RESUMEN

Sporotrichosis associated with exposure to domestic cats is hyperendemic in Rio de Janeiro, Brazil. A review of the clinical records at our institute revealed four patients with clinical signs of dacryocystitis and a positive conjunctival culture for Sporothrix who were diagnosed with Sporothrix dacryocystitis. Three patients were children (< 13 years of age) and one patient was an adult. Two patients reported contact with a cat that had sporotrichosis. Dacryocystitis was associated with nodular, ulcerated lesions on the face of one patient and with granulomatous conjunctivitis in two patients; however, this condition manifested as an isolated disease in another patient. All of the patients were cured of the fungal infections, but three patients had chronic dacryocystitis and one patient developed a cutaneous fistula. Sporotrichosis is usually a benign disease, but may cause severe complications when the eye and the adnexa are affected. Physicians, especially ophthalmologists in endemic areas, should be aware of the ophthalmological manifestations and complications of sporotrichosis.

RESUMEN

The incidence of pulmonary embolism (PE) rises markedly with age, and only a few cases have been reported in younger adults. Thrombophilia has been reported as one of the predisposing factors for PE in younger adults. Here we report an extraordinary case of PE complicated with dysplasminogenemia, a rare genetic disorder resulting in hypercoagulability, in a young male. An 18-yr-old male visited an emergency room in the United States complaining chest discomfort. He was diagnosed as PE with deep vein thrombosis without apparent risk factors. Anticoagulation therapy with warfarin had been initiated and discontinued after 6 months of treatment. After returning to Korea he was tested for thrombophilia which revealed decreased activity of plasminogen and subsequent analysis of PLG gene showed heterozygous Ala620Thr mutation. He was diagnosed with PE complicated with dysplasminogenemia. Life-long anticoagulation therapy was initiated. He is currently under follow-up without clinical events for 2 yr.

This is a case report of a patient with Down's syndrome with an important mechanical ectropion and elongation of the lower lid related to chronic allergic conjunctivitis, besides cataract and keratoconus. The tarsal strip technique and ressection of redundant and thick conjunctiva was the surgical approach chosen for this case. Functional and cosmetic outcomes were considered satisfactory.

Fifty patients complaining of bilateral keratitis secondary to acute conjunctivities "one hundred eyes" were treated by either gentamicin 13.6 mg/ml., "25 eyes", or tobramycin 13.6 mg/ml. "25 eyes" or combination of gentamicin 5 mg/ml, penicillin G 31000 units/ml. "50 eyes". The latter treatment was significantly more effective "P< 0.005" and of much less complications

RESUMEN

The differences between allergic rhinitis were evaluated in 127 individuals from tropical urban region of Venezuela. The study design had historic data obtained regarding exacerbation of initial symptoms on exposure to common precipitants, physical examination, nasal cytology, immunodiagnostic test, routine laboratory test (fresh stool test, white cell count and urine test) and sinus x-rays. Forty nine patients were diagnosed as having allergic rhinitis (AR), 10 as probably having AR, 13 had infectious rhinitis, 4 had mixed components, 1 as non-allergic non-eosinophilic rhinitis, and 50 had no evidence of immunological nasal repiratory allergy and were taken as controls. The associated findings with allergic rhinitis were: conjunctivitis 62%, sinusitis 23%, and bronchial asthma 24%. Our results support a significative prevalence of allergic rhinitis in this particular group, with similar symptom patterns to that of industrialized countries
