Résumé

Objective To detect the gene expression changes between urethra plates from hypospadias patients and foreskins from non-hypospadias patients by microarray and to investigate the underlying mechanisms of hypospadias.Methods Twelve hypospadias patients,aged 6-12 months (mean,8 months),were enrolled as the test group,including 5 moderate and 7 severe hypospadias patients.Six age-matched patients underwent circumcision were enrolled as controls.Samples from hypospaidas patient's urethra plates during hypospadias repair and samples from the foreskins during circumcision were obtained and processed into Tri-Reagent immediately for RNA extraction.Oligonucleotide expression microarrays were used to detect genes expression changes in tissues from patients with and without hypospadias.This microarray analysis incorporated 22 000 genes.The intensity of all genes present was analyzed by one-way ANOVA (P<0.01) and Tukey's test.Four estrogen responsive genes,CYR61,CTGF,ATF3 and GADD45β,were tested by RT-PCR in 8 controls,8 moderate hypospadias and 8 severe hypospadias as well.Results Ninty-four genes were detected differentially expressed in hypospadias patients compared with phimosis patients.There were 47 genes upregulated in moderate hypospadias compared with controls(P<0.01),68 genes up-regulated in severe hypospadias compared with controls(P<0.001),17 genes up-regulated in severe hypospadias compared with moderate hypospadias(P>0.05).These genes were involved in different cell functions such as growth regulation and signal transduction.CYR61,CTGF,ATF3 and GADD45β,known to be estrogen responsive or to interact with estrogen receptor were found up-regulated in microarray and the up-regulations were confirmed by RT-PCR.Conclusions The up-regulated genes contribute to the development of hypospadias.Up-regulation of estrogen responsive genes may play important roles in the development of hypospadias.

<p><b>OBJECTIVE</b>To determine the feasibility of single dose intravesical epirubicin in the prevention of recurrent superficial bladder carcinoma.</p><p><b>METHODS</b>We compared the effect of intravesical epirubicin or mitomycin C on tumor recurrence and disease free interval and their side effects after treatment of superficial bladder tumor. 47 postoperative patients with stages Ta to T1 primary superficial bladder carcinoma of grades 1 or 2 were randomized into groups A: single 80 mg epirubicin; B: 40 mg consecutive epirubicin; C: 40 mg consecutive mitomycin C. Patients were followed up for clinical, analytical, and cystoscopic evaluations every 3 months.</p><p><b>RESULTS</b>The disease free intervals of the three groups were found no significant differences (F = 3.25, P > 0.05). The recurrence rate was 6.25% (1/16), 13.3% (2/15), 12.5% (2/16) (chi(2) = 0.496, P > 0.05) in groups A, B, and C at 1 year, and 33.3% (5/15), 26.7% (4/15), 25% (4/16) (chi(2) = 0.290, P > 0.05) at 3 years after operation, respectively. Side effects of group A (13.3%) were lower than those of group B (46.7%) or C (43.8%) (chi(2) = 14.56, P < 0.01).</p><p><b>CONCLUSIONS</b>Single dose of epirubicin given intravesically immediately after tumor resection is effective in preventing tumor recurrence.</p>

ObjectiveTo evaluate nerve-sparing semi-cystectomy for bladder cancer at trigone or bladder neck.Methods6 patients suffering from multi and invasive cancer at trigone or bladder neck underwent semi-cystectomy and in 5 of which the nerve-sparing technique was carried out.The post-operative outcome was evaluated.ResultsAll patients were followed up for 9~36 months.With nerve sparing surgery,sexual function has been preserved in 4 of the 5.Radical cystectomy was performed for 1 case because of tumor relapse.Unilateral hydronephresis was noted in another one.ConclusionsQuality of life has been good and erectile function can be preserved by nerve-sparing technique.

Résumé

Objective To study the management of prostate sarcoma. Methods Four cases of prostatic sarcoma were presented with review of the literature. Results Of the 4 cases,two were spindle cell sarcoma,one leiomyosarcoma and the other rhabdomyosarcoma.Combined modality therapies were used,including surgical,radiation,chemotherapy,Chinese herb medicine and immunotherapy.One case has been out of follow up and one has survived 22 months without tumor.The other two patients died within 7 and 19 months after operation. Conclusions Prostatic sarcoma is an aggressive malignancy with a bad prognosis.Individualized treatment should be formulated.

Résumé

Purpose:To study the diagnosis and treatment of urachal carcinoma, and to improve its prognosis.Methods:Three patients with urachal carcinoma are described. In addition, many related literatures from Medline and Chinese journals are reviewed. Results:All three cases were presented with subumbilical masses, and received urachectomy and partial cystectomy. One of them had Bricker's sugery. Two patients survived for 53 and 25 months without tumor. Conclusions:History and physical examination should be given special emphasis. Early detection and well planned regimen can improve the patients' prognosis.
