Objetivo: Determinar la asociación de la pediculosis a variables socio-nutricionales en los alumnos del 1° y 2° grado de primaria de la Escuela 22318 Pampa de Tate en Pachacutec, Ica ­ octubre 2019. Materiales y métodos:Estudio realizado en 184 alumnos de una población de 252 alumnos de 1° y 2° grado de primaria de la Escuela 22318 Pampa de Tate en Pachacutec, a quienes se les lavó el cabello con Champú Nopucid 1 sachet por 2 niños y se usó un peine chino por niño por ser ello es de uso personal. La identificación de los piojos se realizó en el lavatorio y en el peine chino con la ayuda de una lupa en los casos de duda. Resultados: La prevalencia de pediculosis en estudiantes del 1° y 2° de primaria de la Escuela 22318 Pampa de Tate en Pachacutec, Ica en octubre del 2019 fue de 58.7% con un IC95%: (65.3% ­ 51.6%). La proporción de desnutridos fue de 9.8%, de bajo peso de 14.1%, de normo peso 73.9% y de sobrepeso de 2.2%. La pediculosis está asociada (valor de p= 0.000089) al sexo siendo más frecuente en las mujeres. La pediculosis está asociada al grado de estudios (valor de p= 0.007038) siendo más frecuente en los de 2° grado. No existe asociación entre pediculosis y estado nutricional (valor de p= 0.1377071). Conclusiones: La prevalencia de pediculosis fue de 58.7% con un IC95%: (65.3% ­ 51.6%), asociada al sexo femenino, al 2° grado de estudios. (AU)


Objective: To determine the association of pediculosis with socio-nutritional variables in students of the 1st and 2nd grade of primary school 22318 Pampa de Tate in Pachacutec, Ica - October 2019. Materials and methods: Study carried out on 184 students from a population of 252 1st and 2nd grade primary school students at School 22318 Pampa de Tate in Pachacutec, who had their hair washed with Nopucid Shampoo 1 sachet for 2 children and used a Chinese comb per child because it is for personal use . The identification of the lice was carried out in the lavatory and in the Chinese comb with the help of a magnifying glass in cases of doubt. Results: The prevalence of pediculosis in students of the 1st and 2nd grade of the School 22318 Pampa de Tate in Pachacutec, Ica in October 2019 was 58.7% with a 95% CI: (65.3% - 51.6%). The proportion of malnourished was 9.8%, underweight 14.1%, normal weight 73.9% and overweight 2.2%. Pediculosis is associated (p value = 0.000089) with sex, being more frequent in women. Pediculosis is associated with the degree of studies (p value = 0.007038), being more frequent in those of the 2nd degree. There is no association between pediculosis and nutritional status (p value = 0.1377071). Conclusions: The prevalence of pediculosis was 58.7% with a 95% CI: (65.3% - 51.6%), associated with the female sex, at the 2nd degree of studies. (AU)

RESUMEN

Abstract Pseudomycetoma is an extremely rare deep mycosis, caused by dermatophytic fungi that penetrate the tissue from infected follicles of tinea capitis. Both clinically and histopathology are similar to eumycetoma, being distinguished through the isolation of the fungus, which in the case of pseudomycetoma can be Microsporum spp. or Trichophyton spp. genre. We present a 24-year-old man with an exuberant tumor in the occipital region with fistula, whose histopathological examination evidenced grains composed of hyaline hyphae and the culture for fungi isolated the agent Microsporum canis. Combined treatment of surgical excision followed by oral griseofulvin for two years was performed, with resolution of the condition.

Abstract Secondary osteoma cutis is a phenomenon that may occur in several conditions. When it occurs in a melanocytic nevus it is named osteonevus of Nanta, an event considered uncommon and characterized by the presence of bone formation adjacent or interposed with melanocytic cells. There are reports of its occurrence in various melanocytic lesions, being more frequently associated with intradermal nevus. We report a case of osteonevus of Nanta in combined nevus, possibly the first description of this association.

RESUMEN

Abstract Pachydermoperiostosis (PDP) or primary hypertrophic osteoarthropathy (PHO) is a rare hereditary disease characterized by digital clubbing, pachydermia, and periostosis. Its pathogenesis is uncertain and the diagnosis is based on clinical and radiological data. A complete form of the syndrome is reported in a male patient with disease onset in adolescence, with compatible clinical and radiological findings, presenting the three cardinal findings as well as other associated manifestations, such as hyperhidrosis and acne.

Introducción: la psoriasis consiste en trastorno de la proliferación celular epidérmica, caracterizada clínicamente, por una excesiva descamación cutánea y la presencia de lesiones en forma de placas hiperqueratosicas y pruriginosas localizadas en el tronco, las articulaciones y el cuero cabelludo. Presentación de casos: caso 1: paciente femenina de 62 años de edad de procedencia urbana que acude a la consulta de dermatología, presentando proliferación celular epidérmica, con excesiva descamación cutánea y placas hiperqueratosica, pruriginosas localizadas en todo el cuerpo, incluyendo el cuero cabelludo. Caso 2: paciente femenina de 26 años de edad, procedencia rural que acude a la consulta de dermatología de su área de salud, paciente que desde el embarazo comenzó con una lesión eritomatoescamosa en abdomen y luego se generalizo, acompañado de prurito y que a pesar del tratamiento tópico no mejoraba. Discusión: Seguimiento de las pacientes por psoriasis vulgar. Se indicó tratamiento con acupuntura y auto hemoterapia en puntos seleccionados de acupuntura, en varios ciclos programados. Conclusiones: se logró evolución altamente satisfactoria con desaparición progresiva de las lesiones en la piel, debido a la respuesta clínica obtenida.


Introduction: psoriasis consists of a disorder of epidermal cell proliferation, clinically characterized by excessive skin peeling and the presence of lesions in the form of hyperkeratosic and pruritic plaques located on the trunk, joints and scalp. Presentation of cases: case 1: 62-year-old female patient of urban origin who comes to the dermatology consultation, presenting epidermal cell proliferation, with excessive skin desquamation and hyperkeratosic, pruritic plaques located throughout the body, including the scalp. Case 2: 26-year-old female patient, rural origin who comes to the dermatology consultation of her health area, patient who, since pregnancy, began with an erythematous squamous lesion in the abdomen and then generalized, accompanied by itching and which, despite topical treatment, did not improve. Discussion: Follow-up of patients with vulgar psoriasis. Treatment with acupuncture and auto hemotherapy was indicated at selected acupuncture points, in several programmed cycles. Conclusions: highly satisfactory evolution was achieved with progressive disappearance of the skin lesions, due to the clinical response obtained.

RESUMEN

Abstract Melanoacanthoma is a rare variant of seborrheic keratosis, which is notable for dark pigmentation and fast radial growth, making it difficult to distinguish from melanoma. Histologically, it is characterized by proliferation of keratinocytes and dendritic melanocytes. The authors report a scalp lesion, fast growing, suspected by dermoscopy and confocal microscopy examination, with dendritic cells distributed throughout the lesion. Based on these findings, it was not possible to classify this lesion as clearly benign, so it was excised. Histopathologic evaluation and immunostain were consistent with melanoacanthoma.

RESUMEN

Abstract Dissecting cellulitis is an inflammatory, chronic, and recurrent disease of the hair follicles that mainly affects young Afro-descendent men. Trichoscopy is a method of great diagnostic value for disorders of the scalp. Clinical and trichoscopic findings of dissecting cellulitis are heterogeneous and may present features common to non-cicatricial and scarring alopecia. This article presents the trichoscopic findings of dissecting cellulitis that help in the diagnosis and consequent institution of the appropriate therapy and better prognosis of the disease.

Abstract: Background: Actinic keratosis (AK) represents a risk of progression to squamous cell carcinoma. Ingenol mebutate gel is a novel therapeutic option for field-directed treatment. Objectives: To evaluate the safety, tolerability and patients' perspectives, related to the therapeutic success of managing AKs on the face and scalp with ingenol mebutate gel in Brazilian individuals. Methods: This was an observational, retrospective and descriptive study of 68 areas of actinic keratosis on the face and scalp treated with Ingenol mebutate gel involving a total of 37 patients. The drug was applied for three consecutive days on an area of of 25 cm2 and documentation was performed on baseline and days 4, 8, 15, 60 and 180. On day 4, the composite local skin reaction score was calculated. At the end, a questionnaire was applied to evaluate patients' perspectives about the treatment. Results: Adherence was 100%, no serious adverse events were recorded and the mean composite local skin reaction score (standard deviation) was 8.61±4.22. The treatment was considered optimum by 75.68% of the patients. Study limitations: Calculation of composite local skin reaction score performed only on the fourth day. Conclusions: Treatment with ingenol mebutate gel was considered safe and tolerable in Brazilian subjects. Patients had a maximum adherence rate and a great improvement in self-esteem. The results of this research reproduce the findings of the literature.

This is a case report of lichen planus (LP) with multiple system involvement. A 35-year-old female patient was admitted in November 2014 with a 5-year history of painful/difficult sexual intercourse and loss of oral mucosa, and an 8-year history of focal hair loss. Earlier, the patient had been unable to adhere to corticosteroid therapy because of severe adverse side effects. In September 2014, labia minora mucosa defects and stricture of the urethral orifice (with dysuria), vaginal orifice, and vagina were identified. Biopsy was performed and a diagnosis of erosive LP was made. The patient was treated with an oral immunosuppressant (cyclosporine A) and urethral/vaginal dilatation. Urine flow rate and sex life were improved after 6 months and she discontinued medication. Four years later, the patient reported a good overall treatment efficacy. LP can involve multiple systems and should be considered in patients with dyspareunia. Immunosuppressive agents can achieve a satisfactory effect in patients with contraindication to corticosteroid.

OBJECTIVE@#To detect pathogenic mutation of DOCK6 gene in a patient with convulsive seizure and refractory epilepsy.@*METHODS@#CytoScan HD-Array and next generation sequencing were used to detect the potential mutation in the patient.@*RESULTS@#The proband has carried compound heterozygous mutations of c.188C>T (p.Arg63Gln) and c.5374C>T (p.Glu1792Lys) of the DOCK6 gene, which were respectively inherited from his mother and father. Neither mutation was reported previously. Bioinformatic analysis indicated that the two amino acids are highly conserved. Based on the ACMG guidelines, the c.188C>T mutation was predicted to be likely pathogenic, while the c.5374C>T mutation was of uncertain significance.@*CONCLUSION@#The compound heterozygous mutations of c.188C>T (p.Arg63Gln) and c.5374C>T (p.Glu1792Lys) of the DOCK6 gene probably underlie the disease in this patient.

RESUMEN

Abstract Herein we report a case of juvenile xantogranuloma, an inflammatory disease more commonly diagnosed during childhood and is characterized by cutaneous and ocular manifestations. Iris is the main target, presenting as local or diffuse yellowish lesions. Iris involvement may precipitate not only glaucoma but also amblyopia. Treatment is based on corticosteroids therapy, either local or systemic aiming disease control.


Resumo É relatado um caso raro de xantogranuloma juvenil, doença de natureza inflamatória diagnosticada mais frequentemente na infância, com manifestações cutâneas e oculares. A íris é o principal sítio extracutâneo da doença, apresentando-se como lesão amarelada, difusa ou localizada. O acometimento iriano pode acarretar surgimento de glaucoma, além de ambliopia. O manejo clínico da lesão ocular no presente caso foi baseado no necessidade no uso contínuo de corticoide tópico e sistêmico para estabilização da doença.

RESUMEN

Abstract: Although wound or traumatic myiasis is common in tropical countries, only recently cases associated with underlying dermatoses, such as seborrheic dermatitis and psoriasis, have been reported. We describe a patient with seborrheic dermatitis and an ulcerated lesion on the scalp, in which the dermatological examination with the aid of dermoscopy allowed the identification of larvae (maggots) compatible with infestation by Cochliomyia hominivorax. Treatment was performed with oral and topical ivermectin, followed by manual extraction of the larvae.

RESUMEN

Abstract: Lichen simplex chronicus is a skin disease that mostly affects female patients, with a peak incidence between ages 35 and 50 years. On the scalp, it appears as a single or multiple oval lesions, showing scaling and hair shaft loss or breakage. An important dermoscopic feature of the disease are the "broom fibers." Histopathology reveals the "hamburger" and the "gear wheel" signs. The aim of this report is to demonstrate a case of lichen simplex chronicus on the scalp with typical and exuberant clinical, dermoscopic, and histopathological findings.

Les manifestations cutanées au cours de l'infection à VIH étaient beaucoup rapportées, celles du cuir chevelu l'étaient moins. Notre objectif était de déterminer les types de dermatoses du cuir chevelu survenant au cours de l'infection à VIH. Une étude prospective était menée sur 10 mois (juillet 2008 - avril 2009) dans des structures de prise en charge de personnes vivant avec le VIH à Dakar. Était inclus, tout patient infecté par le VIH présentant une atteinte du cuir chevelu et suivi dans l'un des sites de l'étude durant la période de l'étude. Un total de 63 patients présentant 85 pathologies du cuir chevelu était colligé, dont 65 % de sexe féminin et 35 % de sexe masculin. L'âge moyen était de 38 ans. La prévalence des atteintes du cuir chevelu était de 5,7 % à la clinique dermatologique de l'hôpital Aristide Le Dantec et 22 % au Centre de Traitement ambulatoire (CTA) du CHU de Fann. La trichopathie soyeuse représentait 32 % des pathologies, la teigne 24 %, la dermite séborrhéique 15 %. La trichopathie soyeuse, était associée au stade C chez 19 patients et au stade B chez 8 patients. Parmi les 20 patients ayant la teigne, 8 patients avaient moins de 200 CD4/mm3. Le délai moyen avant le recours au soin était de 74 jours. L'atteinte du cuir chevelu entrainait un retentissement psychologique chez 63 % des patients. Le cuir chevelu était atteint en cas d'immunodépression sévère et de dénutrition importante

Cutaneous metastases of malignant tumors are relatively rare, and breast cancer is the most common malignancy in women with cutaneous metastases. Since newly developed cutaneous lesions can be the first signs of metastases in breast cancer patients, it is crucial to rule out the possibility of malignancy. Although only a few cases have been reported, breast cancer contributes to a large portion of scalp metastases. This case report demonstrates a rare case of breast cancer metastasis only confined to the scalp. The patient was a 55-year-old woman who was diagnosed with scalp metastasis from breast cancer 10 years after the first curative surgery. The scalp lesion was palpable for 4 years and showed a sudden increase in size over a few months. The patient underwent wide excision with flap coverage. After surgery the patient received radiotherapy, but she has refused additional hormonal therapy. To date, there is no evidence of disease recurrence.
