O Diabetes Mellitus Gestacional é definido como doença que se caracteriza pelos altos níveis de glicemia sanguínea, diagnosticada durante a gestação. Este adoecimento pode acarretar várias complicações maternas e fetais, muitas vezes, necessitando de internamento precoce e cuidados avançados. Objetivou-se caracterizar o perfil epidemiológico de gestantes com diabetes mellitus gestacionais atendidas em serviço de referência. Trata-se de estudo descritivo, documental, retrospectivo, de caráter quantitativo, realizado com gestantes atendidas na maternidade do Hospital Regional do Sudoeste ­ PR, Francisco Beltrão. A amostra foi constituída por 216 gestantes, cujos dados foram coletados dos prontuários das pacientes. Incluíram-se as gestantes atendidas e diagnosticadas com diabetes mellitus gestacional no período de 2020 e com pelo menos um exame de glicose em jejum ou um teste de tolerância oral à glicose para comprovação diagnóstica. Foram exclusas as gestantes dos anos de 2019 e 2021 e oito transferências. A amostra teve maior porcentual do Diabetes mellitus gestacional (90,7%), com prevalência na raça branca (69,9%), faixa etária de 15- 35 anos (68,5%). Ademais,65,7% realizaram controle com dieta e 32,4 % necessitaram realizar o uso de insulina e 51,9%delas eram obesas. A presente pesquisateve considerável relevância, pois permitiu obter perfil epidemiológico de gestantes diagnosticadas com diabetes mellitus, trazendo benefícios, como identificação precocemente da doença, de modo a evitar complicações para gestantes e bebês. Gestational Diabetes Mellitus is defined as a disease characterized by high levels of blood glucose, which is diagnosed for the first time during pregnancy. It can cause several maternal and fetal complications, often requiring early hospitalization and advanced care. The aim of thestudy was to characterize the epidemiological profile of pregnant women with gestational diabetes mellitus seen at a reference service. This is a descriptive, documentary, retrospective, quantitative study, carried out with pregnant women attended at the maternity hospital of the Hospital Regional do Sudoeste - PR in the city of Francisco Beltrão. The sample consisted of216 pregnant women, and data were collected from the patients' medical records. The study included all pregnant women who were attended and diagnosed with GDM in the period described, and who had at least one fasting glucose test or an oral glucose tolerance test for diagnostic confirmation. All pregnant women from the year 2019 and 2021 were excluded fromthe study. The sample had a higher percentage of GDM 90.7% according to race 69.9% werewhite, aged 15-35 years 68 , 5%, while 65.7% performed control with diet and 32.4% neededto use insulin and 51.9% of them were obese. This research had great results because it had an epidemiological profile of pregnant women diagnosed with Diabetes Mellitus, bringing benefitsand thus being able to identify gestational Diabetes mellitus early, aiming to avoid complications for the pregnant woman and the baby.


La diabetes mellitus gestacional se define como una enfermedad caracterizada por niveles elevados de glucosa en sangre, diagnosticada durante el embarazo. Esta enfermedad puede dar lugar a varias complicaciones maternas y fetales, que a menudo requieren una hospitalización temprana y cuidados avanzados. El objetivo es caracterizar el perfil epidemiológico de las gestantes con diabetes mellitus atendidas en el servicio de referencia. Se trata de un estudio descriptivo, documental, retrospectivo, de carácter cuantitativo, realizado con gestantes atendidas en la maternidad del Hospital Regional del Sudoeste - PR, Francisco Beltrão. La muestra estaba formada por 216 mujeres embarazadas, cuyos datos se recogieron de las historias clínicas de las pacientes. Se incluyeron las mujeres embarazadas atendidas y diagnosticadas de diabetes mellitus gestacional en el periodo 2020 y con al menos una prueba de glucosa en ayunas o una prueba de tolerancia oral a la glucosa para su diagnóstico. Se excluyeron las embarazadas de los años 2019 y 2021 y ocho traslados. La muestra tuvo un mayor porcentaje de Diabetes mellitus gestacional (90,7%), con prevalencia en la raza blanca (69,9%), grupo de edad 15-35 años (68,5%). Además, el 65,7% se controlaba con la dieta y el 32,4% necesitaba utilizar insulina y el 51,9% era obeso. La presente investigación tiene una relevancia considerable, ya que permite obtener el perfil epidemiológico de las gestantes diagnosticadas con diabetes mellitus, lo que conlleva beneficios, como la identificación precoz de la enfermedad, a fin de evitar complicaciones para las gestantes y los bebés.

INTRODUCCIÓN: La etapa prepuberal es un periodo crítico del desarrollo de la grasa corporal, en el cual la leptina y la resistencia a la insulina han sido asociados, sin embargo, hay pocos estudios en prepúberes normo- peso. OBJETIVO: Evaluar la relación de leptina con composición corporal y resistencia a la insulina en un grupo de prepúberes normopeso. PACIENTES Y MÉTODO: Estudio transversal analítico en 128 pre púberes saludables, normopeso, entre 6 y 10 años. Se midieron, talla, peso, índice de masa corporal (IMC), porcentaje de grasa corporal (PGC), perímetro abdominal (PA) y perímetro de cadera (PC). Se determinó leptina (ng/ml) e Insulina (μU/L) en plasma por inmunoensayo y glicemia (mmol/L) por método enzimático. Se calculó HOMA-IR. Se hizo análisis de comparación y de correlación por sexo. RESULTADOS: Se confirmó en niñas un mayor nivel de leptina (6,8 ± 5 vs 3,3 ± 3,7; p = 0,000), insulina (7,1 ± 4,5 vs 5,2 ± 2,5; p = 0,016), PGC (22,4 ± 4,3 vs 18,6 ± 3,9; p = 0,000) y PC (67 ± 5,7 vs 65,0 ± 4,5; p = 0,019), y un menor índice cintura/cadera (0,84 ± 0,04 vs 0,88 ± 0,04; p = 0,000) comparado con varones. Las correlaciones de leptina con variables antropométricas fueron signifi cantes en ambos sexos, con mayor asociación en sexo femenino. La asociación del HOMA-IR con la leptina fue similar en ambos sexos. CONCLUSIONES: En prepúberes normopeso de 6 a 10 años, hay diferencias por sexo en adiposidad y en niveles de leptina, no asociadas con diferencias en el IMC ni en la resistencia a la insulina. La mayor asociación de leptina con adiposidad en las niñas podría estar relacionada con una elevada tasa de adipogénesis inducida por esta hormona.


INTRODUCTION: The prepubertal stage is a critical period of body fat development, in which leptin and insulin re sistance has been associated, however, there are few studies in normal-weight prepubescents. OBJECTIVE: To assess the relationship between leptin and body composition and insulin resistance in a group of normal-weight prepubescents. PATIENTS AND METHOD: Analytical cross-sectional study with 128 healthy prepubescents of normal weight, aged between 6 and 10 years. Height, weight, body mass index (BMI), body fat percentage (BFP), waist circumference (WC), and hip circumference (HC) were measured. Plasma leptin (ng/mL) and insulin (mU/L) were evaluated by immunoassay and glycemia (mmol/L) by enzymatic method. HOMA-IR was calculated. A comparison study and correlation analysis by sex were performed. RESULTS: Females presented higher values than males of leptin (6.8 ± 5 vs 3.3 ± 3.7; p = 0.000), insulin (7.1 ± 4.5 vs 5.2 ± 2.5; p = 0.016), BFP (22.4 ± 4.3 vs 18.6 ± 3.9; p = 0.000), and HC (67 ± 5.7 vs 65.0 ± 4.5; p = 0.019), and a lower waist/hip ratio (0.84 ± 0.04 vs 0.88 ± 0.04; p = 0.000). Leptin correlations with anthropometric variables were significant in both sexes, with greater association in females. The association of HOMA-IR with leptin was similar in both sexes. CONCLUSIONS: in normal-weight prepubescents aged between 6-10 years, there are sex differences in adiposity and leptin levels not associated with differences in BMI or insulin resistance. The greater association of leptin with adiposity in girls could be related to a high rate of adipogenesis induced by this hormone.

RESUMEN Introducción: La prueba de tolerancia de comida mixta es considerada la prueba de oro para la medición de la producción de insulina endógena en pacientes con diabetes tipo 1. Objetivo: Determinar la utilidad de la prueba de tolerancia de comida mixta con Nutrial I para evaluar la función de las células ß en diabéticos tipo 1 de diagnóstico reciente y la relación de esa función con algunas características clínicas y bioquímicas. Métodos: Se estudiaron variables bioquímicas como la glucemia, hemoglobina glucosilada (HbA1c), péptido C y fracciones lipídicas. La prueba de tolerancia de comida mixta con Nutrial I se aplicó a 18 sujetos con diabetes tipo 1 de diagnóstico reciente y a 8 voluntarios con edades comprendidas entre 19 y 35 años. El consumo del suplemento Nutrial I se calculó según el peso del paciente. Se obtuvieron muestras para glucemia y péptido C a los -10, 0, 30, 60, 90 y 120 minutos. Resultados: Se observaron concentraciones elevadas de glucemia y disminuidas de péptido C durante la prueba de tolerancia de comida mixta en los diabéticos tipo 1 de diagnóstico reciente, en comparación con los voluntarios, así como, diferencias en las áreas bajo la curva de péptido C (AUC-pc) (p= 0,001). En los diabéticos tipo 1 de diagnóstico reciente se evidenció una correlación negativa entre el AUC-pc con los niveles de glucemia en ayunas (r= -0,747; p ( 0,0001) y la HbA1c (r= -0,535; p= 0,022). Por el contrario, se encontró una correlación positiva entre el AUC-pc y el péptido C en ayunas (r= 0,722; p= 0,001). El AUC-pc después de la prueba de tolerancia de comida mixta es mayor en los sujetos con glucemia en ayunas si GA < 7 mmol/L con respecto a los sujetos con glucemia en ayunas ( 7 mmol/L (p= 0,012). Conclusiones: El empleo del Nutrial I en la prueba de tolerancia de comida mixta fue útil en la evaluación de la función de las células β en diabéticos tipo 1 de diagnóstico reciente. Los valores bajos de glucemia en ayunas durante esta prueba son marcadores indirectos de una función residual de células ( más conservada en los diabéticos tipo 1 de diagnóstico reciente(AU)


ABSTRACT Introduction: The tolerance test of mixed food is considered the gold standard for the measurement of endogenous insulin production in patients with diabetes type 1. Objective: To determine the usefulness of the tolerance test of mixed food with Nutrial I to assess the ß-cells function in patients with diabetes type 1 of recent diagnosis and the relation of this function with some clinical and biochemical characteristics. Methods: There were studied biochemical variables as the blood glucose, glycosylated haemoglobin (HbA1c), C-peptide and lipid fractions. The tolerance test of mixed food with Nutrial I was applied to 18 individuals with diabetes type 1 of recent diagnosis and in 8 volunteers aged between 19 and 35 years old. The consumption of Nutrial I supplement was calculated according to the weight of the patient. Samples were obtained for blood glucose and C-peptide at -10, 0, 30, 60, 90 and 120 minutes. Results: There were observed high concentrations of glycemia and decreased amounts of C-peptide during the tolerance test of mixed food in recently diagnosed type 1 diabetics in comparison with the volunteers, as well as differences in areas under the curve of C-peptide (AUC-pc) (p= 0.001). In the recently diagnosed type 1 diabetics was evident a negative correlation between the AUC-pc with fasting plasma glucose levels (r= -0,747; p(0.0001) and HbA1c (r= -0,535; p= 0.022). On the contrary, it was found a positive correlation between the AUC-pc and fasting C-peptide (r = 0.722; p = 0.001). The AUC-pc after the tolerance test of mixed food was greater in subjects with fasting blood glucose < 7 mmol/L with respect to the subjects with fasting blood glucose ( 7 mmol/L (p= 0.012). Conclusions: The use of Nutrial I in the tolerance test of mixed food was useful in the assessment of the role of the β-cells in patients with recently diagnosed diabetes type 1. Low values of fasting blood glucose during this test are indirect markers of a residual function of (cells more preserved in type 1 diabetics of recent diagnosis(AU)

ABSTRACT Objective We denote the four major factors related to the development of type 2 diabetes (T2D) as "diabetes factor" (DF); increased insulin resistance (IR); decreased glucose effectiveness (GE); and the first-and-second-phase of insulin secretion (FPIS, SPIS). The level of hemoglobin (Hb) was found to be related to IR and FPIS, but no-known studies focused on its role in relation to SPIS and GE. In this study, we aim to evaluate the relationships between Hb and all four DFs in the same individual. Subjects and methods We randomly enrolled 24,407 men and 24,889 women between 30 and 59 years old. IR, FPIS, SPIS and GE were measured according to equations published in our previous studies. To compare the slopes between Hb and the four DFs with different units, we converted their units to percent of change per unit of increased Hb. Results Age, HDL-cholesterol and GE were higher in women; BMI, blood pressure, LDL-cholesterol, TG, Hb, FPIS, SPIS and IR were higher in men. After they were converted into percentage, the closeness of their relationships to Hb, from the highest to the lowest, were GE, IR, FPIS and SPIS for women and IR, GE, FPIS and SPIS for men. GE was the only one negatively related to Hb. Conclusions Our data showed that IR, FPIS and SPIS were both positively and, GE negatively, related to the Hb in adult Chinese. For women, GE had the closest association with Hb; for men, it was IR. Both phases of insulin secretion had relatively weaker relationships than IR and GE.

Abstract Neotropical nectar-feeding bats consume large amounts of sugar and use most of their energy-rich diet directly from the bloodstream, suggesting an adaptation towards lower body energy reserves. Here we tested the hypothesis that bats Glossophaga soricina spare the energy costs of storing energy reserves, even if this would represent a risky susceptibility during fasting. Blood glucose concentrations in 18 h fasted bats showed a 40% decrease. Breast muscle and adipose tissue lipids, as well as carcass fatty acids and liver glycogen, were also decreased following fasting. The inability to keep normoglycemia following a short-term fasting (i.e. 28 h) confirm that nectar bats invest little on storing energy reserves and show a severe fasting susceptibility associated to this pattern. Our study also support the general hypothesis that evolutionary specializations towards nectar diets involve adaptations to allow a decreased body mass, which reduces the energy costs of flight while increases foraging time.


Resumo Morcegos nectarívoros que ocorrem na região Neotropical consomem grandes quantidades de carboidratos, e usam a energia obtida da dieta diretamente, a partir da glicose na circulação sanguínea. Esta adaptação sugere que morcegos nectarívoros tenham evoluído no sentido de apresentar adaptações fisiológicas que permitam o baixo armazenamento de reservas energéticas corporais. Nós testamos a hipótese de que morcegos Glossophaga soricina poupam o gasto energético envolvido com a formação de reservas energéticas teciduais, mesmo que isso represente uma arriscada suscetibilidade da espécie frente ao jejum. As concentrações de glicose apresentaram uma diminuição de 40% após 18 h de jejum. As concentrações de lipídios do músculo peitoral e do tecido adiposo, bem como as de ácidos graxos da carcaça e glicogênio hepático também diminuíram após 18 h de jejum. A incapacidade de manter a normoglicemia observada após o jejum de curto-prazo confirma que morcegos nectarívoros desta espécie não investem na formação de reservas energéticas, e apresentam, consequentemente, uma severa susceptibilidade ao jejum. Este estudo suporta a hipótese de que adaptações evolucionárias da espécie envolvem diminuição da massa corporal, reduzindo o custo energético do voo e aumentado o tempo de forrageamento.

Newborn animals, in the fetal-to-neonatal transition, usually face several challenges in their first 24 hours, including issues with acid-base balance, glycemic levels and oxygenation. Difficulties to overcome such issues have caused several deaths among newborns. Therefore, studies have been carried out in order to evaluate them. The main purpose of this study is to evaluate the correlation between the serum lactate level in the mother, in the placenta and in the newborn. Moreover, the study measured the lactate level, blood glucose level and blood gas level in the first 24 hours. Tests were carried out right after birth, and at 4, 8, 12 and 24 hours after. Lactate levels were quite similar to the placental levels (p=0.991) which, in turn, were significantly different from the mother's (p=0.011). Results showed that, shortly after birth, the production of lactate in the placenta is part of the issue. Along the first 24 hours, the study observed a reduction of the levels of lactate in newborns; the levels were closer to the normal index levels for the species. Regarding the blood gas test results, we observed mild metabolic acidosis at birth; acid-base balance was completely stable at the end of the period.(AU)


Os principais desafios da transição fetal-neonatal são a estabilização do equilíbrio ácido-básico, glicemia e oxigenação ao longo das primeiras 24 horas de vida. Falhas nesse sistema são a maior causa de morte nesse período e os estudos de viabilidade neonatal tem se concentrado nas avaliações destas variáveis. O objetivo do presente estudo foi avaliar a comparação da lactatemia materna, placentária e neonatal ao nascimento e, ainda, observar o comportamento do lactato, glicemia e hemogasometria nas primeiras 24 horas de vida do neonato ovino. Para tanto, as avaliações foram realizadas ao nascimento, 4, 8, 12 e 24 horas após. A lactatemia neonatal se mostrou próxima à placentária (p=0,991) e significativamente diferente da materna (p=0,011), o que sugere uma influência da produção de lactato pela placenta na lactatemia neonatal ao nascimento. Ao longo das 24 horas de vida foi possível observar a depuração de lactato pelo organismo do neonato, aproximando assim dos valores de referência para a espécie. Na hemogasometria foi possível observar discreta acidose metabólica, com normalização já nas primeiras 4 horas e estabilização completa do equilíbrio ácido-básico ao final das 24 horas de vida.(AU)

ABSTRACT Objective: The role of bone markers on insulin resistance (IR) remains controversial. The objective of this study is to evaluate the association between bone mineral density (BMD) and glucose metabolism and investigate if visceral hyperadiposity, evaluated by waist circumference (WC), is an effect modifier of this association. Subjects and methods: Cross-sectional analysis with 468 young adults from the fourth follow-up of the 1978/79 Ribeirão Preto prospective birth cohort, Brazil. BMD, total osteocalcin (OC), fasting plasma glucose and insulin concentrations were assessed. IR, sensitivity (S) and secretion (β) were estimated by homeostasis model assessment (HOMA) indexes. Multiple linear regression models were constructed to estimate the association between BMD and glucose metabolism. Beta coefficient, R2 and p-values were provided. WC was tested as an effect modifier and OC as a confounder. The covariates were selected based on Direct Acyclic Graph. Results: Significant interaction between BMD (femoral neck and proximal femur areas) and WC on glucose metabolism was observed in the adjusted models. Subjects with increased WC presented a positive association between BMD and log HOMA1-IR while an inverse association was found in those with normal WC (femoral neck R2 = 0.17, p = 0.036; proximal femur R2 = 0.16, p = 0.086). BMD was negatively associated with log HOMA2-S in individuals with increased WC and positively in those with normal WC (femoral neck R2 = 0.16, p = 0.042; proximal femur R2 = 0.15, p = 0.097). No significant associations between BMD, log HOMA2-β and OC and glucose metabolism markers were observed. Conclusions: BMD was associated with glucose metabolism, independently of OC, and WC modifies this association.

BACKGROUND: chronic hypoxia increases basal ventilation and pulmonary vascular resistance, with variable changes in arterial blood pressure and heart rate, but it's impact on heart rate variability and autonomic regulation have been less well examined. We studied changes in arterial blood pressure, heart rate and heart rate variability (HRV) in rabbits subjected to chronic normobaric hypoxia (CNH; PB ~ 719 mmHg; FIO2 ~ 9.2%) for 14 days and assess the effect of autonomic control by acute bilateral vagal denervation. RESULTS: exposure to CNH stalled animal weight gain and increased the hematocrit, without affecting heart rate or arterial blood pressure. Nevertheless, Poincaré plots of the electrocardiographic R-R intervals showed a reduced distribution parallel to the line of identity, which interpreted as reduced long-term HRV. In the frequency domain, CNH reduced the very-low- (< 0.2 Hz) and high-frequency components (> 0.8 Hz) of the R-R spectrograms and produced a prominent component in the low-frequency component (0.2-0.5 Hz) of the power spectrum. In control and CNH exposed rabbits, bilateral vagotomy had no apparent effect on the short- and long-term HRV in the Poincaré plots. However, bilateral vagotomy differentially affected higher-frequency components (> 0.8 Hz); reducing it in control animals without modifying it in CNH-exposed rabbits. CONCLUSIONS: These results suggest that CNH exposure shifts the autonomic balance of heart rate towards a sympathetic predominance without modifying resting heart rate or arterial blood pressure.

Abstract Background: The hypertriglyceridemic waist (HTW) phenotype is defined as the simultaneous presence of increased waist circumference (WC) and serum triglycerides (TG) levels and it has been associated with cardiometabolic risk in children and adolescents. Objective: The objective was to evaluate the influence of HTW phenotype in the fasting glycemia and blood pressure in children and adolescents over one-year follow-up period. Methods: It is a cohort study involving 492 children and adolescents from 7 to 15 years old, both genders, who were submitted to anthropometric, biochemical and clinical evaluation at the baseline, and also after 6 and 12 months of follow-up. Generalized Estimating Equation (GEE) models were calculated to evaluate the longitudinal influence of the HTW phenotype in the glycemia and blood pressure over one-year. Results: It was observed a prevalence of 10.6% (n = 52) of HTW phenotype in the students. The GEE models identified that students with HTW phenotype had an increase of 3.87 mg/dl in the fasting glycemia mean (CI: 1.68-6.05) and of 3.67mmHg in the systolic blood pressure (SBP) mean (CI: 1.55-6.08) over one-year follow-up, after adjusting for confounding variables. Conclusions: The results of this study suggest that HTW phenotype is a risk factor for longitudinal changes in glycemia and SBP in children and adolescents over one-year follow-up period.


Resumo Fundamento: O fenótipo de cintura hipertrigliceridêmica (CHT) é definido como a presença simultânea de circunferência de cintura (CC) e níveis séricos de triglicérides (TG) aumentados e tem sido associado com risco cardiometabólico em crianças e adolescentes. Objetivo: Avaliar a influência do fenótipo CHT na glicemia de jejum e na pressão arterial em crianças e adolescentes em um período de acompanhamento de um ano. Métodos: Trata-se de um estudo de coorte envolvendo 492 crianças e adolescentes de 7 a 15 anos de ambos os sexos, que foram submetidos à avaliação antropométrica, bioquímica e clínica no início e também após 6 e 12 meses de seguimento. Os modelos de Equação de Estimulação Generalizada (GEE) foram calculados para avaliar a influência longitudinal do fenótipo CHT na glicemia e na pressão arterial ao longo de um ano. Resultados: Foi observada uma prevalência de 10,6% (n = 52) do fenótipo CHT nos estudantes. Os modelos GEE identificaram que os estudantes com fenótipo CHT apresentaram aumento de 3,87 mg/dl na média de glicemia em jejum (IC: 1,68-6,05) e de 3,67 mmHg na pressão arterial sistólica media (PAS) (IC: 1,55-6,08) depois de um ano de acompanhamento, após ajuste para variáveis de confusão. Conclusões: Os resultados deste estudo sugerem que o fenótipo CHT é um fator de risco para alterações longitudinais da glicemia e da PAS em crianças e adolescentes em um período de um ano de seguimento.

Objective: This study aims to investigate whether pre-operative Homeostasis Model Assessment Insulin Resistance (HOMA-IR) value is a predictor in non-diabetic coronary artery bypass grafting patients in combination with hemoglobin A1c, fasting blood glucose and insulin levels. Methods: Eighty one patients who were admitted to Cardiovascular Surgery Clinic at our hospital between August 2012 and January 2013 with a coronary artery bypass grafting indication were included. Patients were non-diabetic with <6.3% hemoglobin A1c and were divided into two groups including treatment and control groups according to normal insulin resistance (HOMA-IR<2.5, Group A; n=41) and high insulin resistance (HOMA-IR>2.5, Group B; n=40), respectively. Pre-operative fasting blood glucose and insulin were measured and serum chemistry tests were performed. The Homeostasis Model Assessment Insulin Resistance values were calculated. Statistical analysis was performed. Results: There was a statistically significant difference in fasting blood glucose and HOMA-IR values between the groups. Cross-clamping time, and cardiopulmonary bypass time were longer in Group B, compared to Group A (P=0.043 and P=0.031, respectively). Logistic regression analysis revealed that hemoglobin A1c was not a reliable determinant factor alone for pre-operative glucometabolic evaluation of non-diabetic patients. The risk factors of fasting blood glucose and cardiopulmonary bypass time were more associated with high Homeostasis Model Assessment Insulin Resistance levels. Conclusion: Our study results suggest that preoperative screening of non-diabetic patients with Homeostasis Model Assessment Insulin Resistance may improve both follow-up visit schedule and short-term outcomes, and may be useful in risk stratification of the high-risk population for impending health problems. .


Objetivo: Este estudo tem como objetivo investigar se a resistência de valor do Modelo de Avaliação da Homeostase da Resistência à Insulina (Homeostasis Model Assessment Insulin Resistance - HOMA-IR) no pré-operatório é um preditor de revascularização do miocárdio para pacientes não diabéticos em combinação com a hemoglobina A1c, glicemia em jejum e insulina. Métodos: Oitenta e um pacientes que foram internados no serviço de Cirurgia Cardiovascular em nosso hospital entre agosto de 2012 e janeiro de 2013, com indicação para cirurgia de revascularização do miocárdio indicação foram incluídos. Os pacientes não diabéticos com <6,3% de hemoglobina A1c foram divididos em dois grupos, incluindo os grupos de tratamento e controle de acordo com a resistência à insulina normal (HOMA-IR <2,5, Grupo A, n=41) e alta resistência à insulina (HOMA-IR> 2,5, Grupo B, n=40), respectivamente. Glicemia de jejum pré-operatório e insulina foram medidas e testes de química do soro foram realizados. Os valores de HOMA-IR foram calculados. A análise estatística foi realizada. Resultados: Houve diferença estatisticamente significativa em jejum de valores de glicose no sangue e HOMA-IR entre os grupos. Tempo de pinçamento e tempo de circulação extracorpórea foi maior no grupo B, em relação ao Grupo A (P=0,043 e P=0,031, respectivamente). A análise de regressão logística revelou que hemoglobina A1c não foi um fator determinante para a avaliação confiável sozinho glicometabólico pré-operatória de pacientes não diabéticos. Background: Resistance exercise effects on cardiovascular parameters are not consistent. Objectives: The effects of resistance exercise on changes in blood glucose, blood pressure and vascular reactivity were evaluated in diabetic rats. Methods: Wistar rats were divided into three groups: control group (n = 8); sedentary diabetic (n = 8); and trained diabetic (n = 8). Resistance exercise was carried out in a squat device for rats and consisted of three sets of ten repetitions with an intensity of 50%, three times per week, for eight weeks. Changes in vascular reactivity were evaluated in superior mesenteric artery rings. Results: A significant reduction in the maximum response of acetylcholine-induced relaxation was observed in the sedentary diabetic group (78.1 ± 2%) and an increase in the trained diabetic group (95 ± 3%) without changing potency. In the presence of NG-nitro-L-arginine methyl ester, the acetylcholine-induced relaxation was significantly reduced in the control and trained diabetic groups, but not in the sedentary diabetic group. Furthermore, a significant increase (p < 0.05) in mean arterial blood pressure was observed in the sedentary diabetic group (104.9 ± 5 to 126.7 ± 5 mmHg) as compared to that in the control group. However, the trained diabetic group showed a significant decrease (p < 0.05) in the mean arterial blood pressure levels (126.7 ± 5 to 105.1 ± 4 mmHg) as compared to the sedentary diabetic group. Conclusions: Resistance exercise could restore endothelial function and prevent an increase in arterial blood pressure in type 1 diabetic rats. .


Fundamento: Os efeitos do exercício resistido sobre os parâmetros cardiovasculares não são consistentes. Objetivos: Foram avaliados os efeitos do exercício resistido sobre as alterações na glicemia, reatividade vascular e pressão arterial de ratos diabéticos. Métodos: Ratos Wistar foram divididos em três grupos: grupo controle (n = 8), diabético sedentário (n = 8) e diabético treinado (n = 8). O exercício resistido foi realizado no aparelho de agachamento para ratos e consistiu em três séries de dez repetições com uma intensidade de 50%, três vezes por semana, durante 8 semanas. As alterações na reatividade vascular foram avaliadas em anéis de artéria mesentérica superior. Resultados: Foi observada uma redução significativa da resposta máxima dos relaxamentos induzidos por acetilcolina no grupo diabético sedentário (78,1% ± 2) e um aumento do grupo diabético treinado (95 ± 3%), sem alterar a potência. Na presença de NG-nitro-L-arginina metil éster, os relaxamentos induzidos por acetilcolina foram significativamente reduzidos nos grupos controle e diabético treinado, mas não no grupo diabético sedentário. Além disso, foi observado um aumento significativo (p < 0,05) da pressão arterial média no grupo diabético sedentário de 104,9 ± 5 para 126,7 ± 5 mmHg, quando comparado ao grupo controle. Por outro lado, o grupo diabético treinado apresentou redução significativa (p < 0,05) nos níveis da pressão arterial média de 126,7 ± 5 mmHg para 105,1 ± 4 mmHg, quando comparado ao diabético sedentário. Objetivo: Avaliar o crescimento neonatal dos descendentes de ratas espontaneamente hipertensas (SHR) submetidas a exercício em esteira motorizada. Metodologia: Ratas SHR e Wystar Kyoto (WKY) foram distribuídas em: sedentárias (SHR e WKY), exercitadas durante a prenhez (SHR EXP e WKY EXP) e exercitadas previamente - antes e durante a prenhez (SHR EXPP e WKY EXPP). Durante a prenhez, pesos corpóreos, glicemia e nível de lactato e triglicérides e pressão arterial foram determinados. No 10º dia de vida pós-natal, peso, glicemia e comprimento corporal e cranial dos filhotes e triglicéride hepático foram analisados. Resultados: Não houve diferença no peso e lactato durante a prenhez e no número de filhotes entre os grupos. O grupo WKE EXPP apresentou maior pressão arterial comparado aos grupos WKY EXP E WKY. SHR EXPP e WKY EXPP apresentaram maior nível de triglicéride que os grupos exercitados na prenhez e sedentários. SHR EXPP apresentou menor glicemia que os SHR EXP e SHR. Não houve diferença nos parâmetros dos filhotes, mas o comprimento corpóreo foi maior nos grupos WKY EXPP e SHR EXPP. Conclusão: O exercício prévio foi exaustivo para a linhagem WKY. Purpose: To evaluate the neonatal growth of the offspring of spontaneously hypertensive rats (SHR) subjected to treadmill exercise. Methods: SHR and Wystar Kyoto (WKY) rats were distributed into: sedentary group (SHR and WKY), exercised during pregnancy (SHR EXP and WKY EXP) and exercised previously and during pregnancy (SHR EXPP and WKY EXPP). During pregnancy, body weight, blood glucose, lactate and triglycerides levels and blood pressure were determined. At 10th day of life, body weight, blood glucose, and body and cranial length and hepatic triglycerides were analyzed. Results: There was no difference in body weight and lactate level during pregnancy, and number of pups among groups. The WKY EXPP group presented higher blood pressure than WKY and WKY EXP groups. SHR EXPP and WKY EXPP showed higher triglyceride level compared to groups trained in pregnancy and sedentary. SHR EXPP showed lower blood glucose than SHR and SHR EXP. There was no difference in the parameters of the newborns. In addition, body length was higher in WKY EXPP and SHR EXPP groups. Conclusion: The previous exercise was exhausting for the WKY strain. Exercise during pregnancy is not harmful to SHR rats and benefited the body length of offspring...

Since 1964, the hypothesis of Pedersen has been used to explain fetal macrosomia observed in gestational diabetes mellitus (GDM), by a mechanism involving maternal hyperglycemia - fetal hyperglycemia - fetal hyperinsulinemia. However,since the 1980-89 decade, it is known that pregnant women with pre-gestationaloverweight not suffering from GDM still have a higher frequency of fetal macrosomia. Furthermore, pregnant women with GDM, despite being subjected to optimalglycemic control, still show unacceptably high frequencies of fetal macrosomia, aphenomenon that is concentrated in pregnancies with overweight or obesity priorto pregnancy. If glucose is not the single nutrient responsible for fetal macrosomiain pregnant women with gestational diabetes that undergo strict glycemic control,other nutrients may cause excessive fetal growth in pre-pregnancy overweightmothers. In this review, we propose that triglycerides (TG) could be responsible forthis accelerated fetal growth. If this hypothesis is validated in animal models andclinical studies, then normal and pathological ranges of TG should be defined, andmonitoring of triglyceride levels during pregnancy should be advised as a possiblenew alternative, besides a good glycemic control, for the management of fetal macrosomia in GDM women with overweight prior to pregnancy.

Background: Plasma insulin and HOMA (homeostasis model assessment) index, used to determine insulin resistance, do not have local standard values for children and adolescents in Chile. Aim: To establish the normal reference intervals for insulin and HOMA in children and adolescents aged 10-15 years, according to sex and puberal maturation. Material and Methods: A cross-sectional study of 2,153 children and adolescents from Puente Alto County was performed, during 2009 and 2010. Anthropometry and self-report of puberal maturation were assessed. Fasting glucose (hexoquinase) and insulin blood levels (chemiluminiscence), were determined and HOMA index was calculated. Percentile distributions of these variables were calculated. Results: The reference group included only subjects with normal body mass index and fasting blood glucose (n = 1,192). Girls had higher insulin and HOMA values than boys (12.5 ± 6.0 and 9.1 ± 4.9 μϋ/mL (p < 0.01) and 2.7 ± 1.4 and 2.1 ± 1,1 (p < 0.01), respectively). Subjects with Tanner I and IIpuberal stages had lower insulin and HOMA mean values than subjects with Tanner III and IV (9.0 ± 4.3 and 12.5 ± 6.2μϋ/ml (p < 0.01) and2.0 ± 1 and2.8 ± 1.4 (p < 0.01), respectively). Conclusions: The 90th percentile of insulin and HOMA distributions according to sex and maturation, was selected as the upper cut-off point to identify individuals with insulin resistance. HOMA cutoff point for Tanner I and II boys was 3.2, for Tanner I and II girls was 4.1, for Tanner III and IV boys was 4.2 and for Tanner III and IV girls was 5.0.

Background: Adipose tissue is a source of inflammatory cytokines which may influence insulin action. Aim: To evaluate exercise effects on plasma (p)-levels of tumor necrosis factor (TNF)-α , and interleukin (IL)-6 in Peruvian Amerindian women. Material and Methods: After five years of observation during which p-glucose deteriorated and weight increased, 44 Peruvian Amerindian women aged 20-59 years took part in a 6-month exercise program. Plasma TNF-α and IL-6 had been measured 5 years earlier, and were reassessed before and after exercise, using ELISA kits. Cardiorespiratory fitness was derived from the results of the twelve minutes’ walk and expressed as maximum oxygen uptake (VO2max). Results: After the training program, estimated VO2max increased from 18.55 ± 3.79 to 20.91 ± 4.61 mL kg-1 min-1 (p < 0.01) and p-TNF-α increased from 3.2 ± 3.4 to 4.3 ± 6.6 pg/mL (p = 0.03). There were reductions in p-glucose from 5.19 ± 1.59 to 4.32 ± 1.62 mmol/L (p < 0.01) and in waist circumference (p=0.01). Weight, p-insulin, and IL-6 did not change. Levels of p-glucose and p-TNF-α observed after the training period were identical to values 5 years earlier. P-TNF-α was positively associated with estimated VO2max, prior to (r = .414, p < 0.01) as well as after (r = .362, p < 0.05) exercise, independently of adiposity. P-IL-α was associated with waist circumference but not with estimated VO2max. Conclusions: P-TNF-a correlated with estimated VO2max independently of adiposity. Beneficial effects of exercise on p-glucose were observed in spite of an increase in p-TNF-α.


Antecedentes: El tejido adiposo es una fuente de citoquinas inflamatorias, que pueden influenciar la acción de insulina. Objetivo: Evaluar el efecto del ejercicio sobre los niveles de factor de necrosis tumoral α (TNFα) e interleukina α (IL-α) en mujeres amerindias. Material y Métodos: Después de cinco años de observación, en los cuales la glicemia y el peso aumentaron, 44 mujeres amerindias de 20 a 59 años participaron en un programa de ejercicio que duró seis meses. TNFα e IL-α se habían medido cinco años antes y se re evaluaron antes y después del ejercicio, usando kits de ELISA. La capacidad cardiorespiratoria se estimó a partir de la prueba de marcha de 12 minutos y se expresó como capacidad aeróbica máxima (VO2max). Resultados: Después del programa de ejercicio, la VO2max estimada aumentó de 18,55 ± 3,79 a 20,91 ± 4,61 mL kg-1 min-1 (p < 0,01) y el TNFα aumentó de 3,2 ± 3,4 a 4,3 ± 6,6 pg/mL (p = 0,03). Se observó una reducción de la glicemia de 5,19 ± 1,59 a 4,32 ± 1,62 mmol/L (p < 0,01) y de la circunferencia de cintura (p = 0,01). El peso, insulinemia e IL-α no cambiaron. Los niveles de glicemia y TNFα observados después del período de entrenamiento, eran iguales a los determinados cinco años antes. Los niveles de TNFα se correlacionaron con el VO2max estimado antes y después del período de entrenamiento, y no dependieron de la adiposidad. Los niveles de IL-α se correlacionaron con la circunferencia de cintura pero no con el VO2max estimado. Conclusiones: Los niveles de TNFα se correlacionaron con el VO2max estimado independientemente del nivel de adiposidad. Los efectos beneficiosos del ejercicio sobre la glicemia se observaron a pesar de un aumento de los niveles de TNFα.

Background: The higher prevalence of chüdhood obesity has led to search for metabolic syndrome (MS) in this age group. Aim: To study the prevalence of MS in obese children and adolescents. Material ana Methods: Cross sectional study of 255 obese children and adolescents aged 11.3 ± 2.4 years, 45 percent males, 60 percent pubertal, with a body mass Índex (BMI) z score of 2.7 ± 0.6, who were evaluated for obesity. MS was defined as the presence of at least three of the following criteria, according to Ferranti: fasting glucose (FG) ≥ 100 mg/dl, triglycerides (TG) ≥ 100 mg/dl, HDL < 50 mg/dl, waist circumference (WC) > percentile (p) 75 and blood pressure (BP) > p90. Patients were also classified using Cook criteria: FG ≥100 mg/dl, TG ≥ 110 mg/dl, HDL < 40 mg/dl, WC > p 90, BP > p 90. Results: MS was observed in 45 and 22.7 percent of patients, according to Ferranti and Cook definitions, respectively. WC was the most frequent criteria and glucose was the most uncommon. Males had higher body mass Índex, WC and TG levéis than femóles. According to Ferranti and Cook áefinitions, MS prevalence was 53.5 and 28 percent in males and 37.6 andl8.4 percent in fernales (p < 0.05). Fifty and 26.1 percent of pubertal patients exhibited MS vs 36.9 and 17.5 percent in pre-pubertal subjects (p < 0.05) using Ferranti and Cook criteria, respectively. The frequency of MS increased along with a higher BMI. Conclusions: MS is a prevalent condition in obese children and adolescents, especially in males and pubertal children. It is necessary to have a better and universal definition for MS in pediatrics including all ages, in order to be focused in obesity prevention and treatment.

OBJECTIVES: We investigated the effects of chronic (eight weeks) low-to moderate-intensity swimming training on thermal pain sensitivity in streptozotocin-induced diabetic female rats. METHODS: Female Wistar rats (n = 51) were divided into the following groups: trained streptozotocin-induced diabetic rats [hyperglycemic trained (HT)], sedentary streptozotocin-induced diabetic rats [hyperglycemic sedentary (HS)], normoglycemic trained rats (NT) and normoglycemic sedentary rats (NS). Diabetes was induced by a single injection of streptozotocin (50 mg/kg, i.p.). One day after the last exercise protocol (60 min/day, five days/week for eight weeks) in the trained groups or after water stress exposure (ten min/twice a week) in the sedentary groups, the rats were subjected to a hot plate test. RESULTS: After eight weeks of swimming training, streptozotocin-induced diabetic rats presented a significantly lower body mass (trained: 219.5±29 g, sedentary: 217.8±23 g) compared with the normoglycemic groups (trained: 271±24 g, sedentary: 275.7±32 g). Interestingly, we did not find differences in blood glucose levels (mg/dl) between the trained and sedentary groups of the hyperglycemic or normoglycemic rats (HT: 360.2±6.6, HS: 391.7±6.7, NT: 83.8±14.0, NS: 77.5±10.1). In the hot plate test, the rats from the HT group presented a significantly lower latency than the other rats (HT: 11.7±7.38 s, HS: 7.02±7.38 s, NT: 21.21±7.64 s, NS: 22.82±7.82 s). CONCLUSION: Low-to-moderate swimming training for a long duration reduces thermal hyperalgesia during a hot plate test in streptozotocin-induced diabetic female rats.

FUNDAMENTO: O processo de envelhecimento associa-se ao desenvolvimento de várias doenças, que podem ser amenizadas pela prática de atividades físicas. O treinamento aeróbico é um meio efetivo para manter e melhorar as funções cardiovasculares. Além disso, desempenha um papel fundamental na prevenção e tratamento de diversas doenças crônico-degenerativas, em especial o diabete melito. OBJETIVO: Verificar os efeitos de 13 semanas de treinamento aeróbico sobre a pressão arterial, o índice de massa corpórea e a glicemia em idosas com diabete tipo 2. MÉTODOS: Onze mulheres idosas diabéticas (61,0 ± 9,1 anos de idade), sedentárias, realizaram 13 semanas de treinamento aeróbico, compondo o grupo G2. Onze idosas (60,2 ± 6,8 anos de idade) controladas não realizaram exercícios físicos durante a pesquisa, constituindo o grupo-controle (G1). O grupo G1 foi submetido somente a orientações educativas uma vez na semana, e o grupo G2, a caminhadas três vezes na semana. RESULTADOS: Houve redução significativa da glicemia e da pressão arterial diastólica nos dois grupos. Não foram encontradas reduções significativas no IMC após o treinamento aeróbico em ambos os grupos. CONCLUSÃO: Treze semanas de treinamento aeróbico foi suficiente para promover reduções significativas na pressão arterial diastólica e glicemia, portanto, esse tipo de exercício reduz os fatores de risco para doenças cardiovasculares e metabólicas.


BACKGROUND: The aging process is associated with the development of several diseases, which can be attenuated by the practice of physical activities. Aerobic training is an effective method to maintain and improve cardiovascular function. Additionally, it has a crucial role in the prevention and treatment of several chronic-degenerative diseases, especially diabetes mellitus. }OBJECTIVE: To verify the effect of a 13-week aerobic training program on blood pressure (BP), body mass index (BMI) and glycemia levels in elderly women with type-2 diabetes mellitus (DM2). METHODS: Eleven sedentary elderly women with DM2, aged 61.0 ± 9.1 years, were submitted a 13-week aerobic training program, constituting group G2. Eleven controlled elderly women (aged 60.2 ± 6.8 years) were not submitted to the aerobic training, constituting the control group (G1). G1 attended educational lectures once a week, whereas G2 walked three times a week. RESULTS: Both groups presented a significant decrease in glycemia and diastolic blood pressure levels. No significant decreases in BMI were observed after the aerobic training in either group. CONCLUSION: The 13-week aerobic training program was enough to promote significant decrease in the diastolic blood pressure and glycemia levels; therefore, this type of exercise training decreases the risk factors for cardiovascular and metabolic diseases.
