Résumé

Objective To observe the clinical results of treatment for ischemic necrosis of the femoral head with autogenous spongy bone morselized and BMP implantation after arthroscopic core decompression.Methods Retrospectively analyzing the clinical data of 23 cases(30 hips),who were treated with autogenous cancerous bone morselized and BMP implantation after arthroscopic core decompression.Results 23 patients were followed up after discharge,follow-up time was 12 to 36 months,with an average of20.4 months.X-ray demonstrate good bone restoration in femoral head.In the observation period,femoral fractured or collapsed in 1 case of stageⅡ C、1 case of stage ⅢB,the original pain symptom aggravating,so artificial joint replacement were implemented.The other cases can maintain the results obtained in postoperative 6 months.According to the comprehensive evaluation.all the cases were 40~92scores after treatment.The rate of excellent and good results Was 76.7%.The results were better in stage Ⅰ(5/5),Stage Ⅱ(12/16),and Stage Ⅲ(6/9).Hip pain disappeared or lightening obviously,function improved obviously,Xray demonstrate femoral head contour smoothly and joint clearance normal.Conclusion Autogenous spongy bone morselized and BMP implantation after arthroscopic core decompression had good clinic curative effects in treatment of inchoate ischemic necrosis of the femoral head.

Objective To search for a better internal fixation method in the treatment of humeral shaft fracture by comparing biomechanics and clinical results of fracture fixation with rectangle intramedullary nail and compressed plate. Methods ①Fracture models were produced in 24 wet humeral bones of adult cadavers. The models were randomly divided into two groups fixed with rectangle intramedullary nail and compressed plate respectively. Four-point bending test, twisting test and pull test were performed on the facture models after fixation and another 12 intact wet humeral bone specimens. ②The humeral shaft fracture patients were treated with rectangle intramedullary nail (92 cases) and compressed plate (78 cases). The experimental and clinical data were analyzed and compared. Results The statistical results of bending test and twisting test revealed that the rectangle intramedullary nail was better than the compressed plate for the fixation of humeral shaft fracture (P0.05). Fracture healing time was 13 weeks in average and the rate of excellent and better function was 91.3% in the rectangle intramedullary nail fixation group, while both were 18.2 weeks and 80.8% respectively in the compressed plate fixation group. Conclusion The rectangle intramedullary nail fixation is more suitable than the compressed plate fixation for the treatment of humeral shaft fracture.

Résumé

Objective To assess the clinical outcome of retrograde intramedullary interlocking nail in the treatment of femoral suplracondylar or intercondylar fracture with femoral shaft fracture.Methods 3 cases of femoral supracondylar fracture with femoral shaft fracture and 6 cases of intercondylar femoral fracture with femoral shaft fracture experienced the fixation with retrograde intramedullary interlocking nail.Results The patients were followed up for 7~ 20 months,on an average of 10 months.All the patients recovered in an average time of 6 months,with excellent knee joint movement.Conclusion Retrograde intramedullary interlocking nail has obvious advantages in the treatment of femoral supracondylar or intercondylar fracture with femoral shaft fracture and the clinical outcome is satisfactory.
