Résumé

Zollinger-Ellison syndrome is a very rare disease that's caused by tumor having gastrin-producing cells, and this is accompanied by hypergastrinemia leading to gastric acid hypersecretion, peptic ulcer and their complications. A 60-year-old man presented with epigastric pain and soreness he had experienced for 7 days. The endoscopic findings showed reflux esophagitis and multiple active ulcers at the antrum, the duodenal bulb and the 2nd and 3rd portions of the duodenum. The fasting serum gastrin level was markedly elevated above 1,470 pg/mL and this was consistent with the findings of gastrinoma. We confirmed the presence of gastrinoma via the abdomen CT scan and selective intra-arterial calcium injection. This is the first Korean case report of gastrinoma that was localized by selective intra-arterial calcium injection.

Résumé

Most patients with hypertension and no definable cause are said to primary or essential hypertension, while other patients with hypertension and specific causes are siad to secondary hypertension. Approximately 5% fo patients with hypertension have specific causes. The patients of secondary hypertension should not be ignored for the reason: The elimination of cause for hypertension can normalize the blood pressure. Juxtaglomerular cell tumor is a rare condition of secondary hypertension, with only approximately 70 cases reported to date. The higher the level of blood pressure, the most likely that various cardiovascular diseases will develop prematurely through acceleration of atherosclerosis. But Acute myocardial infarction in young hypertensives has been reported rarely. We describe the rare case of a young patient with acute myocardial infarction associated with secondary hypertension by renal juxtaglomerular cell tumor.

Résumé

We experienced a rare case of trachea diverticula combined with the sequela of tuberculosis and a fungus ball. The patient had complained of coughing and hemoptysis for a long time after experiencing tuberculosis. He was admitted due to hemoptysis and the aggravation of coughing. The CT scan showed a variable sized trachea diverticula combined with tuberculosis sequela and a fungus ball in the right lung fields. The diagnosis was made by bronchoscopy and a CT scan. After bronchial artery embolization and conservative treatment, the patient's symptoms improved and the patient was discharged.
