Rhinophyma is a pathology characterized by hypertrophy of the sebaceous glands with proliferation of fibrous tissue, the nose has a lobed appearance, as a result there is a deformity of the nasal tip; rhinophyma is a form of rosacea. The prevalence of this variety of rosacea is approximately 5-7% in the population and predominantly in males from the fifth to seventh decade of life, the etiology is not well known, however there are several predisposing genetic and environmental factors. We present the case of an 84-year-old male patient with a pathological history of arterial hypertension and hypothyroidism; his lesion began 10 years ago, as an erythematous lesion with presence of telangiectasia in the wings and nasal tip, not painful. Treatment was performed with a cold scalpel and radiofrequency, transverse cuts were made until the tumor was gone, and finally radiofrequency was used on the entire surface of the remaining lesion. The outpatient procedure was performed without immediate or late complications.

Different aspects of rhinophyma were described: history, etiopathogeny, histopathology, epidemiology, clinical, differential diagnoses, treatments and prognosis. It is emphasized that it is a localized disease in the nose associated to general disorders, and that it brings psychological and social problems. We present a case operated with plastic surgery. The authors present a hypothesis about the etiopathogeny and pathophysiology of the disease and consider rhinophyma treatments as palliative and non-curative.

RESUMEN

@#INTRODUCTION: Rhinophyma, aside from persistent centrofacial redness is a major diagnostic criteria for rosacea. Phyma may be mistaken for hypertrophy of tissue due to an underlying hemangioma. CASE REPORT: A 35-year-old female presented with few erythematous papules on the face and nose 19 years prior to consult. Lesions evolved into multiple erythematous nodules on nose and was mistaken for tissue hypertrophy due to an adjacent congenital hemangioma. Her hemangioma was treated with pulsed dye laser 16 years prior with noted decrease in size and erythema. Recently she noticed enlargement of her nose with persistent redness. She presented with multiple firm, thick irregularly shaped erythematous nodules with prominent pilosebaceous pores and telangiectasia on the nose. Skin punch biopsy was done which revealed hypertrophy and lysis of sebaceous lobules with a moderately dense inflammatory infiltrate of lymphocytes. Histopathological diagnosis was rhinophyma. Patient was treated with low dose isotretinoin (0.20 mkd) with marked flattening of lesions in just one month. CONCLUSION: Distinguishing phyma from tissue hypertrophy caused by hemangioma poses as a diagnostic challenge. Careful dermatological examination and histopathological findings will aid in correct diagnosis. Low dose oral isotretinoin is an effective treatment for rhinophyma.

INTRODUCTION: The rhinophyma is an extremely stigmatizing condition for a patient. The first descriptions of this disease dates back to the time of Hippocrates, which was popular known as alcoholic nose or nasal elephantiasis is responsible for generating a huge deformity in the nose. This deformity derives from a process of hyperplasia and hypertrophy of the sebaceous glands in nasal skin associated with fibrosis and dilated local blood vessels and connective tissue. Surgery is considered the treatment of choice for aesthetic improvement of the injury. METHODS: This was a retrospective study based on analysis of results from 11 patients (10 men and 1 woman) who underwent treatment for rhinophyma between January 2010 to January 2016 at the Plastic Surgery Service of the Teaching Hospital of Federal University of Juiz de Fora and Clinical Plastic Center. RESULTS: Of 11 patients who underwent surgery, a 72-year-old man had basal expansive invasive carcinoma with positive surgical margins after pathological examination of the surgical specimen, however, the patient had pathological free margins after reoperation. Other 3 patients had hypercromy in the intervention region during the immediate postoperative. CONCLUSION: Despite technological advances, the removal of the injury with a cold scalpel technique and the use of postoperative dressings are highly efficient, when combined, provide excellent aesthetic results with low complication rates and high rates of patients' satisfaction, whom mostly had improvement in quality of life after the procedure.

The Rhinophyma isn´t a common disease, however it´s a condition that significantly disturbs the facial aesthetics of people; for that reason, its understanding, appropriate diagnosis and precise management are so relevant to achieve the best possible recovery. Between the management options are numerous tools, since prevention to aggressive surgical procedures. Despite this, patients with rosacea, initial pathological condition, move through different stages of disease´s evolution without adequate medical treatment, causing rhinophyma, final condition of rosacea process. The aim of this paper was to review the current literature about this subject.

RESUMEN

Rhinophyma is a progressively growing benign tumor that has the ability to distort the nasal form. Even though, it is considered a rare subtype of rosacea, it is of great importance for the ability to create: psychic, aesthetic and functional disorders. This report describes a severe case of rhinophyma in a 52 years old male patient, the medical treatment and the surgical technique used for removal.

Sebaceous carcinoma is very rare, occurring in less than 1% of cutaneous neoplasm. It may have numerous clinical presentations but typically appears as yellow or orange firm nodule in periorbital region. In the absence of usual clinical manifestations, diagnosis is difficult which calls for more studies to help with definitive diagnosis. We report an 81-year-old man whose sebaceous carcinoma was diagnosed by histopathologic and immunohistochemical studies. Our patient had clinical presentation as erythematous tumoral lesion, the unusual location of the lesion on the upper lip, very close to the nose, and nasal deformation similar to rosacea without any extension of tumour cells into nasal area. The mechanism by which sebaceous carcinoma resulted in nasal deformation is not clear

RESUMEN

Nasal symptoms are a major health problem for elderly people and affect their quality of life. Inflammatory conditions and influence of age on nasal physiology determine its pathogenesis. Changes of immune function, structural characteristics of nasal mucus and moisture, as well as alterations of mucociliary clearance and nasal air flow are present in these patients. Among the most prevalent diseases are sinonasal rhinitis, both allergic and non allergic, epistaxis, decreased olfactory function and rhinophyma, final stage of rosacea. Improving our understanding of the pathophysiology behind these disorders, allow us to make a proper assessment of these patients and develop effective therapies to give them proper solution, thus improving their quality of life.

A rhinophyma lesion can mask the existence of coexisting occult skin cancers, and many types of tumors can mimic a rhinophyma. Although several coexistent malignant tumors have been reported in patients with rhinophyma, few reports have described the coexistence of rhinophyma and cutaneous squamous cell carcinoma (SCC). Herein, we report on a case of SCC in a 45-year-old man who presented with 2-month-history of a painful, erythematous, globular plaque on the tip of his nose and this lesion mimicked a rhinophyma lesion. The histopathologic examination showed a proliferation of atypical squamous cells that extended into the dermis, which was consistent with SCC. He was treated with surgical excision, and there has been no evidence of recurrence during the follow-up period of 1 year.

RESUMEN

Rhinophyma is a localized swelling of the soft tissue of the nose due to variable combinations of fibrosis, sebaceous hyperplasia and lymphoedema. It develops almost entirely in males. It is often regarded as a complication or end stage of rosacea. It may become grossly distorted in contour. The treatment of rhinophyma involves surgical removal of excess tissue or other means of physical ablation [1]. The primary reason for excision of rhinophyma is cosmetic deformity. Recently, carbon dioxide [CO2] laser has been advocated for the incomplete excision [vaporization] of rhinophyma followed by re-epithelialization from the remaining glandular epithelium [1, 2]. This study aims to evaluate the efficiency of CO2 laser for the treatment of rhinophyma, report the complications and follow-up the patients for a sufficient period. To our knowledge, it is the first to be done at Damascus University and in Syria. This study included 3 patients with moderate rhinophyma seen at Dermatology and Venereology Hospital at Damascus University during a period of two years between 19/2/2006-16/3/2008. A complete clinical history and examination were done for each patient, and the diagnosis of rhinophyma was made depending on the characteristic clinical feature. Patients underwent a single treatment session, and were followed-up for a sufficient period for reporting immediate and late complications, and to determine the degree of improvement of their cosmetic appearance and the recurrence rate. Vaporization of rhinophyma was done by using CO2 laser, 10600nm in the invisible far infrared range of electromagnetic spectrum of radiation. A continuous wave defocused and collimated beam of CO2 laser [spot size of 0.2mm] was used in a drilling mode by the free manual hand piece for the removal of the excess of tissue and remodeling of the nose, and in a scanning mode by the special hand piece of computerized pattern generator, which was used for doing skin resurfacing of the surface of the nose and feathering effect of its periphery at the end of the operation. All patients showed a very good or excellent result which consequently reflected in the improvement of their psychological status. No important complications were reported, and no obvious recurrence occurred after a follow-up period ranged between 1-18 months.. Using CO2 laser for the treatment of rhinophyma is greatly effective, and could be considered among the treatments of choice

RESUMEN

Rhinophyma is characterized by a disfiguring soft tissue hypertrophy of the nose. Severe cosmetic deformity and impairment of breathing may coexist, making the surgical treatment necessary. We present a case of giant rhinophyma, who had difficulty in nasal breathing and took preventive aspirin for CVA. We treated this patient with scalpel tangential excision and skin graft. A giant rhinophyma involved two thirds of nasal dorsum, nasal tip, and both alae. The patient withdrew aspirin 7 days prior to surgery, and rhinophyma was excised with scalpel. The defect was covered with full-thickness skin graft from right supraclavicular area. Five days after surgery, the patient resumed to take aspirin. A skin graft was taken completely and a pathologist confirmed an excised specimen to be a rhinophyma. Within 1 month postoperatively, the patient showed more natural nasal contour and improvement of nasal respiration. Six months after the operation, the nose represented a good appearance without recurrence of rhinophyma. We present a case of giant rhinophyma which had bleeding tendency because of preventive aspirin. We treated this case with scalpel excision and full-thickness skin graft, instead of secondary intention wound healing.
