RÉSUMÉ

Objective: Through the use of high-density polyethylene implants in adults with cleft lip and palate nasal deformities and abnormal functions, to evaluate the clinical effect of postoperative patients on improving nasal shape and symptoms. Methods: A retrospective study of 12 patients with nasal deformities after cleft lip and palate surgery in the Shanghai Ninth People's Hospital Affiliated to Shanghai JiaoTong University School of Medicine from January 2018 to January 2022 was completed, including 7 males and 5 females, with the age ranging from 18 to 29 years. All the patients underwent nasal deformity correction, and nasal septum correction was performed if necessary. High-density polyethylene implants (MEDPOR/Su-Por) were used intraoperatively. Follow-up for at least 6 months was performed to measure the relevant appearance indicators and subjective Visual Analog Scale (VAS) scores, and to compare the clinical effects before and after surgery. SPSS 22.0 software was used for statistical analysis. Results: Before and after surgery, the average VAS score of nasal obstruction decreased by (4.83±0.94) points; the average VAS score of appearance satisfaction increased by (3.92±1.08) points; the height of nasal columella was increased by (1.79±0.78)mm; the height of nasal tip was increased by (2.79±1.50)mm; the height of ipsilateral nostril was increased by (1.83±0.62)mm; the width of the ipsilateral nasal floor was reduced by (0.42±0.47)mm. All of above were statistically significant (all P<0.05). Conclusion: High-density polyethylene implants can effectively improve the shape and function of the nose in operations related to cleft lip and palate nasal deformity and abnormal functions, and are an ideal synthetic material.

ObjectiveTo analyze the effects of respiratory control measures before and after COVID-19 epidemic on influenza virus. MethodsThe percentage of influenza-like cases, the positive rate of influenza virus and the change of influenza outbreaks before and after the COVID-19 pandemic were compared and analyzed by selecting the data of influenza surveillance sentinel-points in Shanghai. ResultsThe percentage of influenza-like illness after the outbreak of COVID-19 in 2020 was significantly higher than that during the same period between 2017 and 2019. The positive rate of influenza virus detection in 2020 was significantly lower than the average rate of influenza virus detection from 2017 to 2019 with significant statistical difference (χ 2=2 359.07, P<0.001). The number of outbreaks in 2020 was significantly lower than that from 2017 to 2019. ConclusionDuring the respiratory season, personal protection and reduction of human aggregation can effectively reduce the infection of influenza and the incidence of influenza in the population.

This article summarized the laws and regulations on the prevention and control of infectious diseases, classification of infectious diseases, and measures for the prevention and control of infectious diseases in Japan. Ⅰt focused on the containment of spread of infectious diseases, classification and categorization in the treatment and quarantine. Ⅰn addition, the response to the novel coronavirus pneumonia in the early stage of transmission in Japan was introduced. Moreover, we propose specific suggestions to improve the system of laws and regulations on the prevention and control of infectious diseases in China, improve the classification of infectious diseases, clarify the rights and obligations of governments at all levels, establish medical facilities with"classification and combination of normal and emergency situations"for the treatment of infectious diseases, practice emergency simulation exercises regularly, and improve risk communication.

Objective: To investigate the clinical effects of single-stage auricular reconstruction and hearing rehabilitation in children with microtia and external auditory canal atresia. Methods: Sixty eight cases of microtia with external auditory canal atresia (53 males and 15 females, age from 7 to 12 years, with a median age of 8.8 years), who received operations in Shanghai Ninth People's Hospital, Shanghai Jiaotong University School of Medicine from July 2017 to December 2019 were collected.A total of 28 cases received auricle reconstruction with high-density polyethylene (Medpor) framework and hearing reconstructions, among which 20 patients received the traditional external auditory canal and middle ear repair (EACR), and eight patients were implanted bone conduction device bone bridge(BB) simultaneously.In the control group, 40 patients only received Medpor frame implantation for auricle plasty. Postoperative changes in auricle morphology and auditory function and postoperative complications were evaluated. Results: After three to thirty months follow-ups, the auricles shape recovered well in all three groups. The average scores of 14 fine structures in the auricles were 9.43(EACR) and 10.67(BB) points. The average score of auricle symmetry were 6.83(EACR) and 6.00(BB) points. There was no significant difference compared to the auricle reconstruction group (8.23/6.20 points). P>0.05. After surgery, the average hearing improvement in the BB group was 43.33 dB HL and the average speech recognition threshold declined 42.28 dB HL. In the EACR group, the average hearing improvement was 4.13 dB HL and the average speech recognition threshold declined 11.36 dB HL. No vertigo, tinnitus, cerebrospinal fluid leakage and other complications occurred in all the patients. In the EACR group, sensorial hearing loss, auricle stent fracture, ear canal restenosis and ear canal atresia occurred in one patient respectively. In the auricle group, one auricle stent exposure and one facial branch nerve injury occurred. Nearly ten patients had difficulty in hair growth at scalp incisions. Conclusions: The operation of single-stage auricular reconstruction and hearing rehabilitation for microtia is feasible. The methods of hearing reconstruction should be determined by evaluating the development of the inner and middle ear of the patients. For those with poor mastoid development, bone bridge implantation is recommended to achieve a stable and significant hearing effect.

Objective A/H1N1(pdm09) viruses were the dominant strains in Shanghai during 2018-2019 influenza surveillance year.This study is to provide a scientific reference for clinical drug use by investigating the susceptibility of A/H1N1(pdm09) viruses to neuraminidase inhibitors(NAIs). Methods Sixty strains of A/H1N1(pdm09) viruses were randomly selected for testing the susceptibility and drug resistance to Oseltamivir and Zanamivir by means of neutaminidase inhibition and neuraminidase (NA) gene sequencing. Results The 60 epidemic strains all proved to be susceptible to Oseltamivir and Zanamivir and the susceptibility was not observed to be decreased or remarkably decreased.In genetic sequencing, NA was not observed to present amino acid mutation at the key sites and auxiliary sites in catalytic activity, which confirmed the results of the phototypic detection of neuraminidase inhibition. Conclusion The subtype influenza viruses A/H1N1(pdm09) circulating in Shanghai during 2018-2019 surveillance year are still sensitive to NAIs, which provides a scientific reference for clinical use of drugs.However, we monitored only a number of strains and think that the work monitoring antiviral susceptibility should be continued with the wide use of the drugs.

ATG5 and BECLIN-1 belong to two kind of crucial autophagy-related proteins included the formation of autoph-agosomes,in addition to the promotion of autophagy,enhances susceptibility towards apoptotic stimuli,therefore,ATG5 and BECLIN-1 are considered to be a molecular link between autophagy and apoptosis.Preliminary data revealed that ATG5 media-ted the formation of autophagosomes by ATG5-ATG12-ATG16L ubiquitination system,while BECLIN-1 induces autophagy by phosphatidylino3-kinase (PtdIns3KC3)complex.But now it is believed that truncated ATG5(tATG5-N)and truncated BEC-LIN-1(BECLIN-1-C),an amino-terminal cleavage product of ATG5 and carboxyl-terminal cleavage product of BECLIN-1, could induce apoptosis.The research summarize the progress on ATG5 and BECLIN-1 regulated autophagy and apoptosis,so as to further reveal the molecular mechanism of they regulate autophagy and apoptosis.

Objective To study the epidemiologic characters of influenza in Shanghai from 2013 to 2015 . Methods Data of outpatient visits for influenza-like illness ( ILI ) were collected through influenza surveillance hospitals .Pathogenic surveillance to part of ILI samples was conducted . Results From 2013 to 2015, the ILI consultation rate ranged from 0.68%~3.73%, with an average rate of 1 .46%.In total 51 445 samples were collected , and 12673 samples were positive , with the positive rate of 24 .63%.The ratio of the patient visited for ILI aged from 25 to 60 was higher than other groups . Conclusion The peak of epidemic of influenza in Shanghai was in winter and summer .The ratio of the patients visited for ILI aged over 60 increased with years .More measures should be taken to protect the elder people .

The aim of the current research topic was to test the generalized additive method (GAM),using data from the analysis and prediction on influenza-like illness (ILI) in Shanghai.Through collecting the meteorological data as well as the ILI from 2006 to 2010,we established several nonlinear regression candidate models based on the GAM.These models considered factors as:the nonlinear dependence on the meteorological data,i.e.weekly average temperature and weekly average (maximum) temperature differences and the ILI.The AIC (Akaike information criterion) involved two simplified models which were implemented for further analysis and prediction.Finally,numerical examples showed that the proposed models could shed light on the connection between the meteorological data and the ILI.GAM could be used to fit the frequencies of ILI and meteorological factors in Shanghai.The proposed models were able to accurately analyze the onset of ILI,implying that GAM might be suitable for the prediction and analysis of those meteorological correlative diseases.

Objective To describe the epidemiological characteristics of patients with Creutzfeldt-Jakob disease (CJD) in Shanghai from 2006 to 2012.Methods Clinical and epidemiological information on CJD patients from Shanghai CJD Surveillance Network was analyzed.Cerebral spinal fluid (CSF) and blood specimens from patients were collected and used for detecting the 14-3-3 protein,and polymorphism of 129 amino acid and mutation of PRNP genes.Data was processed by EpiData (V3.0) and analyzed by SPSS (V17.0).Results In totally,one definite CJD patient together with 56 probable and 17 possible sporadic CJD patients were identified.One E200K genetic CJD case was diagnosed and another one was clinically diagnosed.No period-or geographic-related events were observed for these cases,but the houses of the two genetic CJD cases were close to each other.The mean age of onset of the probable CJD patients was 62 years old which was significantly older than that of those possible CJD patients.Conclusion Most of the CJD patients identified in Shanghai were sporadic and the number was stable from 2006 to 2012.The mean age of onset of those probable CJD patients was older than that of the possible CJD patients.
