RESUMEN Paciente de 56 años quien acudió a la consulta por disminución de la visión, dolor y enrojecimiento ocular izquierdo, con antecedentes patológicos personales de hipertensión arterial esencial y antecedentes patológicos oculares de trauma ocular con fragmento de madera al cortar leña hace 37 años. El cuerpo extraño penetró hasta la cámara anterior y permaneció intracorneal, ya que en aquel momento no pudo ser removido en su totalidad. Se describe la técnica quirúrgica empleada, sobre la cual la información revisada ha sido escasa referente a los procederes quirúrgicos de remoción de cuerpos extraños semejantes, por lo que la técnica descrita debe contribuir a enfrentar futuros casos similares(AU)


ABSTRACT A 56-year-old male patient attends consultation with left eye vision reduction, pain and redness. The patient has a history of essential hypertension and eye trauma caused by a wood splinter while chopping wood 37 years ago. The foreign body went in as far as the anterior chamber and remained intracorneal, since at that time it could not be totally removed. A description is provided of the surgical technique used. Scant information was found about surgical procedures to remove similar foreign bodies. Therefore, the technique herein described should be useful to treat analogous cases(AU)

RESUMEN

Ocular trauma is an important cause of vision loss. The agents incriminated in such injuries are diverse. We present a case of ocular trauma with a metallic spoon causing deep laceration of lid and temple region with sclerocorneal laceration. After assessment of the general condition and stabilization of the systemic parameters the operative procedure was undertaken on elective basis. Though the final visual outcome was not rewarding due to the severity of the injury, any potential hemostatic catastrophe was averted.

RESUMEN

The purpose of this article is to report a case of an asymptomatic intralenticular metallic foreign body that was retained for 6 months. A 66-year-old male visited our ophthalmology department because of decreased visual acuity in his left eye 6 months after he suffered ocular trauma while mowing. He had not been treated because he did not experience any discomfort. His corrected visual acuity was 0.4. Central corneal opacity, an intralenticular metallic foreign body, and an intact posterior capsule were observed on slit lamp examination. Phacoemulsification with posterior chamber lens implantation and simultaneous removal of the intralenticular foreign body was performed. Seventeen days after the operation, his corrected visual acuity was 1.0, the intraocular lens was well-seated, and there was no intraocular inflammation. In this case report, a patient was found to have an intralenticular metallic foreign body retained for 6 months. During this time he did not experience any ocular dysfunction due to the foreign body. Mowing accidents are common in Korea. Despite the absence of symptoms, patients reporting a history of lawn mowing should be thoroughly examined.

RESUMEN

The aim of this study is to define the epidemiologic characteristics and clinical profile and evaluation of final visual outcome of corneal scleral laceration. A prospective survey was conducted over a two year period from March 2000 to March 2002 on 79 consecutively admitted patients who underwent corneal scleral repairment at Yazd Eye Hospitals. This study included 79 eye injuries. Fifty nine of cases [74.7%] occurred in males and 20 cases [25.3%] in females. Mean age was 17.08 years. Left eye was more commonly involved [57%]. Initial VA was an important predictor of final VA. Good initial VA > 20/200 leading to excellent final VA. 40.6% of traumas occurred in the first decade of life. Injuries less than 10 mm and related to zone I and II predicted a good visual outcome. In this study endophthalmitis [5.1%] was much less likely to develop in eyes with primary repairment within 24 hours from the injury [1.5%] than in eyes being repaired after 24 hours [21.4%] and the difference was significant [P==0.016]. Significant predictive factors for final VA after corneal scleral injury include: initial VA, wound length, wound and location,, and the time of primary repair

RESUMEN

Fishing is recreational activity enjoyed by millions of people worldwide. Although uncommon in developing countries it is a popular pastime in the Western world. and, given this popularity, a number of associated personal injuries are probably inevitable. We report a case of fish hook injury to the eyelid and discuss the various techniques available for removal of fish hooks. The terminologies associated with various parts of the fish hook are also illustrated.

RESUMEN

A case of bilateral accidental globe penetration during administration of retrobulbar steroid for bilateral optic neuritis is discussed. One eye with bisected macula was managed successfully by vitrectomy, internal gas tamponade, and postoperative laser to the edges of the retinal tear. The fellow eye was blind due to central retinal artery occlusion.

RESUMEN

PURPOSE: To analyze the clinical presentation and outcome of treatment for globe perforation secondary to peri-bulbar anaesthesia. METHODS: Eight patients (3 females and 5 males) aged 66-84 years were included in the study. Ocular perforations were suspected in 3 cases before or during surgery, in 4 cases diagnosis was established within one week and in one case at 3 weeks. Three patients underwent indirect argon laser photocoagulation to seal the retinal break, one patient had cryotherapy, 3 patients underwent a pars plana vitrectomy with fluid gas exchange and endo-laser; and one patient refused any further treatment. RESULTS: The final visual acuity after a mean follow up of 14 months was better than 6/9 in 2 patients, between 6/9-6/12 in 4 patients, and perception of light in 2 patients. CONCLUSION: If diagnosed early and treated adequately, a majority of patients with globe perforation during periocular anaesthetic could be saved.

RESUMEN

In order to determine the prognosis of perforating eye injuries caused by BB guns in our patients the visual and anatomic results of 14 patients with gun injuries seen between September 1996 and February 1998 in Farabi Hospital and private office in Tehran were reviewed. Five patients had nonperforating eye injuries. All perforated eyes underwent scleral buckling lensectomy vitrectomy and silicone injection. All injured eyes had a visual acuity of light perception or better at presentation. Among 9 cases of perforating injuries 7 patients had double and 2 had single perforation. All patients had a final visual acuity of hand motion or better 7 patients achieved a visual acuity of >/= 1/10 and 5 >/= 3/10> This study shows that in some ocular injuries caused by BB guns vision may be retained with vitreo-retinal surgeries

RESUMEN

Foram estudadas 42 perfurações do globo ocular. Apenas um paciente apresentou trauma binocular. Identificamos com facilidade os fatores etiológicos. Como atendimento primário, a maioria foi levada a tratamento cirúrgico o mais rápido possível.


The authors have studied 42 perforating ocular injuries. Only one patient presented bilateral trauma. As the primary care, the majority undergone surgical repair as soon as possible. Preventive maesures are discussed.

RESUMEN

A retrospective study was done of fifty cases with penetrating eye injuries treated at Al Shifa Eye Hospital, Rawalpindi presenting between August, 1992 and August, 1994. Out of the fifty cases, 36 [72%] were male and 14 [28%] female. There were 29 [58%] children in this study. The number of lost eyes were 14 [28%]. Men follow up duration was 4.7 months with a range of 01 month-20 months. The final vision was 6/12 or better in only 6 cases. Poor prognostic factors in this study were poor vision on presentation i.e. perception of light only, large wound size, corneal wounds, vitreous haemmorrhage, retinal detachment, uveal prolapse, and infection [endophthalmitis]. While increased time interval between injury and treatment statistically did not affect the final vision, poor follow up was an important negative factor in this study regarding the final vision
