RESUMEN

Genital dissatisfaction is well known in female and adults. Less is known about male adolescents and their genital satisfaction. The aim of this study was to investigate and report the role of surgery in male adolescents to improve the evaluation of their genitalia. We considered all patients treated for external genital pathology in the period of adolescence. Inclusion and exclusion criteria were created. Patients underwent an evaluation test before and after surgery. During the study period, 137 patients were treated, and at the end of the study, 98 cases were considered for analysis. The most frequent pathologies were webbed penis and penile curvature. A postoperative score improvement was noted and patients with concealed penis and webbed penis showed a better postoperative outcome. Overweight was considered an important factor associated with a worse preoperative score. Evaluation of the external genitalia is important in adolescents, and it is an understudied problem. Overweight may be associated with a worse evaluation of one's genital and should be clinically considered to avoid related social problems in adulthood. Therefore, cosmetic genital surgery should be considered even in male adolescents.

RESUMEN

A 70-year-old male presenting with a mass in the right inguinal area was treated with surgery, and was diagnosed pathologically as spermatic cord metastasis of pancreatic cancer. He was given systemic chemotherapy. Unfortunately, he died of ascites and cachexia three months later.

Objetivos: Diagnosticar e manejar patologias comuns do trato genital masculino. Métodos: Revisão da literatura do ano de 2011 ao 2017 em base de dados PubMed. Resultados: Hipospádia é a localização anômala do meato uretral. Seu diagnóstico é clínico. As indicações cirúrgicas são anormalidades funcionais e estéticas. Fimose é descrita como impossibilidade de retração do prepcio para exposição da glande peniana, sendo um evento fisiológico quando apresenta resolução espontânea. Nos casos patológicos, o tratamento é primeiramente realizado com corticoide tópico. Se falha, o tratamento é cirúrgico. Hidrocele é o acúmulo de líquido na bolsa escrotal. O diagnóstico é clínico e por transiluminação escrotal. A maioria dos casos regride espontaneamente até os 24-36 meses. A cirurgia é considerada padrão-ouro para hidrocele comunicante na criança. Criptorquidia é a falha na migração do testículo até a bolsa escrotal. Seu diagnóstico é clínico, através da palpação testicular bilateral. Caso os testículos não estejam presentes na bolsa escrotal até os 6 meses de idade, a cirurgia é imprescindível, pelo risco de infertilidade e malignização. Conclusões: É essencial que o médico generalista saiba reconhecer tais condições com o intuito de iniciar prontamente o tratamento adequado, evitando suas complicações.


Aims: Diagnose and manage common pathologies of the male genital tract. Methods: Literary review of the last 6 years in the PubMed database. Results: Hypospadia is an anomalous location of the urethral meatus. The diagnosis is clinical. Surgical indications are functional and aesthetic abnormalities. Fimosis is described as impossibility of retraction of the foreskin to expose the penile glans, it is a physiological event when it presents spontaneous resolution. In pathological cases, treatment is first performed with topical corticosteroids. If it fails, the treatment is surgical. Hydrocele is the accumulation of fluid in the scrotal sac. The diagnosis is clinical and scrotal transillumination. Most cases regress spontaneously up to 24-36 months. Surgery is considered gold standard in cases of communicant hydrocele in children. Cryptorchidism is the failure of the migration of the testis to scrotal sac. The diagnosis is clinical through bilateral testicular palpation. If the testicles are not present in the scrotal sac until 6 months of age, surgery is essential, due to the risk of infertility and malignancy. Conclusions: It is imperative that the general practitioner knows how to recognize such conditions in order to promptly initiate appropriate treatment, avoiding complications.

Epididymal agenesis is defined as the absence of the epididymis totally or segmentally, unilateral or bilateral, which is secondary to the Wolffian duct malformation (1). Rete testis, epididymis, vas deferens and seminal vesicle are believed to develop from Wolffian ducts.

RESUMEN

Membrana penescrotal ou fusão penescrotal na forma isolada é uma rara anomalia de etiologia obscura, na qual a pele do pênis e o escroto estão fundidos. A fusão pode ser completa, com total ausência de angulação do pênis e escroto, ou incompleta, com uma membrana de extensão variável ligando o pênis proximal ao escroto. O objetivo da correção cirúrgica é separar o pênis do escroto, o que poderá ser realizado facilmente, utilizando-se plástica com retalhos em ?Z?, plásticas com retalhos ?V-Y?, ou incisão transversal e sutura vertical. Os resultados com a correção precoce são mais gratificantes e auxiliam a prevenir vários problemas psíquicos.

RESUMEN

Los condilomas acuminados corresponden a la patología ano rectal más frecuente en los pacientes portadores del virus de la inmuno deficiencia humana (VIH+). Su tratamiento genera controversias por la posible progresión a carcinoma epidermoide y su alta recurrencia. Se presenta nuestra experiencia en el tratamiento de los condilomas acuminados en pacientes VIH+. La serie corresponde a veinte pacientes todos homosexuales de sexo masculino, con edad promedio de 32 años, once de ellos recibían terapia anti retroviral al momento de la cirugía. Los síntomas más frecuentes fueron el sangrado, el prurito y la sensación de masa anal. Quince pacientes presentaron condilomas en todo el margen anal con extensión al canal anal. En cuatro casos se realizó fulguración de las lesiones y los restantes fueron resecados. La anatomía patológica de estos casos mostró infección por virus papiloma en todos ellos, con displasia moderada en solo un caso. No se registraron casos de displasia intensa ni carcinoma invasor. El seguimiento alejado se logró en 16 pacientes, dos de ellos presentaron recidiva de las lesiones debiendo ser sometidos a un nuevo procedimiento quirúrgico sin nuevas recidivas y lográndose el control sintomático en todos ellos. Los cuatro pacientes restantes se encuentran fallecidos por su patología de base. En esta serie aunque la histología no muestra displasia intensa ni carcinoma, el control sintomático justifica el tratamiento quirúrgico de estas lesiones.

OBJECTIVE: To study the incidence and relevance of ectopic adrenal tissue in pediatric patients who underwent groin surgical explorations. MATERIALS AND METHODS: We studied 1120 patients with groin surgical explorations during a period of 8 consecutive years. PatientsÆ clinical data and histological findings were analyzed. RESULTS: We found ectopic adrenal tissue in 13 patients in 1120 groin surgical exploration (1.16 percent). Of the 13 cases, 5 were diagnosed as having undescended testes, 6 inguinal hernia and 2 communicating hydrocele. Median age at diagnosis was 5.6 years. Histological sections showed adrenal cortical tissue with no medulla present. CONCLUSION: Based on the clinical implications of those adrenal rests it is mandatory the removal of this ectopic tissue whenever encountered during surgical interventions in the groin region in children.

RESUMEN

Spermatic cord leiomyosarcomas are rare tumors and standard treatment consists of radical orchiectomy and high cord ligation. We report a case of a paratesticular leiomyosarcoma successfully treated by enucleation. A 22-year-old man presented with a 6-months history of inguinal pain. Physical examination revealed a right paratesticular nodule about 0.5 cm in diameter. Inguinal exploration and nodule biopsy were performed. It was thought to be a benign epididymal nodule on a quick section and the tumor was enucleated and sent for paraffin section. Histology and immunohistochemistry were compatible with leiomyosarcoma. The patient was advised to undergo radical orchiectomy with high cord ligation. However, he refused surgery. An alternative approach with clinical, biochemical and radiological follow-up was adopted. The patient has been followed up for thirteen years and shows no evidence of disease.

INTRODUCTION: Genital skin loss in men may be caused by avulsion injuries of the penis and scrotum or by gangrene of the male genitalia. Reconstruction of the scrotum after complete loss of the overlying skin is a challenging problem. We report our experience on the management of this problem. MATERIAL AND METHOD: Medical records of all male patients with massive scrotal skin loss and exposed testes treated at Ramathibodi Hospital and Noparat Rajthanee Hospital from 1990 to 1999 were reviewed. The etiologies of scrotal skin loss, technique of treatment, post-operative consequence as well as complications were noted. RESULTS: Twelve patients were described in this study. Nine patients had avulsion injuries of the penile and scrotal skin secondary to agricultural machinery accidents. Three patients were after extensive debridement of Fournierrís gangrene. The exposed testes had been placed in thigh pouches and scrotal reconstruction using thigh pedicle flaps was done 4-6 weeks later. No immediate and delayed complications were detected in all of the patients. They recovered without any sequelae and had a satisfactory cosmetic result. CONCLUSION: Extensive scrotal skin loss should be immediately treated surgically. Implantation of the exposed testes in the upper thigh pouch and delayed reconstruction of the scrotum using thigh pedicle flaps can provide excellent results

Se presentan 2 variantes técnicas del tratamiento quirúrgico del linfedema primario de pene y escroto en 2 pacientes con características clínicas diferentes. Una de las técnicas quirúrgicas empleadas consistió en 2 incisiones laterales en las bolsas escrotales con resección del tejido linfedematoso y la otra con una incisión única en la bolsa y reimplantación de la base del pene con resección de todo el tejido linfedematoso. En ambos casos antes de intentar la resección del tejido linfedematoso se extrajeron de su lecho el cordón espermático y los testículos para facilitar la operación, minimizar el tiempo quirúrgico y producir menos complicaciones. La propia piel del cuello de la bolsa escrotal se emplea en su reconstrucción e incluso para acomodar los testículos. No se hizo necesario, con estas técnicas, realizar injerto libre ni pediculado de piel. Los pacientes recuperaron la capacidad funcional del pene, tuvieron mejoría estética y desapareció la angustia que esta enfermedad les ocasionaba(AU)


Two technical variants of the surgical treatment of primary penile and scrotal lymphoedema used in 2 patients with different clinical characteristics are presented. One of the surgical techniques used consisted in 2 lateral incisions in the scrotal bursae with resection of the lymphoedematous tissue and the other in a single incision in the bursa and reimplantation of the basis of the penis with resection of all the lymphoedematous tissue. In both cases, before attempting the resection of the lymphoedematous tissue the spermatic cord and the testes were removed from their bed to facilitate the operation, to reduce the surgical time and to cause less complications. The own skin of the neck of the scrotal bursa is used in its reconstruction and even to accomodate the testes. By using these techniques, it was not necessary to make a free or pediculate skin graft. Patients recovered the functional capacity of the penis, had an aesthetic improvement and the anguish produced by this disease disappeared(AU)

This prospective study was carried out to see the age incidence of varicocele in our population and to compare the results of two commonly practiced surgical procedures i.e., Ivanissevich's procedure and Paloma's procedure. Seventy-two patients with the mean age of 24.7 years, having left-sided varicocele, were studied over two years. Among them 58.33% had Grade-II varicocele. Operative time and hospital stay was almost same in both the groups. However, postoperative complications rate with Ivanissevich's procedure was comparatively more. Postoperative improvement in semen quality was found in 60% patients having history of infertility or sub-fertility

Os autores relatam sua experiência com 26 casos de torção do cordão espermático em dois hospitais universitários, de janeiro de 1992 a dezembro de 1997. O presente estudo foi realizado com o objetivo de avaliar dados como idade, sinais/sintomas e relação entre demora do tratamento e perda do testículo. O diagnóstico diferencial também é discutido. Concluem que em casos duvidosos a exploração cirúrgica precoce é necessária para excluir torção do cordão espermático e para prevenir a perda testicular

RESUMEN

Paciente de 38 anos, evoluindo há 10 anos com nódulos e fístulas no pênis, bolsa escrotal, períneo, regiöes glúteas e inguinais, apresentava elefantíase da genitália externa. Os diagnósticos de linfogranuloma venéreo, donovanose e tuberculose foram excluídos através da histopatologia, bacteriologia e imunofluorescência indireta para Chlamydia trachomatis. O caso ilustra o potencial de gravidade e complicaçöes da hidradenite supurativa, assim como os resultados da cirurgia reconstrutiva
