Résumé

Background: Imipenem-resistant Acinetobacter spp. resulting from metallo-beta-lactamase [MBLs]-producing strains have been reported to be among important causes of nosocomial infections and of serious therapeutic problem worldwide


Objective: To determine the prevalence of metallo-beta-lactamase among imipenem-resistant Acinetobacter isolates based on phenotypic methods


Methods: This was an descriptive study in which 140 clinical isolates of Acinetobacter spp. Were initially tested for imipenem susceptibility and later for metallo-beta-lactamase production using combined disk diffusion, double disk synergy test, and Hodge test during 2010. Antibiotic susceptibility of positive metallo-beta-lactamase isolates were further evaluated by disk diffusion technique using CLSI methodology


Findings: Of 140 isolates, 29 [20.7%] were imipenem-resistant Acinetobacter. Positive phonetypic test for metallo-beta-lactamase was 24 [17.1%]


Conclusion: The result of this study is indicative of growing number of nosocomial infections associated with multidrug-resistant Acinetobacter spp. in this region leading to difficulties in antibiotic therapy

Résumé

To determine the prevalence of keratoconus [KCN] in patients with mitral valve prolapse [MVP]. This descriptive study was conducted on patients with MVP at Shahid Rajaei Hospital during 2003-2004. Corneal topography was performed in 392 subjects with absolute mitral valve prolapse based on Perloff's major criteria. Orbscan was carried out in suspected KCN cases. Results: Patient consisted of 247 men [63%] and 145 women [37%] with mean age of 37.27 +/- 11.29 years. The diagnosis of two topographically KCN suspect patients was not confirmed with Orbscan. Positive family history of keratoconus in first-degree relatives was found in three patients. No cases of keratoconus were found. This study suggests that the prevalence of KCN in patient's with MVP could be less than previously reported. A large multicenter study is needed to detect the association of these disorders. Genetic studies are suggested

Résumé

Delayed evaluation and management of penetrating cardiac injuries especially mortar fragments were performed in 30 war victims in the Shahid Rajai Heart Hospital, Tehran. All were men with a mean age of 20.7 years. Pleuritic chest pain was the most common symptom [53.3%], while physical examination was negative in the majority of cases [66.8%]. 50 percent of the cases had pericardial effusion on the echocardiogram. The right ventricle was the most frequent site of involvement [26.6%] followed by the left ventricle [16.6%], right atrium, left atrium, aorta [each 13.3%], pulmonary trunk [10%], and inferior surface of the heart [6.6%]. More than half of the cases had associated hemothorax. Shell fragments were removed in all cases but two. Fragments larger than one centimeter in the vicinity of the heart structures in the pericardium are recommended to be removed
