Introduction: Endo-periodontal lesions can lead to the formation of severe intraosseous defects in the periodontium; which can lead to tooth loss. Objective: Demonstrate that surgical periodontal therapy with the help of gelatin sponges, with colloidal silver nanoparticles, restores bone tissue lost in endo-periodontal lesions. Case presentation: 55-year-old male patient without systemic alterations, diagnosed with grade 3 endo-periodontal lesion in patients with periodontitis. It was treated first with ducts and then with periodontal surgery combined with gelatin sponges, which contain colloidal silver nanoparticles, and were placed filling the 2-wall bone defect involving > 80 percent of the root length, with 24 months of radiographic and clinical follow-up. Conclusions: Based on the case report, surgical periodontal therapy and filling of bone defects with gelatin sponges, which contain colloidal silver nanoparticles, were sufficient to restore the lost bone at a 24-month follow-up. However, further studies are needed to assess the clinical benefit of this material for the treatment of intraosseous defects(AU)

RESUMEN

El diagnóstico molecular, mediante la aplicación de técnicas molecu- lares, ha permitido estudiar microorganismos presentes en el inicio y progresión de la caries dental, enfermedad periodontal, y en los fracasos endodónticos. Las técnicas moleculares permiten la detección y cuan- tificación del material genético del ácido desoxirribonucleico (DNA), ácido ribonucleico (RNA) o proteínas, lo que posibilita el estudio del genoma completo o secuencias de DNA específicas. Estas técnicas surgen como una necesidad de detectar microorganismos de difícil o lento crecimiento en cultivos; la técnica más utilizada es la reacción en cadena de la polimerasa (PCR) que permite la amplificación de peque- ños segmentos de material genético al utilizar cebadores, por lo que es un método económico, preciso, sensible y rápido para la detección de microorganismos. El presente artículo de revisión bibliográfica servirá para informar sobre los avances de las técnicas moleculares utilizadas para el diagnóstico en la práctica odontológica (AU)


Molecular diagnosis, through the application of molecular techniques, has made it possible to study microorganisms present in the onset and progression of dental caries, periodontal disease, and endodontic failures. Molecular techniques allow the detection and quantification of the genetic material of deoxyribonucleic acid (DNA), ribonucleic acid (RNA) or proteins, allowing the study of the complete genome or specific DNA sequences, they arise as a need to detect difficult or slow growing microorganisms in cultures. The most widely used technique is the polymerase chain reaction (PCR) that allows the amplification of small segments of genetic material using primers, it is an economical, precise, sensitive and fast method for the detection of microorganisms. This bibliographic review article will serve to report on the advances in molecular techniques used for diagnosis in dental practice (AU)

Introducción: La enfermedad periodontal comprende un conjunto de trastornos asociados a la inflamación y a la pérdida de estructuras del soporte dental de origen infeccioso multifactorial. La investigación científica expone un abanico de cepas bacterianas probióticas en el tratamiento periodontal, con propiedades en la inhibición del crecimiento de periodontopatógenos, y mejoras significativas en los parámetros clínicos, cuando se combina con el tratamiento periodontal mecánico. Objetivo: Describir el efecto de la terapia combinada entre el tratamiento periodontal mecánico tradicional y un gel probiótico multicepa, aplicado a un caso de periodontitis. Presentación de caso: Paciente femenino de 53 años de edad, procedente de la comunidad de El Portachuelo, zona rural aledaña a la ciudad de Mérida. Presentó agrandamiento gingival, enrojecimiento y dolor espontáneo en la encía palatina, con concomitantes de sangrado y exudado purulento desde hace seis meses. La realización de un examen clínico periodontal, análisis radiográfico y análisis microbiológico permitieron diagnosticar periodontitis estadio IV grado B. Se aplicó tratamiento periodontal convencional no quirúrgico en combinación con cepas probióticas de Lactobacillus rhamnosus, L acidophilus y L. plantarum, en presentación de un gel adhesivo para la aplicación supra y subgingival y en suspensión vía oral para obtener un efecto sistémico durante 45 días. Los resultados obtenidos fueron favorables con mejoramiento de los parámetros clínicos como índice de sangrado periodontal y profundidad de los sacos periodontales. Conclusiones: En concordancia con diversos estudios, se ha demostrado que la terapia probiótica puede ser usada como tratamiento coadyuvante en casos de periodontitis. Se observa una franca y rápida mejoría al utilizar terapias multicepas(AU)


Introduction: Periodontal disease comprises a set of disorders associated with inflammation and the loss of dental support structures of multifactorial infectious origin. Scientific research exposes a range of probiotic bacterial strains in periodontal treatment, with properties in inhibiting the growth of periodontal pathogens, and significant improvements in clinical parameters, when combined with mechanical periodontal treatment. Objective: Describe the effect of combined therapy between traditional mechanical periodontal treatment and a multi-strain probiotic gel, applied to a case of periodontitis. Case presentation: A 53-year-old female patient from the community El Portachuelo, a rural area next to the city of Mérida. She presented gingival enlargement, redness and spontaneous pain in the palatine gum, with concomitant bleeding and purulent exudate for six months. A clinical periodontal examination, radiographic analysis and microbiological analysis made it possible to diagnose stage IV grade B periodontitis. Conventional non-surgical periodontal treatment was applied in combination with probiotic strains of Lactobacillus rhamnosus, L. acidophilus and L. plantarum, in presentation of an adhesive gel for supra and subgingival application and in oral suspension to obtain a systemic effect for 45 days. The results obtained were favorable with improvement of clinical parameters such as periodontal bleeding index and depth of periodontal sacs. Conclusions: In accordance with several studies, it has been shown that probiotic therapy can be used as an adjuvant treatment in cases of periodontitis. A clear and rapid improvement is observed when using multi-strain therapies(AU)

Objetivo: Avaliar a frequência de hipertensão arterial sistêmica e sua associação com parâmetros periodontais e do índice de dentes cariados, perdidos, obturados (CPOD) em indivíduos atendidos na clínica do Curso de Odontologia da Unigranrio. Materiais e métodos: Noventa e cinco indivíduos de ambos os sexos foram incluídos no estudo entre março e maio de 2021. Os participantes responderam a questionários anamnésicos, tiveram sua pressão arterial sistêmica aferida e foram examinados para obtenção do índice CPOD e de parâmetros periodontais. Resultados: A frequência de hipertensão arterial sistêmica na população estudada foi 23,15%. Em pacientes com periodontite, essa prevalência foi 27%, e, em pacientes com gengivite, 19%. Pacientes com periodontite tiveram médias superiores de pressão arterial sistólica (PAS) e diastólica (PAD) comparado ao grupo com gengivite (p<0,0001). Foram identificadas correlações significativas positivas entre PAS e bolsas periodontais moderadas (rho=0,356) e profundas (rho=0,342), perda de inserção avançada (rho=0,538), CPOD (rho=0,365) e quantidade de dentes ausentes (rho=0,477), p < 0,001. A PAD apresentou correlações significativas (p < 0,001) positivas com bolsas periodontais moderadas (rho=0,331) e profundas (rho=0,283), perda de inserção avançada (rho=0,465), CPOD (rho=0,361) e dentes ausentes (rho=0,348). Conclusões: A frequência de hipertensão arterial sistêmica autorrelatada é relativamente alta na população estudada e, em especialmente, dentre as pessoas com periodontite. Pressão arterial sistólica e diastólica são correlacionadas positivamente com parâmetros periodontais indicadores de severidade de doença, assim como piores escores do CPOD.


Aim: The study evaluated the prevalence of systemic arterial hypertension and its association with periodontal parameters and decayed-missing-filled teeth (DMFT) index in individuals treated in a Dental School clinic. Material and methods: Ninety-five individuals of both genders were included in the study in the period between March and May 2021. All participants answered anamnestic questionaries, had their systemic blood pressure measured, and were examined to obtain the DMFT and periodontal parameters. Results: The prevalence of systemic arterial hypertension in the study population was 23.15%. In periodontitis individuals, that prevalence was 27%, and, in gingivitis patients, 19%. Patients with periodontitis have higher mean systolic (SBP) and diastolic (DBP) blood pressure compared with gingivitis individuals (p<0.0001). Significant positive correlations were found between SBP and moderate (rho=0.356) and deep (rho=0.342) periodontal pockets, severe attachment loss (rho=0.538), DMFT (rho=0.365), and amount of missing teeth (rho=0.477), p ? 0.001. The DBP showed significant (p?0.001) positive correlations with moderate (rho=0.331) and (rho=0.283) deep periodontal pockets, severe attachment loss (rho=0.465), DMFT (rho=0.361), and missing teeth (rho=0.348). Conclusions: The prevalence of systemic arterial hypertension is relatively high in the study population and, in particular, among individuals with periodontitis. Systolic and diastolic blood pressure are positively correlated with periodontal parameters that indicate the severity of disease, as well as with worse DMFT scores.

Resumen La periodontitis es una enfermedad multifactorial caracterizada por una respuesta inflamatoria desarrollada por el huésped frente a los microorganismos de la biopelícula bacteriana. El proceso localizado en la cavidad oral puede influir en el huésped a nivel sistémico provocando resultados adversos en el embarazo como parto prematuro, hipertensión materna y bajo peso del niño al nacer. Objetivo: Analizar el estado actual del conocimiento sobre los mecanismos probables que vinculan la periodontitis con resultados adversos en el embarazo. Material y métodos: Se efectuó una revisión de la literatura y una búsqueda de artículos publicados durante el periodo comprendido entre los años 2016 y 2021 en las bases de datos MEDLINE, PUBMED, SciELO, LILACS, idioma inglés y/o español. Las búsquedas se realizaron utilizando los siguientes términos: Periodontitis, mediadores de inflamación, complicaciones del embarazo, parto prematuro. Resultados: Se analizaron un total de 24 publicaciones, incluidas revisiones de la literatura, revisiones sistemáticas, estudios de cohorte, estudios de casos y controles y estudio observacionales, que cumplieron con los criterios de inclusión. Conclusión: La periodontitis es un factor de riesgo evitable por lo que se recomienda intensificar las medidas de prevención y tratamiento, en embarazadas y en mujeres en edad fértil.


Abstract Periodontitis is a multifactorial disease characterized by an inflammatory response developed by the host against the microorganisms of the bacterial biofilm. The localized process in the oral cavity can influence the host systemically causing adverse pregnancy outcomes such as premature delivery, maternal hypertension, and low birth weight. Objective: To analyze the current state of knowledge about the probable mechanisms that link periodontitis with adverse pregnancy outcomes. Material and methods: A literature review and a search for articles published during the period between 2016 and 2021 was carried out in the MEDLINE, PUBMED, SciELO, LILACS, English and / or Spanish databases. Searches were conducted using the following terms: Periodontitis, inflammatory mediators, pregnancy complications, preterm delivery. Results: 24 publications were analyzed, including literature reviews, systematic reviews, cohort studies, case-control studies, and observational studies, which met the inclusion criteria. Conclusion: Periodontitis is an avoidable risk factor, therefore it is recommended to intensify prevention and treatment measures in pregnant women and women of childbearing age.

To verify the presence of periodontitis, its severity, and their association with associated factors based on medical records of patients who attended the Dental Clinic of the Faculty of Southern Brazil over 4 years. This is a cross-sectional study, which included 422 medical records of patients aged ≥ 18 years. The clinically analyzed data were: plaque index (PI),bleeding on probing (BOP), probing pocket depth (PPD), and clinical attachment level (CAL). Sociodemographic data, dental hygiene, harmful habits and chronic systemic diseases were described. Statistical analysis of binary logistic regression was used to verify the relationship between the severity of periodontitis and the exposure variables. The older adults [odds ratio (OR) = 2.36; 95% confidence interval (CI): 1.23-4.54 ­41 to 55 years and OR = 3.0; 95% CI: 1.49-6.09 ­56 to 87 years], and men (OR = 1.9; 95% CI: 1.18-3.14) showed higher chances of periodontitis severity. Smokers (OR = 3.54; 95% CI: 2.05-6.12), those with hypertension (OR = 2.11; 95% CI: 1.23-3.63) and with diabetes (OR = 2.10; 95% CI: 1.08-4.12) showed higher chances of developing advanced periodontitis. Advanced or severe periodontitis occurred in one-third of the patients. The findings showed that men, older adults, with systemic arterial hypertension, diabetes mellitus, and smokers are more susceptible to severe periodontitis.

El objetivo de este trabajo fue establecer la relación entre el grado de enfermedad periodontal y el control metabólico de la glucemia en pacientes que padecen diabetes tipo 2. Es un estudio de tipo observacional, descriptivo, transversal. Se seleccionaron 42 pacientes con diagnóstico de diabetes mellitus tipo 2 de ambos sexos, entre 25 y 75. Se realizó odontograma y se evaluó el grado de enfermedad periodontal para determinar el grado de compensación metabólica de los pacientes se tomaron muestras de sangre venosa y se midieron los valores de hemoglobina glicosilada. Un 23,55% de los pacientes no presentaron enfermedad periodontal y el resto se dividió en leve, moderada y severa presentándose la enfermedad leve en primer orden. Los pacientes con mayor grado de enfermedad periodontal presentaron mayor número de piezas dentarias perdidas (p 0,0334). La enfermedad periodontal se distribuye de la misma manera en ambos grupos de control glucémico (p=0,1211). En el grupo estudiado, el grado de enfermedad periodontal es independiente del control glucémico de los pacientes (AU)


The objective of this work was to establish the relationship between the degree of periodontal disease and the metabolic control of glycemia in patients with type 2 diabetes. It is an observational, descriptive, cross-sectional study. 42 patients with a diagnosis of type 2 diabetes mellitus of both sexes, between 25 and 75, were selected. An odontogram was performed and the degree of periodontal disease was evaluated; venous blood samples were taken and glycosylated hemoglobin values were measured to determine the degree of metabolic compensation of the patients. 23.55% of the patients did not present periodontal disease and the rest were divided into mild, moderate, and severe, with mild disease presenting in the first order. The patients with a higher degree of periodontal disease had a higher number of missing teeth (p 0.0334). Periodontal disease is distributed in the same way in both glycemic control groups (p = 0.1211). In the group studied, the degree of periodontal disease is independent of the glycemic control of the patients (AU)

Resumen El objetivo de este trabajo fue estudiar la relación entre la concentración de mucina salival y la enfermedad periodontal. La muestra se dividió en tres grupos de 20 individuos cada uno: Grupo 1 sin enfermedad periodontal; Grupo 2 con gingivitis; y Grupo 3 con periodontitis. En todas las muestras salivales se confirmó la presencia de mucina, el Grupo 1 presentó un valor promedio de 1,27 mg/ml. En el Grupo 2 se registró un promedio de 1,93 mg/ml. En el Grupo 3 se observó un promedio de 3,01 mg/ml. El Análisis de la Variancia y posterior prueba de F (F = 25,01, p < 0,0001) confirman diferencias significativas en los contenidos de mucina entre grupos. El aumento de la concentración de mucina salival en pacientes periodontales podría representar un marcador químico de utilidad como coadyuvante en el diagnóstico clínico de esta enfermedad.


Resumo O objetivo deste trabalho foi estudar a relação entre a concentração de mucina salivar e a doença periodontal. A amostra foi dividida em três grupos de 20 indivíduos cada: Grupo 1 sem doença periodontal; Grupo 2 com gengivite; e Grupo 3 com periodontite. Em todas as amostras salivares foi confirmada a presença de mucina, o Grupo 1 apresentou valor médio de 1,27 mg / ml. No Grupo 2, foi registrada uma média de 1,93 mg / ml. No Grupo 3 foi observada uma média de 3,01 mg / ml. A Análise de Variância e o teste F subsequente (F = 25,01, p <0,0001) confirmam diferenças significativas nos conteúdos de mucina entre os grupos. O aumento da concentração de mucina salivar em pacientes periodontais pode representar um marcador químico útil como adjuvante no diagnóstico clínico desta doença.


Abstract This work aimed to study the relationship between salivary mucin concentration and periodontal disease. The sample was divided into three groups of 20 individuals each: Group 1 with no periodontal disease, Group 2 with gingivitis, and Group 3 with periodontitis. Mucin was detected in all the saliva samples. Group 1 had an average value of 1.27 mg/ml. Group 2 had an average value of 1.93 mg/ml. Group 3 had an average value of 3.01 mg/ml. The analysis of variance and subsequent F test (F = 25.01, p < 0.0001) confirmed significant differences in mucin content between the groups. Increased salivary mucin concentration in periodontal patients could be a useful chemical marker for the clinical diagnosis of periodontal disease.

La asociación entre Enfermedad Periodontal y Disfunción Eréctil se ha estudiado limitadamente. Sin embargo, hoy en día los estudios disponibles han reportado una posible asociación en base a factores de riesgo compartidos entre ambas patologías, pero fundamentalmente se atribuye la asociación al fenómeno de disfunción endotelial que se vincula con ambas enfermedades. Se ha propuesto que la enfermedad periodontal crónica puede inducir una respuesta inflamatoria sistémica que resulta en el deterioro de las condiciones fisiológicas y bioquímicas del endotelio generando disfunción endotelial. Por otro lado, se reconoce a la disfunción eréctil como una enfermedad de origen multifactorial, en la que prevalece la condición vasculogénica como el factor etiológico más frecuente, esto atribuido a un defecto vascular (disfunción endotelial). Es así como el punto de enlace más fuerte es la alteración de la función vascular, que sin duda demuestra argumento sólido de asociación entre las dos enfermedades. El objetivo de esta revisión bibliográfica es informar sobre los posibles mecanismos de asociación entre enfermedad periodontal y disfunción eréctil, enfocándose en el proceso de disfunción endotelial como el principal vínculo existente.(au)


The association between periodontal disease and erectile dysfunction has not been sufficiently studied. However, recent studies have reported a possible link based on mutually shared risks factors between both pathologies, but the association is fundamentally attributed to endothelial dysfunction, phenomenon that is related to both diseases. It has been suggested that chronic periodontal disease can induce systemic inflammatory response that results in deterioration of the physiological and biochemical conditions of the endothelium causing endothelial dysfunction. On the other hand, erectile dysfunction is a multifactorial disease, but the vasculogenic factor is considered as the most frequent one, attributed to a vascular defect (endothelial dysfunction). So, the strongest link is the vascular function alteration, which undoubtedly demonstrates demonstrates a solid argument of association between both diseases. The objective of this literature review is to report the possible mechanisms of association between periodontal disease and erectile dysfunction, focusing mainly on endothelial dysfunction as the main existing link.(au)

Abstract: Periodontal diseases are considered a worldwide public health problem, owing to their high prevalence in developed and developing countries. Periodontitis may lead to tooth loss, which can impact oral health-related quality of life. Gingivitis and periodontitis have been extensively studied regarding their etiopathogenesis, epidemiology, prevention and treatment outcomes. However, most of these aspects are studied and discussed globally, which may hamper a clear interpretation of the findings and the design of effective plans of action for specific regions or populations. For example, in Latin America, epidemiological data about the distribution of periodontal diseases is still scarce, mainly when it comes to nationwide representative samples. This Consensus aimed to address the following topics related to periodontal diseases in Latin America: a) The impact of the global burden of periodontal diseases on health: a global reality; b) Periodontal diseases in Latin America; c) Strategies for the prevention of periodontal diseases in Latin America; d) Problems associated with diagnosis of periodontal conditions and possible solutions for Latin America; e) Treatment of Periodontitis. This consensus will help to increase awareness about diagnosis, prevention and treatment of periodontal diseases, in the context of Latin American countries.

Abstract Objective: To evaluate periodontal tissue condition on systemic lupus erythematosus (SLE) patients and its characteristics. Material and Methods: This descriptive and cross-sectional study involved 61 SLE patients. Clinical examination of the oral cavity was performed using periodontal index (PI), gingival index (GI), clinical attachment loss (CAL) and number of loose teeth. Also, we evaluated SLE duration, treatment duration, ethnics, marital status, educational background, family income, and occupation. Results: In the evaluation of periodontal tissue, 93.4% had bleeding on probing, 80.3% clinical attachment loss, and 16.3% loose teeth. A total of 54 patients (88.5%) with SLE had periodontitis. Seven subjects had no periodontitis, 11 mild periodontitis, 29 moderate periodontitis and 14 severe periodontitis. Mean Periodontal Index score, Gingival Index, Clinical Attachment Loss (mm), and the number of mobility teeth, Plaque Index and Calculus Index respectively were 2.66 ± 1.20, 1.95 ± 1.02, 0.75 ± 0.59 mm, 1,49 ± 1.77. There was a significant difference in periodontal index score, shown periodontitis between employment and unemployment subjects (p=0.004) and a moderate correlation between periodontitis and occupation. Conclusion: Periodontitis found as manifestations SLE patients, followed by bleeding on probing and loose teeth. Its characteristics is playing a role in periodontitis in SLE patients.

A endocardite infecciosa (EI) corresponde a uma infecção microbiana do endocárdio com presença de coágulo de plaquetas e fibrina em seu interior. Quando é de origem periodontal, a Aggregatibacter actinomycetemcomitans (Aa) é a bactéria mais relacionada com a ocorrência dessa doença. Foi avaliada a presença de Aa em gengivites e periodontites de pacientes hospitalizados e sua associação na predisposição à EI. Métodos: Realizou-se a sondagem periodontal em 15 pacientes de um hospital de Itajaí, SC com gengivites e periodontites, entre 18 a 75 anos de idade, de ambos os sexos, considerados em risco de EI. Usou-se um dente de cada paciente como amostra, coletado em cones de papel esterilizado, transferência em recipientes e processamento no laboratório. As amostras foram homogeneizadas e alíquotas de 0,1 ml foram semeadas em duplicata em placas de ágar sangue, incubadas por cinco dias em microaerofilia a 37 O C. Depois da visualização das colônias bacterianas características, foram realizadas provas bioquímicas e reação de catalase. O diagnóstico de EI foi realizado seguindo os critérios de Duke modificado. Conclusão: Não houve incidência de pacientes com risco de EI relacionada com presença da Aa nos participantes avaliados


Objective: Infective endocarditis (IE) corresponds to a microbial infection of the endocardium with the presence of platelet and fibrin clotting in its interior. When it is of periodontal origin, Aggregatibacter actinomycetemcomitans (Aa) is the bacterium most often related to the occurrence of this disease. The presence of Aa in the gingivitis and periodontitis of hospitalized patients and its association with predisposition to IE were evaluated. Methods: Periodontal probing was performed in 15 patients with gingivitis and periodontitis, aged 18 and 75 years, of both genders, and considered at risk for IE at a hospital in Itajaí ­ SC. One tooth of each patient was used for the sample, which was collected in cones of sterilized paper, transferred in containers, and processed in the laboratory. The samples were homogenized and aliquots of 0.1 ml were seeded in duplicate in blood agar plates, incubated for five days in microaerophylia at 37ºC. Following visualization of characteristic bacterial colonies, biochemical and catalase reaction tests were conducted. The diagnosis of IE was performed following the modified Duke criteria. Conclusion: There was no incidence of patients at risk of IE related to the presence of Aa in the participants evaluated

Introducción: El agrandamiento gingival es el aumento exagerado y desfigurante del volumen de la encía. Su aparición se asocia a fármacos, entre los que se encuentran los inmunosupresores y los bloqueadores de los canales de calcio como la ciclosporina A y amlodipino. Objetivo: Describir un caso clínico de agrandamiento gingival asociado a ciclosporina A y amlodipino, con periodontitis crónica subyacente, su tratamiento y prevención de recidiva. Presentación del caso: Paciente masculino, de 50 años de edad, antecedentes de hipertensión arterial, asma bronquial y hepatitis C, además de presentar insuficiencia renal crónica para la cual se le realizó un trasplante renal. Recibe tratamiento con ciclosporina A y amlodipino. Al examen clínico se observaron aumento de volumen generalizado en la encía, que cubría completamente la corona de los dientes, bolsas periodontales de 5 a 8 mm, sangramiento gingival y movilidad dentaria. Principales comentarios: El proceso diagnóstico permitió comprobar que además del agrandamiento gingival generalizado existía una periodontitis crónica generalizada. Conclusiones: La ingestión de un inmunosupresor como la ciclosporina A con el uso de un bloqueador de los canales de calcio, el amlodipino, y la influencia de factores de connotación local, parecen ser los responsables de la aparición combinada del agrandamiento gingival generalizado y la periodontitis crónica concomitante. La fase higiénica contribuyó considerablemente a mejorar el estado periodontal, cuya solución definitiva se alcanzó con la cirugía periodontal convencional. Se corrobora la importancia del examen periodontal en pacientes candidatos a trasplantes de órganos(AU)


Introduction: Gingival enlargement is an exaggerated and disfiguring increase in gum volume, associating its appearance with drugs like immunosuppressants and calcium channel's blockers such as cyclosporine A and Amlodipine. Objective: To describe a clinical case of gingival enlargement associated to cyclosporine A and amlodipine, presenting chronic underlying periodontitis, its treatment and prevention in case of recurrence. Case Presentation: Male patient, 50 years old with a history of arterial hypertension, bronchial asthma and hepatitis C, and presenting chronic renal failure leading renal transplant. The patient was treated with cyclosporine A and amlodipine. In the clinical examination was observed an increased volume in the gum, which completely covered the crown of the teeth, also periodontal bags of 5 to 8 mm, gingival bleeding and dental mobility. Main Comments: The diagnostic process allowed to verify that in addition to the generalized gingival enlargement there was a generalized chronic periodontitis. Conclusions: The ingestion of an immunosuppressant such as Cyclosporin A with the use of a calcium channel's blocker, amlodipine, and the influence of local connotation factors seem to be responsible for the combined appearance of generalized gingival enlargement and concomitant chronic periodontitis. The hygienic phase contributed considerably to improve the periodontal state, whose definitive solution was achieved with conventional periodontal surgery. The importance of periodontal examination in patients who are candidates for organ transplants is corroborated(AU)

Abstract Objective The aim of this study was to evaluate the levels of salivary biomarkers IL-1β, IL-10, RANK, OPG, MMP-2, TG-β and TNF-α in individuals with diagnosis of peri-implant mucositis in the absence or presence of periodontal and peri-implant maintenance therapy (TMPP) over 5 years. Material and Methods Eighty individuals diagnosed with peri-implant mucositis were divided into two groups: one group that underwent periodontal and peri-implant regularly maintenance therapy, called GTP (n=39), and a second group that received no regular maintenance GNTP (n=41). Each participant underwent a complete periodontal and peri-implant clinical examination. Collection of saliva samples and radiographic examination to evaluate peri-implant bone levels were conducted at two times: initial examination (T1) and after 5 years (T2). The salivary samples were evaluated through ELISA for the following markers: IL-1β, IL-10, RANK, OPG, MMP-2, TGF and TNF-α. Results A higher incidence of peri-implantitis was observed in the GNTP group (43.9%) than in the GTP group (18%) (p=0.000). All individuals (n=12) who presented peri-implant mucositis and had resolution at T2 were in the GTP group. After 5 years, there was an increase in the incidence of periodontitis in the GNTP group compared to the GTP group (p=0.001). The results of the study revealed an increase in the salivary concentration of TNF-α in the GNTP group compared to the GTP group. The other salivary biomarkers that were evaluated did not show statistically significant differences between the two groups. Conclusions The salivary concentration of TNF-α was increased in individuals with worse periodontal and peri-implant clinical condition and in those with a higher incidence of peri-implantitis, especially in the GNTP group. Longitudinal studies in larger populations are needed to confirm these findings and elucidate the role of this biomarker in peri-implant disease.

Abstract Objective: To determine the expression of TLR4 and MMP8 in gingival crevicular fluid [GCF] in patients with periodontitis. Material and Methods: Clinical samples were collected from 23 gingival crevicular fluid of periodontal disease subjects (n = 14) and healthy periodontal subjects (n=9). Measurement of Clinical parameters of probing pocket depth (PPD), bleeding on probing (BOP), and clinical attachment loss (CAL) were included as diagnostic criteria. Pocket Depth (PD) and CAL were defined as present if the PPD was ≥ 4 mm and the CAL ≥ 1 mm. Expression of TLR4 and MMP8 in the gingival crevicular fluid of deep pockets (PD≥ 6mm), shallow pockets (PD 4-5 mm) and healthy periodontal sulcus (0-3 mm) were evaluated by quantitative real-time polymerase chain reaction (qRT-PCR). Statistical analysis to compare the pocket was using Independent t-test and Mann-Whitney test. Correlation between mRNA expression and clinical parameters was analyzed using Spearman's correlation test Results: Expression of TLR4 was higher in shallow pockets compared to the control group, but the difference was not statistically significant (p>0.05). The expression of MMP8 was higher in shallow pockets compared to the control group, but the difference was not statistically significant (p>0.05) either. There is no significant correlation between TLR4 and MMP8 with clinical periodontal parameters Conclusion: TLR4 and MMP8 mRNA expression levels should not be used as a clinical biomarker in periodontitis diagnostic tools.

RESUMEN Introducción: la falsa vía es la apertura artificial en las paredes del conducto debido a penetración de la dentina; lo que provoca comunicación entre el espacio pulpar y el periodonto. Estos accidentes suelen ocurrir durante la realización de tratamientos endodónticos o al preparar el lecho para colocación de pernos. Esto provoca daño al periodonto, pudiendo desarrollarse bolsas periodontales. Objetivo: identificar la frecuencia de estos accidentes, y determinar su asociación con la presencia de periodontitis. Materiales y métodos: se realizó un estudio descriptivo y transversal en la Facultad de Estomatología de La Habana, entre enero de 2011 y enero de 2016. El universo estuvo constituido por 50 dientes con falsa vía, correspondientes a 50 pacientes. La información se obtuvo a través de: interrogatorio, examen clínico, radiografías e historias clínicas, previo consentimiento de los pacientes. Se procedió al llenado de un formulario confeccionado al efecto. Resultados: el 52,0 % de falsas vías correspondió al grupo de 35 a 59 años, los dientes más afectados fueron los incisivos, con 40,0 %. El 60,0 % de las perforaciones fueron ocasionadas por terapia endodóntica. Se detectó periodontitis en el 30,0 % de los dientes perforados. Conclusiones: las falsas vías fueron predominantes en el grupo de edad de 35 a 59 años, los incisivos fueron los dientes más afectados, la mayoría de las lesiones se produjo durante la terapia endodóntica. Más de la cuarta parte de las lesiones estuvo asociada a periodontitis (AU).


ABSTRACT Introduction: the false way is the artificial opening in the walls of the conduit due to penetration of the dentin; which causes communication between the pulp space and the periodontium. These accidents often occur during endodontic treatments or when preparing the bed for bolt placement. This causes damage to the periodontium, and periodontal pockets may develop. Objective: to identify the frequency of these accidents, and to determine their association with the presence of periodontitis. Materials and methods: a descriptive and cross-sectional study was carried out in the Faculty of Dentistry of Havana, between January 2011 and January 2016. The universe consisted of 50 teeth with false pathway, corresponding to 50 patients. The information was obtained through: interrogation, clinical examination, radiographs and medical histories, with the consent of the patients. A completed form was filled out. Results: 52,0 % of false pathways corresponded to the group of 35 to 59 years, the teeth most affected were the incisors, with 40,0 %. 60,0 % of the perforations were caused by endodontic therapy. Periodontitis was detected in 30,0 % of the perforated teeth. Conclusions: the false pathways were predominant in the age group of 35 to 59 years, the incisors were the most affected teeth, most of the lesions occurred during endodontic therapy. More than a quarter of the lesions were associated with periodontitis (AU).

RESUMEN Introducción: la enfermedad periodontal es el conjunto de enfermedades inflamatorias que afectan los tejidos de soporte del diente. Se considerada un problema de salud, que se comporta como indicador de riesgo que influye en enfermedades sistémicas como la diabetes mellitus. Objetivo: determinar el comportamiento de la enfermedad periodontal en pacientes diabéticos. Materiales y métodos: se realizó estudio observacional, descriptivo, transversal, en policlínico "Juan Gualberto Gómez" Los Arabos, Matanzas, de enero a mayo 2016 en un universo conformado por 12 pacientes diabéticos mayores de 20 años, sin distinción de sexo y color de piel, con plenitud de sus facultades físicomotoras. Se estudiaron variables demográficas, prevalencia y gravedad de la enfermedad periodontal y áreas bucales con afectación periodontal. Resultados: resaltó la edad 61 y más años con 50 %. Predominó sexo femenino con un 75 %, el 66,6 % presentó enfermedad periodontal. La gingivitis de leve a moderada se presentó en 25 %. El 41,6 % presentó formas benévolas de periodontitis. Las superficies vestibulares se afectaron en el 75 %, el área de incisivos inferiores destacó sobre los superiores al igual que los molares superiores ambas con 50 %. Conclusiones: la enfermedad periodontal mostró una alta prevalencia en la población estudiada, resaltaron las féminas en la edad de 61 y más año en correspondencia con el envejecimiento poblacional actual. Predominó la gingivitis de tipo leve y la periodontitis moderada. Las zonas vestibulares de incisivos inferiores y molares superiores se afectaron por enfermedad periodontal (AU).


ABSTRACT Introduction: The periodontal diseases are the joint of the inflammatory diseases that affect the supporting tissues of the tooth. It´s considered health problem that behave as a risk factor that influences upon systemic conditions like diabetes mellitus. Objective: Determine behave of the periodontal diseases in diabetic patients. Materials and methods: An observant, descriptive and transversal studied was carried out in the policlinic Juan Gualberto Gomez from Los Arabos Matanzas provinces from January to May 2016 in a universe of 12 diabetic patients older than 20 years, of both sex, without distinction in the color of the skin and in a full physical and motor conditions. They were analyzed variables than: age, sex, prevalence and severity of the periodontal disease also oral areas with periodontal affection. Results: The more significant age from 61 years old and more. The female sex prevailed with 75 %; the 66 % had periodontal disease. Gingivitis was presented from mild to moderate in a 25 %. The 41,6 % presented moderate levels of periodontitis. Thebuccalareas were affected in a 75 %. The area of the lower incisors was the most significant compared with theupper incisors and upper molars with 50 %. Conclusions: The periodontal disease showed a high prevalence in the population studied. The female with 61 years old and more prevailed in correspondence with the current aging in population mild gingivitis and moderate periodontitis prevailed. Buccal areas of lower incisors and upper molars were affected by periodontal disease (AU).

Objective: To evaluate the periodontal status of teeth adjacent to the posterior single tooth implant during one year after restoration. Material and Methods: In this crosssectional study, 36 patients, which were systematically and had received the implant(s) of single posterior teeth over the past year were selected. The teeth adjacent to singleposterior tooth implants were evaluated regarding bleeding on probing, mobility, gingivitis and periodontal pockets during the follow up period of 3, 6 and 9 months under the supervision of two specialists. To investigate the relationship between periodontal statuses, the Chi-square test was used, and the Kruskal-Wallis test was used to assess statistical significance of periodontal indexes in the period under assessment. The p-value of less than 0.05 was considered significant. Results: There was no significant relationship between the variable of bleeding on probing, mobility, gingivitis and periodontal pocket in the studied distal and mesial teeth. Out of studied patients, after 9 months, only 11.1% had one of the periodontal problems, as a result, 88.9% of crowns replacement of single-posterior tooth does not make any periodontal damages to adjacent teeth. Conclusion: Crowns replacement of implant does not have any destructive impact on periodontal condition of adjacent teeth.

Aim: To evaluate glycosylated hemoglobin (HbA1c) before and after non-surgical periodontal treatment in patients with type 2 diabetes mellitus (DM-2). Methods: Twenty subjects diagnosed with periodontitis and DM-2 were treated using an NSPT protocol. Periodontal examination and blood measurements were performed at baseline and after three months. Patients with DM-2 treated for at least a year, with at least 10 teeth and with probing depths between 4-6 mm in more than three regions were included. The variables evaluated were HbA1c in blood and periodontal measures (probing depths, insertion level, gingival bleeding on probing, dental plaque, calculus, inflammation, clinical attachment and mobility). All patients were informed of the conditions of the therapy used. Scaling and root planning (SRP) of the full mouth was performed using an ultrasonic scaler and hand instrument under local anesthesia, supragingival prophylaxis and oral hygiene instruction. Also, 0.12% chlorhexidine digluconate was formulated twice a day for two weeks. Statistical analyses were performed using StataIC 14. The values are shown as the mean, median and standard deviation (SD) or interquartile rank (IR), and p<0.05 was considered significant. Results: Twenty subjects completed the three-month follow-up and were included in the analysis. Three months after the non-surgical periodontal treatment, most periodontal parameters had a meaningful reduction (p< 0.05) and a large effect size >0.8. Clinical attachment level showed no improvement. The HbA1c values were not significantly decreased (p=0.94). Conclusions: Although non-surgical periodontal therapy eliminates local inflammation, it is insufficient to significantly reduce HbA1c levels in a short time period

Se realizó un estudio cuasi experimental, longitudinal y prospectivo, con el objetivo de evaluar el tratamiento de los defectos óseos complejos con coralina y membrana de colágeno. El universo estuvo constituido por 18 pacientes de ambos sexos entre 25 y 54 años de edad atendidos en la Clínica "Lidia Doce Sánchez" de Guantánamo desde 2014 hasta 2017. Los pacientes se distribuyeron según grupos de edades, fueron evaluados luego del año de ser intervenidos. Se evidenció mayoritariamente el grupo de edades de 35-44 años, la ausencia de bolsas periodontales, la movilidad dentaria grado 0. La osteointegración grado III y una evaluación buena del tratamiento con coralina y membrana de colágeno en defectos óseos complejos, coincidieron en primacía(AU)


A quasi-experimental, longitudinal and prospective study was carried out, with the aim of evaluating the treatment of complex bone defects with coralina and collagen membrane. The universe consisted of 18 patients of both sexes between 25 and 54 years of age attended in the Clinic "Lidia Doce Sánchez" of Guantanamo from 2014 to 2017. The patients were distributed according to age groups, were evaluated after the being operated. The age group of 35-44 years, the absence of periodontal pockets, the grade 0 tooth mobility were evidenced. The osseo integration grade III and a good evaluation of the treatment with coralina and collagen membrane in complex bone defects coincided in primacy(AU)
