RESUMEN

Objective To study the influence of different suture methods on the uterine diverticulum without suture of subcutaneous fat in cesarean section .Methods 106 cases of cesarean section were randomly divided into two groups according to the digital chart ,53 cases in each group .The control group received single layer suture in cesarean section without suture of subcutaneous fat ,while the observation group received double suture with cesarean section without suture of subcutaneous fat .The incision adverse reactions were observed in the two groups .After 1 year of follow-up,the scar diverticulum in the incision of the uterus was observed .Results In the control group,1 case, 13 cases,9 cases and 0 cases were found fat liquefaction ,subcutaneous induration ,incision redness and swelling ,and the incision union was bad.The incidence rate of adverse reaction was 43.4%.In the observation group,the fat lique-faction,subcutaneous induration ,incision redness and swelling ,incision union were 0 case,2 cases,3 cases,0 case,the incidence rate of adverse reactions was 9.4%.The side effects of incision in the observation group were significantly less than those in the control group(χ2 =13.620,P<0.01).After follow-up for 1 year,the number of diverticula, the muscularis of the diverticulum and the volume of the diverticulum in the control group were 6 cases,(5.51 ± 2.01)mm,(0.46 ±0.13)mL,which in the observation group were 2 cases,(7.63 ±1.63)mm,and (0.31 ±0.11)mL. The incidence of diverticularization in the observation group was significantly less than that in the control group (χ2 =8.960,P<0.05).The diverticulum muscle thickness of the observation group was significantly higher than that of the control group(t=4.631,P<0.05).The diverticulum volume of the observation group was significantly less than that of the control group(t=2.914,P<0.05).Conclusion Cesarean section incision without suture subcutaneous fat bilayer suture is more conducive to the healing of the incision , the formation of scar diverticulum less chance , the formation of scar diverticulum degree is light .

Objective To investigate the efficacy of oxytocin combined with hemabate in the prevention of postpartum hemorrhage and diverticulum in scarred uterus. Methods 88 patients with scar uterus, postpartum hemorrhage and diverticulum in Chang'an Hospital from May 2016 to May 2017 were divided into two groups by maternal numbering and randomization, with 44 patients in each group. The control group was treated with oxytocin, and the observation group was treated with oxytocin combined with hemabate. It must compare the amount of bleeding, the amount of 2 h bleeding, the amount of postpartum 24 h bleeding and the incidence of adverse reactions in the two groups after the treatment. Results Compared with the control group, the amount of bleeding, the amount of two hours' bleeding and the amount of postpartum 24 hours' bleeding in the observation group were significantly lower than that in the control group(P<0.05). The postpartum hemorrhage volume of the observation group was 6.81%, which was lower than that of the control group, and the rate of postpartum hemorrhage was 27.27% (P<0.05). The incidence of adverse reactions such as nausea, vomiting, heart rate and blood pressure in the observation group was significantly lower than that in the control group (4.54% vs. 18.18%), the difference was statistically significant(P<0.05). Conclusion In the scar uterus, postpartum hemorrhage and diverticulum, the use of oxytocin combined with hemabate treatment has obvious curative effect. It is an important measure to reduce the amount of bleeding and ensure safety.

RESUMEN

Here we report an incidental huge uterine-cervical diverticulum from a total abdominal hysterectomy specimen in a perimenopausal woman who presented with acute abdominal pain. The diverticulum was mimicking with various cysts present in the lateral side of the female genital tract. Histopathological examination confirmed this to be a cervical diverticulum with communication to uterine cavity through two different openings. They can attain huge size if left ignored for long duration and present a diagnostic challenge to clinicians, radiologists, as well as pathologists because of its extreme rarity. Therefore, diverticula should also be included as a differential diagnosis. Its histopathological confirmation also highlights that diverticula can present as an acute abdomen, requiring early diagnosis with appropriate timely intervention. Immunohistochemistry CD 10 has also been used to differentiate it from a mesonephric cyst.
