RESUMEN
Bajo la teoría de vía aérea unificada, se ha observado que el asma y la rinosinusitis crónica (RSC) tienen una estrecha relación, con efectos importantes de una enfermedad sobre el control de la otra. El objetivo de esta revisión bibliográfica es clarificar cómo ambas enfermedades se relacionan desde su origen, epidemiología, fisiopatología y tratamiento. Sabemos que la presencia de RSC se asocia con peores resultados del asma, mayor frecuencia de exacerbaciones, hospitalizaciones y mayor uso de corticoides sistémicos. Varios mecanismos parecen tener un rol en la disfunción de la vía aérea inferior en pacientes con RSC, dentro de los cuales se plantea que la respuesta inflamatoria en común de tipo Th2 juega un papel principal. Existe amplia literatura respecto al efecto que tiene el tratamiento de la RSC en el control del asma, en esta revisión se expondrá la evidencia disponible del tratamiento médico con corticoides nasales, montelukast y macrólidos, así como también del tratamiento quirúrgico de la RSC y el uso de biológicos.
Under the unified airway theory, asthma and chronic rhinosinusitis (CRS) have a close relationship, with significant effects of one disease on the control of the other. This bibliographic review aims to clarify how both diseases relate to each other from their origin, epidemiology, pathophysiology, and treatment. CRS is associated with worse asthma outcomes, higher frequency of exacerbations, hospitalizations, and increased use of systemic corticosteroids. Several mechanisms play a role in lower airway dysfunction in patients with CRS, among which the common Th2-type inflammatory response plays a substantial role. There is extensive literature regarding the effect of the treatment of CRS in the control of asthma. We present the available evidence regarding the effect of medical treatment with nasal corticosteroids, montelukast, and macrolides, as well as the surgical treatment and use of biologics.

Objective: To explore the correlation between eosinophils (Eos) and the incidence of chronic sinusitis with nasal polyps (CRSwNP) in Xinjiang region of China by comparing the proportion of inflammatory cells in the pathological tissues and peripheral blood. Methods: Retrospective analysis was performed on 582 patients with CRSwNP who underwent endoscopic nasal surgery in the First Affiliated Hospital of Xinjiang Medical University from January 2012 to March 2018, including 367 males and 215 females, aged (45.5±13.4) years (x¯±s). Patients were divided into groups according to demographic characteristics, recurrence and complication of allergic rhinitis (AR). Preoperative blood routine and postoperative pathological section data of nasal polyps were collected to compare the ratio of inflammatory cells in pathological tissue and the ratio of peripheral blood Eos in each group. The correlation between the proportion of inflammatory cells in the pathological tissue of nasal polyps and the recurrence of CRSwNP was analyzed, as well as the distribution of (eosCRSwNP) in Uygur and Han CRSwNP patients in Xinjiang region. Statistical analysis was performed by SPSS 19.0 software. Results: Compared with non-recurrent CRSwNP patients, the ratio of Eos in nasal polyp tissue and peripheral blood was increased significantly, (Z value was -3.142 and -2.344, respectively, both P<0.05). Compared with CRSwNP patients without AR, the ratio of Eos in nasal polyps and peripheral blood was also increased significantly in patients with AR (Z value was -6.664 and -4.520, respectively, both P<0.05). There was a positive correlation between tissue Eos and CRSwNP recurrence (r=0.130, P=0.002). The majority of CRSwNP patients were both eosCRSwNP in Uygur and Han ethnic groups. Conclusions: Eos is associated with the recurrence of CRSwNP in Xinjiang region, and eosCRSwNP is the dominant factor in both Uygur and Han patients.

ABSTRACT Objective: To evaluate the association between nutritional status, physical activity, sedentary behavior, diet, and lifestyle in childhood with respiratory diseases during adolescence. Methods: Prospective study conducted in São Caetano do Sul, São Paulo - Brazil, as part of the International Study of Childhood Obesity, Lifestyle and the Environment (ISCOLE). During childhood, indicators of lifestyle (body composition, physical activity, sedentary behavior, diet) and family environment were measured in 2012 and 2013. After five years, participants answered the International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire for the diagnosis of respiratory diseases (asthma and/or rhinitis). Analyses were determined by logistic regression. Results: A total of 168 schoolchildren (56% boys) were evaluated, and the prevalence of asthma and rhinitis accounted for 15.5 and 25.6%, respectively. Whole milk consumption (Odds Ratio [OR]=1.24; 95% confidence interval [95%CI] 1.03-1.49), having a television in the bedroom (OR=0.29; 95%CI 0.12-0.71), and attending physical education classes ≥2 times/week (OR=0.30; 95%CI 0.11-0.81) in childhood were associated with the presence of asthma in adolescence. Factors significantly associated with rhinitis were as follows: female participants (OR=2.45; 95%CI 1.20-4.98) and whole milk consumption (OR=1.21; 95%CI 1.04-1.40). Conclusions: Higher consumption of whole milk, not having a television in the bedroom, few physical education classes, and being a girl were factors associated with respiratory diseases. Public policies should be directed toward a healthier lifestyle and the prevention of respiratory diseases.
RESUMO Objetivo: Avaliar a associação entre estado nutricional, atividade física, comportamento sedentário, dieta e estilo de vida na infância e doenças respiratórias durante a adolescência. Métodos: Estudo prospectivo realizado em São Caetano do Sul, São Paulo, como parte do International Study of Childhood Obesity, Lifestyle and the Environment. Indicadores do estilo de vida (composição corporal, atividade física, comportamento sedentário, dieta) e do ambiente familiar foram mensurados durante a infância, em 2012 e 2013. Após cinco anos, os escolares responderam o questionário do International Study of Asthma and Allergies in Childhood (ISAAC) para o diagnóstico de doenças respiratórias (asma e/ou rinite). Análises foram determinadas por meio da regressão logística. Resultados: Foram avaliados 168 escolares (56% meninos), e as prevalências de asma e rinite observadas foram 15,5 e 25,6%, respectivamente. Consumir leite integral (Odds Ratio [OR]=1,24; intervalo de confiança 95% [IC95%] 1,03-1,49), ter televisão no quarto (OR=0,29; IC95% 0,12-0,71) e praticar aulas de educação física ≥2 vezes/semana (OR=0,30; IC95% 0,11-0,81) na infância foram associados com a presença de asma na adolescência. Os fatores significativamente associados à rinite foram: ser do sexo feminino (OR=2,45; IC95% 1,20-4,98) e consumir leite integral (OR=1,21; IC95% 1,04-1,40). Conclusões: Consumir mais leite integral, não ter televisão no quarto, ter tido poucas aulas de educação física e ser do sexo feminino foram fatores associados às doenças respiratórias. Políticas públicas devem ser dirigidas para um estilo de vida mais saudável e a prevenção de doenças respiratórias.

RESUMO Objetivo Descrever características clínicas e identificar fatores associados a maior gravidade da asma, em uma amostra de pacientes acompanhados em um centro de referência em Salvador. Métodos Estudo transversal de 473 adultos, acompanhados regularmente no Programa para Controle da Asma na Bahia (ProAR), reavaliados de forma sistemática entre 2013 e 2015. Os pacientes foram admitidos por preencher critérios anteriores de asma grave e reclassificados de acordo com a definição mais atual, proposta por um documento conjunto da European Respiratory Society/American Thoracic Society (ERS/ATS 2014). Resultados Foram reclassificados como portadores de asma grave pelos critérios da ATS/ERS (AG-ERS/ATS) 88/473 (18%). Destes, 87% eram mulheres, 48% obesos, com mediana do índice de massa corporal (IMC) de 29 kg/m2 (IQ 26-34), 99% tinham sintomas de rinite crônica e 83%, sintomas de doença do refluxo gastroesofágico (DRGE). Nenhum se declarou fumante atual. Os principais corticosteroides inalatórios utilizados foram beclometasona (88%) e budesonida (69%). A maioria relatou adequada adesão (77%) e a minoria das avaliações (0,6%) revelou erros graves na técnica inalatória. A mediana do volume expiratório forçado no primeiro segundo pós-broncodilatador (VEF1pós-BD) foi 67% do predito (IQ 55-80). A mediana do número de eosinófilos no sangue periférico foi menor nos pacientes com AG-ERS/ATS [209 células/mm3 (IQ 116-321)] do que nos demais pacientes estudados [258 células/mm3 (IQ 154-403)]. Sintomas de doença do refluxo gastroesofágico (DRGE) foram associados a mais gravidade [OR = 2,2; IC95% (1,2-4,2)]. Conclusões Neste grupo de pacientes, sintomas de RGE foram associados a AG-ERS/ATS e contagem de eosinófilos > 260 células/mm3 esteve associada a 42% menos chance de AG-ERS/ATS.
ABSTRACT Objective To describe the clinical features and to identify factors associated with significant severe asthma in samples of patients followed in a reference center in Salvador. Methods A cross-sectional study of 473 adults, regularly followed in the "Asthma Control Program" in Bahia (Programa de Controle da Asma e da Rinite Alérgica na Bahia (ProAR)), reassessed systematically between 2013 and 2015. The patients were admitted for meeting previous criteria of severe asthma and were reclassified according to the most current definition proposed by a joint document of the "European Respiratory Society/American Thoracic Society" (ERS/ATS) (ERS/ATS 2014). Results Only 88/473 (18%) were reclassified as having severe asthma by ERS/ATS criteria (SA-ERS/ATS). Among these patients, 87% were women, 48% obese, with a median Body Mass Index (BMI) of 29 kg·m2 (IQ 26-34), furthermore, 99% had symptoms of chronic rhinitis and 83% had symptoms of Gastroesophageal Reflux Disease (GERD). None of the 88 patients claimed to be current smokers. The most frequently corticosteroids were beclomethasone dipropionate (BDP) (88%) and budesonide (BUD) (69%). The majority of the evaluations reported adequate adherence (77%), however, the minority (0,6%) detected serious errors in inhalation techniques. The median Forced Expiratory Volume (FEV1) associated with post-bronchodilator test (post-BD) was 67% predicted (IQ 55-80). The median number of eosinophils in the peripheral blood was lower in patients with SA-ERS/ATS (258 cells/mm3 (IQ 116-321) than in the other patients studied [258 cells/mm3 (IQ 154-403)]. Gastroesophageal reflux symptoms were associated with a higher severity [OR = 2.2 95% CI (1.2-4.2)]. Conclusion In this group of patients, symptoms of GERD were associated with SA-ERS/ATS and eosinophil count > 260 cells/mm3 were associated 42% with less chance SA-ERS/ATS

Primary Ciliary Diskinesia (PCD) is a heterogeneous, rare genetic disease that can be present in up to 5% of the patients with recurrent respiratory infections. The underlying pathogenesis is disrupted ciliary function which results in delayed mucus transportation leading to chronic inflammation in the upper and lower respiratory tract. Almost all PCD patients have otolaryngologic manifestations, characterized by recurrent ear and sinus infections, chronic inflammation at this level, sensorioneural and conductive hearing loss, and sleep-disordered breathing. This article reviews the diagnostic and therapeutic aspects of these manifestations.
La Disquinesia Ciliar Primaria (DCP) es una enfermedad genética heterogénea rara que puede estar presente en hasta un 5% de los pacientes que presentan infecciones respiratorias a repetición. La patogenia es secundaria a una alteración de la función ciliar que a su vez provoca una alteración del transporte de moco, resultando en una condición inflamatoria crónica en la vía aérea superior e inferior. Las manifestaciones clínicas de la esfera otorrinolaringológica en los pacientes portadores de DCP están presentes prácticamente en la totalidad de los mismos, y se caracterizan por infecciones recidivantes de oídos y cavidades perinasales, inflamación crónica a este nivel, hipoacusia neurosensorial y conductiva, y alteraciones respiratorias durante el sueño. En este artículo se revisarán los aspectos diagnósticos y terapéuticos de dicho compromiso.

Abstract Background: Epidemiological studies have shown that children who grow up on traditional farms are protected from allergic diseases. However, less is known about if the environment influences the pharmacotherapy in these patients. Objective: To compare the treatment of asthmatic and rhinitis children from urban and rural areas in Medellín, Colombia. Methods: During one year, we follow up a group of children (6 to 14 years) with diagnostic of asthma or rhinitis living for more than five years in urban or rural area. A questionnaire with socio-demographic characteristics, pharmacotherapy treatments, was obtained each three months. Atopy evaluation, spirometry and clinical test for asthma and rhinitis severity were done at the beginning and one year later. Results: Eighty six point four percent patients completed the follow up (rural n: 134, urban n: 248). Patients in rural location required less salbutamol (p: 0.01), visit to emergency department (p <0.01) and have a less number of patients with FEV1 <80% (p: 0.05). For clinical control rural children require less pharmacotherapy than urban children (p: 0.01) and more patients with rhinitis (18% vs 8% p: 0.03) and asthma (23% vs 12% p: 0.01) in the rural group could suspended pharmacotherapy. Atopy (p: <0.07) and poli-sensitization (p: <0.08) was a little higher in urban than rural area. We observe that poverty/unhygienic indicators were risk factors for higher levels of specific IgE among patients from urban area. Conclusion: Patients with respiratory allergies located in urban area require more pharmacotherapy and have less clinical response than rural children.
Resumen Introducción: Los estudios epidemiológicos han demostrado que los niños que crecen en las granjas suelen tener menos frecuencia de enfermedades alérgicas. Sin embargo, se sabe menos si el tipo de ambiente (rural vs urbano) también puede influir en la respuesta clínica de a la farmacoterapia. Objetivo: Comparar un grupo de niños localizados en área rural y área urbana de Antioquia, Colombia, en cuanto al tratamiento farmacológico recibido para el asma y/o la rinitis. Métodos: Fueron incluidos niños con asma y/o rinitis que llevaran viviendo al menos 5 años en la misma zona rural o urbana con edades entre 6 a 14 años. A todos los pacientes se les realizó un seguimiento clínico cada 3 a 4 meses. La evaluación de la atopia, la espirometría y test para evaluar la gravedad del asma y la rinitis se realizaron al principio y al final del estudio. Resultados: De los pacientes candidatos, 382 (86.4%) completaron el seguimiento (rural n= 134 urbano n= 248). Los pacientes en área rural requirieron menos salbutamol (p: 0.01), visitas al departamento de emergencias (p <0.01) y tenían un menor número de pacientes con FEV1 <80% (p <0.05). Para el control clínico, los niños en zonas rurales requieren menos farmacoterapia que los niños en zona urbana (p: 0.01). Igualmente, para la rinitis (18% vs 8% p: 0.03) y el asma (23% vs 12% p= 0.01) un mayor número de los pacientes en zona rural pudieron suspender la farmacoterapia. La atopia (p <0.07) y la poli-sensibilización (p <0.08) fue mayor en las zonas urbanas que en las rurales. Se observó que los indicadores de pobreza y los servicios de aseo, eran factores de riesgo para mayores niveles de IgE entre los pacientes de área urbana. Conclusión: Los pacientes con asma o rinitis localizado en el área urbana tienen síntomas más severos y refractarios al tratamiento farmacológico, por lo que requieren más farmacoterapia que los niños rurales. Algunos factores ambientales intra y extra domiciliarios propios de la zona rural y urbana podrían influir en estos resultados.

RESUMEN Objetivo Determinar la prevalencia de síntomas asociados a asma, rinitis y eccema relacionada con la ingesta de alimentos en menores de seis a siete años. Metodología Estudio descriptivo de prevalencia de asma, rinitis y eccema y la relación con el consumo de alimentos. Información obtenida en 1 520 escolares del Distrito de Barraquilla y su área metropolitana. Se empleó metodología ISSAC procesándose la información mediante el software SPSS versión 24 El cuestionario fue contestado por el acudiente del menor y solo se tuvieron en cuenta los que tenían debidamente diligenciado los consentimientos informados Se hizo análisis empleando Chi-cuadrado, y el valor P de significancia con un 95% de confianza. Resultados Para los 1 520 menores encuestados el alimento que presentó relación con alergias fue el pescado asociándolo con el asma, evidenciando que en la población puede ser factor protector para la aparición de eccema mostrando estadísticamente una P<0,05, un Chi-cuadrado: 4,566, IC 95% [0,3;0,8]. Conclusiones De los alimentos analizados el pescado es uno que presentó relación y se asoció como factor protector para el eccema, el consumo de la leche de vaca se relacionó como factor protector para rinitis; en lo referente a los demás alimentos y las alergias el estudio no es conclusivo en cuanto a si la ingesta temprana es un factor predisponente o desencadenante de ellas en los menores, o no lo son.(AU)
ABSTRACT Objective To determine the prevalence of symptoms associated with asthma, rhinitis and eczema with food intake in children under six years of age. Materials and Methods Descriptive study on the prevalence of asthma, rhinitis and eczema and their correlation with food consumption. Information was obtained from 1 520 schoolchildren of the District of Barranquilla and its metropolitan area. The ISSAC method was used to process the information, which was provided by the child's guardian, using the software SPSS version 24. Only duly completed questionnaires with informed consents were considered. Analysis was made using a chi-squared test, with a significant P value and 95% confidence. Results In the 1 520 children surveyed, the type of food related to allergies was fish, associated with asthma, showing that this may be a protective factor for the population regarding the onset of eczema, with a statistically a P value of <0.05, chi-square of 4.566, and CI of 95% [0.3;0.8]. Conclusions The analyzed foods showed that fish is related and associated as a protective factor for eczema, while the consumption of cow's milk was related as a protective factor for rhinitis. Regarding other foods and allergies, the study is not conclusive as to whether their early intake is a predisposing factor or trigger in children.(AU)

Objective: To describe the frequency and etiology of rhinitis, oral breathing, types of malocclusion and orofacial disorders in patients treated for dental malocclusion. Methods: Patients with poor dental occlusion (n=89, 8-15 years) undergoing orthodontic treatment at the Postgraduate Orthodontics Center (São Paulo, Brazil) participated in the study. Rhinitis and oral breathing were diagnosed by anamnesis, clinical assessment and allergic etiology of rhinitis through immediate hypersensitivity skin prick test with airborne allergens. The association between types of breathing (oral or nasal), rhinitis and types of dental malocclusion, bruxism and cephalometric alterations (increased Y axis of facial growth) compared to standard cephalometric tracing (Escola de Odontologia da Universidade de São Paulo) were assessed. Results: The frequency of rhinitis in patients with dental malocclusion was 76.4% (68), and, of these, 81.7% were allergic (49/60 positive skin prick test), whereas the frequency of oral breathing was 62.9%. There was a significant association between an increased Y axis of facial growth and oral breathing (p<0.001), as well as between oral breathing and rhinitis (p=0.009). There was no association between rhinitis and bruxism. Conclusions: The frequency of rhinitis in children with dental malocclusion is higher than that in the general population, which is approximately 30%. Patients with oral breathing have a tendency to a dolichofacial growth pattern (increased Y axis of facial growth). In patients with rhinitis, regardless of the presence of oral breathing, the dolichofacial growth tendency was not observed.
Objetivo: Descrever a frequência e etiologia da rinite, da respiração oral, os tipos de má oclusão e as alterações orofaciais em pacientes tratados por má oclusão dentária. Métodos: Pacientes com má oclusão dentária (n=89, oito a 15 anos) em tratamento ortodôntico em centro de pós-graduação em ortodontia (São Paulo, Brasil) participaram do estudo. Rinite e respiração oral foram diagnosticadas por anamnese e exame clínico e a etiologia alérgica dessa por teste cutâneo de hipersensibilidade imediata (TCHI) com aeroalérgenos. Avaliou-se a relação entre tipos de respiração (oral ou nasal), rinite e tipos de má oclusão dentária, bruxismo e alterações cefalométricas (aumento do eixo Y de crescimento facial) em comparação com o traçado cefalométrico padrão (Escola de Odontologia da Universidade de São Paulo). Resultados: A frequência de rinite nos pacientes com má oclusão dentária foi de 76,4% (68), desses 81,7% eram alérgicos (49/60 TCHI positivo) e a frequência de respiração oral foi de 62,9%. Houve associação significativa entre ter o eixo Y de crescimento facial aumentado e respiração oral (p<0,001), o mesmo entre respiração oral e rinite (p=0,009). Não houve associação entre rinite e bruxismo. Conclusões: A frequência de rinite em crianças com má oclusão dentária é superior à da população geral, que gira ao redor de 30%. Os pacientes com respiração oral têm tendência de crescimento dólico facial (eixo Y de crescimento aumentado). Nos pacientes com rinite, independentemente da presença da respiração oral, a tendência dólico facial não foi observada.

OBJECTIVE: To determine the prevalence of symptoms of asthma, rhinitis, and atopic eczema in adolescents (AD; 13-14 years) living in seven Brazilian cities, by applying the standardized written questionnaire (WQ) of the International Study of Asthma and Allergies in Childhood (ISAAC), and to evaluate the time trend nine years after the last assessment of ISAAC phase 3 (ISP3). METHODS: The ISAAC-WQ was answered by 20,099 AD from the Northern, Northeastern, Southeastern, and Southern Brazilian regions. Values obtained were compared to those observed in ISP3 using nonparametric (chi-squared or Fisher) tests, and the ratio of annual increment/decrement was established for each of the centers, according to the symptom assessed. RESULTS: Considering the national data and comparing to values of ISP3, there was a decrease in the mean prevalence of active asthma (18.5% vs. 17.5%) and an increase in the frequency of severe asthma (4.5% vs. 4.7%) and physician-diagnosed asthma (14.3% vs. 17.6%). An increase in prevalence of rhinitis, rhinoconjunctivitis, and atopic eczema was also observed. CONCLUSIONS: The prevalence of asthma, rhinitis, and atopic eczema in Brazil was variable; higher prevalence values, especially of asthma and eczema, were observed in regions located closer to the Equator. .
OBJETIVO: Determinar a prevalência de sintomas relacionados à asma, à rinite e ao eczema atópico em adolescentes (13-14 anos, AD) residentes em sete cidades brasileiras com o questionário escrito (QE) padronizado do International Study of Asthma and Allergies in Childhood (Isaac) e verificar a tendência temporal passados nove anos da última avaliação do Isaac fase 3 (ISF3). MÉTODOS: O QE Isaac foi respondido por 20.099 AD (13-14 anos) moradores em centros das regiões Norte, Nordeste, Sudeste e Sul. Os índices obtidos foram comparados aos do ISF3 com o teste não paramétrico (qui-quadrado ou Fisher) e foi estabelecida a taxa de incremento/decremento anual para cada um dos centros segundo o sintoma avaliado. RESULTADOS: Em relação ao ISF3, considerando-se os dados nacionais, houve queda da prevalência média de asma ativa (18,5% vs. 17,5%) com elevação da frequência de asma grave (4,5% vs. 4,7%) e de asma diagnosticada por médico (14,3% vs. 17,6%). Aumento da prevalência de rinite e rinoconjuntivite e de eczema flexural também ocorreram. CONCLUSÕES: A prevalência de asma, rinite e eczema atópico no Brasil foi variável. Valores mais altos, sobretudo de asma e eczema, foram observados nos centros localizados mais próximos ao Equador. .

A asma, a rinite e o eczema atópico têm sido associados à exposição a poluentes ambientais. Em Açailândia-MA há forte atividade industrial siderúrgica com visível poluição atmosférica, particularmente no bairro Piquiá, onde se situa o distrito industrial do município. Objetivos: Determinar a prevalência de sintomas de asma, rinite e eczema atópico e fatores associados em crianças de 6-7 anos e adolescentes de 13-14, residentes no Piquiá de Cima, Açailândia-MA; analisar a correlação entre as concentrações médias mensais de MP2,5 e o número de internações per capita por doenças respiratórias na população total e na faixa etária de 5 a 14 anos, em Açailândia-MA e São Luís-MA, no triênio 2012-2014. Metodologia: Realizou-se estudo observacional seccional na população residente no bairro, nas faixas etárias de 6 e 7 anos e 13 e 14 anos,totalizando 165 indivíduos, fazendo-se uso dos questionários escrito e complementar do estudo ISAAC (International Study of Asthma and Allergies in Childhood), validados para a população brasileira. Asthma affects 10% of the world population and is associated with various factors, such as genetic, and environmental allergens. The asthma, rhinitis and atopic eczema have been associated with exposure to environmental pollutants. In Açailândia-MA's strongs teel industrial activity with visible air pollution, particularly in Piquiá district, where lies the industrial district of the city. Objectives: To determine the prevalence of symptoms of asthma, rhinitis and atopic eczema and associated factors in children aged 6-7 years and adolescents aged 13-14 residing in Piquiá de Cima, Açailândia-MA; analyze the correlation between the monthly average concentrations of PM 2.5 and the number of admissions percapita for respiratory diseases in the population and in the age group 5-14 years in Açailândia-MA and São Luís, in the triennium 2012-2014. Objective: to evaluate the prevalence of rhinitis symptoms among 6 and 7-year old schoolchildren in Fortaleza, Brazil. Methods: a cross-sectional study using the rhinitis module of the ISAAC (International Study of Asthma and Allergies in Childhood) questionnaire. The sample included 2,020 students from public and private schools, in 2010. Results: the prevalence of cumulative rhinitis, active rhinitis, allergic rhinoconjunctivitis and physician-diagnosed rhinitis was 49.9%, 42%, 15.4% and 28.1%, respectively, being significantly more prevalent among private school students (OR = 0.55, 95CI: 0.46 - 0.66, p < 0.001; OR = 0.50, 95CI: 0.41 - 0.60, p < 0.001; OR = 0.67, 95CI: 0.52 - 0.85, p < 0.001; OR = 0.15, 95CI: 0.12 - 0.19, p < 0.001, respectively), without differences between genders. Interference with daily activities was reported by 25.3%, while 5.7% reported to be moderately or severely affected, without a difference between genders. Conclusion: the prevalence of rhinitis symptoms and associated morbidity among 6-7-year old schoolchildren in Fortaleza was above the Brazilian average, with predominance among private school students, without difference between genders. It was observed that rhinitis was underdiagnosed in this population. .
Objetivo: avaliar a prevalência de sintomas de rinite em crianças de 6 e 7 anos de idade, em Fortaleza. Métodos: foi aplicado o questionário ISAAC (International Study of Asthma and Allergies in Childhood), módulo rinite, em estudo de desenho transversal de base populacional, com amostragem probabilística de 2.020 escolares de escolas públicas e privadas, em 2010. Resultados: a prevalência de rinite cumulativa, rinite ativa, rinoconjuntivite alérgica e rinite diagnosticada foi, respectivamente, 49,9, 42, 15,4 e 28,1%, com predomínio de forma significativa entre as crianças das escolas particulares (RC = 0,55, IC95%: 0,46 a 0,66, p < 0,001; RC = 0,50, IC95%: 0,41 a 0,60, p < 0,001; RC = 0,67, IC95%: 0,52 a 0,85, p < 0,001; RC = 0,15, IC95%: 0,12 a 0,19, p < 0,001, respectivamente), e sem diferença entre os gêneros. Interferência dos sintomas com as atividades diárias foi relatada por 25,3% dos pesquisados, e 5,7% relataram ser moderada ou intensamente afetados, sem diferença entre os gêneros. Conclusão: a prevalência de sintomas de rinite e da morbidade associada, entre crianças de 6 e 7 anos de idade, em Fortaleza, mostrou-se acima da média nacional, com predomínio no grupo das escolas privadas, e sem diferença entre os gêneros. Observou-se também que rinite é subdiagnosticada entre as crianças de 6 e 7 anos de idade, em Fortaleza. .

Symptoms of sleep-disordered breathing, especially obstructive sleep apnea syndrome (OSAS), are common in asthma patients and have been associated with asthma severity. It is known that asthma symptoms tend to be more severe at night and that asthma-related deaths are most likely to occur during the night or early morning. Nocturnal symptoms occur in 60-74% of asthma patients and are markers of inadequate control of the disease. Various pathophysiological mechanisms are related to the worsening of asthma symptoms, OSAS being one of the most important factors. In patients with asthma, OSAS should be investigated whenever there is inadequate control of symptoms of nocturnal asthma despite the treatment recommended by guidelines having been administered. There is evidence in the literature that the use of continuous positive airway pressure contributes to asthma control in asthma patients with obstructive sleep apnea and uncontrolled asthma.
Tem-se observado que sintomas dos distúrbios respiratórios do sono, especialmente a síndrome da apneia obstrutiva do sono (SAOS), são comuns em asmáticos; além disso, associam-se com a gravidade da asma. Sabe-se que durante a noite tende a haver maior gravidade dos sintomas da asma, assim como uma maior proporção de mortalidade durante a noite e as primeiras horas da manhã. Sintomas noturnos ocorrem entre 60-74% dos pacientes com asma e são marcadores de controle inadequado da doença. Vários mecanismos fisiopatológicos são relacionados a esse agravamento. A SAOS está incluída entre os fatores mais importantes. A investigação da SAOS em pacientes com asma deve ser realizada sempre que não houver um controle adequado dos sintomas noturnos da asma com o tratamento recomendado por diretrizes. Há evidências da literatura que sugerem que o uso de pressão positiva contínua nas vias aéreas pode contribuir para o controle da asma, quando o paciente asmático tem apneia obstrutiva do sono e sua asma não está controlada.

OBJETIVO: A prevalência de conjuntivite alérgica (CA) não foi estabelecida. Estimativas sugerem que alergias oculares afetam de 15 a 20% da população mundial, ainda que a maioria dos estudos epidemiológicos abranjam sintomas de alergia nasal e ocular e não sejam específicos a respeito da CA. O objetivo deste estudo foi verificar a prevalência de sintomas, comorbidades e o impacto da alergia ocular em adolescentes. MÉTODOS: Os adolescentes foram selecionados de uma amostra de escolas e preencheram, em sala de aula, um questionário previamente validado sobre os sintomas da CA. O seu diagnóstico foi considerado quando mais de três episódios de prurido ocular foram relatados nos últimos 12 meses. Sintomas relacionados, como lacrimejamento, fotofobia, sensação de corpo estranho, impacto sobre as atividades diárias e diagnóstico de conjuntivite alérgica, foram analisados. RESULTADOS: Foram obtidos questionários de 3.120 adolescentes (média de 13,3±1,1 ano). Nos últimos 12 meses, 1.592 (51%) adolescentes tiveram prurido ocular. O sintoma relacionado mais frequente foi lacrimejamento (74%), seguido de fotofobia (50,1%) e sensação de corpo estranho (37,1%). A prevalência de conjuntivite alérgica foi de 20,7%, afetando mais pessoas do sexo feminino do que do masculino (56,1% em comparação a 45,9%; p = 0,01). O risco de um adolescente com alergia ocular apresentar asma, rinite e eczema atópico foi (RC = 5,7; IC de 95%: 4,5 a 7,1); (RC = 3,6; IC de 95%: 3,0 a 4,3) e (RC = 2,6; IC de 95%: 2,0 a 3,5), respectivamente. Uma interferência grave nas atividades diárias foi relatada por 30,5%. CONCLUSÕES: Sintomas de alergia ocular são comuns, frequentemente relacionados a outras doenças alérgicas, e causam impacto sobre as atividades diárias de adolescentes.
OBJECTIVE: The prevalence of allergic conjunctivitis (AC) has not been established. Estimates suggest that ocular allergies affect 15% to 20% of the worldwide population, yet most epidemiological studies encompass nasal and ocular allergy symptoms and have not been specific to AC. The aim of this study was to verify the prevalence of ocular allergy symptoms, co-morbidities, and their impact on adolescents. METHODS: Adolescents were selected from a sample of schools, and completed in classrooms a previously validated questionnaire on symptoms of AC. AC diagnosis was considered when more than three episodes of ocular itching were reported in the past 12 months. Related symptoms such as tearing, photophobia, foreign body sensation, impact on daily activities, and diagnosis of AC were analyzed. RESULTS: Questionnaires were obtained from 3,120 adolescents (mean age 13.3 ± 1.1 years). Ocular itching in the past 12 months occurred in 1,592 (51%). The most frequently associated symptom was tearing (74%), followed by photophobia (50.1%) and foreign body sensation (37.1%). The prevalence of AC was 20.7%, affecting more females than males (56.1% versus 45.9%; p = 0.01). The risks of an adolescent with ocular allergy to present asthma, rhinitis, and atopic eczema were (OR = 5.7; 95% CI: 4.5 to 7.1), (OR = 3.6; 95% CI: 3.0 to 4.3), and (OR = 2.6; 95% CI: 2.0 to 3.5), respectively. Severe interference in daily activities was reported by 30.5%. CONCLUSIONS: Symptoms of ocular allergy are common, frequently associated to other allergic diseases, and impact the daily activities of adolescents.

Objetivo Establecer la prevalencia de síntomas respiratorios, asma y rinitis, posiblemente asociados a la contaminación del aire en niños entre 5 y 14 años, en la localidad de Bosa, año 2012-2013. Métodos Se tomó una muestra de 553 niños residentes en la localidad de Bosa. Resultados Cuando el niño habita con personas que fuman tiene 1,5 veces más de riesgo de toser en la noche respecto a los niños cuyos contactos no fuman. Los niños que habitan en viviendas con chimeneas a menos de 100 m de distancia tienen 1,6 veces la probabilidad de presentar el síntoma. Quienes asisten al colegio de mayor exposición y además tienen edificaciones en construcción o vías en mal estado a menos de 100 m de sus viviendas, presentan 2,5 veces la posibilidad de manifestar el evento. En cuanto a presentar sibilancias en el último año, los niños que tienen humedades en su habitación presentan 4 veces la probabilidad de manifestarlas. Hay un incremento del riesgo de sibilancias en un 80 % cuando el niño vive a menos de 100 m de edificaciones en construcción o vías sin pavimentar y además asiste al colegio de mayor exposición. Conclusiones Son tan importantes las intervenciones gubernamentales para la modificación de los factores de riesgo extramurales asociados a la enfermedad respiratoria como las mejoras que deben llevarse a cabo extramuralmente.(AU)
Objective Establishing the prevalence of respiratory symptoms, asthma and rhinitis, possibly associated with air pollution, in 5- to 14-year-old children in Bosa (a conurbation of Bogota), between 2012 and 2013. Methods A sample was taken of 553 children living in the conurbation. Results The results indicated that when a child lives with people who smoke there was a 1.5 times risk of coughing at night (compared to living in a non-smoking home) such night-time coughing being different to that produced by respiratory infections such as colds, bronchitis and pneumonia. Children living in homes having fireplaces/open cooking areas located less than 100 m apart had 1.6 times greater probability of presenting symptoms. Children attending schools having greater PM10 exposure and living near buildings being constructed or having roads in a poor state of repair less than 100 meters from their homes were 2.5 times more likely to suffer respiratory disease. Children living in damp rooms were 4 times more likely to have wheezed during the past year. The risk of wheezing became increased by 80 % when a child lived within 100 meters of buildings being constructed or near unpaved roads and attended a school having greater exposure. Conclusion Government intervention is critical for changing respiratory disease-associated extramural risk factors, such as improvements benefitting children which should be carried out in urban areas.(AU)

OBJETIVO: Descrever as prevalências de asma e rinite em adolescentes de 13-14 anos de idade em Fortaleza (CE) em 2010 e compará-las com as prevalências obtidas em um inquérito em 2006-2007. MÉTODOS: Estudo transversal envolvendo uma amostragem probabilística de 3.015 e 3.020 adolescentes, respectivamente, em 2006-2007 e 2010, utilizando o protocolo do International Study of Asthma and Allergies in Childhood. RESULTADOS: Na comparação entre os dois períodos, não houve diferenças significativas em relação a sibilos cumulativos, asma ativa, quatro ou mais crises de sibilos no último ano, prejuízo do sono por sibilos > 1 noite/semana e crises limitando a fala. Em 2010, houve um aumento significativo na prevalência de sibilos após exercícios, tosse seca noturna e asma diagnosticada (p < 0,01 para todos). Em 2010, houve uma redução significativa na prevalência de rinite diagnosticada (p = 0,01), enquanto não houve diferenças significativas entre os dois períodos nas prevalências de rinite cumulativa, rinite atual e rinoconjuntivite. Em ambos os períodos, tosse seca noturna, rinite atual e rinoconjuntivite foram significativamente mais prevalentes nas mulheres que nos homens (p < 0,01 para todos). Também nos dois períodos, asma ativa, rinite atual e rinoconjuntivite foram significativamente mais prevalentes nos alunos das escolas particulares do que naqueles das escolas públicas (p < 0,01 para todos). CONCLUSÕES: Nossos dados mostram que as prevalências de sintomas de asma e rinite continuam altas entre os adolescentes de 13-14 anos em Fortaleza, com predomínio no gênero feminino e em alunos de escolas particulares.
OBJECTIVE: To describe the prevalences of asthma and rhinitis in adolescents (13-14 years of age) in the city of Fortaleza, Brazil, in 2010, comparing the results with those obtained in a prevalence survey conducted in 2006-2007. METHODS: This was a cross-sectional study involving probabilistic samples of 3,015 and 3,020 adolescents in surveys conducted in 2006-2007 and 2010, respectively. The International Study of Asthma and Allergies in Childhood protocol was used on both occasions. RESULTS: Comparing the two periods, there were no significant differences regarding cumulative wheezing, active asthma, four or more wheezing attacks within the last year, sleep disturbed by wheezing more than one night per week, and speech-limiting wheezing. The prevalences of exercise-induced wheezing, dry cough at night, and physician-diagnosed asthma were significantly higher in 2010 than in the 20062007 period (p < 0.01 for all). The prevalence of physician-diagnosed rhinitis was significantly lower in 2010 (p = 0.01), whereas there were no significant differences between the two periods regarding cumulative rhinitis, current rhinitis, and rhinoconjunctivitis. In both periods, dry cough at night, current rhinitis, and rhinoconjunctivitis were significantly more prevalent in females than in males (p < 0.01 for all). Also in both periods, active asthma, current rhinitis, and rhinoconjunctivitis were more prevalent in private school students than in public school students (p < 0.01 for all). CONCLUSIONS: Our data show that the prevalences of asthma and rhinitis symptoms remain high among females and private school students.

Introduction: The respiratory tract is one of the main points of entry of foreign substances into the body. Because of its location, the respiratory tract is heavily exposed to harmful agents, such as gases, vapors, or aerosols. Aim: Our objective was to evaluate the symptoms of occupational rhinitis in workers exposed to cotton dust. Method: The prospective study population consisted of workers from the "Nova Esperança" Cooperative of Nova Odessa (Sao Paulo), who were studied between September and December 2008. Data were collected through an individually and privately answered questionnaire designed by the author considering the clinical criteria for rhinitis. Results: Using the questionnaire, we evaluated a total of 124 workers. Among these patients, 63.7% complained of nasal obstruction, 57.2% of nasal itching, 46.7% of rhinorrhea, and 66.1% of sneezing. Of the patients considered to have very serious symptoms, 9% had nasal obstruction; 9%, itching; 4%, rhinorrhea; and 6.4%, sneezing. Discussion: Aerosol agents in the environment can clearly aggravate and even initiate rhinitis. From the standpoint of pathogenesis, the mechanisms of classical allergic airway inflammation involving mast cells, IgE, histamine, eosinophils, and lymphocytes may be responsible for the development of rhinitis after exposure to high molecular weight allergens such as proteins derived from animals and plants. This study showed a strong relationship between the occupational exposures associated with work in the cotton textile industry and the symptoms of rhinitis. Conclusion: Analysis of the data clearly showed the occurrence of rhinitis symptoms in these patients, demonstrating that the prevention and treatment of this condition in the workplace is extremely important...

Objective. The present study was designed to estimate the prevalence of rhinitis symptoms among secondary school students in Saudi Arabia Methods. A cross-sectional study was conducted among children from secondary schools in the city of Riyadh. The study utilised the International Study of Asthma and Allergies in Children (ISAAC) questionnaire. Results. Among the 3073 students (1504 boys and 1569 girls), the prevalence of life-time rhinitis, rhinitis in the past 12 months, and hay fever were 43.8%, 38.6% and 21.3%, respectively. There was a significant difference between boys and girls in the prevalence of life-time rhinitis (41% versus 46.5%, p=0.02) and the prevalence of rhinitis in the past 12 months (35.1% versus 41.9%, p=0.001). There was no significant difference between boys and girls in the prevalence of hay fever (22.5% versus 20.2%, p=0.144). The prevalence of asthma symptoms among students with life-time rhinitis and hay fever were 35.4% and 39.9%, respectively. Asthma symptoms were strongly linked with life-time rhinitis (Odds Ratio [OR]=2.5, p<0.001) and hay fever (OR=2.4, p value<0.001). Conclusions. The prevalence of rhinitis symptoms is high among 16 to 18 years old adolescents in Saudi Arabia, and symptoms are more common in girls compared to boys. Rhinitis symptoms are also associated with a high frequency of asthma symptoms.
