RESUMEN

La litiasis vesicular en la infancia y adolescencia ha acusado un notable incremento en los últimos años. En países europeos, como Inglaterra, las colecistectomías se han triplicado desde 1997 y en el 2012 en el Hospital Infantil de Santiago de Cuba representó el 96,4 por ciento de todas las operaciones mayores electivas. En el IV Simposio Nacional de Cirugía Pediátrica (Varadero, Matanzas, 1- 3 de julio de 2019) fue presentada, discutida y aprobada esta "Guía de Práctica Clínica de litiasis vesicular en niños y adolescentes" y se recomendó, al concluir dicho evento, compartir dicha guía a través de su publicación, para que los servicios de cirugía pediátrica la empleen como referencia y la apliquen en las instituciones del sistema cubano de salud encargadas de la atención sanitaria de niños y adolescentes(AU)


Vesicular lithiasis in childhood and adolescence has had a marked increase in recent years. In European countries such as England, cholecystectomies have tripled since 1997 and in 2012 at the Children's Hospital in Santiago de Cuba accounted for 96.4 percent of all major elective operations. At the IV National Symposium on Pediatric Surgery (Varadero, Matanzas, July 1-3, 2019) this " Clinical Practice Guidelines of Vesicular Lithiasis in Children and Adolescents" was presented, discussed and approved, and it was recommended, at the conclusion of that event, to share this guidelines through its publication, for pediatric surgery services to use it as a reference and to be applied in the institutions of the Cuban health system which are responsible for the health care of children and adolescents(AU)

RESUMEN Fundamento: El dispositivo intrauterino ha sido utilizado durante muchos años como método anticonceptivo; una complicación infrecuente posterior a su inserción es la migración fuera del útero. La localización vesical y la formación de vesicolitiasis, son complicaciones asociadas a la migración. Objetivo: Presentar un caso de migración de un dispositivo intrauterino a vejiga con litiasis sobreañadida como inusual etiología de una cistitis recurrente. Presentación del caso: Caso clínico de un dispositivo intrauterino en vejiga en una paciente de 43 años, cuyo diagnóstico se realizó incidentalmente en estudio de cistitis recurrente; se diagnosticó imagenológica y endoscópicamente en consulta de Urología; se decidió tratamiento quirúrgico mediante cistolitotomía a cielo abierto y se extrajo un cálculo de 4x5 cm de diámetro. La paciente evolucionó satisfactoriamente. Conclusiones: Considérese la posibilidad de migración del dispositivo intrauterino a la vejiga con litiasis sobreañadida como causa de cistitis recurrente, en pacientes femeninas que tengan antecedente de uso de este método anticonceptivo, lo que constituye un elemento importante en el diagnóstico y tratamiento de la infección urinaria baja.


ABSTRACT Background: The intrauterine device has been used for years as a contraceptive method; a non-frequent complication after its insertion is migration out of the uterus. The bladder location and the formation of vesicolithiasis are complications associated with migration. Objective: To present a migration case from an intrauterine device to the bladder with overadded lithiasis as an unusual etiology of recurrent cystitis. Case report: Clinical case of an intrauterine device in the bladder in a 43-year-old patient, whose diagnosis was made incidentally in a recurrent cystitis study, it was diagnosed by imaging and endoscopy in the Urology consultation; surgical treatment was decided by means of open cystolithotomy and a stone 4x5 cm in diameter was extracted. The patient evolved satisfactorily. Conclusions: To consider the possibility of migration of the intrauterine device to the bladder with overadded lithiasis as a cause of recurrent cystitis in female patients who have a preceding use of this contraceptive method, thus it constitutes an important element in the diagnosis and treatment of urinary lower infection.

Introducción: La litogénesis biliar, proceso de sobresaturación de colesterol en la bilis vesicular, es prevenible. Objetivo: Describir las nuevas evidencias biomoleculares de la litogénesis biliar de colesterol como base de la futura terapia preventiva de la litiasis vesicular. Método: Se realizó una revisión sistemática y crítica de las evidencias de impacto sobre la litogénesis biliar. Se consultaron artículos publicados entre 2015-2020 en las bases de datos PubMed, Medline, SciELO, LILACS y Elsevier. Resultados: Se recuperaron evidencias actuales de los mecanismos biomoleculares relacionados con las futuras terapias preventivas de la litiasis vesicular, propuestos como fundamentos teóricos. Conclusiones: La descripción actualizada de la litogénesis biliar de colesterol, con los nuevos conceptos biomoleculares incorporados, aporta a su comprensión el papel de los genes de receptores nucleares, la intervención de estos últimos y de los transportadores de la secreción biliar. Dirigida a médicos generales, cirujanos, gastroenterólogos y fisiólogos, la descripción actualizada de La litogénesis biliar impacta como nuevo paradigma con los conceptos biomoleculares que intervienen en pro de su prevención(AU)


Introduction: Biliary lithogenesis is a preventable process of cholesterol supersaturation in gallbladder bile. Objective: Describe the new biomolecular evidence of biliary cholesterol lithogenesis serving as a basis for future preventive therapy for gallbladder lithiasis. Methods: A systematic critical review was conducted of impact evidence about biliary lithogenesis. The papers consulted were published in the databases PubMed, Medline, SciELO, LILACS and Elsevier from 2015 to 2020. Results: Current evidence was retrieved of biomolecular mechanisms proposed as theoretical foundations for future preventive therapies for gallbladder lithiasis. Conclusions: Intended for general practitioners, surgeons, gastroenterologists and physiologists, the updated description of biliary lithogenesis including the role of nuclear receptors, biliary lipid transporters and the biological value of enterohepatic circulation in the integrity and functioning of the hepatobiliary system as regulators of the cholesterol mechanism, makes an impact as a new paradigm with the biomolecular concepts involved in biliary lithogenesis prevention(AU)

Urethral stricture disease is a relatively common urologic condition with an estimated incidence of 9/100,000 men younger than 65 years. The risk of stricture is markedly increased in patients older than 65 years with an incidence of 21/100,000, which is almost half the incidence for urolithiasis in this population. This indicates the importance and high burden of strictures.[1] Symptomatic patients typically present with obstructive urinary symptoms (e.g., incomplete emptying, straining, weak stream, etc.) and occasionally have a history of recurrent infections, hematuria, bladder stones, or renal failure.[2] More than half of strictures arise in bulbar urethra.


La estenosis uretral es una afección urológica relativamente frecuente, con una incidencia estimada de 9/100.000 varones menores de 65 años. El riesgo de estenosis aumenta notablemente en los pacientes mayores de 65 años, con una incidencia de 21/100.000, que es casi la mitad de la incidencia de urolitiasis en esta población. Esto indica la importancia y la elevada carga de las estenosis.[1] Los pacientes sintomáticos suelen presentar síntomas urinarios obstructivos (por ejemplo, vaciado incompleto, esfuerzo, chorro débil, etc.) y, en ocasiones, tienen antecedentes de infecciones recurrentes, hematuria, cálculos vesicales o insuficiencia renal.[2] Más de la mitad de las estenosis surgen en la uretra bulbar.

El dispositivo intrauterino (DIU), ha sido utilizado en nuestro medio y en el mundo durante muchos años como método anticonceptivo. Una complicación infrecuente posterior a su inserción es la migración fuera del útero. La localización vesical y complicación con la vesicolitiasis, forman parte de las complicaciones asociadas a la migración. Presentamos un caso de migración de un DIU a la vejiga asociado a la formación de un cálculo entorno a ese dispositivo. Se describe el cuadro clínico, los estudios realizados y el tratamient


The intrauterine device (IUD) has been used in our environment and in the world for many years as a contraceptive method. One of the complications is the migration outside the uterus, being the bladder location unusual and much more the secondary bladder stone. We present a case of migration of an IUD to the bladder associated with the formation of a calculus around this foreign body. The clinical record, study and treatment are described.

@#BACKGROUND: This is a rare case of two large urinary bladder stones causing severe infection of the urinary tract affecting the bone marrow due to chronic immune stimulation in a patient with recurrent anemia. Urinary bladder calculi are hard masses of minerals. They develop when the minerals in concentrated urine crystallize. This often happens when the bladder cannot be emptied. Signs and symptoms can vary from severe abdominal pain to blood in the urine. Sometimes, bladder stones don't cause any symptoms. If left untreated, bladder stones may lead to infections and other complications such as hepatic abscess via a hematogenous route. CASE: This is a case of a seventy-eight-year-old man with a history of multiple blood transfusions secondary to anemia of unknown cause. He came into our institution for a second opinion. We worked up the patient, which showed hepatic abscess and two large urinary bladder calculi. Further investigation of the anemia later led to a diagnosis of primary myelofibrosis. DIAGNOSTICS: Ultrasound showed a complex mass on the left hepatic lobe measuring 7.5 cm x 6.0 cm x 2.1 cm consistent with a hepatic abscess. The culture of the abscess was positive for E. coli. Computed Tomography (CT) scan of the lower abdomen showed heterogeneous mass measuring 8.6 cm x 8.7 cm x 9.2 cm within the urinary bladder (see Figure 2). Urinalysis was consistent with a urinary tract infection. Urine culture showed E. coli. Video-assisted cystoscopy showed two urinary bladder calculi, measuring 1.5 cm x 3.2 cm x 4.2 cm weighing 30 grams each (see Figure 3). The calculi were composed of 100% Calcium Oxalate. He underwent a series of diagnostic examinations for anemia including gastroscopy to rule out a bleeding ulcer. Complete blood count showed hemoglobin of 77 g/L and a hematocrit of 0.23. Finally, bone marrow core biopsy was done which is consistent with primary myelofibrosis. CONCLUSION: Urinary bladder stones can be asymptomatic and may present only with vague abdominal pain. It should be one of the considerations in asymptomatic patients with long-standing prostatitis or benign prostatic hyperplasia. Detailed history, thorough physical examinations, and cautious diagnostic tests are mandatory to confirm the diagnosis. A hepatic abscess may arise from infections in the urinary tract such as prostatitis through hematogenous extension. Therefore, it is important to address the origin of the infection to prevent such complications. This is a rare case of an elderly man who presented with chronic anemia and later found out to have large urinary bladder stones that caused severe infection leading to immune stimulation of the bone marrow, hence the diagnosis of primary myelofibrosis by bone marrow biopsy. Such a rare case must be thought of holistically and analytically.

Introducción: La fibrosis quística es una enfermedad genética que constituye causa frecuente de neumopatía crónica grave en adultos jóvenes. Se asocia a cirrosis de vías biliares y cálculos vesiculares, por lo que el anestesiólogo debe estar familiarizado con las características clínicas de la enfermedad a fin de disminuir el índice de complicaciones perioperatorias que pueden presentarse. Objetivo: Presentar las características clínicas y la conducta perioperatoria en una paciente con diagnóstico de fibrosis quística anunciada para colecistectomía videolaparoscópica. Caso clínico: Paciente femenina de 19 años de edad, raza negra, con múltiples infecciones respiratorias durante la infancia, que se diagnosticó como caso nuevo de fibrosis quística y durante los estudios se observó litiasis vesicular sintomática por lo que se anunció para colecistectomía videolaparoscópica. Se describe la evaluación y preparación preoperatoria, así como la conducta anestésica intraoperatoria. Conclusiones: Los resultados anestésicos dependen de la atención a una enfermedad compleja que afecta a múltiples órganos y el control estrecho y el tratamiento oportuno de la enfermedad pulmonar previa(AU)


Introduction: Cystic fibrosis is a genetic disease that is a frequent cause of severe chronic lung disease in young adults. It is associated with biliary tract cirrhosis and gallstones, a reason why the anesthesiologist must be familiar with the clinical characteristics of the disease in order to reduce the rate of perioperative complications that may occur. Objective: To present the clinical characteristics and perioperative behavior in a patient diagnosed with cystic fibrosis announced for videolaparoscopic cholecystectomy. Clinical case: Female patient of 19 years of age, black race, with multiple respiratory infections during childhood, who was diagnosed as a new case of cystic fibrosis. During the studies, symptomatic vesicular lithiasis was observed, that's why the patient what was announced for videolaparoscopic cholecystectomy. The evaluation and preoperative preparation, as well as the intraoperative anesthetic behavior are described. Conclusions: Anesthetic outcomes depend on the attention to a complex disease that affects multiple organs and the close control and timely treatment of previous lung disease(AU)

RESUMEN Introducción: La cirugía de mínimo acceso en general y en particular la abdominal laparoscópica ha revolucionado la cirugía ofreciendo mejoría en la calidad de vida de los pacientes lo cual se hace más evidente en los adultos mayores. Objetivo: Describir la efectividad y seguridad de la cirugía abdominal laparoscópica en el adulto mayor. Métodos: Se realizó una investigación descriptiva en una serie consecutiva de 204 adultos mayores con indicación de cirugía abdominal laparoscópica en el Centro Nacional de Cirugía de Mínimo Acceso en el primer semestre del año 2017. Resultados: La edad mediana fue de 68,0 (60-90) años, predominó el sexo femenino (71,6 por ciento). La comorbilidad en 180 pacientes (88,2 por ciento). La litiasis vesicular (43,2 por ciento), hernia inguinal (14,1 por ciento) y afecciones colorrectales (12,2 por ciento) fueron los diagnósticos más frecuentes mientras que la colecistectomía laparoscópica (44,1 por ciento), hernioplastia inguinal (14,1 por ciento) y la cirugía colorrectal (12,2 por ciento) las técnicas más realizadas. La mediana del tiempo quirúrgico fue 60,0 (15-360) minutos. Hubo una tasa de complicaciones de 6,9 por ciento, de conversión de 3,4 por ciento, reintervención de 2,0 por ciento y mortalidad de 0,5 por ciento. La mediana de la estadía hospitalaria fue de 1,0 (1-52) días. El antecedente de cirugía por cáncer se asoció con la presencia de complicaciones, p=0,000. Conclusiones: La cirugía abdominal laparoscópica constituye una opción efectiva y segura para el adulto mayor(AU)


ABSTRACT Introduction: Minimal access surgery in general, and particularly laparoscopic abdominal surgery, have revolutionized surgery that offers improvement in the quality of life of patients, which is more evident in older adults. Objective: To describe the effectiveness and safety of laparoscopic abdominal surgery in the aged adult. Methods: A descriptive investigation was carried out in a consecutive series of 204 aged adults with indication for laparoscopic abdominal surgery, at the National Center for Minimal Access Surgery, in the first semester of 2017. Results: The median age was 68.0 (60-90) years. The female sex predominated (71.6 percent). Comorbidity in 180 patients (88.2 percent). Vesicular lithiasis (43.2 percent), inguinal hernia (14.1 percent) and colorectal disorders (12.2 percent) were the most frequent diagnoses, while laparoscopic cholecystectomy (44.1 percent), inguinal hernioplasty (14.1 percent) and colorectal surgery (12.2 percent) were the most performed techniques. The median surgical time was 60.0 (15-360) minutes. There was a complication rate of 6.9 percent, a conversion rate of 3.4 percent, reintervention of 2.0 percent and mortality of 0.5 percent. The median of the hospital stay was 1.0 (1-52) days. The history of surgery for cancer was associated with the presence of complications, p=0.000. Conclusions: Laparoscopic abdominal surgery is an effective and safe option for the aged adult(AU)

BACKGROUND: This study compared the following three endoscopic techniques used to treat bladder stones: transurethral cystoscope used with a pneumatic lithoclast or nephroscope used with a pneumatic lithoclast and nephroscope used with an ultrasonic lithoclast.METHODS: Between January 2013 and May 2016, 107 patients with bladder stones underwent endoscopic treatment. Patients were classified into three groups based on the endoscopic techniques and energy modalities used in each group as: group 1 (transurethral stone removal using a cystoscope with pneumatic lithoclast), group 2 (transurethral stone removal using a nephroscope with pneumatic lithoclast), and group 3 (transurethral stone removal using a nephroscope with ultrasonic lithoclast). Baseline and perioperative data were retrospectively com-pared between three groups.RESULTS: No statistically significant intergroup differences were observed in age, sex ratio, and stone size. A statistically significant intergroup difference was observed in the operation time—group 1, 71.3±46.6 min; group 2, 33.0±13.7 min; and group 3, 24.6±8.0 min. All patients showed complete stone clearance. The number of urethral entries was higher in group 1 than in the other groups. Significant complications did not occur in any patient.CONCLUSION: Nephroscopy scores over cystoscopy for the removal of bladder stones with respect to operation time. Ultrasonic lithoclast is a safe and efficacious modality that scores over a pneumatic lithoclast with respect to the operation time.

Antecedentes: actualmente un punto discutido de la colecistectomía laparoscópica (CL) es la realización sistemática de la colangiografía intraoperatoria (CIO); sin embargo, esta permite el diagnóstico de litiasis coledociana insospechada (LCI). Objetivo: establecer el porcentaje de CIO realizadas, el número de LCI diagnosticadas, describir qué terapéutica se utilizó para resolverlas y establecer si existe relación entre el tamaño de las litiasis diagnosticadas y su tratamiento transcístico. Resultados: de las 1077 CL electivas, la CIO pudo realizarse en el 89,14% de los pacientes. En 2014, el porcentaje de CIO fue el más alto de la serie (95,38%). Se encontraron 38 LCI. El tratamiento realizado incluyó el abordaje transcístico y la colangiopancreatografía retrógrada endoscópica (CPRE) intraoperatoria. La morbilidad global fue del 7,9 % sin mortalidad. Discusión: nuestro porcentaje de CIO se encuentra por debajo del enunciado en otras publicaciones, pero el porcentaje aumentó con los años. La incidencia de LCI en nuestro caso fue del 3,96%. En nuestro servicio primeramente se intenta la resolución transcística (tasa de éxito del 77,42% sin complicaciones). Otra opción es la CPRE intraoperatoria, que se utilizó en 4 casos con una eficacia del 100% sin complicaciones. Conclusión: el tratamiento de la LCI continúa siendo un reto para los cirujanos, debido sobre todo a la imprevisibilidad del cuadro; resulta un factor muy importante para la resolución transcística el tamaño de la litiasis encontrada (más o menos de 6 mm). Consideramos la CPRE intraoperatoria como una herramienta importante en la resolución de esta patología.


Background: currently a discussed point of laparoscopic cholecystectomy (LC) is the systematic implementation of intraoperative cholangiography (IOC); however, it allows the diagnosis of unsuspected common bile duct stones (UBDS). Objective: to establish the percentage of IOC performed, the number of UBDS diagnosed, to describe what therapeutic was used to solve them and to establish if there is a relationship between the size of the diagnostic lithiasis and the transcystic treatment of the same. Results: of the 1077 elective LC, IOC could be performed in 89.14% of patients. In 2014 the percentage of IOC was the highest in the series (95.38%). 38 UBDS were found. The treatment included the transcritical approach and intraoperative ERCP. Overall morbidity was 7.9% without mortality. Discussion: our IOC percentage is below the utterance in other publications, but the percentage has increased over the years. The incidence of UBDS in our case was 3.96%. In our service we first try the transcritical resolution (success rate of 77.42% without complications). Another option is intraoperative ERCP that was used in 4 cases with 100% efficacy without complications. Conclusion: the treatment of the UBDS continues being a challenge for the surgeons, mainly due to the unpredictability of the picture; a very important factor for transcystic resolution is the size of the stone found (more or less than 6 mm). We consider intraoperative ERCP as an important tool in the resolution of this pathology.

Introducción: Evidencias científicas avalan que niveles elevados de ácidos biliares en la luz intestinal se asocian con lesiones inflamatorias, premalignas y malignas de colon. Objetivo: Argumentar científicamente la condición de niveles elevados de ácidos biliares totales en heces, litiasis vesicular y colecistectomizado como factores de riesgo para lesiones de colon. Método: Se realizó una investigación analítica en el Instituto de Gastroenterología y el Centro Nacional de Cirugía de Mínimo Acceso desde 2002 hasta 2015. En el estudio se incluyeron adultos con sintomas digestivos bajos. Se desarrollaron dos momentos: la determinación del riesgo relativo de la presencia de lesiones colónicas diagnosticadas por endoscopía y la identificación por histología. Se utilizaron métodos estadísticos como la prueba chi cuadrado de Pearson, determinación del riesgo relativo, regresión logística, entre otros. Resultados: Se observó mayor probabilidad de presentar algún diagnóstico endoscópico e histológico positivo para todas las edades y sexo, en pacientes con niveles elevados de ácidos biliares totales en heces y los colecistectomizados. Los riesgos relativos comunes y más altos se relacionaron con adenomas tubulares de alto grado de displasia y tubulovellosos. Los diagnósticos más frecuentes fueron pólipos y adenomas. Conclusiones: Los resultados avalan que los niveles elevados de ácidos biliares totales en heces, la litiasis vesicular y estar colecistectomizado, constituyen factores de riesgo para lesiones de colon. Se recomienda incluir estas tres condiciones de riesgo en los programas de pesquisa para el cáncer colorrectal(AU)


Introduction: Scientific evidence supports that elevated levels of bile acids in the intestinal lumen are associated with inflammatory, premalignant and malignant lesions of the colon. Objective: To scientifically support the condition of elevated levels of total bile acids in faeces, vesicular and cholecystectomized lithiasis as risk factors for colonic lesions. Method: An analytical research was carried out in the Gastroenterology Institute and the National Center for Minimally-Invasive Surgery, from 2002 to 2015. The study included adults with low digestive symptoms. Two moments were developed: the determination of the relative risk for the presence of colonic lesions diagnosed by endoscopy and the identification by histologic study. Statistical methods were used, such as the Pearson chi-square test, the relative risk determination, logistic regression, among others. Results: Higher probability was observed for all ages and sexes to present a positive endoscopic and histological diagnosis, in patients with high levels of total bile acids in feces and in cholecystectomized patients. The common and higher relative risks were related to tubular adenomas of high degree of dysplasia and tubulovillous. The most frequent diagnoses were polyps and adenomas. Conclusions: The results support that high levels of total bile acids in faeces, vesicular lithiasis and being cholecystectomized constitute risk factors for colonic lesions. These three risk conditions are recommended to be included in screening programs for colorectal cancer(AU)

Choledocholithiasis is one of the more common benign disorders of the biliary tract with multiple features of presentation and several alternatives for its diagnosis and treatment. Our aim was to perform a based-evidence revision to propose a diagnostic and therapeutic algorithm. The raised values of gamma glutamiltranspeptidase, alkaline phosphatase and total bilirubin, are well predictors for a choledocholithiasis. The image evidence for a pre-operative detection with higher sensibility, specificity and better cost-effectiveness is the cholangioresonance. For its intraoperative detection, the cholangiography is the method most frequently used, though cholangioscopy is likewise useful. In the case of a post-operative suspicious, the cholangiography through the T tube is the gold standard. With regard to the treatment of the choledocholithiasis, the different stages are analyzed. depending if the detection was performed pre, intra or postoperatively. As a conclusion, the approach of the choledocholithiasis in one step seems to be better that to perform it in two steps, being the laparoscopic exploration for bile ducts stones more safety than the use of the intraoperative ERCP (endoscopic retrograde cholangiopancreatography) .The postoperative ERCP is not recommended excepting in very selected cases, and the biliodigestive derivations should be reserved only for the primary lithiasis of the common bile duc

Introducción: las vías clínicas son una de las principales herramientas de la gestión de la calidad asistencial para la estandarización de los procesos asistenciales. Se ha demostrado que su implantación permite disminuir la variabilidad de la práctica clínica. Objetivos: Elaborar una vía clínica para el diagnóstico, tratamiento y seguimiento de enfermos con enfermedades benignas de la vesícula biliar operados por mínimo acceso. Método: se realizó un estudio cuasi experimental en el servicio de cirugía del Hospital Provincial Saturnino Lora de Santiago de Cuba, desde enero del año 2011 hasta septiembre del año 2014 con una muestra de 1287 pacientes operados. Se diseñó la vía clínica mediante el consenso de expertos y se dividió la muestra en dos grupos para llevar a cabo el cuasi-experimento. Resultados: en el grupo B, 97,3 por ciento de los pacientes fueron egresados en 24 h o menos luego de ser operados; 93,9 pr ciento de los pacientes del grupo B fueron operados en 60 minutos o menos. Se observó una disminución significativa del tiempo quirúrgico en el grupo B al compararse ambos grupos (p= 0,004). Hubo una disminución estadísticamente significativa (p= 0,047) de las lesiones de la vía biliar principal en el grupo B. Conclusiones: la aplicación de la vía clínica disminuyó de forma significativa la estadía hospitalaria, los tiempos quirúrgicos y las lesiones iatrogénicas de la vía biliar(AU)


Introduction: The clinical pathways are one of the main tools for the management of the quality of care in the standardization of the caring processes. its implantation has been shown to reduce the variability of clinical practice. Objectives: To elaborate a clinical pathway for the diagnosis, treatment and follow-up of patients with benign diseases of the gall bladder and operated by minimal access. Method: A quasi-experimental study was carried out in the surgery service of Saturnino Lora Provincial Hospital in Santiago de Cuba, from January 2011 to September 2014, with a sample of 1287 operated patients. The clinical pathway was designed upon expert consensus and the sample was divided into two groups, in order to carry out the quasi-experiment. Results: In group B, 97.3 percent of the patients were discharged after 24 hours or less of being operated; 93.9 percent of patients in group B were operated in 60 minutes or less. A significant decrease in surgical time was observed in group B, when both groups were compared (p= 0.004). The group B had a statistically significant decrease (p= 0.047) of the lesions of the main bile duct in. Conclusions: The application of the clinical pathway significantly reduced hospital stay, surgical times and iatrogenic lesions of the bile duct(AU)

La micción es un proceso complejo, que requiere la coordinación entre el sistema nervioso central y periférico. La alteración en ése, aumenta el riesgo para que se produzcan infecciones complicadas y a largo plazo, daño renal. La alteración en el vaciado de la vejiga, obliga a que muchos de esos pacientes, realicen cateterismos intermitentes o sonda vesical permanente, aumentando el riesgo de infecciones polimicrobianas o por gérmenes multiresistentes. Algunos factores implicados en el desarrollo de las infecciones de esos pacientes, son el residuo postmiccional elevado, estasis urinario, litiasis vesical, uso de catéteres, además de las alteraciones en el sistema inmune y las capas de recubrimiento en la mucosa vesical. El diagnóstico de infección se realiza al encontrar: piuria y bacteriuria, según método de vaciado vesical, y un síntoma general que sugiera infección. Conclusión: Las infecciones urinarias en pacientes con lesión medular, deben ser tratadas de acuerdo a sensibilidades de la zona, siempre con la toma previa de un urocultivo y con un diagnóstico adecuado de infección urinaria, teniendo en cuenta las diferentes maniobras de evacuación de la vejiga. No están recomendados los tratamientos cortos ni el tratamiento de las bacteriurias asintomáticas. Una de las formas de prevenir las infecciones, está en evitar situaciones de riesgo como el estasis vesical, las presiones intravesicales elevadas y los vaciamientos incompletos. En la actualidad existen múltiples medicamentos para prevenir las infecciones urinarias, pero faltan estudios con evidencia de más peso y en pacientes con lesión medular, para que puedan ser recomendados.


Urination is a complex process, requiring coordination between the central and peripheral nervous system. The alteration in this, increases the risk for complicated infections and long-term kidney damage. The alteration in the emptying of the bladder, causes many of these patients to perform intermittent catheterization or permanent bladder catheterization, increasing the risk of polymicrobial infections or multiresistant germs. Some factors involved in the development of infections of these patients are high postvoiding, urinary stasis, vesical lithiasis, use of catheters, as well as alterations in the immune system and the layers of lining in the bladder mucosa. The diagnosis of infection is made on finding: pyuria and bacteriuria, according to the method of bladder emptying, and a general symptom that suggests infection. Conclusion: Urinary tract infections in patients with spinal cord injury should be treated according to the sensitivity of the area, always with prior urine collection and an adequate diagnosis of urinary tract infection, taking into account the different maneuvers of bladder evacuation. Short treatments and treatment of asymptomatic bacteriuria are not recommended. One way to prevent infections is to avoid risky situations such as bladder stasis, elevated intravesical pressures and incomplete emptying. At the moment there are multiple drugs to prevent urinary tract infections, but there are no studies with evidence of heaviest weight and in patients with spinal cord injury, so that they can be recommended.

PURPOSE: Augmentation cystoplasty (AC) is a surgical procedure used in adults and children with refractory bladder dysfunction, including a small bladder capacity and inadequate bladder compliance, and in whom conservative and medical treatment has failed. This study was aimed to determine the long-term outcomes of AC in children. METHODS: A retrospective analysis was conducted of 42 patients (31 males; mean age, 14.2±6.2 years) who underwent AC for neurogenic and nonneurogenic bladder dysfunction, with a median 12.0±1.5 years of follow-up. All patients underwent AC using the ileum with or without continent reconstruction. Pre-AC, concurrent, and post-AC procedures and complications were analyzed. Patients who underwent ureterocystoplasty, were lost to follow-up, or had less than 10 years of follow-up were excluded. The primary outcomes were the complication and continence rates, the post-AC linear rate of height and weight gain, and renal function. The Student t-test was used to evaluate between-group differences and the paired t-test was used to evaluate longitudinal changes in measured variables. RESULTS: Renal function was stable or improved in 32 of 42 patients (76.2%), with a post-AC continence rate of 88.1%. Thirty patients (71.4%) required 72 procedures post-AC. There was no statistically significant difference in the mean percentile of height (P=0. 212) or weight (P=0.142) of patients in the pre- and post-AC periods. No cases of bladder perforation or malignancy were detected. CONCLUSIONS: We consider AC to be a safe and effective procedure that does not negatively affect future physical growth, while achieving a good rate of stable renal function. Patients need long-term follow-up to address long-term complications.

The risk of bladder mucosal necrosis and substantial decreases in volume after implantation were known as major complication of subureteral injection therapy. RESUMEN

Situs inversus totalis es una anomalía congénita poco frecuente. Se caracteriza por la transposición de los órganos tóraco-abdominales en sentido sagital al brindar una imagen llamada en espejo. Reportamos paciente de 59 años con diagnóstico de litiasis vesicular y situs inversus totalis resuelto por colecistectomía videolaparoscópica, en el Hospital Universitario Comandante Manuel Fajardo de La Habana. El cuadro clínico tuvo seis meses de evolución, con dolor a tipo cólico en hipocondrio izquierdo acompañado de náuseas, que se relacionaban con la ingestión de alimentos colecistoquinéticos. Los exámenes de laboratorio estuvieron dentro de parámetros normales. La ecografía abdominal informó litiasis vesicular, así como, el situs inversus totalis que se confirmó con Rayos X de tórax. Su evolución posoperatoria, luego de un año, fue satisfactoria, la cual mostró la factibilidad de la técnica quirúrgica empleada(AU)


Situs inversus totalis (SIT) is a rare congenital anomaly characterized by the transposition of the thoracic-abdominal organs towards the sagittal direction, producing an image called in mirror. The case is presented of a 59-year-old female patient, with a diagnosis of vesicular lithiasis and situs inversus totalis for cholelithiasis, treated by laparoscopic cholecystectomy in Comandante Manuel Fajardo University Hospital of Havana. The patient's clinical picture had a natural history of 6 months, with left colic pain in the left hipocondrium, with nauseas, accentuated on ingestion of greasy meals. The laboratory test results showed normal parameters. X-rays of the abdomen confirmed vesicular cholestasis and situs inversus totalis. The patient's postoperative evolution after one year was satisfactory, showing the feasibility of the surgical technique used(AU)

Intrauterine contraceptive devices (IUCDs) are a common form of reversible contraception owing to fewer systemic side effects and low cost, especially in a developing country like India. However, IUCDs are not without complications. Migration of a device into adjacent organs is the most morbid of all the documented complications. A patient who presents with a history that suggests loss or disappearance of an IUCD thread associated with urinary symptoms should raise suspicions that a device may have migrated into the bladder. Physicians should also be aware of possible secondary vesical calculus formation. Further radiological investigations and appropriate management are warranted. We present a case report describing the migration of an IUCD into the bladder with secondary calculus formation.

OBJECTIVE: Holmium laser enucleation of prostate (HoLEP) is as fast-paced treatment that is one of the standard treatment for benign prostate hypoplasia. However, there is a rare case that the satisfaction rate of surgery is lowered due to storage symptoms such as frequency and urgency. We investigated the risk factors of bladder irritation symptoms persisted after holmium laser enucleation of prostate. METHODS: From January 2009 to December 2014, 220 patients underwent HoLEP in Soonchunhyang University Cheonan Hospital. Among them, 133 patients were selected who did not have any problem that could affect the voiding function including urologic cancer, neurogenic bladder, urinary tract infection, and bladder stone disease. At 3 months after surgery patients were divided into two groups: postoperative International Prostate Symptom Score (IPSS) storage symptom scores <8 (group I) and ≥8 (group II). The two groups were analyzed the parameters: preoperative (prostate size, uroflowmetry, IPSS, prostate calcification), intraoperative (operative time, resected prostate weight), and postoperative (IPSS, uroflowmetry). RESULTS: Of the 133 patients, 94 patients were assigned to group I and 39 patients were group II. Age of the patients, the prostate size, and maximal urinary flow rate showed no statistical difference in both groups. Preoperative storage symptom scores were significantly higher in group II (P<0.05) and IPSS voiding-to-storage subscore ratio was lower in group II without significance (P<0.05). The prostate calcification, periurethral prostatic calcification, and intravesical prostatic protrusion was no difference in the two groups. Postoperative IPSS, maximal urinary flow rate, and post-void residual urine volume was improved in two groups but storage symptom score was not improved in group II. Operative time and resected prostate weight were not found the differences, and postoperative parameters (maximal urinary flow rate, post-void residual urine volume) were showed no difference in both groups. However all of the IPSS was higher in group II. CONCLUSION: Many symptom improvements after HoLEP were observed but some patients may have difficulties due to persistent bladder irritation. Because bladder irritation sign could persist after surgery if preoperative storage symptom score was higher, it is necessary for a detailed description before surgery.
