RESUMEN

@#Primary ovarian pregnancy accounts for less than 1% of all cases of ectopic pregnancies. Its diagnosis is most commonly made intra-operatively and the approach in its management has been geared towards conservative measures such as oophorectomy and resection. Use of intrauterine device still remains the most established risk factor for the development of ovarian pregnancy. The diagnosis is established following the criteria first described by Spiegelberg in 1878. Several cases of ovarian gestation have been described in literature, although very few cases with live term fetus have been reported. Majority of the cases were diagnosed intra-operatively, and the management was tailored depending on the complexity of each of the different cases. This paper reports a case of primary ovarian pregnancy with a live term fetus, which was only diagnosed intra-operatively. Total hysterectomy with right salpingo-oophorectomy was performed due to difficulties encountered brought about by dense adhesions.

El embarazo ectópico es en la actualidad un problema de salud a nivel mundial atribuido al aumento de su incidencia y la morbimortalidad que crea. La localización ovárica del embarazo ectópico es un evento raro pero posible y también en ascenso. Presentamos el caso de una paciente que acude al cuerpo de guardia con escasa sintomatología. Se le realizó el diagnóstico ultrasonográfico de la entidad y apoyado en una prueba de embarazo positiva. Luego le practicaron laparotomía de urgencia en la que se comprobó el embarazo ovárico derecho no complicado. También se efectuó una salpingooforectomía y la paciente tiene una evolución satisfactoria trans-posoperatoria(AU)


Ectopic pregnancy is currently a global health problem due to the increase in its incidence and morbidity and mortality. The ovarian location of ectopic pregnancy is a rare but possible event and on the rise. We present the case of a patient who comes in with little symptoms. The ultrasound diagnosis of the entity was made and supported by a positive pregnancy test. Then emergency laparotomy was performed, which proved that the right ovarian pregnancy was not complicated. Salpingo-oophorectomy was also performed and the patient has a satisfactory trans and postoperative evolution(AU)

OBJECTIVE: To analyze the preoperative diagnostic clues to ovarian pregnancy (OP). METHODS: This study conducted a retrospective chart review of 23 patients with OP and 46 patients with tubal pregnancy (TP) from October 1, 2003 to September 31, 2016 in Hanyang University Hospital. RESULTS: There were no significant differences in demographic and clinical characteristics between the two groups. The presence of an ectopic gestational sac and hemoperitoneum was significantly higher in the TP group (13.0% vs. 95.7%, P=0.000; 13.0% vs. 54.3%, P=0.001, respectively) in preoperative ultrasonogram. The OP group had more ruptured ectopic gestational sacs than the TP group (73.9% vs. 45.7%, P=0.039) in surgical findings. CONCLUSION: For the patients in whom a gestational sac is not detected in the uterus or the fallopian tubes, it is important to be aware of the possibility of OP and rupture of an ovarian gestational sac to promote early diagnosis and surgical intervention.

RESUMEN

La gestación ectópica ovárica es muy infrecuente, presentando una incidencia de 1/2100-7000 gestaciones (0,5-3% de todos los ectópicos). Su diagnóstico es difícil y dada la poca experiencia, no está claro el tratamiento más adecuado. Se presentan seis casos clínicos, recogidos en nuestro hospital desde 2001 hasta 2011, destacando el difícil diagnóstico prequirúrgico así como el tratamiento realizado.


Ectopic ovarian pregnancy is very infrequent, with an incidence of 1/2100-7000 pregnancies (0.5-3% of all ectopic pregnancies). Diagnosis is difficult and due to few cases presented in the literature there is no consensus on the appropriate treatment. Here we review six clinical cases of ovarian ectopic pregnancy that presented to our hospital between 2001 and 2011, highlighting the difficult presurgical diagnosis and appropriate course of treatment.
