Objective @#To investigate the effects of Tuina targeting different body parts on the behaviors and gut microflora of rat models with valproic acid (VPA)-induced autistic spectrum disorder(ASD).@*Methods@#Twenty female Sprague Dawley (SD) rats with 12.5 d of pregnancy were randomly divided into VPA model group [intraperitoneal injection of VPA (600 mg/kg), n = 15] and saline group (intraperitoneal injection of equal amount of normal saline, n = 5). The offspring male rats injected with saline were secleted as control group. The offspring male rats injected with VPA were randomly divided into VPA, dorsal, and abdominal groups (n = 7 in each group). On the 21st day after birth, three-chamber social test, open field test, and marbleburying test were carried out to observe the social abilities, anxiety behaviors, and stereotypical behaviors of rats in the four groups. Rats in dorsal and abdominal groups underwent Tuina for 14 d, twice a day. On the 35th day, behavioral tests were conducted again, and intestinal contents were taken for species composition and structural analysis, as well as marker and differential species analysis.@*Result@#(i) According to behavioral observations, compared with VPA group, the social and movement time in the central open field of rats in dorsal group increased significantly (P <0.05), and the number of buried marbles decreased markedly (P < 0.01), indicating improvement on their social abilities, anxiety behaviors, and stereotypical behaviors as consequences of dorsal Tuina; and the number of buried marbles was reduced as well in abdominal group when compared with VPA group (P < 0.05), suggesting the improvement on their stereotypical behaviors following abdominal Tuina. In the marble-burying test, the number of marbles buried in dorsal group was less than in abdominal group, and the stereotypical behaviors were improved more significantly (P < 0.05), and there were no significant differences in the three-chamber social and open field tests between the two groups (P >0.05). (ii) In accordance with intestinal microflora detection results, compared with VPA group, both dorsal and abdominal groups showed increased richness (P < 0.05) and elevated diversity (P < 0.05 in dorsal group and P < 0.01 in abdominal group) in intestinal microflora. The results of differential analysis indicated that at the phylum level, compared with VPA group, the relative abundance of Firmicutes in rats in abdominal group showed a significant reduction trend (P < 0.05); at the genus level, compared with VPA group, the relative abundance of Lactobacillus in rats in dorsal and abdominal groups decreased significantly (P < 0.05). Dorsal group also showed significant increase in the genus Blautia in the analysis of marker species comparedwith VPA group (P < 0.05).@*Conclusion@#Tuina impacted the behavior and gut microflora structure of ASD model rats. Dorsal intervention had a significant effect on social abilities, anxiety behaviors, and stereotypical behaviors of ASD model rats, while abdominal intervention only had an obvious effect on stereotypical behaviors. Both dorsal and abdominal interventions increased the richness and diversity of gut microflora of ASD model rats, with abdominal intervention improving the intestinal microbial diversity more significantly and resulting in a more uniform species distribution.

Objective@#Sialidosis type 2 has variants that are both catalytically inactive (severe), while sialidosis type 1 has at least one catalytically active (mild) variant. This study aimed to discuss the structural changes associated with these variants in a newly reported family carrying N-acetyl-α-neuraminidase-1 (NEU1) variants and explore the clinical characteristics of different combinations of variants in sialidosis type 1. @*Methods@#First, whole-exome sequencing and detailed clinical examinations were performed on the family. Second, structural analyses, including assessments of energy, flexibility and polar contacts, were conducted for several NEU1 variants, and a sialidase activity assay was performed. Third, previous NEU1 variants were systematically reviewed, and the clinical characteristics of patients in the severe-mild and mild-mild groups with sialidosis type 1 were analyzed. @*Results@#We report a novel family with sialidosis type 1 and the compound heterozygous variants S182G and V143E. The newly identified V143E variant was predicted to be a mild variant through structural analysis and was confirmed by a sialidase activity assay. Cherry-red spots were more prevalent in the severe-mild group, and ataxia was more common in the mild-mild group. Impaired cognition was found only in the severe-mild group. Moreover, patients with cherry-red spots and abnormal electroencephalographies and visual evoked potentials had a relatively early age of onset, whereas patients with myoclonus had a late onset. @*Conclusion@#Changes in flexibility and local polar contacts may be indicators of NEU1 pathogenicity. Sialidosis type 1 can be divided into two subgroups according to the variant combinations, and patients with these two subtypes have different clinical characteristics.

Objective: To investigate the effects of exosome derived from miR-133a-3p engineered human umbilical cord blood mesenchymal stem cells (ucMSC) on myocardial repair after acute myocardial infarction (AMI) in rats. Methods: UcMSC was amplified and cultured in vitro. Lentiviral carrying miR-133a-3p and negative control vectors were transfected into ucMSC. Exosomes secreted by the transfected ucMSC were named miR-133a-3p-Exo and miR-NC-Exo, respectively. The AMI model of rats was established by ligation of the left anterior descending coronary artery. MiR-133a-3p-Exo or miR-NC-Exo were then injected into the border zone of the infarct area. Cardiac function was assessed by echocardiography after twenty-eight days of intervention, and Masson staining was used to evaluate the area of myocardial fibrosis post-AMI. The myocardial apoptosis after infarction was evaluated by TUNEL staining and the angiogenesis after infarction was evaluated by immunofluorescence staining in the current study. Results: Compared with the miR-NC-Exo group, the left ventricular ejection fraction in the miR-133a-3p-Exo group was significantly increased ((47.4%±9.8%) vs. (64.2%±8.9%), P<0.05). While the myocardial fibrosis area ((31.2%±7.3%) vs. (18.0%±1.5%), P<0.01) and the percentage of apoptotic cardiomyocytes ((25.6%±3.6%) vs. (15.1%±4.4%), P<0.05) was significantly reduced in the miR-133a-Exo group. Besides, the expression of CD31 and α-smooth muscle actin (α-SMA) were also increased significantly in the miR-133a-3p-Exo group compared to the miR-NC-Exo group (CD31: (2.9±0.9) vs. (13.9±2.0), P<0.000 1, α-SMA: (3.5±0.9) vs. (11.0±1.6), P<0.000 1). Conclusion: Exosome derived from miR-133a-3p engineered ucMSC effectively inhibited myocardial apoptosis and promoted angiogenesis, thus improving the cardiac function after myocardial infarction in rats.

AIM: To investigate long-term efficacy and safety of patients after excimer laser in situ keratomileusis(LASIK)surgery in 20a.METHODS: A retrospective study.Patients who underwent LASIK in our hospital from January 1998 to December 2001 were recruited. The patients were notified by telephone to the outpatient for follow-up. The collected data included demographic characteristics(gender and age), preoperative uncorrected distance visual acuity(UCVA)and best corrected visual acuity(BCVA), preoperative diopter, intraoperative corneal flap thickness and corneal stromal residual thickness(RST). The main indicators were long-term efficacy index, safety index, UCVA, BCVA, corneal thickness and axial length. The slit lamp, fundus and optical coherence tomography(OCT)examination were performed at the same time.RESULTS: A total of 95 patients(190 eyes)were recruited. At the final postoperative visit, there were 71 patients(142 eyes, 74.7%)had UCVA≥1.0, and 82 patients(164 eyes, 86.3%)had BCVA≥1.0. There were 2 eyes among them had bad BCVA(≤0.6)due to macular retinoschisis and glaucoma, respectively, while other patients' BCVA was 0.8. There was no significant correlation between the UCVA and BCVA of patients after surgery in 20a and the factors such as age at surgery, preoperative diopter and corneal thickness(P>0.05), but there was a negative correlation with the increase of axis length(rs=-0.32, -0.31, all P<0.05). UCVA and BCVA were positively correlated with corneal stromal residual thickness at the last postoperative follow-up(P<0.05). The safety and efficacy indexes of LASIK after surgery in 20a were 1.00±0.10 and 0.83±0.27, respectively. During the follow-up, no patients were found to have corneal ectasia and complications related to corneal flap, and no patients underwent secondary surgery. No patients with corneal dryness were found after silt lamp examination.CONCLUSION: LASIK after surgery in 20a shows good safety and efficacy.

AIM: To analyze the changes of uncorrected distance visual acuity(UCDVA)and ocular accommodation before and after excimer laser photorefractive keratectomy(PRK).METHODS: A retrospective study. There were 120 patients(240 eyes)who underwent PRK surgery in our center from December 2017 to December 2019 divided into two groups according to the diopter of spherical equivalent(SE): 70 cases with SE≤-6.00D in the mild to moderate myopia group and 50 cases with -6.00D<SE≤-9.00D in the high myopia group which were observed and analyzed. The UCDVA(LogMAR), monocular accommodative amplitude(AMP), negative relative accommodation(NRA), positive relative accommodation(PRA)and monocular accommodative flipper(AF)were measured before and after operation at 1wk, 1 and 3mo. RESULTS: In the group of low-moderate myopia and high-myopia group, the UCDVA at 1wk, 1 and 3 mo after operation was significantly increased compared with those before operation(all P<0.01). The increase of UCDVA of low to moderate myopia group was better than that in the high myopia group at 3mo after operation(P<0.05). Monocular AMP in the both groups firstly decreased and increased afterwards, and there was a significant decrease than that before operation at 1wk and 1mo after operation(P<0.01). AMP reached and was better than that before operation at 3mo after operation(P<0.01). The increase of low to moderate myopia group was both higher and faster than that of high myopia group. Compared with PRA before operation, the patients of low to moderate myopia group decreased at 1wk after operation(P<0.01), postoperative levels at 1mo recovered to preoperative level(P>0.05)and postoperative at 3mo was superior to preoperative operation(P<0.05); High myopia group did not change significantly at 1wk and 1mo after operation compared with preoperative operation(P>0.05), and increased at 3mo after operation compared with preoperative level(P<0.01). For low to moderate myopia group, there was no significant difference in preoperative and postoperative NRA, and at 1wk after operation, NRA of high myopia group temporarily increased compared with preoperative operation(P<0.01). At 1mo after operation, it returned to the preoperative level(P>0.05), and significantly increased at 3mo after operation compared with preoperative operation(P<0.01). Monocular AF for both groups significantly decreased at 1wk after operation compared with preoperative operation, recovered at 1mo after operation and was significantly better at 3mo after operation(all P<0.01); The low-to-moderate group increased more than the high-myopia group.CONCLUSION: PRK has a good correction effect for myopia below -9.00D, and people in low to moderate myopia have a better postoperative UCDVA correction effect than those in high myopia. The eye accommodation will be abnormal in a short period after operation. The recovery and improvement time of accommodative function in low-moderate myopia group are shorter than that in high myopia group, but both groups can recover and improve at 3mo after operation compared with the preoperative levels.

BACKGROUND@#The association between free triiodothyronine (FT3) and long-term prognosis in dilated cardiomyopathy (DCM) patients has not been evaluated. The purpose of this study was to determine whether the level of FT3 could provide prognostic value in patients with DCM.@*METHODS@#Data of consecutive patients diagnosed with DCM were collected from October 2009 to December 2014. FT3 was measured by fluoroimmunoassay. Other biochemical markers, such as free thyroxin (FT4), thyroid-stimulating hormone, red blood cell, hemoglobin, blood urea nitrogen, and serum creatinine, were tested at the same time. Follow-up was performed every 3 months. The primary endpoint was all-cause mortality. Pearson analysis was used to evaluate the correlation of FT3 and other lab metrics with DCM patients' prognosis. The association of long-term mortality in DCM and FT3 was compared using Cox hazards model.@*RESULTS@#Data of 176 patients diagnosed with DCM were collected. Of them, 24 patients missed FT3 values and six patients were lost to follow-up. Altogether, data of 146 patients were analyzed. During the median follow-up time of 79.9 (53.5-159.6) months, nine patients lost, 61 patients died (non-survival group), and 85 patients survived (survival group). FT3 was significantly lower in non-survival group than that in survival group (3.65 ± 0.83 pmol/L vs. 4.36 ± 1.91 pmol/L; P = 0.003). FT3 also showed a significantly positive correlation with red blood cell and hemoglobin, negatively correlated with age, blood urea nitrogen and serum creatinine (P < 0.05), respectively. Patients in the group of lower FT3 levels (FT3 ≤3.49 pmol/L) suffered from a higher risk of all-cause mortality (P for log-rank = 0.001). In multivariate Cox regression analysis, FT3 level was significantly associated with all-cause mortality (hazard ratio: 0.70, 95% confidence interval 0.52-0.95, P for trend = 0.021).@*CONCLUSION@#Low levels of FT3 were associated with increased all-cause mortality in patients with DCM.

Virtual autopsy is a new technique for investigating the morphological changes of cadaveric tissues and organs by medical imaging technology. It has been widely used in the identification of causes of death. Multislice spiral computed tomography (MSCT) has become a routine inspection method in some identification institutions, although it cannot completely replace traditional autopsy, it plays a key auxiliary or substitute role in the identification of certain abnormal causes of death. Plain MSCT scan cannot visualize cadaveric vessels, but can perform cadaveric angiography to determine vascular injury or disease. This technology has entered a rapid development period in recent years, and has made a considerable progress in contrast agent, perfusion methods and imaging methods. This article summarizes several common cadaveric MSCT angiography methods, such as systemic angiography, angiography through cardiopulmonary resuscitation, targeted angiography, and angiography by cardiac puncture, and analyzes and compares the application prospects.

OBJECTIVE: To determine the attributes of responses of the higher nerve center to acupuncture stimulation of Zusanli (ST36) on the basis of spontaneous brain activity, so as to explore the synchronization level of different brain rejoins after acupuncture. METHODS: All studies using fMRI to investigate the effect of acupuncture stimulation of ST36 and/or other acupoints on the human brain (at least 10 healthy subjects or patients in one group) published in journals from January of 1995 to January of 2018 were searched from databases of CNKI and PubMed by using keywords of acupuncture, electroacupuncture, and resting-state functional magnetic resonance images (rs-fMRI) or regional homogeneity (ReHo). Brain functional image data of acupuncture stimulation of ST36 and/or other acupoints were collected and analyzed with anisotropic effect size-signed differential mapping (AES-SDM) software (Meta-analysis), and those of acupuncture of simple ST36 analyzed as a subgroup. RESULTS: A total of 229 papers in Chinese and 109 in English were collected. According to our inclusive and exclusive standards, 11 papers containing 235 subjects were brought into analysis at last. Meta-analysis of brain image data of acupuncture at ST36 and/or other acupoints (comparison between pre- and post-acupuncture) revealed that the same brain regions (generality) which showed a significant increase in ReHo, are the right and left anterior cingulated gyrus, right caudate, left superior frontal gyrus, right middle frontal gyrus, and the right paracentral lobe, and those which showed a marked decrease of ReHo are the left mid-inferior occipital gyrus, left and right precentral and postcentral gyrus. The brain regions responding to acupuncture at ST36 only (specificity) are the right inferior parietal lobe, left middle inferior gyrus, right posterior lobe of cerebellum, and the left angular gyrus which displayed an increase of ReHo, and the right middle superior frontal gyrus which showed a decrease in ReHo. CONCLUSION: After acupuncturing at ST36, the relative generality and specificity of the central response in healthy subjects reflected as the location of the affected brain regions and the difference in the synchronization level of the corresponding spontaneous brain activities.

BACKGROUND: Acute graft-versus-host disease (aGVHD) is an inflammatory disease mediated by regulatory T cells. It is of certain significance to prevent and treat aGVHD by improving the levels of CD4+CD5+regulatory T cells (CD4+CD5+Treg)and related inflammatory factors in the body. OBJECTIVE: To investigate the effect of granulocyte colony stimulating factor (G-CSF) on CD4+CD5+regulatory T cells (CD4+CD5+Treg) and cytokines in children with hematological malignancies after allogeneic hematopoietic stem cell transplantation (allo-HSCT). METHODS: Seventy children with hematological malignancies undergoing allo-HSCT were enrolled. G-CSF was mobilized in the 45 cases (G-CSF group) before allo-HSCT transplantation. The other 25 cases were assigned to non-G-CSF group. Peripheral blood samples were collected at 4 weeks after allo-HSCT. Another 60 healthy children were selected as control group. The levels of T cells, serum interleukin-35 (IL-35) and interferon-γ (INF-γ) were measured by flow cytometry and ELISA, respectively. According to whether aGVHD occurred after allo-HSCT, the 70 cases were divided into aGVHD positive group and aGVHD negative group. The relationship between the changes of T cells and cytokines with aGVHD was analyzed. RESULTS AND CONCLUSION: (1) The percentage of CD4+CD5+Treg in CD4+T and CD4+FoxP3+T cells as well as serum IL-35 level in children with hematological diseases were lower than those in the control group (P < 0.05), while the levels of INF-γ were significantly higher than that in the control group (P < 0.05). (2) The percentage of CD4+CD5+Treg in CD4+T cells and CD4+FoxP3+T cells as well as serum IL-35 level in the G-CSF group were lower than those in the non-G-CSF group (P < 0.05), while the levels of INF-γ was higher than that in the non-G-CSF group (P < 0.05). (3) There were 7 cases (16%) of aGVHD in the G-CSF group and 10 cases (40%) of aGVHD in the non-G-CSF group at 4 weeks after allo-HSCT, and the incidence of aGVHD was significantly different between the G-CSF and non-G-CSF groups (P=0.022). (4) The percentage of CD4+CD5+Treg in CD4+T and CD4+FoxP3+T cells as well as serum IL-35 level in the aGVHD negative group were higher than those in the aGVHD positive group (P < 0.05), while the levels of INF-γ were significantly lower than that in the aGVHD positive group (P < 0.05). (5) CD4+CD5+Treg and CD4+FoxP3+T cells were positively correlated with the IL-35 level, but negatively correlated with the INF-γ level by Pearson single factor analysis. To conclude, G-CSF mobilization can significantly improve CD4+CD5+Treg cells and its transcription factor Foxp3 after allo-HSCT, which may be useful for improving the therapeutic effect of allo-HSCT and preventing aGVHD in patients with malignant hematological diseases.

Objective By comparing the methylation status of imprinted gene H19 in three different quality spermatozoa and the methylation status of the embryos after the combination of spermatozoa, the sperm methylation status in the ART technique was preliminarily explored with the postnatal embryo methylation. Methods In this study, a total of 91 spermatozoa and 91 low-quality embryos were collected from IVF-ET in the First People's Hospital of Yunnan Province. Among them, sperm samples were divided into three groups according to the sperm concentration parameter in the WHO fourth edition, then the relationship between different quality spermatozoa and postnatal embryo methylation were analyzed. Results The abnormality rate of H19 methylation status in group B was significantly higher than that of the sperm group and the embryo group. Conclusions The abnormalities of H19 methylation mainly concentrated in the abnormal parameters of spermatozoa, suggesting that the abnormal state was related to the decrease of sperm quality;Through IVF, ICSI fertilized embryos have the same proportion of the corresponding abnormal state with no significant difference; Abnormal sperm embryos did not show abnormality, which may be related to the self-repair function of the embryo itself.

BACKGROUND:Human umbilical cord mesenchymal stem cells (hUC-MSCs) can secrete a variety of cytokines and growth factors to support hematopoiesis. They can change the hematopoietic microenvironment, and furthermore, regulate and improve the immune function of the organism.OBJECTIVE:To investigate the effect of hUC-MSCs on immune reconstitution and prognosis in children with acute lymphoblastic leukemia (ALL) after allogeneic hematopoietic stem cell transplantation (allo-HSCT).METHODS:Sixty-four ALL children who had undergo allo-HSCT were enrolled and randomized into non-blood donor allo-HSCT group (n=32) and non-blood donor allo-HSCT+hUC-MSCs group (n=32).The therapeutic effect and complication were observed in the two groups. The changes of immunoglobulins and lymphocyte subpopulations in the two groups were compared before and 1,3 and 6 months after transplantation.RESULTS AND CONCLUSION:Compared with the allo-HSCT group, the time of granulocyte implantation was shorter (P < 0.05),and the incidence of chronic graft versus host disease and cytomegalovirus was lower in the allo-HSCT+ hUC-MSCs group. There were no significant differences in hematopoietic stem cell implantation rate, platelet remodeling rate, cumulative recurrence rate and 1-year survival rate between the two groups (P > 0.05).The levels of serum immunoglobulins IgA,IgG,IgM,IgE and the levels of T lymphocytes (CD4+,CD3+CD4+,CD4+CD8+) and B cells in the peripheral blood of allo-HSCT+hUC-MSCs group were significantly higher than those in the allo-HSCT group at 1,3 and 6 months after transplantation (P < 0.05).In conclusion, the combined use of hUC-MSCs infusion and allo-HSCT can effectively reduce the incidence of chronic graft-versus-host disease, and contribute to granulocyte hematopoietic reconstitution and immune function reconstruction.
