RESUMEN
The standard treatment of tympanic membrane perforation is myringoplasty. We describe a new myringoplasty technique requiring only a partial removal of skin on the ear drum remnant followed by lateral placing of fascia and free skin graft. The overall cure rate in our series is 97 per cent. The hearing gains for the overall cases are 13.9 dB (p < 0.001) for the AC, 13.6 dB (p < 0.001) for the air bone gap closure, 13.1 dB (p < 0.001) for the SRT and 4.2 per cent (p = 0.005) for the SDS. Minor complications are observed in only 14 per cent. Our technique is simple, effective and can be performed on an out-patient basis.

Se presenta un caso clínico correspondiente a un paciente de sexo masculino, de 20 años que, luego de ser operado de una timpanoplastía de ambos oídos por una otitis media crónica con resultado auditivo satisfactorio, comienza con hipoacusia de conducción progresiva bilateral. Esto obliga a una revisión de la cirugía y se encuentra una osificación por sobre el neotímpano el cual aparece indemne. Se realiza una revisión de la literatura sin encontrar un caso similar al descrito. Se plantean algunas interrogantes de los autores

Pre and postoperative hearing functions of 30 patients with successful myringoplasties were compared. A preoperative screening examination as well as an audiological evaluation were done. The tympanic cavities were thoroughly explored during the operation. A postoperative clinical and audiological follow-up for 3 months was carried out for ears with successful myringoplasties. Pure tone audiometry, tympanometry, and stapedial reflex were encompassed. A comparison was held between the clinical data and the different audiological measurements. The size and site of the perforation as well as the technique of tympanoplasty affect postoperative compliance but does not affect postoperative air-bone gap and hearing gain. Compliance should not be taken as an absolute criteria to a corresponding wide air-bone gap unless it is associated with an intratympanic pathology. Absence of stapedial reflex can be associated with normal middle ear function and normal sensory neural function
