RESUMEN

Botriomicose é infecção bacteriana crônica, granulomatosa, supurativa, que afeta a pele e os tecidos subcutâneos. Caracteriza-se por lesão cutânea com múltiplas fístulas, que drena secreção purulenta, na qual se encontram grãos branco-amarelados, PAS-positivos. O principal agente etiológico é o Staphylococcus aureus, e outros germes menos frequentes são Pseudomonas aeruginosa, Escherichia coli, isolados ou associados. Os autores relatam um caso da doença, com lesões exuberantes de consistência fibrosa e múltiplos orifícios fistulosos dos quais drenavam grãos branco-amarelados, localizadas na perna direita e provocando aumento de volume do membro, que tiveram resolução com sulfametoxazol- trimetoprim.


Botryomycosis is a chronic, granulomatous, suppurative bacterial infection involving the skin and subcutaneous tissues. The skin lesion is characterized by multiple fistulae draining purulent secretion with white-yellowish grains PAS positive. The main etiologic agent is Staphylococcus aureus and other less frequent pathogens are Pseudomonas aeruginosa and Escherichia coli, isolated or associated. The authors report a case of this disease with exuberant lesions on the right leg and increased limb volume, fibrous consistency and multiple fistula orifices draining white-yellowish grains. Resolution was achieved with sulphamethoxazole-trimethoprim.

RESUMEN

La infección por Streptococcus dysgalactiae subsp equisimilis es causa frecuente de faringitis, artritis e infecciones de piel y tejidos blandos; excepcionalmente se ha descrito como causa de endocarditis infecciosa. Se presenta el caso de un paciente masculino de 42 años quien ingresa al Hospital Vargas de Caracas con fiebre, astenia, dolor en hombro derecho con limitación funcional y dolor en hipocondrio derecho; a las 48 horas se evidencia soplo mesosistólico en foco mitral, hemorragias en astillas y lesiones de Janeway, por lo que se plantea Endocarditis Infecciosa, y se inicia tratamiento con Vancomicina + Gentamicina. Los hemocultivos muestran crecimiento de Streptococcus equisimilis en ambos frascos; el ecocardiograma transtorácico evidencia una vegetación en valva posterior de válvula mitral. Luego de 48 horas de inicio de tratamiento remite la fiebre. El ecocardiograma control a los 14 días mostró la desaparición de la vegetación en la válvula mitral. El paciente recibió tratamiento por 6 semanas


Streptococcus dysgalactiae subsp equisimilis is frequent in patients with pharyngitis, arthritis and skin infections; it rarely causes infective endocarditis. We describe a 42 year-old male patient complaining of fever, malaise, right shoulder pain and right upper quadrant pain who was admitted to the Hospital Vargas, Caracas, Venezuela. After 48 hours his physical examination revealed mesosystolic murmur at the apex, splinter hemorrhages and Janeway lesions, suggestive of Infective Endocarditis Blood cultures identified Streptococcus equisimilis; transthoracic echocardiogram revealed a vegetation over the posterior leaflet of mitral valve; the patient was treated with Vancomycin + Gentamycin. After 48 hours of treatment the fever subsided. A transthoracic echocardiogram after 14 days showed no vegetations. The patient was treated during 6 weeks

RESUMEN

The purpose of this work was to evaluate the frequency of Malassezia pachydermatis infection and other infectious agents in dogs with external otitis and with healthy auditory tubes. Clinical manifestations of external otitis were evaluated. Samples from the auditory tube of 102 dogs with otitis and from 32 healthy dogs were submitted to direct microscopic examination and cultured in blood agar and Sabouraud dextrose agar with chloramphenicol and cycloheximide. Direct examination showed more than ten cells of M. pachydermatis in 52.0per cente of the samples from dogs with otitis, but in only 21.8per cent of the healthy auditory tube samples. M. pachydermatis was isolated in 37.5per cent of the samples from dogs with healthy auditory tube and 76.5per cent (p<0.01) of the samples from dogs with otitis. There was an association between M. pachydermatis and Staphylococcus aureus (p<0.01), but not with Pseudomonas aeruginosa (p>0.05). Infection by M. pachydermatis was prevalent in the following breeds: Cocker Spainel, German Shepherd and Brazilian Fila. No differences were found in frequency of the infection in relation to age, sex and ear anatomy of the dogs. Otomycosis were predominantly ceruminous and erythematous. M. pachydermatis was the most frequent agent in external otitis.

RESUMEN

É relatado um caso de botriomicose em mulher de 46 anos. A lesäo, com evoluçäo de 12 anos, localizada no braço, tinha aspecto ulcerovegetante. A histopatologia evidenciou gräos contendo cocos Gram-positivos. O cultivo deu crescimento a Staphylococcus aureus. Houve resposta satisfatória à antibioticoterapia
