Statement of Problem: The effects of individual variations in coping strategies have been debated in studies of the association between stress and chronic periodontitis, with conflicting results


Purpose: To investigate the associations between stress, coping styles and periodontal disease in a sample of Iranian population


Materials and Method: Forty patients with chronic periodontitis and forty control subjects with a healthy periodontium were enrolled in this study and matched for age and gender. Participants were patients undergoing periodontal treatment at the Department of Periodontics, Guilan University of Medical Sciences. A single examiner performed periodontal examination. Psychological assessments, including the Life Events Questionnaire and the Ways of Coping Questionnaire were done by a second examiner; both examiners were blind to the study. Bi-variate and multivariate logistic regression analyses were used to compare results for patients and control subjects


Results: Statistically significant differences in the problem-focused coping [p< 0.01], intensity of stress [p< 0.006], as well as escape-avoidance [p< 0.01], and accepting responsibility [p< 0.001] subscales were observed between the patient and control groups. Multivariate logistic regression identified a negative association between periodontitis and tooth-brushing frequency [OR= 3.3, 95% CI: 1.22- 8.69], as well as the accepting responsibility coping style [OR= 1.5, 95% CI: 1.14- 1.98], and a positive association with stress intensity [OR= 1.081, 95% CI: 1.023-1.143]


Conclusion: The results suggest that psychological stress associated with various life events is a significant risk indicator for periodontal disease. Although statistically small, there was a clinically important link between coping strategies and periodontal disease

Postoperative pain after periodontal surgical procedures is a common complaint. Celecoxib, a cyclooxygenase [COX]-2 inhibitor NSAID, is associated with a lower incidence of well known side effects reported for the conventional NSAIDs. Sparse information is available on the efficacy of Celecoxib, or Prednisolone, a steroidal anti-inflammatory drug, following periodontal surgery. The aim of the present study was to compare the analgesic efficacy of Celecoxib and prednisolone after periodontal mucoperiosteal flap with minor osseous surgery. In this randomized, double-blind, cross-over, placebo-controlled, clinical trial, 20 patients with generalized, moderate to severe chronic periodontitis underwent three mucoperiosteal flap with minor osseous surgery with at least 4 week interval. Each quadrant was randomly assigned to receive one of the following medications in two doses starting one hour before surgery: 200 mg celecoxib, 10 mg Prednisolone, and placebo. The patients filled out a visual analogue scale [VAS] and a five-point verbal rating scale [VRS-5] at 11 predetermined time periods. Data analyses were performed, using Friedman, Pearson correlation and Spearman rank correlation tests. There was a statistically significant lower pain reported in the celecoxib than placebo groups during 9 postoperative time periods using VAS [p =0.03]. This value was statistically significant for all the time periods under investigation, using VRS-5 [p =0.01]. The level of pain was lower in Prednisolone than in the placebo groups at the four-hour period [p =0.045], and during the day after surgery [p =0.01]. A statistically significant lower pain perception was reported with the use of celecoxib at three postoperative hours [3, 5 and 6] than prednisolone [p =0.02]. There was a positive correlation between VAS and VRS-5 pain rating scales used in this study [p =0.0001, r =0.938]. Within the limitations of this study, preventive and postoperative uses of Celecoxib and Prednisolone were effective in the control of pain following periodontal flap with minor osseous surgery. However, during the first 8 postoperative hours, Celecoxib showed a superior efficacy in the duration and continuity of pain relief in comparison with Prednisolone

Résumé

Determining students' attitudes towards the goals of doctoral dissertation writing, topic selection, choosing the supervisor, and their opinion about problems in conducting a research project, can be helpful in assisting programmers to better reform and improve the quality of this course. Survey the attitudes towards thesis writing process among students of guilan university of medical sciences, in the year of 2009.In this descriptive-cross sectional study, 164 medical and dental students who being in the last two years of their education or graduated in recently years, were surveyed. We used a self-made questionnaire comprised of 54 questions, which its validity and reliability were tested prior to initiation of the study. The questions were designed in three bases. Data were analyzed as frequency distribution, mean and standard deviation, and X[2] test, using SPSS software. Within the thesis process and topic selection domain, most of the students agreed with the need for learning how to write and collect a thesis [66.4%], guidance in topic selection [71.4%], and access to the resources and facilities [76.2%]. Most of the students [76%] responded that they would refuse to write a dissertation if it was elective, and 71.4% were willing to write an article based on their work. Availability of the supervisor had the highest priority for the students, within the domain of supervision. 12.9% of medical and 22.5% of dental students were satisfied from thesis process there was a statistically significant relationship between the field and satisfaction of performance process [P=0.002]. This preliminary research showed that students inquired to learn basics of dissertation and article writing in the form of a credit course. Encouraging students to attend research methodology workshops, teaching supervision skills to the mentors, providing research funds and laboratory equipments has positive influence to attitudes of students and improve the performance of the doctoral dissertation course
