RESUMEN

Objective: To summarize the preliminary experience in the treatment of esthesioneuroblastoma (ENB) and to explore the effect of age, chemotherapy, modified Kadish stage and pathological grade on the prognosis of ENB. Methods: The clinical data of 87 ENB patients from the First Affiliated Hospital of Sun Yat-sen University and Sun Yat-sen University Cancer Center between June 2002 and November 2017 were retrospectively analyzed. The modified Kadish stage was used to evaluate the extent of the lesions, and the Hyams grading system was used for pathological grading. The patients were followed up regularly to evaluate the recurrence and metastasis of the tumor. Cox proportional hazard model was used for univariate and multivariate analyses. Prognostic factors with P<0.05 in univariate analysis were included in multivariate analysis. After controlling the confounding factors, the model coefficients were used to calculate the hazard ratio (HR) and 95% confidence interval (CI). Results: The median follow-up time of ENB patients was 29 months, and the 5-year overall survival rate was 39.3%. In univariate analysis, age, chemotherapy, modified Kadish stage and pathology grade were independent predictors of overall survival, while gender, radiotherapy and surgery were not prognostic factors. Multivariate analysis showed that modified Kadish stage and pathology grade were independent predictors of overall survival rate after excluding confounding factors. Conclusions: Age, chemotherapy, modified Kadish stage and pathological grade are taking important role in the overall survival rate of patients with ENB. Modified Kadish stage and pathological grade are independent predictors of overall survival rate.

RESUMEN

Resumen El neuroblastoma olfatorio (NBO) es un tumor maligno poco frecuente que se origina de las células neuroepiteliales olfativas. Su diagnóstico precoz es difícil debido a la poca especificidad de los síntomas que presentan los pacientes. Las pruebas de imagen juegan un papel importante en su diagnóstico y en la planificación quirúrgica, por lo que es importante que los radiólogos conozcan sus hallazgos y las diferentes clasificaciones que ayudarán a elegir el tratamiento más adecuado para cada tumor.


Abstract Olfactory neuroblastoma (ONB) is a rare malignant tumor that originates from olfactory neuroepithelial cells. Its early diagnosis is difficult due to the low specificity of the symptoms. Imaging tests play an important role in its diagnosis and surgical planning so it is important that radiologists know their findings and the different classifications that will help to choose the most appropriate treatment for each tumor.

Introduction: Malignant sinonasal tumors comprise less than 1% of all neoplasms. A wide variety of tumors occurring primarily in this site can present with an undifferentiated or poorly differentiated morphology. Among them are esthesioneuroblastomas, sinonasal undifferentiated carcinomas, and neuroendocrine carcinomas. Objectives: We will discuss diagnostic strategies, recent advances in immunohistochemistry and molecular diagnosis, and treatment strategies. Data Synthesis: These lesions are diagnostically challenging, and up to 30% of sinonasal malignancies referred to the University of Texas MD Anderson Cancer Center are given a different diagnosis on review of pathology. Correct classification is vital, as these tumors are significantly different in biological behavior and response to treatment. The past decade has witnessed advances in diagnosis and therapeutic modalities leading to improvements in survival. However, the optimal treatment for esthesioneuroblastoma, sinonasal undifferentiated carcinoma, and neuroendocrine carcinoma remain debated. We discuss advances in immunohistochemistry and molecular diagnosis, diagnostic strategies, and treatment selection. Conclusions There are significant differences in prognosis and treatment for esthesioneuroblastoma, neuroendocrine carcinoma, and sinonasal undifferentiated carcinoma. RESUMEN

El neuroblastoma olfatorio es una neoplasia neuroectodérmica maligna rara que afecta a la bóveda nasal y que tiende a invadir las estructuras vecinas, entre estas a la cavidad intracraneal. Su asociación con aneurismas intracraneales nunca ha sido descrita. Presentamos a una paciente con la coexistencia de un aneurisma del segmento oftálmico de la arteria carótida interna y un neuroblastoma olfatorio con invasión intracraneal. La coexistencia entre neoplasias del sistema nervioso central y aneurismas intracraneales es infrecuente. En este artículo analizamos las posibles causas de la asociación entre neoplasias y aneurismas intracraneales.


The olfactory neuroblastoma is a rare malignant neuroectodermal neoplasia of the nasal roof. It has a highly tendency of invades near structures including intracranial compartment. Associations between olfactory neuroblastoma with intracranial aneurysms have been never described. We show a case of a female with coexistence of an internal carotid artery aneurysm and olfactory neuroblastoma. The coexistence between nervous system tumors with intracranial aneurysms is not common. In this paper we analyze the association between intracranial neoplasia and aneurysms.

RESUMEN

O Estesioneuroblastoma (ENB) é um tumor maligno raro, originado da crista neural, crescendo na cavidade nasal. Objetivo: descrever um caso de ENB, com abordagem multidisciplinar, cuja manifestação clínica inicial foi proptose ocular, que é uma de suas apresentações mais raras. Método: relato de caso de um paciente de doze anos, masculino, portador de ENB, analisando a forma de manifestação clínica, método diagnóstico empregado e tratamento realizado. Resultados: a manifestação clínica inicial foi a proptose ocular esquerda, sendo submetido, em outro serviço médico, à cirurgia com ressecção parcial do tumor e quimioterapia. Em função da persistência tumoral, foi submetido à nova avaliação e ao tratamento definitivo, através de cirurgia craniofacial, com ressecção tumoral completa, reconstrução com retalho microcirúrgico do músculo reto abdominal e radioterapia adjunvante. Atualmente, no nono mês pós-operatório, sem sinais de doença ativa. Conclusão: o trabalho mostra a abordagem terapêutica multimodal, com ênfase no tratamento cirúrgico, manifestação clínica infreqüente, diagnósticos diferenciais e modo de reconstrução empregado...

RESUMEN

O índice proliferativo celular tumoral de 8 casos cirúrgicos de estesioneuroblastomas com extensäo intracraniana (9 com extensäo intracraniana em um total de 15, coletados de 1978 a 1992 no Hospital Nordstadt de Hannover, Alemanha) foi avaliado através do anticorpo monoclonal MIB-1, notando-se uma correlaçäo prognóstica. A maioria dos tumores revelou índice proliferativo alto (3 por cento a 42 por cento, média de 16 por cento) e a classificaçäo histopatológica de Hyam foi realizada. A ressecçäo foi radical em 100 por cento dos casos e o acesso cirúrgico combinado (paranasal e subfrontal) o preconizado. Os autores apresentam revisäo da literatura discutindo a sua casuística
