Escrofuloderma: tuberculosis cutánea y pulmonar asociada a coinfección COVID-19: reporte de caso / Scrofuloderma, cutaneous and pulmonary tuberculosis associated with COVID-19: report of one case

Cutaneous tuberculosis represents 1-1.5% of extrapulmonary tuberculosis, including a variety of clinical conditions. Scrofuloderma and lupus vulgaris are the most common forms. We report a 49-year-old woman who sought medical attention through tele-dermatology concerning a cervical nodule associated with suppuration and cutaneous involvement. The diagnoses of scrofuloderma and pulmonary tuberculosis were confirmed, and during her evolution she presented a coinfection with SARS-CoV-2. The possible associations between tuberculosis and COVID-19 were reviewed.

La tuberculosis (TBC) es producida por el Mycobacterium tuberculosis. Es escrofuloderma es una de sus formas cutáneas e integra el grupo de las TBC colicuativas. El empiema tuberculoso puede ocasionar inflamación de los tejidos de la peripleura o “peripleuritis”, con formación de abscesos fríos peripleuríticos; en su evolución puede abrirse camino a través de los planos profundos hacia piel, constituyendo el denominado empiema por necesidad, que se manifiesta clínicamente con tumoraciones cutáneas que pueden evocar lipomas. Se presenta el caso de un paciente con coexistencia de escrofuloderma a focos múltiples (óseo y ganglionar) y empiema por necesidad.

Simultaneous occurrence of multiple types of cutaneous tuberculosis (TVC and PNT) with internal organ tuberculosis (PTB).

A 40 years old male patient of poor socioeconomic condition presented with a well circumscribed rough surfaced and fissured, verrucous plaque on left axilla and a similar lesion on right foot- which were clinically diagnosed as tuberculosis verrucosa cutis. He also had necrotic papular eruption with varioliform scarring in some lesions on upper back clinically diagnosed as papulonecrotic tuberculid. He had fever and cough and mild weight loss suspicious of pulmonary tuberculosis. On investigation ESR was raised, mild anaemia was present, MT was strongly positive, chest x-ray was suggestive of tuberculosis. So, he was diagnosed as a case of simultaneous occurrence of tuberculosis verrucosa cutis (TVC) with papulonecrotic tuberculid (PNT) with pulmonary tuberculosis (PTB). Standard anti tuberculosis chemotherapy already started to offer cure of all lesions.

Cutaneous tuberculosis and phlyctenular keratoconjunctivitis: a forgotten association.

Cutaneous tuberculosis may be associated with concurrent systemic foci in the body such as lung, lymph node, bone or CNS. Phlyctenular keratoconjunctivitis (PKC) is a manifestation of immunological response to a variety of antigens in the eye, tubercular focus (evident or occult) being the commonest in India. Reports in the existing literature have shown lungs and lymph nodes to be the predominant underlying focus associated with PKC, whereas cutaneous tuberculosis has seldom been found in this situation. We report this forgotten association in two children with cutaneous tuberculosis, one each with lupus vulgaris and scrofuloderma, who also had PKC. Interestingly, one of the cases also had simultaneous lichen scrofulosorum, which is also an immunological response to tubercular antigen and manifests in the skin, thus showing immunological manifestation in two different organ systems along with cutaneous focus of tuberculosis.

Scrofuloderma neck with chest wall abscess

Cutaneous tuberculosis [TB] is among the uncommon varieties of tuberculosis but it is also on the rise, commensurate with the general resurgence of the disease. A case of scrofuloderma of neck with tuberculous abscess of anterior chest wall is, hereby, reported. The patient presented with a non-healing ulcer of neck with swelling of anterior chest wall, which responded to antituberculous treatment. This complication of scrofuloderma neck is very rare and should be kept in mind in the differential diagnosis of such lesions

O objetivo da apresentação é enfatizar a evolução exuberante e rara da infecção cutâneapelo Mycobacterium tuberculosis acometendo pacientes imunossuprimidos

Cutaneous tuberculosis in three HIV-infected patients.

Although tuberculosis is a common disease in patients infected with HIV, cutaneous presentation is not commonly found. The authors report three HIV-infected patients with cutaneous tuberculosis and lung involvement. Patient 1 presented with a nodular skin lesion on the right forearm and the diagnosis was confirmed by histopathology and PCR study. Patients 2 and 3 presented with generalized erythematous papules and vesicopustules on the trunk and extremities. Culture grew M. tuberculosis in patient 2 and M. tuberculosis DNA was detected in the skin lesion of patient 3 by the PCR method.

Tuberculose verrucosa cutis / Tuberculosis verrucosa cutis

A tuberculose verrucosa cutis representa uma forma indolente, verrucosa de tuberculose cutânea, decorrente da inoculaçäo do Mycobacterium tuberculosis na pele de pacientes previamente infectados. Nesta apresentaçäo, relatamos o caso de um paciente com tuberculose verrucosa cutis que demonstra a dificuldade que muitas vezes ocorre para confirmar o diagnóstico definitivo de uma lesäo verrucosa suspeita.

Tuberculosis cutánea: revisión de diez años / Cutaneous tuberculosis: 10 years revision

Se realiza un estudio de 25 pacientes internados en la Unidad de Dermatología del Hospital F. J. Muñiz, durante un período de 10 años (1-1-1988 al 1-12-1997) con diagnóstico de tuberculosis cutánea. En nuestra casuística predominó el sexo masculino con 14 casos. Del total de pacientes, 4 tenían serología para HIV. En lo que respecta a la clínica, 6 casos correspondieron a escrofudermas (1 óseo y 5 ganglionares), 1 gomosa, 2 seudomicetomas, 5 lupus vulgares (2 a focos múltiples), 2 verrugosas, 2 periorificiales, 1 miliar diseminada y 6 formas combinadas. Se analizaron otras variables como: baciloscopía, cultivos PPD y tuberculosis concomitante en otros órganos. Finalmente todos recibieron tratamiento completo, teniendo 18 pacientes buena evolución
