Trombosis de la vena porta e hidrocele purulento secundario a diverticulitis con peritonitis: informe de un caso y discusión / Thrombosis of the portal vein and purulent hydrocele following diverticulitis with peritonitis

Se presenta paciente de 53 años de edad con antecedentes de enfermedad diverticular colónica de dos años de evolución al diagnóstico, que ingresa por síndrome febril prolongado. Se constata en su evolución trombosis de la vena porta. A las 48 horas presenta aumento de volumen de la bolsa escrotal derecha. Una ecografía abdominal y testicular evidenció trombosis de ambas ramas de vena porta y un piohidrocele. La TC abdominal demostró además engrosamiento de las paredes del colon en fosa ilíaca izquierda con persistencia del conducto peritoneo vaginal, permeable e hidrocele. La punción testicular corroboró el diagnóstico de piohidrocele y los hallazgos intraoperatorios confirmaron la presencia de una diverticulitis colónica perforada y bloqueada con peritonitis. An unusual infected hydrocele: a case report

Of acute scrotal lesions in the neonates, cases needing emergent exploration are quite rare and differential diagnosis of them is not easy. Only a few cases of inflamed hydroceles occurring in young boys have been reported to have been caused by bacterial infection in the literature. We couldn't find any case concerning simultaneous bacterial infection in urine and hydrocele by the same organisms. We present an unusual case of infected hydrocele in a neonate with bacteriuria.

Hydrocele in filarial and non-filarial patients-histopathological, histochemical and ultrastructural studies

Twenty biopsies material of the testicular tissue were obtained from patients with unilateral hydrocele [eight due to lymphatic filariasis and twelve of idiopathic causes]. Twenty biopsies lymphatic were also obtained from the normal unaffected testes and were used as control. The data obtained from histopathological, histochemical and ultrastructural studies revealed alternation in the testicular tissue with hydrocele than in the normal control testis. In the filarial hydrocele, 37.5% of the cases showed thickening of the basement membrane and fibrosis with arrest of the spermatogenesis at the level of secondary spermatocytes. In addition, 12.5% showed thickening with interstitial fibrosis. Microfilariae were seen in hydrocele fluid 3/8 cases [37.5%]. In the idiopathic hydrocele, 50% of the testicular tissue showed marked collagen deposits and testicular atrophy with fibrosis and thickening of basement membrane. The second 50% of cases showed variable degree of maturation arrests. Explanation for these variations in the pathological and anatomical pictures in both types of hydrocele is suggested. In the filariasis hydrocele, it is due to the inflammatory response that may subside and recurrent. In the idiopathic hydrocele, it is due to the irreversible hypoxia caused by the continuous mechanical pressure due to huge amount of hydrocele fluid resulting in irreversible degenerative process with marked fibrosis

Prevençäo da hidrocele pós-varicocelectomia pela sutura da fásica cremastérica / Pós-varicocelectomy hydrocele prevention by the suture of the cremasteric fascia

Uma das intercorrências que pode ser encontrada no pós-operatório de uma cirurgia de varicocelectomia é a formaçäo de hidrocele, uni ou bilateral, devido ao trauma cirúrgico. Num trabalho retrospectivo fizemos um levantamento de 76 cirurgias de varicocelectomia, sendo 58 sem sutura da fáscia cremastérica e 18 com sutura da mesma. Do total de casos 9(11,8%) apresentaram hidrocele, de vários tamanhos, uni ou bilateral, no pós-operatório, e destes casos 1(1,3%) ocorreu com sutura da fáscia e 8 (10,6%) ocorreram sem a sutura da fáscia cremastérica. De acordo com estes dados entendemos que a sutura da fáscia cremastérica parace ser um método profilático eficiente para a prevençäo da formaçäo de hidrocele no pós-operatório da cirurgia de varicocelectomia
