RESUMEN

El xantogranuloma juvenil es un tumor benigno secundario a una proliferación de células histiocíticas que se presenta en lactantes y niños; su aparición en adultos es ocasional. Se inicia en forma repentina por la aparición de lesiones cutáneas papulonodulares rojizo anaranjadas, redondeadas, de 2 a 6 mm, que se localizan preferentemente en cara y parte superior del cuerpo. Se resuelve en forma espontánea en un período variable. Las lesiones suelen estar limitadas a la piel pero pueden tener otra localización. La ocular es una de las que puede presentar severas complicaciones que incluyen glaucoma, hipema, proptosis y amaurosis como en la paciente de 4 meses que presentamos.


Juvenile xantogranuloma (JXG) is a benign tumor due to the prolifera-tion of histiocytic cells, which appears in infants and children and occasionanlly in adults. It has a sudden onset which consists of erythematous or yellowish, papulonodular, slightly raised lesions, with a diameter varying from 2 to 6 mm. It is usually located on the face and upper part of the body, and has a spontaneous remission in a variable period of time. Although lesions are usually limited to the skin, other organs may be involved. Ocular involvement may lead to severe complications including glaucoma, hyphema, proptosis and blindness, as the four months-old patient we report.

RESUMEN

A 60-year-old diabetic patient transferred to our retina clinic for a regular follow-up for diabetic retinopathy. He had uneventful cataract surgery at the time of pars plana vitrectomy in the right eye due to diabetic retinopathy at a private ophthalmologic hospital. Six months after the surgery, neovascular glaucoma with hyphema developed in the right eye and an Ahmed valve was implanted at our hospital. Ten months after cataract surgery, we found opacification of the intraocular lens (IOL) which was causing significant visual disturbance. At the time, the best corrected visual acuity (BCVA) in the right eye was hand motion. The IOL was explanted 45 months after the operation. Five months after explantation, the BCVA was 0.06. Unfortunately, pathologic analysis was not performed. Patient-related factors such as an anterior chamber reaction caused by hyphema might have been responsible for the opacification. To our knowledge, there are no previous reports of opacification of the Akreos Adapt IOL.

RESUMEN

Blunt trauma is usually associated with severe ocular damage particularly in eyes following cataract surgery. Small, self-sealing incision cataract surgery is changing the pattern of presentation of such cases, with relatively favorable outcomes. We describe a case of blunt trauma following phacoemulsification leading to aniridia.

RESUMEN

This prospective, randomised study was performed to determine the best treatment available for mild noncomplicated traumatic hyphema. This study comprised 120 patients who fulfill the criteria of our study. The patients were admitted in our center through the eye casulty department and they were randomly divided into three equal and similar groups, one group received Predforte [registered sign] eye drops, the other received Predforte [registered sign] and Cyclogel [registered sign] 1% eye drops and the control group received Tears Natural [registered sign] eye drops. For each patient, the following characteristics were recorded at presentation: age, sex, size of hyphema, initial visual acuity [IVA], intraocular pressure [IOP] and fundus examination. The outcome, hyphema resorption time, and occurrence of complications such as rebleeding and secondary glaucoma were recorded and compared between the groups. the resorption time was almost the same [4 days] regardlless of the treatment modality the patient received. Four [3.3%] patients developed a rebleed, two [5%] in the first [steroid only] group and 2 [5%] in the third [control] group. the final visual acuities [FVA] were 0.3 log MAR on discharge, the mean log MAR is 0.1.P value of [0.04]. The cause of the decline in final visual acuity in these 4 patients was the development of traumatic cataract rather than the treatment used. A total of 14 patients [11.6%] developed a mild had FVAto moderate elevation of IOP [23-29 mmHg]; 10 [25%] in the first [steroid only] group, 2 [5%] in the second [steroid cyclo] group and 2 [5%] in the third [control] group. In all cases, the IOP returned to normal either without treatment or with short term Timolol eye drops. In cases of mild simple traumatic hyphems not exceeding 50%, simple lubricating drops probably is most efficient and safe treatment. Using other such as Predforte and cyclopentolate did have any addional beneficiary effect

RESUMEN

Air-bags have received widespread support as an effective means of enhancing automotive safety, and they are becoming more common as standard automobile equipment on many cars. Although air-bag induced ocular injuries are rare, they present a serious concern because of the possibility of permanent damage or visual impairment. To date, most reports have investigated ocular injury from high velocity motor vehicle accidents and reports of ocular injury from low speed motor vehicle accidents have been rare. We describe a patient who sustained severe ocular injury, including periorbital fracture, hyphema, vitreous hemorrhage, and choroidal rupture of the macular area, due to an inflated air-bag in a low speed motor vehicle accident.

An unusual complication following extracapsular cataract extraction with implantation of an intraocular lens is reported. One day after removal of the sutures three months after the original surgery, the patient returned with blood within the capsular bag. This resorbed, on conservative treatment within five days

RESUMEN

En un período de 6 meses estudiamos 11 casos de hipema traumático y tratamos de establecer su causa. La mayoría de ellos fueron accidentes de trabajo en personas jovenes que no tenían protección adecuada para el trabajo que estaban realizando. Todos los pacientes recibieron tratamiento médico y en tres de ellos fue necesario tambien paracentesis y drenaje de la camara anterior

RESUMEN

Late complications following penetrating keratoplasty are multiple. They include, among others, cataract, glaucoma, graft vascularization, suture abscess, graft rejection, epithelialization of the anterior chamber, high astigmatism, transmission of donor disease to recipient, recurrence of original host pathology in the graft, vitreous adhesion and sequelae and cystoid macular edema. We treated a patient who developed a spontaneous active bleeding in the anterior chamber six months after corneal transplantation

RESUMEN

Se realizó un estudio retrospectivo de egresos hospitalarios por hifema traumático durante 1988 ocurridos en la Asistencia Pública de Santiago. Se analizaron algunas características epidemiológicas y evolución clínica. Se registraron 158 egresos, el 84,2% correspondió al sexo masculino, predominando como grupo etario los menores de 18 años con un 53,7%. El 79,8% fueron hifemas menores de un tercio de cámara, y el 94,3% del total fueron catalogados como primarios al momento del ingreso. Predominaron los causados por proyectiles. Un 79,1% evolucionó sin complicaciones, se observó un 3,8% de resangramiento y sólo un 3,2% requirió de tratamiento quirúrgico. Desarrollaron complicaciones más frecuentes aquellos pacientes menores de 5 años de edad, con latencia de hospitalización mayor, con hifemas mayores de media cámara e hifemas secundarios
