RESUMEN

Background: Increasing chronic diseases challenges the health systems of low- and middleincome countries, including Cameroon. Type 1 diabetes (T1D), among the most common chronic diseases in children, poses particular care delivery challenges. Aim: We examined social representations of patients' roles and implementation of T1D care among political decision-makers, healthcare providers and patients within families. Setting: The study was conducted in Yaoundé, Cameroon. Methods: Eighty-two individuals were included in the study. The authors conducted semistructured interviews with policy makers (n = 5), healthcare professionals (n = 7) and patients 'parents (n = 20). Questionnaires were administered to paediatric patients with T1D (n = 50). The authors also observed care delivery at a referral hospital and at a T1D-focused nongovernmental organisation over 15 days. Data were analysed using thematic content analysis and descriptive statistics. Results: Cameroonian health policy portrays patients with T1D as passive recipients of care. While many practitioners recognised the complex social and economic determinants of adherence to T1D care, in practice interactions focused on specific biomedical issues and offered brief guidance. Cultural barriers and policy implementation challenges prevent patients and their families from being fully active participants in care. Parents and children prefer an ongoing relationship with a single clinician and interactions with other patients and families. Conclusion: Patients and families mobilise experience and lay knowledge to complement biomedical knowledge, but top-down policy and clinical practice limit their active engagement in T1D care. Contribution: Children with T1D and their families, policy makers, healthcare professionals, and civil society have new opportunities to contribute to person-centred care, as advocated by the Sustainable Development Goals.

Abstract Starch samples from eight legumes cultivars instar of one variety of Vigna unguiculata L. (Cowpea), one variety of Vigna subterrenea V. (Bambara groundnut) and six varieties of Phaseolus vulgaris L. (Common bean), grown in Cameroon were isolated, and their physicochemical and pasting properties were evaluated. The objectives of the study were to investigate the starch properties and processing characteristics of different bean varieties, and to establish the basic foundation of improving the functionality of beans and their starch grown in the region. The result revealed significant differences amongst the properties of the starches. The swelling power of the legume starch isolates put them in the category of highly restricted-swelling starch. This characteristic is desirable for the manufacture of value-added products such as noodles and composite blends with cereals. The pasting properties were determined using a rapid visco analyzer, and various legumes bean starches exhibited different pasting profiles. The high breakdown viscosity (BV) was founded for Cowpea and Bambara groundnut and confirmed their low. ability to resist heat and shear stress when compared to Common bean varieties studies. The factors which influence the pasting characteristics resulting to decrease in peak viscosity (PV), trough viscosity (TV) and final viscosity (FV) of starch are attributed to the interaction of starch with the protein, fat, etc. which depended to their variety.


Resumo Foram isoladas amostras de amido de oito cultivares de leguminosas ínstar de uma variedade de Vigna unguiculata L. (feijão caupi), uma variedade de Vigna subterrenea V. (amendoim Bambara) e seis variedades de Phaseolus vulgaris L. (feijão comum), cultivadas nos Camarões, e suas propriedades físico-químicas e de pasta foram avaliadas. Os objetivos do estudo foram investigar as propriedades do amido e as características de processamento de diferentes variedades de feijão e estabelecer as bases básicas para melhorar a funcionalidade do feijão e do amido cultivado na região. O resultado revelou diferenças significativas entre as propriedades dos amidos. O poder de inchamento dos isolados de amido de leguminosas os coloca na categoria de amido com inchamento altamente restrito. Esta característica é desejável para o fabrico de produtos de valor acrescentado, tais como macarrão e misturas compósitas com cereais. As propriedades de pasta foram determinadas usando um analisador rápido de visco, e vários amidos de feijão leguminosos exibiram diferentes perfis de pasta. A alta BV foi fundada para o amendoim Cowpea e Bambara e confirmou sua baixa. capacidade de resistir ao calor e tensão de cisalhamento quando comparado com estudos de variedades de feijão. Os fatores que influenciam as características colantes resultantes da diminuição do pico de viscosidade (PV), da viscosidade mínima (TV) e da viscosidade final (FV) do amido são atribuídos à interação do amido com a proteína, gordura, etc., que dependem da sua variedade.

RESUMEN

Introduction. Nous décrivons un cas d'agénésies dentaire multiples associées à une ectopie bilatérale des troisièmes molaires ainsi de la deuxième prémolaire mandibulaire gauche. Cas clinique: Une patiente de 30ans était référée par son médecin gastroentérologue pour une prise en charge odontologique. La patiente décrit de ne jamais avoir bénéficié d'avulsion dentaire et ne souffrait d'aucun douleur ou gène au niveau orofacial. Une exploration clinique suivie d'examens radiologiques était réalisé. L'examen clinique révèle l'absence de plusieurs dents. La radiographie panoramique avait permis de confirmer le diagnostic d'agénésies multiples de huit dents. Les deux troisièmes molaires mandibulaires incluses en position verticale au niveau de la branche mandibulaire. La 35 aussi incluse en position apicale de la 38. L'examen CBCT montre une déhiscence des corticales des faces interne et externe du ramus en regarde des couronnes des troisièmes molaires. Discussion. L'oligodontie associée à une inclusion dentaire multiple reste un cas rare. Les éléments de l'examen clinique associés à la radiographie panoramique permettent de confirmer le diagnostic d'agénésie et d'ectopie dentaires. La radiologie tri dimensionnelle est capitale dans la prise de décision thérapeutique des dents ectopiques en position condy lienne.Conclusion. Sans symptomatologie clinique et radiologique, la décision thérapeutique était de s'abstenir de traitement chirurgical et de surveiller

Objectives:The aim of the study was to determine the prevalence and factors associated to poor adherence to antiepileptic drugs in a referral hospital of Douala, Cameroon.Method:This was a cross-sectional and descriptive study of antiepileptic drugs (AED) adherence carried out in a group of selected patients with epilepsy. Patients were recruited from January to June 2018 at the Neurology Department of the Douala Laquintinie Hospital. Non-adherence to antiepileptic drugs was measured by the 8-item Morisky Medication Adherence Scale and logistic regression was used to look for significant associations.Results:Participants (n=102) were of mean age 28.39 ± 19.74 years-old with 55% of male sex. Non-adherence was observed in 84 patients giving a prevalence rate of 82.35%. The factors associated to poor adherence to AEDs were forgetfulness (p<0.0001), lack of financial resources (p<0.0001), not having medication on hand, shortage of drugs at the pharmacy (p<0.0001) and lack of information on the disease (p<0.0001).Conclusion:Non-adherence to AEDs is common in Cameroon. Targeted management programs and communication strategies as well as health workers training are necessary to improve adherence to AED treatment in patients with epilepsy and avoid consequences such as seizure recurrence, status epilepticus and poor quality of life

Introduction : La déshydratation sub-clinique se définit par une perte de 1% à 3% du poids corporel en eau. Elle touche un à deux tiers des élèves en Europe. Peu de données sont disponibles dans notre contexte (climat tropical), justifiant le présent travail. Objectif : Evaluer l'état d'hydratation d'enfants scolarisés au cours de la saison sèche à Yaoundé.Patients et Méthodes : Nous avons mené une étude transversale en février 2017, sur une population d'enfants âgés de 7 à11 ans. L'état d'hydratation était évalué en début et fin de journée par bioimpédancemétrie et analyse d'urine. Par ailleurs les données sur les habitudes d'hydratation ont été recueillies grâce à un questionnaire.Résultats : Nous avons inclus 167 enfants dont 55,1% de filles. Les enfants déclaraient des 86,2% des cas boire de l'eau mais de manière spontanée dans 67,7% de cas. La consommation moyenne d'eau par les enfants au cours de 8 heures passées à l'école était de 527 ± 301 ml. A la bioimpédancemétrie, la masse hydrique était faible chez 52,3% d'élèves. La densité urinaire quant à elle était élevée chez 76% des élèves à l'arrivée à l'école et cette proportion atteignait 81,4% en fin de journée.Conclusion : La consommation d'eau semble faible dans notre population. Plus de la moitié de notre population d'étude est en déshydratation sub-clinique à l'arrivée à l'école. Cette proportion varie selon la méthode utilisée

RESUMEN

Le personnel soignant en première ligne dans la riposte contre l'infection à COVID-19 et les familles des défunts au COVID-19 vivent pour certains des troubles anxieux, des burn-outs et des troubles dépressifs caractérisés, suite à la perte soit d'un patient soit un proche. Ces troubles sont relevés dans la littérature mondiale. Le sujet africain vit dans un environnement culturel plus communautaire qu'individualiste. Il peut donc souffrir davantage lorsqu'il est privé de ses commémorations culturelles au cours de la mise en terre des morts, dans un contexte de mesures barrières physiques et communautaires des gouvernements. Notre cas clinique se propose de montrer une autre affection « psychiatrique » étiquetée de deuil pathologique, qui pourrait se développer dans notre continent, contrairement à d'autres dans les mois avenir. Nous montrerons ainsi l'intérêt de l'accompagnement psychologique à long terme dans les centres de crise

RESUMEN

Introduction: ectopic pregnancy (EP) is the leading cause of maternal mortality in the first trimester of pregnancy in our environment. This study aimed at evaluating the incidence, risk factors, clinical presentation and treatment of ectopic pregnancy in the Limbe and Buea Regional Hospitals in Cameroon. Methods: this was a retrospective nested case control study carried out from December 2006 to December 2016. A ratio for control vs cases of 3:1 was obtained. Any pregnancy implanted outside the normal uterine cavity was considered as an ectopic pregnancy. Student's t-test was used to compare continuous variables and Pearson's Chi-square test for categorical variables. The association between EP and the demographical and clinical variables was estimated using logistic regression. Statistical significance was set at p-values<0.05.Results: a total of 247 cases of EP were registered out of 17221 deliveries giving an incidence of 1.43% in ten years. History of pelvic inflammatory disease (OR = 3.10, CI (1.76-5.44), p < 0.001), previous EP (OR = 10.22, CI (2.61-14.82), p < 0.001), History of induced abortion (OR = 2.68, CI (3.32-9.73), p< 0.001), history of adnexa surgery (OR = 4.37, CI (2.17-10.32), p < 0.001) and history of appendectomy (OR = 2.16, CI (0.99-6.64) p< 0.001), were also found to be associated with increased risk of EP. More than five percent (5.52%) of the patients were in shock at presentation. Diagnosis was confirmed mainly by use of ultrasound (78.53%) and treatment was principally by laparotomy (97.55%) with salpingectomy (95.60%). Most (90.18%) of ectopic pregnancies were ruptured at presentation. Only 2.45% of cases were manage medically with the use of methotrexate. Conclusion: the incidence of ectopic pregnancy (EP) in our environment is within the global range (hospital-based incidence of 1.43%) and is rising. Late presentation, lack of modern diagnostic and management tools have made laparotomy with salpingectomy the principal method of management of ectopic pregnancy in our environment
