Tratamiento con fenestración: Reporte de un caso y revisión / Los quistes hepáticos congénitos pueden ser únicos, múltiples, o distribuirse difusamente en el parénquima hepático („poliquistosis hepática‰). En la poliquistosis hepática, el número, tamaño de los quistes puede asociarse a poliquistosis renal, con la gravedad de la enfermedad renal, la cual condiciona el pronóstico. El dolor y el aumento progresivo de la cavidad abdominal, asociado al crecimiento de los quistes empeoran la calidad de vida llegando a incapacitar al paciente. El tratamiento médico conservador consiste en modificaciones de la dieta, AINES, uso de antihipertensivos y diuréticos si se desarrolla hipertensión arterial. Otros incluyen el empleo de drenaje percutáneo dirigido por ultrasonido, fenestración, hepatectomía parcial y finalmente el trasplante hepático y/o hepático-renal. Describimos el caso de una paciente femenino de 39 años de edad, con poliquistosis hepática y renal quien no responde a tratamiento médico conservador; presentando dolor abdominal difuso persistente, disnea a moderados esfuerzos, limitación para la actividad de la vida diaria e incapacidad laboral, a quien se le practicó fenestración y destechamiento de los quistes hepáticos con evolución postoperatoria satisfactoria, y mejoría de la calidad de vida.

The congenital liver cysts may be unique, multiple or diffusely distributed in the liver parenchyma ("polycystic liver"). The pain and progressive increase in the abdominal cavity, associated with the growth of cysts worsen the quality of life coming to incapacitate the patient. Medical treatment consists of conservative amendments to the diet, AIN, use of antihypertensive and diuretics if developed hypertension. Others include the use of percutaneous drainage led by ultrasound, fenestration, partial hepatectomy and eventually liver transplantation and / or liver-kidney. The following case is a patient of 39-year-old female, with polycystic kidney and liver who does not respond to conservative medical treatment; presenting diffuse persistent abdominal pain, breathlessness on moderate efforts to limit the activity of daily living and incapacity to work, who was practiced remove the wall and fenestration of liver cysts with satisfactory postoperative evolution and improvement of the quality of life.
