stones with less trau -ma, less bleeding, shorter operative time, rapid recovery, the advantages of the low rate of complications , treatment of ureteral orifice cysts and stones safe and effective method .

Objective To discuss the clinical efficacy of transurethral electroresection in the treatment of ureteral orifice tumors. Methods A total of 65 cases of ureteral orifice tumor were treated with transurethral electroresection and chemotherapy.Results The operation time was 5~90 min(mean,35 min),and the intraoperative blood loss was 5~100 ml(mean,50 ml).No blood transfusion was required.No fatal case was encountered.Out of the 65 cases,there were 53 cases of transitional cell carcinoma(grade Ⅰ,43 cases;grade Ⅱ,10 cases) and 12 cases of papilloma.The 65 cases were followed for 6~24 months(mean,15 months).Postoperative urethral stricture happened in 4 cases,which were cured by dilation of the urethra.Recurrence of tumor at the trigone and lateral wall of the bladder occurred in 5 cases,which were given a re-operation of transurethral electroresection and chemotherapy and were followed for 10~20 months(mean,15 months) without recurrence.Conclusions Transurethral electroresection of ureteral orifice tumors gives satisfactory effects and should be further recommended.

RESUMEN

Renal dysplasia results from aberrant metanephric histogenesis caused fundamentally by a defect in inducer tissue or responding tissue. Dysplastic kidneys vary tremendously in gross and microscopic appearance but are characterized by abnormal organization and a mixed population of primitive structures, such as fetal or immature cartilage, dysplastic ducts, immature tubules, and undifferentiated mesenchyme. We report a case of unilateral multicystic renal dysplasia associated with an ipsilateral ectopic ureteral orifice entering a seminal vesicle cyst in a 33-year-old man. He was admitted due to primary infertility which had developed three years ago. The his semen analysis revealed oligospermia. No evidence of a family history of renal dysplasia was reported. Microscopic examination showed that the entire kidney was composed of cysts lined by flattened cells, dysplastic ducts and immature tubules surrounded by collars of spindle cells, primitive mesenchyme, and a few aberrantly formed glomeruli.

RESUMEN

Lumbar kidney is one of renal ectopia which rests opposite the sacral promontory in the iliac fossa and anterior to the iliac vessels. We have experienced a case of hypoplastic kidney associated with ipsilateral vaginal ectopic urethral opening which had been discovered by abdominal exploration.

RESUMEN

An ectopic ureteral orifice inserts at a point other than the trigone of the bladder. Ectopic ureteral orifices in a girl commonly are associated with complete ureteral duplication comprises only 10 to 12 per cent of such patients. Continuous incontinence in a girl with an otherwise normal voiding pattern after toilet training is the classic sign of an ectopic ureteral orifice. Here, we report a 3-years-old girl who was admitted to our hospital due to continuous urinary incontinence and managed with the diagnosis of bilateral ectopic ureteral orifices associated with bilateral complete ureteral duplication.

A case of unilateral multicystic renal dysplasia was reported in view of rarity, and a review of literature 23-year-old female was admitted to Kyung Hee University Hospital with a complaint of urinary incontinence. An excretory urogram revealed non-visualization of the right kidney. The right ureter had opening at the 2 cm above the vaginal introitus. The resected right kidney revelaed multilobulated outer surface with multiloculated cystic struture on cut surface. The associated anomaly was uterine didelphy. The condition was treated by nephroureterectomy.

RESUMEN

The ectopic ureteral orifice is commonly associated with duplicated ureter But it is relatively rare that duplicated ureter has ectopic ureteral orifices in company. Herein, in 2 year old girl, we report a case of ectopic ureteral orifices in unilateral duplicated ureter which are located in bladder neck, distal urethra. She has been suffered from continual dribbling despite normal pattern of voiding, treated with ureteral reimplantation in common sheath.

RESUMEN

We present a case of left ectopic ureteral orifice to the vagina with a hypoplastic kidney in a 15 year old girl and a case of ectopic ureteral orifice to the vaginal vestibule associated with bilateral complete double ureter in a 5 year old girl. They presented with urinary incontinence while normally secured voluntary voiding. Fifteen year old girl was treated with left nephrectomy and the other girl with heminephrectomy.

The renal agenesis and ectopic ureter were relatively rare congenital disease, but their combined form was more extremely rare condition. RESUMEN

Vesicoureteral reflux bas been recognized to be an abnormal phenomenon in man, often associated with urinary infection and with both developmental and acquired renal abnormalities. Thus, meticulous endoscopic observation on ureteral orifice configuration is thought to be essential to the accurate diagnosis and the selection of the therapeutic method in management of the patients with diseases in bladder, ureter and kidney. So, endoscopic observation was made on 90 cases of the selected persons with normal findings on I. V. P. & retrograde cystogram, in the Department of Urology. Korea General Hospital, from March I, 1979 to August 31, 1979. The results obtained were as follows; 1) The ureteral orifice configuration of the patients belong to the tunnel shape (66 cases: 36.6%) & the slit shape (64 cases: 35.6%) in the majority. And the next was the oval shaped(32 cases: 17. 8%), the gutter shaped (18 cases: 10.0% in order. 2) There was some difference between male & female in the tunnel shaped (M: 43.1%, F: 25.0%) and the gutter shaped (M:5.2%, F:18.7%) but its difference was negligible, we thought. 3) Most patients (68 cases: 75.6%) had the same ureteral orifices in its configuration.
