Estruma ovárico / Struma ovarii

Introducción: 20 por ciento de los tumores ováricos son de células germinales; y, aunque muchos no pueden ser clasificados por citología, los teratomas sí, y entre estos, el estruma formado por tejido tiroideo. Reconocerlo, ayuda al patólogo en el manejo de la biopsia intraoperatoria y define el proceder clínico quirúrgico. Presentación del caso: mujer de 29 años atendida en urgencias por dolor en región inguinal derecha. Al examinarla se constató tumoración de alrededor de 8 cm en región pélvica derecha, dolorosa al palparla. En el tacto vaginal se percibió masa tumoral gonadal de consistencia firme. El ultrasonido arrojó en el ovario derecho una imagen ecolúcida de paredes gruesas con imágenes hipoecogénicas en su interior, con bordes irregulares de 67 x 97 mm, e imagen ecogénica que ocupa el 20 % de la lesión, con pobre patrón vascular. Conclusiones: los datos clínicos, y los exámenes imagenológicos y citológicos, permiten diagnosticar un estruma ovárico benigno(AU)

Introduction: twenty percent of ovarian tumors are of germinal cells and although many cannot be classified through cytology, teratomes can, and among them, the struma formed by the thyroid tissue. Recognizing this fact helps the pathologist to manage intraoperative biopsy and to define the surgical clinical procedure. Case presentation: a 29 years-old woman seen in the emergency service to relieve pain in her right inguinal region. On the physical exam, an 8 cm tumor was found in the right pelvic region, causing pain when palpating it. The vaginal exam revealed solid gonadal tumor mass. The ultrasound test showed in the right overy an echolucent image of thick walls with hypoechogenic images inside with 67 x 97 mm irregular contours and echogenic image covering 20 percent of lesion, with poor vascular pattern. Conclusions: clinical data and imaging and cytological exams allow diagnosing benign struma ovarii(AU)

Estruma ovarii maligno / Malignant struma ovarii

The Malignant Struma ovarii (SO) is a thyroid carcinoma generated in a mature teratoma, constitutes 0.8-3percent of ovarian tumors. Then, two cases will be reviewed. Case 1: 46 year old woman with no known morbid consulted for right iliac fossa pain requiring exploratory laparotomy with findings of an ovarian tumor, the biopsy reported mature ovarian teratoma SO type microfocus papillary thyroid carcinoma. Case 2: 59 year old female with a history of dyslipidemia and knee osteoarthritis in controls in gynecology since 2010 on the right adnexal mass of cystic type of 5x3 cm. The study showed only free fluid dissemination ovarian, oofectomía bilateral was performed and biopsy showed minimally invasive follicular carcinoma (2.8 cm) developed by SO, serosa without invasion of the ovary. Is derived of endocrinology and the study showed: thyroid stimulating hormone (TSH) 3.3 uIU/ ml; Peroxidase antibody (anti-TPO) and thyroglobulin antibody (antiTG) negative. Ultrasonography exhibited thyroid nodule of 0.8 x 0.6 cm, hipovascularizado with normal thyroid cintigraphy and cytology negative for neoplastic cells in fine needle aspiration (FNA). Struma ovarii con un foco de micro-carcinoma papilar variante folicular de tiroides: reporte de caso y revisión de la literatura / Struma ovarii with follicular variant of papillary thyroid carcinoma: report of one case

Struma ovarii is an ovarian teratoma composed mainly of thyroid tissue, which can occasionally develop a malignant thyroid tumor. We report a 61 years old female consulting for a metrorrhagia in whom an ovarian cyst was discovered. The patient was subjected to a hysterectomy and bilateral oophorectomy. The pathological study of the surgical piece revealed a focus of papillary thyroid carcinoma, follicular variety in a right struma ovarii. Three months after surgery, an abdominal CAT scan did not show any abnormality.

Unilateral malignant struma ovarii in a case of bilateral ovarian teratoma with raised CA-125 level: A rare case with treatment dilemmas.

Struma ovarii are specialized form of mature ovarian teratoma comprised predominantly of thyroid tissue (>50%). Most of the struma ovarii are benign; rarely can they undergo malignant transformation. Elevated CA-125 levels with benign struma ovarii have been seen in only 5 cases in literature. The association of malignant struma ovarii and high CA-125 levels with pseudo-Meig syndrome has been reported in only 2 cases in English literature. We describe a case of a 46-year-old multigravida who presented with an abdominal mass and raised CA-125 levels. Radiological investigations revealed bilateral cystic adnexal masses with ossified elements on left side suggesting a teratoma. Intraoperative frozen section and final pathology revealed bilateral teratoma with follicular variant of papillary thyroid carcinoma arising in the left ovary. To the best of our knowledge, this is the first case of malignant struma ovarii in combination with bilateral teratoma. The dilemmas related to preoperative diagnoses with elevated CA-125 levels, mimicking an epithelial ovarian neoplasm; intraoperative frozen section consultation; management and follow-up issues in this rare malignancy are discussed.

Malignant Struma Ovarii: A Case Report

We present a case of a 40-yr-old woman diagnosed with a primary malignant struma ovarii. The patient was admitted with the complaint of pelvic pain and a large pelvic mass in the mid-portion of lower abdomen on gynecological examination. Pre-operative tumor markers and routine biochemistry were unremarkable. She was treated with total abdominal hysterectomy and right salpingo-oopherectomy. Post-operatively, she was diagnosed with a malignant struma ovarii through the usage of histopathological criteria similar to the guidelines for primary thyroid gland disease. The patient was subsequently performed left salpingo-oopherectomy and retroperitoneal pelvic lympadenectomy for re-staging. Although, left ovary and lymph nodes were histopathologically normal, she was offered thyroidectomy but she refused to accept the offer. Thyroglobulin level was monitored in the post-operative period. She is free of the disease for 18 months.

Malignant struma ovarii: a case report and review of the literature.

A 45-year-old woman with papillary carcinoma arising in struma ovarii which involved one ovary without dissemination is presented. Total abdominal hysterectomy and bilateral salpingo-oophorectomy were performed. Histopathological examination revealed malignant struma ovarii of the papillary type. The flow cytometric analysis revealed an hypodiploid DNA content (DNA index:0.67). Since these are rare tumors, there is no universal exact protocol for the treatment and follow-up of patients with malignant struma ovarii. More data are needed to determine the prognosis and management in preferring mode of therapy.

Fibrotecoma (Fibroma)de ovario / Fecofibrona (fibrona) of ovary

Se describe un caso de fibrotecoma de ovàrio en una mujer de 48 años, con la presencia de tumoraciòn pèlvica dolorosa y ascitis. Microscopicamente muestra la presencia de cèlulas fusiformes con disposiciòn entrecruzada sin mitosis anormales entremezcladas con nidos de cèlulas tecales. Paraplejia secundaria a metástasis de un carcinoma folicular de Strumma ovarii / Secundary paraplegia a to metastasis of follicular carcinoma of ovarian struma

Se presenta el caso clínico de una paciente de 47 años de edad a la que dos años antes se les practicó histerectomía con salpingo-oforectomía bilateral por tumor ovárico reportado como un tumor de la granulosa. Acude nuevamente con cuadro de paraplejía secundaria a comprensión medular por fractura patológica de vértebra dorsal (T7-8). Se le practica laminectomía de la lesión vertebral osteolítica, la que se reporta como correspondiente a un carcinoma folicular de tiroides metastásico. Posteriormente es sometida a una tiroidectomía total, la pieza de esta ocasión se reportó como "glándula tiroidea normal". En busca de la lesión primaria se reexamina la pieza y laminillas de histerectomía, diagnosticándose un carcinoma folicular y struma ovarii. Es tratada con Iodo 131, recupera la motilidad y cinco años después se encuentra en perfectas condiciones, haciendo vida normal. Se hacen comentarios sobre dicha entidad

Struma ovarii benigno / Benign struma ovarii

Säo descritos dois casos de struma ovarii benignos examinados e tratados no Departamento de Ginecologia do Hospital A.C. Camargo da Fundaçäo Antônio Prudente. O diagnóstico anatomopatológico de congelaçäo já demonstrava histopatologia sugestiva de struma ovarii, ratificado posteriormente através do exame com parafina. Procedeu-se ainda ao estudo com imunoperoxidase (antitiroglobulina), como propedêutica complementar. Os aspectos clínicos diferenciais e tratamento deste tumor raro säo discutidos
