RESUMEN

Os autores relatam o caso de uma paciente de 46 anos de idade, que, após ter sido submetida a tratamento radioterápico por neoplasia de colo uterino, desenvolveu cistite actínica com episódios frequentes de hematúria franca. A paciente necessitou ser submetida a repetidos cateterismos vesicais por retenção urinária, hemotransfusões e internacões hospitalares. As medidas conservadoras e as tentativas de hemostasia por cistoscopia não foram bem-sucedidas no controle do sangramento. A paciente foi então submetida a tratamento endovascular com embolização superseletiva das artérias vesicais e outros pedículos vasculares, que se demonstraram associados ao sangramento. O procedimento foi bem-sucedido e a paciente vem sendo acompanhada há nove meses sem a necessidade de novas hemotransfusões nem de novas internações hospitalares. De acordo com a revisão da literatura, o uso dessa técnica ainda não havia sido descrito em trabalhos brasileiros.


This article describes the case of a 46-year-old female patient who had been treated with radiotherapy for cervical cancer. She developed actinic cystitis with frequent episodes of severe hematuria. She required repeated catheterization to manage urinary retention, blood transfusions and hospital admissions. Conservative measures and attempts to achieve hemostasis by cystoscopy were unsuccessful at controlling bleeding. The patient therefore underwent endovascular treatment with superselective embolization of the vesical arteries and other vascular pedicles found to be linked with the bleeding. The procedure was successful and the patient has been in follow-up for 9 months with no need for further blood transfusions or admission to hospital. According to a review of the literature, use of this technique has not previously been described in Brazil.

The aim of this study was to assess histological alterations and perform immunolabeling of Leishmania infantum in the kidneys and urinary bladder of naturally infected dogs. Twenty-five urinary bladder and kidney samples of serologically positive animals (ELISA S7® Biogene and IFAT ≥ 1:40 - Biomanguinhos/Fiocruz) were analyzed by means of immunohistochemical and histological techniques. Cystitis was found in 44% (11/25) of the bladder samples and membranoproliferative glomerulonephritis in 92% (23/25) of the kidney samples. Immunolabeling of the parasite revealed that 32% (8/25) of the bladders and 8% (2/25) of the kidneys were positive. In conclusion, the immunohistochemical technique is a useful tool for detecting amastigote forms of L. infantum in organs of infected dogs. In addition, this was the first report of detection of amastigote forms ofL. infantum in the bladders of dogs.


Objetivou-se neste estudo avaliar as alterações histológicas e realizar a imunomarcação de Leishmania infantum em rins e bexiga de cães naturalmente infectados. Vinte e cinco amostras de bexiga e rins de animais sorologicamente (ELISA S7® Biogene and IFAT ≥ 1:40 - Biomanguinhos/Fiocruz) positivas foram analisadas histologicamente e por meio da técnica de imuno-histoquímica. Os resultados revelaram cistite em 44% (11/25) das amostras de bexiga e glomerulonefrite membranoproliferativa em 92% (23/25) das amostras de rins. A imunomarcação do parasito revelou 32% (8/25) e 8% (2/25) de positividade em bexiga e rins, respectivamente. Conclui-se que a técnica de imunohistoquímica é uma útil ferramenta para detecção de formas amastigotas de L. infantum em órgãos de cães infectados. Além disso, o presente trabalho reporta a primeira descrição de formas amastigotas de L. infantum em bexiga de cães.

RESUMEN

A bladder infection of Aspergillus with no evidence of dissemination is rare. We present a case of Aspergillus infection with transitional cell carcinoma of the urinary bladder without any evidence of systemic involvement. A 65-year-old male diabetic whose main complaints were intermittent painful haematuria and nocturia had undergone nephroureterectomy a year and a half back for transitional cell carcinoma of right renal pelvis. Cystoscopy revealed bladder mucosa having fixed broad tumour with encrustation and bleeding on touch at the right vesico-ureteric junction. The histopathologic diagnosis was a high-grade transitional carcinoma with Aspergillus infection. Fungal culture of urine obtained after bladder wash yielded Aspergillus fumigatus.

PURPOSE: Cyclophosphamide (CYP) is an antineoplastic agent used for the treatment of many neoplastic and inflammatory diseases. Hemorrhagic cystitis is a frequent side effect of CYP. Several studies show that simvastatin has important pleiotropic (anti-inflammatory and immunomodulatory) effects. The purpose of the study was to investigate the effect of simvastatin on bladder, ureter and kidney injury caused by CYP. METHODS: Adult male Wistar rats were randomly divided into three groups. The CYP/SIM group received simvastatin microemulsion by gavage during 7 days (10 mg/kg body wt) before the administration of CYP and the CYP/SAL group rats received saline 0.9 percent. The control rats were not treated. After that, all rats were treated with a single dose of CYP 200 mg/kg body wt intraperitoneally. The rats were killed 24 h after CYP administration. Plasma cytokines (TNF-α, IL-1β, IL-6) were measured by ELISA. Macro and light microscopic study was performed in the bladder, kidney and ureter. RESULTS: In the bladders of CYP/SIMV treated rats edema of lamina propria with epithelial and sub-epithelial hemorrhage were lower than in CYP/SAL treated rats. The scores for macroscopic and microscopic evaluation of bladder and ureter were significantly lower in CYP/SIMV rats than in CYP/SAL rats. The kidney was not affected. The expression of TNF-α, IL-1β and IL-6 was significatly lower in CF/SINV rats (164.8±22, 44.8±8 and 52.4±13) than in CF/SAL rats (378.5±66, 122.9±26 e 123.6±18), respectively. CONCLUSION: The results of the current study suggest that simvastatin pretreatment attenuated CYP-induced urotelium inflammation and decreased the activities of cytokines.


OBJETIVO: Ciclofosfamida (CF) é um agente antineoplásico frequente implicado na etiologia da cistite hemorrágica. Vários estudos mostram que a sinvastatina tem importantes efeitos pleiotrópicos (anti-inflamatórios e imunomoduladores). O objetivo do trabalho foi estudar os efeitos da sinvastatina na prevenção de cistite hemorrágica e lesões uroteliais induzidas por CF em modelo experimental. MÉTODOS: Doze ratos Wistar foram distribuídos de forma randomizada em três grupos: nos ratos do grupo experimental (CF/SINV), foi administrada microemulsão de sinvastatina 10mg/Kg, por via oral (gavagem), durante 7 dias antes da administração de CF e os ratos do grupo controle (CF/SAL), foram tratados com solução salina 0,9 por cento nas mesmas doses e prazos. O grupo controle não foi tratado. Todos os ratos foram tratados com CF 200mg/Kg intraperitonial (dose única) e 24 horas após foram sacrificados. Exame macro e microscópico foi feito na bexiga e os rins e ureteres foram avaliados microscopia. Foram realizadas dosagens plasmáticas de TNF-α, IL-1β, IL-6 (ELISA). RESULTADOS: O escore para avaliação macroscópica do dano à bexiga e o escore para avaliação do dano histológico na bexiga e nos ureteres mostraram-se significativamente menores no grupo CF/SINV em comparação ao grupo CF/SAL (p<0,05). Os rins não foram afetados. A expressão de TNF-α, IL-1β, IL-6 também foi significativamente menor (p<0,05) no grupo CF/SINV (164,8±22, 44,8±8 e 52,4±13) em comparação ao grupo CF/SAL (378,5±66, 122,9±26 e 123,6±18), respectivamente. CONCLUSÃO: O estudo demonstrou eficácia da sinvastatina na atenuação da cistite hemorrágica e lesão ureteral induzidas por CF em ratos Wistar, através da interferência nas citocinas plasmáticas e nas lesões uroteliais.

Introdução: Cistite glandular é um processo proliferativo benigno e infrequente da mucosa vesical, caracterizado por proliferação do epitélio e, em alguns casos, formação de glândulas intestinais. Alterações metaplásticas na cistite glandular são bem documentadas na literatura, embora sua etiologia não seja totalmente esclarecida. Relato do caso: Relatamos um caso de cistite glandular em um paciente de 55 anos, apresentando sintomas miccionais irritativos e obstrutivos persistentes sem resposta à terapia com alfabloqueadores. Ultrassonografia evidenciou lesão vegetante no trígono vesical e o paciente foi submetido à ressecção endoscópica por duas vezes e evoluiu com ureterohidronefrose bilateral. Dado o extenso acometimento vesical e a persistência dos sintomas, o paciente foi submetido a cistoprostatectomia e neobexiga ileal com boa evolução pós-operatória. Discussão: Há duas formas de cistite glandular: típica e intestinal. A forma típica é a mais comum e a intestinal é marcada pela produção de mucina, mais frequentemente associada ao adenocarcinoma de bexiga. A maioria dos casos de cistite glandular é assintomática, sendo que os pacientes sitomáticos normalmente apresentam hematúria, sintomas urinários irritativos e típicos de cistite crônica. Há controvérsias sobre o tratamento precoce agressivo, sendo que vários estudos propõem a ressecção transuretral e o acompanhamento com biópsias.


Introduction: glandular cystitis is a benign proliferative process and infrequent mucosal bladder, characterized by proliferation of the epithelium and in some cases, formation of intestinal glands. Metaplásticas changes in glandular cystitis are well documented in the literature, although its etiology is not fully understood. Case report: A case of glandular cystitis in one patient of 55 years, with irritative and obstructive urinary symptoms persisted without response to therapy with alpha blockers. Ultrasonography revealed a vegetative lesion in the trigonal and the patient underwent endoscopic resection and twice progressed to bilateral ureterohidronefrose. Given the extensive bladder involvement and persistence of symptoms, the patient underwent ileal neobladder cistoprostatectomia and with good postoperative evolution. Discussion: There are two types of glandular cystitis: typical and intestinal tract. The typical form is the most common and is characterized by intestinal mucin production, most often associated with adenocarcinoma of the bladder. Most cases of glandular cystitis is asymptomatic, and patients usually present sitomáticos hematuria, urinary symptoms and typical of chronic cystitis. There is controversy over early aggressive treatment, and several studies suggest transurethral resection and follow-up biopsies.

OBJECTIVE: To investigate the possible protective effect of recombinant human interleukin-11 (rhIL-11) against ifosfamide (IFS)-induced hemorrhagic cystitis (HC) MATERIALS AND METHODS: Male Swiss mice (20-30g) were pretreated with rhIL-11 (25-625 mg, subcutaneously.) 30 min before intraperitoneal injection of IFS (400 mg/kg) or with saline (control group). Twelve hours later, HC was evaluated by bladder wet weight (BWW) to quantify edema, Evans blue extravasation (EBE) to measure vascular permeability, and macroscopic and microscopic analysis. All bladders were assessed by histopathological analysis RESULTS: rhIL-11 (at 125 and 625 mg) attenuated the IFS- induced increase of BWW (37.48 percent and 45.44 percent, respectively, p < 0.05) and EBE (62.35 percent and 56.47 percent, respectively, p < 0.05). IFS- induced macroscopic edema and hemorrhage and microscopic alterations, were also prevented by rhIL-11 at 625 mg. (p < 0.05) CONCLUSION: Our results demonstrate a protective effect of rhIL-11 on experimental IFS- induced HC, not previously reported.

A cistite xantogranulomatosa (CX) é uma rara doença inflamatória crônica e benigna de etiologia desconhecida. É apresentado um caso de cistite xantogranulomatosa em um paciente masculino de 53 anos. A evolução do paciente incluiu estudos clínicos, radiológicos e histológicos. Não houve recorrência em 32 meses de seguimento após o tratamento. Este caso é provavelmente o 21o a ser relatado no mundo, e o primeiro caso na literaturabrasileira


Xanthogranulomatous cystitis (XC) is a rare benign chronic inflammatory disease of unknown etiology. A case of Xanthogranulomatous cystitis in a 53 year-old male is presented. Patient evolution included clinical, radiological, and histological studies. He had no recurrence of XC 32 months after treatment. The present case is probably the 21st to be reported in the world, and the first case in the Brazilian literature

RESUMEN

To report a case of bilateral emphysematous pyelonephritis [EPN] and emphysematous cystitis in a 64-year-old diabetic male with autosomal-dominant polycystic kidney disease [ADPKD]. A 64-year-old diabetic male presented with worsening of renal function and fluid overload. Diagnosis was confirmed by computerized tomography [CT scan] and conservative management with broad-spectrum antibiotics was instituted. There was good clinical response and repeated CT scan showed complete resolution. This case shows that conservative management is an acceptable alternative to surgery in EPN occurring with ADPKD. However, it is recommended that patients should be closely monitored, both clinically and radiologically, and percutaneous catheter drainage or surgical intervention carried out whenever deemed necessary

To study the relationship between bacterial infection and the etiology of cystitis glandularis, 36 female Wistar rats were divided into 3 groups. No intervention was given to the rats in the blank group. NS was infused into the bladder of the rats of the control group, and solution containing E. coli was injected into the bladder of experimental group. Three months later, tissue samples of bladder were collected and observed visually and under light microscope. The results showed that tissues of the blank group were normal; one sample in the control group showed Brunn's nests and cystitis cystica, and 10 in the experimental group had the change of cystitis glandularis. Compared to the blank and control group, samples in the experimental group showed significant change (P0.05). It is concluded that bladder instillation of E. coli can induce cystitis glandularis, which confirms that infection is the cause of cystitis glandularis.

RESUMEN

Las derivaciones urinarias usando el abordaje laparoscópico han ido desarrollándose en la última década. La combinación del abordaje laparoscópico con asistencia manual extracorpórea ha mostrado grandes ventajas en el desarrollo de este tipo de cirugías. El reservorio continente es una excelente opción en pacientes que no son candidatos a derivaciones urinarias ortotópicas. Presentamos nuestra experiencia en la realización de una neovejiga continente tipo Indiana, video asistida, en una paciente con cistitis actínica, siendo, según nuestros conocimientos, la primera derivación de este tipo en publicarse.

In order to study the association between lower urinary tract infection and cystitis glandularis (CG), 120 cases of CG were diagnosed by cystoscopic biopsy in the suspicious foci of the bladder. Among them, 72 cases were subjected to bacterial counting culture of urine and microscopic examination of urinary sediment, and 60 cases to fluorescence quantitative polymerase chain reaction (FQ-PCR) assay to detect HPV, CMV and HSV DNA in urine samples. In the 72 cases of CG, the positive rate of bacterial counting culture of urine was 15.3% (11/72), and gray zone rate was 18.1% (13/72). 31.9% (23/72) patients were positive in bacterioscopy of urinary sediment. There was statistically significant difference as compared with the control group (P0.05). It was concluded that the genesis of CG was closely correlated with the chronic lower urinary tract infection, especially caused by Esch coli.

We conducted a retrospective study to investigate the incidence, risk factors, and clinical features of hemorrhagic cystitis (HC) following allogeneic hematopoietic cell transplantation (allo-HCT). Adult patients who developed HC after allo-HCT were identified from the HCT database of the Asan Medical Center and their medical records were reviewed. From December 1993 to August 2001, a total of 210 adult patients underwent allo-HCT. Fifty-one patients developed HC with a cumulative incidence of 25.7%. The median onset of HC was post-transplant day 24 (range, -2 to 474), and the median duration was 31 days (range, 8 to 369). Significant risk factors for HC by univariate analysis included diagnosis of chronic myelogenous leukemia (p=0.028), unrelated HCT (p=0.029), grade III-IV acute graft-versus-host disease (GVHD) (p<0.001), extensive chronic GVHD (p=0.001), and positive cytomegalovirus antigenemia between post transplant days 31 and 60 (p=0.031). Multivariate analysis showed that grade III-IV acute GVHD was the most important risk factor for the occurrence of HC after allo-HCT (odds ratio, 3.38; 95% CI, 1.36-8.39). Late-onset HC, which occurred beyond 3 weeks after allo-HCT, was more frequently associated with GVHD than earlyonset HC (p=0.007). Our data suggest that a portion of late-onset HC might be a manifestation of GVHD.

Treze casos (8,44 porcento) de síndrome urológica felina foram diagnosticados em 154 necropsias de gatos realizadas de 1991 a 1997. Dados epidemiológicos, sinais clínicos, lesöes de necropsia e achados histopatológicos foram revisados. Sete animais eram machos inteiros e seis eram machos castrados. Obstruçäo uretral, hematúria, disúria e distensäo vesical foram os principais sinais clínicos. Os principais achados de necropsia e histopatológicos foram cistite hemorrágica, pielonefrite, edema perirrenal, hidronefrose, necrose tubular e uretrite.

The cytologic findings of cystitis cystica glandularis--a form of proliferative cystitis which can be mistaken for a tumor clinically and radiologically have hardly been recorded. We describe a case of cystitis cystica where the bladder washings showed a spectrum of findings which were later corroborated on biopsy.
