Résumé

Introduction: A cross-sectional study of 1094 (boys = 665; girls = 429) rural school children aged 11- 18 years of Midnapore Sadar North subdivision, Paschim Medinipur District, West Bengal, India, was undertaken to evaluate their growth pattern and nutritional status. Methodology: Anthropometric measurements including weight and height were measured following standard techniques. Underweight and stunting were used as indicators of nutritional status. Underweight and stunting were defined as weight-for-age (WAZ) and height-for-age (HAZ) <-2 z-scores, respectively, of the National Centre for Health Statistics (NCHS) guidelines. Results: Results revealed that boys were significantly heavier than girls from age 16 onwards; they were also significantly taller from age 14 years. The mean WAZ for boys and girls were -1.488 and -1.417, respectively. The corresponding mean values for HAZ were -1.317 and -1.486. The overall rates of underweight and stunting were 28.3% and 27.8%, respectively. The prevalence of underweight was significantly higher among boys (31.0%) than girls (24.2%). These rates for stunting were 27.4% and 28.4%, for the boys and girls. The rate of underweight and stunting was more in late adolescents (15-18 years) than early adolescents (11-14 years). In boys, the prevalence of stunting was significantly (1.5 times) more in late adolescents than early adolescents. According to the WHO classification for assessing severity of malnutrition, the rates of stunting were medium in both sexes. The rates of underweight were high and very high for girls and boys, respectively. Conclusion: In conclusion, present study provided evidence that the nutritional status of these adolescents were not satisfactory especially among late adolescents.

Résumé

Sigmoid volvulus complicating pregnancy is an extremely rare complication with fewer than 76 cases reported in literature. We report a case of sigmoid volvulus complicating pregnancy. The sigmoid colon was resected and Hartman's colostomy was performed. The patient had a successful recovery. Aggressive resuscitation followed by early surgical intervention should be undertaken to reduce maternal and fetal morbidity and mortality.

Résumé

Axillary bud explants were induced to form shoots on Murashige and Skoog's (MS)' basal medium. Best yield (9 shoots per explant) was obtained when the medium was supplemented with adenine sulphate (40 mg/L) and 6-benzylaminopurine (2.25 mg/L). The shoots were rooted on half strength MS basal medium supplemented with indole butyric acid (0.5 mg/L) and having thiamin-HCl (800 mg/L). Regenerated plantlets were successfully acclimatized. This is the first report of micropropagation in the genus Mussaenda without callus intervention.

Résumé

BACKGROUND & OBJECTIVES: Biotyping of Vibrio cholerae O1 using multiplex PCR (ctxA-tcpA) exploits the nucleotide sequence differences of the major subunit protein of the toxin co-regulated pilus (TCP) gene (tcpA) to differentiate between the classical and El Tor biotypes. However, the presence of classical biotype specific tcpA amplicon with the El Tor strains often complicates the interpretation. The effect of PCR variables on the amplification of biotype specific tcpA in the multiplex PCR has been investigated. METHODS: Reference strains of toxigenic V. cholerae O1 belonging to classical and El Tor biotypes were selected to optimize the PCR variables for the unambiguous biotype determination by multiplex PCR. RESULTS: In the multiplex PCR assay, a reduction in the reaction volume from 100 microliters to 25 microliters and the annealing temperature of 64 degrees C, the El Tor strain produced ctxA amplicon (302 bp) along with tcpA amplicons of 618 bp and 472 bp which are specific for classical and El Tor tcpA respectively. The simplex PCR with biotype specific tcpA primer pairs showed the amplification of either 472 bp or 618 bp tcpA amplicon with El Tor template. With the classical biotype strain, the specific primer pair yielded tcpA amplicon of the expected size. Lowering of PCR annealing temperature from 64 to 60 degrees C resulted in the elimination of the amplification of the nonspecific tcpA amplicon with El Tor strain. INTERPRETATION & CONCLUSION: A comparison of the theoretical melting temperature (Tm) values of the reacting primers, and their alignment to the biotype specific tcpA revealed the basis of unambiguous biotyping of V. cholerae O1 at a PCR annealing temperature of 60 degrees C.

Résumé

BACKGROUND: Obstruction of the suprahepatic inferior vena cava (IVC) by a membrane or stricture is the commonest cause of Budd-Chiari syndrome in the eastern hemisphere. We present our experience with the outcome of balloon cavoplasty in such cases. METHODS: We followed up 40 consecutive cases of Budd-Chiari syndrome over seven years. Doppler study of hepatic venous outflow tract (in all cases), liver biopsy (30 cases) and necropsy (two cases) were performed. Balloon cavoplasty was done in selected cases. RESULTS: Of 40 patients with BCS (mean age 35.2 [SD 8.7] years; 26 men) 5, 5 and 30 had fulminant, acute and chronic presentation, respectively. Inferior vena cavography was performed in 32 cases, and showed membranous obstruction of the IVC in 12, segmental occlusion of the IVC in 11 cases, and block in both the IVC and the main hepatic veins in the rest. Successful balloon cavoplasty was done in 18 cases with obstruction of the IVC (membrane or stricture); 15 of them are well over a mean follow up of 56 (14.6) months. Three patients developed restenosis; two of them, treated with redilatation, are doing well, and one died of septicemia and hepatic failure following a surgical bypass. Pressure gradient between the IVC and right atrium decreased significantly after cavoplasty (15.4 [2.8] vs 6.6 [2.0] mmHg; p< 0.001). CONCLUSION: Balloon cavoplasty gave encouraging results in the management of Budd-Chiari syndrome due to membranous obstruction or stricture of the IVC.

Résumé

BACKGROUND: Diagnosis of Budd-Chiari syndrome (BCS) is often missed unless its possibility has been kept in mind. Obstruction of inferior vena cava (IVC) is reportedly the most frequent cause of BCS in Afro-Asian variety. AIM: An attempt was made to classify BCS (in an eastern Indian population) etiopathologically. PATIENTS AND METHODS: Thirty consecutive cases of BCS presenting over a period of five years were included. Following a thorough physical examination, necessary investigations (including coagulation profile, ultrasonography (with Doppler study) of hepatobiliary tract, hepatic vein and IVC angiography (n = 22) and liver biopsy (n = 26, including autopsy in two cases) were performed. RESULTS: Mean age at presentation was 32.7 +/- 10.36 years (range 12-60 years) with M:F = 21:9. Clinical presentations included, hepatomegaly in 28 (93.3%), ascites in 27 (90%), splenomegaly in 15 (50%), pain in abdomen in 26 (86.6%), jaundice in 10 (33.3%), back veins in 20 (66.6%) and gastrointestinal bleeding in three (10%) cases. Amongst the total of 30 patients, four, six and 20 cases presented as fulminant, acute and chronic BCS respectively. Twenty four cases of BCS could be diagnosed by ultrasonography alone, while the remainder required angiography for diagnosis. IVC and hepatic vein angiography revealed membranous obstruction in nine, partial stricture of IVC in six, and IVC and/or hepatic vein block in others. The etiopathological nature in 30 cases were as follows: idiopathic membranous obstruction in nine (30%), hepatocellular carcinoma in six (20%), idiopathic stricture in six (20%) cases and one case (3.3%) each of the following: cholangiocarcinoma, renal cell carcinoma, chronic pancreatitis, hydatid cyst in liver, protein S deficiency, oral contraceptive use, nephrotic syndrome (with antithrombin III deficiency), polycythemia rubra vera and chronic lymphatic leukemia. CONCLUSION: Idiopathic membranous obstruction and stricture of IVC are the commonest cause of BCS in the eastern part of India. Hepatocellular carcinoma is also a common cause, presenting in the fulminant form. Ultrasonography may be a helpful screening test for BCS, but IVC and hepatic vein catheterisation is essential for a complete work up of these patients.

Résumé

Effect of Thyroxine was studied in histamine induced bronchospasm in guinea pigs. Chronic treatment with the drug significantly protected against experimental bronchospasm. Thyroxine also potentiated salbutamol evoked bronchodilation in this experimental model. Up-regulation of beta-2 adrenoceptors in bronchial smooth muscle may be the probable mechanism of action of thyroxine.

Résumé

40 women suffering from 'dysfunctional uterine bleeding' (DUB) were treated by progesterone (P) and signs and symptoms of 'extragenital effects' of P were noted. In addition to the previous ones reported from this laboratory, a new crop of effects, which, as far as we are aware of, have never been reported in the literature, were found and included: (i) Changes in the frequency of EEG waves, (ii) changes in the ECG (iii) changes in psychoanalytical scoring. Further (iv) with most of the parametres, the intensity of the changes showed considerable waning with passage time, despite the fact that the subjects were still receiving P. Blood P levels similarly fell considerably in the initial phase of the therapy, but recovered to some extent afterwards, despite the continuance of P therapy.

Résumé

The effects of very low-dose X-ray on collagen and Zn and Fe contents of rat skin were studied. CFS male rats were exposed to chronic, intermittent low dose X-radiation at the dose rate of 0.015 cGy/sec/day (7 days irradiation alternating with 7 days rest) for periods of three months and six months. Transmission electron microscopy showed hyalinization collagen on days 180 and 360 in the six months group. Atomic absorption spectrometry revealed significantly decreased zinc levels (P < 0.001) only on day 360 in six months group whereas iron increased significantly (P < 0.001) in all treated groups. This radiation induced redistribution of zinc and iron and subsequent changes in dermal collagen possibly confirm the hazardous effect of long term exposure to X-rays. Our findings also provide some information on the mammalian response threshold at low dose X-irradiation.

Résumé

There were 53 cases where caesarean hysterectomy was done during the last 5 years at Eden Hospital, Calcutta. Majority of the cases were in the age group of 22-32 years and parity varied from 0 to 6. There were 54.71% cases which were due to rupture of previous caesarean section scar mostly lower uterine segment done in teaching institutions. Seventy per cent cases were booked. Only 8 babies survived following caesarean hysterectomy. Maternal mortality was nil. By good intranatal monitoring, more vigilance, performing less caesarean section and better health education one can lower the incidence of caesarean hysterectomy.

Résumé

The antiulcerogenic property of Ocimum sanctum Linn (Tulsi) was studied in pyloric ligated and pyloric ligated & aspirin treated rats. The extract of OSL reduced the ulcer index, free & total acidity on acute and chronic administration. Seven days pretreatment with the drug increased the mucous secretion also. It may be concluded that OSL extract has antiulcerogenic property against experimental ulcers, and it is due to its ability to reduce acid secretion and increase mucous secretion.

Résumé

Detection of toxoplasma IgM antibodies by employing enzyme linked immunosorbent assay (ELISA) technique was developed using different enzymes viz, horseradish peroxidase (HRP) (EC. 1.11.17), urease (EC. 3.5.15) and penicillinase (EC 3.5.2.6) as markers. Of these enzymes, HRP is light sensitive and needs dark chamber, also inactivated by preservative sodium azide. Similarly urease test system is extremely pH sensitive and demands special care during ELISA technique. Whereas penicillinase showed certain distinct advantages viz. stable at room temperature, high specific activity and economical. In the present studies it was observed that the sensitivity of penicillinase is similar to HRP and urease, marker enzymes used in commercially available diagnostic kit. The prominent feature of detection of toxoplasma IgM antibodies involving these three enzymes are: a) Shorter incubation time (About 2.5 hours) b) No false positive reaction. Moreover, these enzyme conjugates were prepared from F (ab')2 fragments of antitoxoplasma rabbit serum to elicit specific interaction with IgM antibodies only, avoiding cross interaction with other non-specific proteins like compliment systems and rheumatoid factor.
