OBJECTIVES@#To observe the effects of electroacupuncture (EA) at "Jiaji" (EX-B 2) combined with neurodynamic mobilization (NM) on the cross-sectional area of the gastrocnemius muscle fibers after sciatic nerve injury in rabbits, and the expression of nuclear factor κB (NF-κB) and muscle-specific ring-finger protein 1 (MuRF1).@*METHODS@#A total of 180 common-grade New Zealand rabbits (half male and half female) were randomly divided into five groups, i.e. a normal control group, a model control group, a NM group, an EA group and a combined intervention group, 36 rabbits in each group. Except in the normal control group, clipping method was used to prepare the model of sciatic nerve injury in the rest groups. On the 3rd day of successful modeling, NM was delivered in the NM group. In the EA group, EA was exerted at bilateral "Jiaji" (EX-B 2) of L4 to L6, stimulated with disperse-dense wave and the frequency of 2 Hz/100 Hz. In the combined intervention group, after EA delivered at bilateral "Jiaji" (EX-B 2) of L4 to L6 , NM was operated. The intervention in each group was delivered once daily, for 6 days a week, and lasted 1, 2 or 4 weeks according to the collection time of sample tissue. After 1, 2 and 4 weeks of intervention, in each group, the toe tension reflex score and the modified Tarlov test score were observed; the morphology of the gastrocnemius muscle was observed by HE staining and the cross-sectional area of muscular fiber was measured; using Western blot method, the expression of NF-κB and MuRF1 of the gastrocnemius muscle was detected.@*RESULTS@#After 1, 2 and 4 weeks of intervention, the toe tension reflex scores and the modified Tarlov scores in the model control group were lower than those of the normal control group (P<0.05), and these two scores in the NM group, the EA group and the combined intervention group were all higher than those of the model control group (P<0.05); the scores in the combined intervention group were higher than those in the EA group and the NM group (P<0.05). The gastrocnemius fibers were well arranged and the myocyte morphology was normal in the normal control group. In the model control group, the gastrocnemius fibers were disarranged, the myocytes were irregular in morphology and the inflammatory cells were infiltrated in the local. In the NM group, the EA group and the combined intervention group, the muscle fibers were regularly arranged when compared with the model control group. After 1, 2 and 4 weeks of intervention, the cross-sectional areas of the gastrocnemius muscle fibers in the model control group were smaller than those of the normal control group (P<0.05). The cross-sectional areas in the NM group, the EA group and the combined intervention group were larger than those of the model control group (P<0.05), and the cross-sectional areas in the combined intervention group were larger than those in the NM group and the EA group (P<0.05). After intervention for 1, 2 and 4 weeks, the protein expressions of NF-κB and MuRF1 in the gastrocnemius muscle were higher in the model control group in comparison of those in the normal control group (P<0.05). In the NM group, the EA group and the combined intervention group, the expressions of NF-κB after intervention for 1, 2 and 4 weeks and the expressions of MuRF1 after 2 and 4 weeks of intervention were lower when compared with those in the model control group (P<0.05). In the combined intervention group, the protein expressions of NF-κB after intervention for 1, 2 and 4 weeks and the expressions of MuRF1 after 2 and 4 weeks of intervention were decreased when compared with those in the NM group and the EA group (P<0.05).@*CONCLUSIONS@#Electroacupuncture at "Jiaji" (EX-B 2) combined with NM may increase the muscle strength and sciatic function and alleviate gastrocnemius muscle atrophy in the rabbits with sciatic nerve injury. The underlying mechanism is related to the inhibition of NF-κB and MuRF1 expression.

SUMMARY: To describe the physical therapy protocols used in critically ill patients to attenuate skeletal muscle atrophy. We conducted a search in PubMed and Embase from inception to November 2020. Observational or experimental studies published in English or Spanish that evaluated the effect of physical therapy protocols on the attenuation of skeletal muscle atrophy in critically ill patients through muscle strength or mass measurement were considered eligible. Studies were only included if they reported a detailed description of the dosing of the interventions. Seventeen studies met the eligibility criteria. We included randomised clinical trials (n = 16) and observational studies (n = 1). The total population of the included studies was 872 critically ill patients. The studies aimed to evaluate the reliability, safety or effectiveness of neuromuscular electrical stimulation (n = 10) protocols, early mobilisation (n = 3), ergometer training (n = 2), transfers in tilt table (n = 1), and blood flow restriction (n = 1). Physical therapy protocols are part of the critically ill patient's integral management. Strategies such as passive mobilisation, in-bed and out-of-bed transfers, gait training, ergometer training, and neuromuscular electrical stimulation substantially impact critically ill patients' prognoses and quality of life after hospital discharge.


RESUMEN: Describir los protocolos de terapia física usados en pacientes críticos para atenuar la atrofia muscular esquelética. Realizamos una búsqueda en PubMed y Embase desde el inicio hasta noviembre de 2020. Se consideraron los estudios observacionales o experimentales publicados en inglés o español que evaluaron el efecto de los protocolos de terapia física en la atenuación de la atrofia del músculo esquelético en pacientes críticos a través de la medición de la fuerza o la masa muscular. Los estudios solo se incluyeron si informaron una descripción detallada de la dosificación de las intervenciones. Diecisiete estudios cumplieron los criterios de elegibilidad. Se incluyeron ensayos clínicos aleatorizados (n = 16) y estudios observacionales (n = 1). La población total de los estudios incluidos fue de 872 pacientes en estado crítico. Los estudios tuvieron como objetivo evaluar la confiabilidad, seguridad o efectividad de los protocolos de estimulación eléctrica neuromuscular (n = 10), movilización temprana (n = 3), entrenamiento con ergómetro (n = 2), transferencias en mesa basculante (n = 1) y restricción del flujo sanguíneo (n = 1). Los protocolos de terapia física forman parte del manejo integral del paciente crítico. Es- trategias como la movilización pasiva, los traslados dentro y fuera de la cama, el entrenamiento de la marcha, el entrenamiento con ergómetro y la estimulación eléctrica neuromuscular tienen un impacto sustancial en el pronóstico y la calidad de vida de los pacientes críticos después del alta hospitalaria.

Introdução: A utilização da estimulação elétrica (EE) no tratamento de músculos desnervados tem sido muito estudada, pois auxilia na reinervação tardia e minimiza a atrofia muscular. Objetivo: Neste estudo, avaliamos a EE através da corrente alternada simétrica sinusoidal na manutenção dos aspectos morfológicos dos músculos Extensor longo dos dedos (EDL) e Sóleo desnervados experimentalmente. Método: Quarenta ratos da linhagem wistar foram divididos em quatro grupos experimentais (n=10): Em grupo controle inicial (GCI), controle Final (GCF), desnervado (GD), e em desnervado e estimulado (GDE). A aplicação EE no músculo EDL e sóleo teve início 48 horas após a lesão nervosa, sendo aplicada três vezes por semana durante quarenta e cinco dias. Cortes transversais foram corados com HE para a mensuração da morfometria das fibras musculares. Para a análise estatística foi utilizado a análise de variância (one-way - ANOVA) seguido pelo teste de tukey (p < 0,05). Resultados: Os animais do grupo D e DE apresentaram uma diminuição da área de secção transversal (AST) quando comparadas ao CF. Porém o grupo DE minimizou a atrofia muscular, pois houve diferença estatisticamente significativa quando comparado ao D, tanto no músculo EDL quanto no sóleo. Conclusão: A EE foi eficiente na manutenção dos aspectos morfológicos dos músculos EDL e sóleo, minimizando a atrofia muscular. Podendo ser um tratamento útil para as lesões nervosas periféricas.


Introduction: the use of electrical stimulation (ES) in the treatment of denervated muscles has been studied as it assists in the late reinnervation and minimizes muscle atrophy. Purpose: this study assessed the ES through the symmetric sinusoidal alternating current in the maintenance of the morphological aspects of the experimentally denervated Extensor digitorum longus (EDL) and Soleus muscles. Method: Forty Wister rats were divided into four experimental groups (n = 10): initial control group ( ICG), final control group (FCG), denervated group (DG) and denervated and stimulated group (DSG). ES was first applied in the soleus and EDL muscle 48 hours after nerve injury, the applications took place three times per week for forty-five days. Cross-sections were stained with HE to perform the morphometric analysis of the muscle fibers. We used the analysis of variance (one-way - ANOVA) followed by the Tukey test (p < 0.05) for statistical analysis. Results: the animals belonging to group D and DE showed a decrease in the crosssectional area (CSA) when compared to FCG. However, the group DE had muscular atrophy minimized, as there was a statistically significant difference when compared to D, both for EDL and soleus muscles. Conclusion: the ES was effective in maintaining the morphological aspects of the EDL and soleus muscles, minimizing muscle atrophy. This may be a useful treatment for the peripheral nerve injuries.

Abstract The reduction of skeletal muscle loss in pathological states, such as muscle disuse, has considerable effects in terms of rehabilitation and quality of life. Since there is no currently effective and safe treatment available for skeletal muscle atrophy, the search for new alternatives is necessary. Resistance exercise (RE) seems to be an important tool in the treatment of disuse-induced skeletal muscle atrophy by promoting positive functional (strength and power) and structural (hypertrophy and phenotypic changes) adaptive responses. Human and animal studies using different types of resistance exercise (flywheel, vascular occlusion, dynamic, isometric, and eccentric) have obtained results of great importance. However, since RE is a complex phenomenon, lack of strict control of its variables (volume, frequency, intensity, muscle action, rest intervals) limits the interpretation of the impact of the manipulation on skeletal muscle remodeling and function under disuse. The aim of this review is to critically describe the functional and morphological role of resistance exercise in disuse-induced skeletal muscle atrophy with emphasis on the principles of training.

BACKGROUND: Lower limb amyotrophy is a serious deformity that is often disabling and causes esthetic problems such as asymmetries and changes in the shape and contour of the legs. The present study evaluated the use of silicone implants for the correction of lower limb amyotrophy. METHODS: Women who underwent surgical correction of lower limb amyotrophy with silicone implants between 2002 and 2009 at the Plastic Surgery Service of Instituto Ivo Pitanguy at the 38th Nursing Service of Santa Casa da Misericórdia, Rio de Janeiro and at their associated services were retrospectively reviewed. RESULTS: Five cases were studied and the results were retrospectively assessed. Follow-up ranged from 3 months to 2 years after surgery until the last postoperative visit. In 4 of 5 patients, the desired result was obtained after a single surgical procedure, and no patient developed capsular contracture, compartment syndrome, circulatory disorders, neurological disorders, or infections. CONCLUSIONS: Calf augmentation in patients with lower limb amyotrophy did not correct the associated functional deficit, but resulted in a high level of satisfaction with regard to the esthetic improvement of the limb, thus improving the quality of life of these patients.


INTRODUÇÃO: As amiotrofias dos membros inferiores são deformidades por vezes incapacitantes, mas que trazem aos pacientes problemas estéticos, como assimetrias e alterações da forma e contorno das pernas. Este estudo teve como objetivo avaliar a aplicabilidade do uso de implantes de silicone para correção de amiotrofias de membros inferiores. MÉTODO: Estudo retrospectivo de pacientes, todas do sexo feminino, submetidas a correção cirúrgica de amiotrofia de pernas com a utilização de implantes de silicone, entre 2002 e 2009, no Serviço de Cirurgia Plástica do Instituto Ivo Pitanguy, na 38ª Enfermaria da Santa Casa da Misericórdia do Rio de Janeiro e em serviços associados. RESULTADOS: Foram analisados 5 casos. A maioria dos resultados foi avaliada de maneira retrospectiva. O tempo de acompanhamento variou de 3 meses a 2 anos, desde a cirurgia até a última consulta pós-operatória. Do total de pacientes, 4 apresentaram resultado desejado em apenas um procedimento cirúrgico e nenhuma paciente apresentou contratura capsular, síndrome compartimental, alterações circulatórias, alterações neurológicas ou infecção. CONCLUSÕES: O aumento das panturrilhas em pacientes portadoras de amiotrofias de membros inferiores não proporcionou modificações no tocante ao deficit funcional prévio. Todavia, essas pacientes, geralmente bastante castigadas pela doença de base, apresentam índice de satisfação muito elevado com a melhoria estética do membro, otimizando, dessa forma, a qualidade de vida dessas pacientes.

A estimulação elétrica neuromuscular (EENM) é um importante recurso utilizado em medicina esportiva para acelerar processos de recuperação. O objetivo deste estudo foi analisar os efeitos da EENM durante a imobilização do músculo gastrocnêmio, em posições de alongamento (LP) e encurtamento (SP). Para tanto, 60 ratas fêmeas jovens Wistar foram distribuídas em seis grupos e acompanhadas durante sete dias: controle (C), eletroestimuladas (EE), imobilizadas em encurtamento (ISP), imobilizadas em alongamento (ILP), imobilizadas em encurtamento e eletroestimuladas (ISP + EE) e imobilizadas em alongamento e eletroestimuladas (ILP + EE). Para a imobilização, o membro posterior direito foi envolvido por uma malha tubular e ataduras de algodão juntamente à atadura gessada. A EENM foi utilizada com freqüência de 50Hz, 10 minutos por dia, totalizando 20 contrações em cada sessão. Após sete dias os animais foram submetidos a eutanásia e os músculos gastrocnêmios retirados para a realização do ensaio mecânico de tração em uma máquina universal de ensaios (EMIC®). A partir dos gráficos carga versus alongamento, foram calculadas as seguintes propriedades mecânicas: alongamento no limite de proporcionalidade (ALP), carga no limite de proporcionalidade (CLP) e rigidez. As imobilizações SP e LP promoveram reduções significativas (p < 0,05) nas propriedades de ALP e CLP, sendo mais acentuada no grupo ISP. Quando utilizada a EENM, houve acréscimo significativo (p < 0,05) dessas propriedades somente no grupo ISP. Já em relação à rigidez, foi observada redução significativa (p < 0,05) somente do grupo C para o grupo ISP. Quando utilizada a EENM, a rigidez do grupo ILP + EE foi significativamente (p < 0,05) maior e mais próxima do grupo C que a do grupo ISP + EE. Neste modelo experimental, a imobilização dos músculos em alongamento atrasou o processo de atrofia, e a estimulação elétrica, realizada durante a imobilização, contribuiu para a manutenção das propriedades...


The neuromuscular electric stimulation (NMES) is an important tool used in sports medicine to accelerate the recovery process. The objective of this study was to analyze the effects of NMES during immobilization of the gastrocnemius muscle, in lengthened (LP) and shortened positions (SP). Sixty young female Wistar rats were distributed into six groups and followed for 7 days: control (C); electric stimulation (ES); immobilized in shortening (ISP); immobilized in lengthening (ILP); immobilized in shortening and electric stimulation (ISP + ES) and immobilized in lengthening and electric stimulation (ILP + ES). For the immobilization, a tubular mesh and cotton rolls together with the plaster were wrapped around the rat's right posterior paw. NMES in a frequency of 50 Hz was used 10 minutes a day, totaling 20 contractions in each session. After 7 days the animals were sacrificed and their gastrocnemius muscles of the right paw were submitted to a mechanical test of traction in a universal test machine (EMIC®). From the load versus elongation curves the following mechanical properties were obtained: elongation in the yield limit (EPL), load in the yield limit (LPL) and stiffness. The SP and LP immobilizations promoted significant reductions (p < 0.05) in the EPL and LPL properties, being more remarkable in the ISP group. When the NMES was used, there was significant increase (p < 0.05) of these properties only in the ISP group. As for stiffness, significant reduction was observed (p < 0.05) only of the C group for the ISP group. When the NMES was used, the stiffness of the ILP + EE group was significantly (p < 0.05) higher and closer to the C group than of the ISP + EE group. We conclude that in this experimental model the immobilization of the muscles in the lengthened position delayed the atrophy process and the electric stimulation during the immobilization contributed to the maintenance of the mechanical properties during the immobilization...


La estimulación eléctrica neuromuscular (EENM) es un importante recurso utilizado en medicina deportiva para acelerar procesos de recuperación. El objetivo de este estudio ha sido analizar los efectos de la EENM durante la inmovilización del músculo gastrocnemio, en posiciones de alongamiento (LP) y contracción (SP). Para tal fin, 60 ratones hembras jóvenes Wistar fueron distribuidas en seis grupos y monitoreadas durante 7 días: control (C), electro estimuladas (EE), inmovilizadas en contracción (ISP), inmovilizadas en alongamiento (ILP), inmovilizadas en contracción y electro estimuladas (ISP + EE) e inmovilizadas en alongamiento y electro estimuladas (ILP + EE). Para la inmovilización, el miembro posterior derecho fue envuelto por una malla tubular y vendas de algodón en conjunto con vendas de escayola. La EENM fue utilizada con una frecuencia de 50 Hz, 10 minutos por día, totalizando 20 contracciones en cada sesión. Después de 7 días los animales fueron sometidos a eutanasia y los músculos gastrocnemios fueron retirados para la realización del ensayo mecánico de tracción en una máquina universal de ensayos (EMIC®). A partir de los gráficos carga versus alongamiento se calculó las siguientes propiedades mecánicas: alongamiento en el límite de proporcionalidad (ALP), carga en el límite de proporcionalidad (CLP) y rigidez. Las inmovilizaciones SP y LP dieron reducciones significativas (p < 0,05) en las propiedades de ALP y CLP, siendo más acentuada en el grupo ISP. Cuando utilizamos la EENM, hubo un crecimiento significativo (p < 0,05) de estas propiedades solamente en el grupo ISP. Por otro lado, en relación a la rigidez, se observó una reducción significativa (p < 0,05) solamente del grupo C respecto al grupo ISP. También se observó que al usarse EENM, la rigidez del grupo ILP + EE fue significativamente (p < 0,05) mayor y más próxima del grupo C que la del grupo ISP + EE. RESUMEN

A prevalência de incapacidade e dependência funcional é maior em idosos e está intimamente associada à redução da massa muscular, que ocorre, até mesmo, em indivíduos saudáveis. A sarcopenia parece decorrer da interação complexa de distúrbios da inervação, diminuição de hormônios, aumento de mediadores inflamatórios e alterações da ingestão protéico-calórica que ocorrem durante o envelhecimento. A perda de massa e força muscular é responsável pela redução de mobilidade e aumento da incapacidade funcional e dependência. Quando associada à fragilidade, esta perda gera custos econômicos e sociais. Neste artigo, pretende-se avaliar aspectos relacionados à gênese da sarcopenia, bem como analisar possíveis opções terapêuticas e de prevenção.


The high prevalence of inability and functional dependence is an important problem in elderly people. It is closely related with aging decrease of lean muscle mass that occurs even in healthy subjects. Skeletal muscle mass deficiency, or sarcopenia, results from complex interactions between innervations disturbances, hormones deficiency, inflammatory cytokines and restriction in caloric-proteic ingestion. Loss of skeletal muscle mass and strength results in disability and functional dependency that are associated to frailty in many elderly people. These conditions represent enormous economic and social budget. In this article, we evaluate pathogenesis of sarcopenia and discuss potential therapies.
