Parvovirus B19 presenting with persistent pancytopenia in a patient of T-ALL post induction chemotherapy diagnosed on bone marrow examination.

Manifestations of parvovirus B19 vary even in the normal host from asymptomatic or subclinical infection to a spectrum of illness with symptoms during viremic and immune complex mediated stage of disease. We report the morphological findings of parvovirus B19 infection (confirmed on serology) in a patient of T-acute lymphoblastic lymphoma (T-ALL) who underwent induction phase of chemotherapy (MCP 842 protocol). Persistent pancytopenia in the bone marrow aspirate with mild increase in blasts was thought to be due to failure to achieve marrow remission. However, giant pronormoblasts with prominent intranuclear inclusions confirmed on trephine biopsy led to the suspicion of parvovirus B19 infection which was later confirmed on serology. This case is presented to report the rarely seen classical morphological feature of parvovirus infection on bone marrow examination which was incidentally the first investigation to diagnose the viremic phase of the infection, indicating that a high index of suspicion needs to be kept in mind while examining bone marrows of susceptible patients.

Manifestações reumáticas associadas à parvovirose / Rheumatic manifestations associated with parvovirus infection

O parvovírus B19 pode levar a diferentes quadros clínicos nos pacientes por ele infectados. Ainda que a maioria dos casos apresente-se assintomática ou oligossintomática, o espectro do acometimento clínico inclui a hidropsia fetal, o eritema infeccioso, crises aplásicas em pacientes com anemia hemolítica e anemia crônica naqueles imunodeprimidos, além de artropatia. Esta geralmente é autolimitada, porém vêm sendo descritos quadros que evoluem para cronicidade. Mais recentemente, tem sido estudado o papel do parvovírus B19 na gênese de várias enfermidades reumáticas, como a artrite reumatóide (do adulto e juvenil), o lúpus eritematoso sistêmico, algumas vasculites e fibromialgia. É pois, a infecção pelo parvovírus B19,assunto extremamente importante para o reumatologista, devendo tal infecção ser lembrada e pesquisada por intermédio de sorologia apropriada.

El efecto que ocasionan los variados gérmenes sobre el sistema hematológico en el organismo reconoce dos aspectos diferentes. Por una parte, y es lo que ocurre con mayor frecuencia, la acción no es muy severa, y si bien, por ejemplo, se pueden observar alteraciones en el hemograma, que permite con frecuencia tener una orientación diagnóstica, no se aprecian efectos nocivos. Diferente es lo que acontece en otros casos, en que además de lo señalado, el efecto sobre la hematopoyesis es severo dando origen a alguna patología, la acción de los gérmenes en el organismo provoca respuestas múltiples y complejas que comprometen diversos sistemas ocasionando con ello, alteración de sus funciones. La infección con cierta frecuencia da como resultado cambios, a veces severos, en el sistema hematológico. Cualquiera que sea su origen, puede perturbar la hematopoyesis en forma global en sus tres sectores o indistintamente a la serie roja, a la blanca o a las plaquetas en forma aislada. Las células sanguíneas son comúnmente afectadas en grado variable por las infecciones, en forma directa pero en la mayoría de las veces no se conoce hasta que punto esto ocurre. La rubéola, por ejemplo es un conocido causante de trombocitopenia, pero el porcentaje en que esto ocurre no se conoce. Existen así mismo, diversas bacterias que causan hemólisis, pero que raramente provocan manifestaciones clínicas evidentes

Study of the role of parvo virus B19 in arthropathies of egyptian adult cases

This work was done to assess the relation between HPV B19 infection and arthropathies in Egyptian adults cases. For this purpose 40 rheumatoid arthritis [RA] cases, 10 osteoarthritis cases and 10 systemic lupus erythematosus [SLE] cases were selected to represent different types of arthropathies. The selection of cases was based on clinical diagnosis and laboratory tests [ESR, Hb level, Rose waller, detection of antinuclear antibodies and detection of hidden rheumatoid factor]. HPV B19 IgM and IgG were searched for by ELISA test in their sera as indicator of the state of HPV B19 infection. A control group was also included in this study, as 30 healthy persons with no previous complaint of rheumatic symptoms. HPV B19 IgM was detected in 60% of patients with RA, 40% of osteoarthritis cases and none of the SLE cases. Positive cases were more among females with long duration of illness affecting both big and small joints. These results indicated a possible causal association between acute HPV B19 and arthropathy. As regards HPV B19 IgG, it was detected in 57% of RA cases, 60% of osteoarthritis cases and 40% of SLE. The difference in IgG was statistically insignificant from the control group [46.7%]. The presence of HPV B19 IgG antibodies indicates previous exposure to the virus but does not indicate its time. Detection of HPV B19 IgM or rising titer of HPV B19 IgG may lead to early diagnosis of HPV B19 arthropathies
