Résumé

Introduction: Excessive gummy smile affects the aesthetics of the patient and can be the result of several factors, including altered passive eruption, which can be surgically corrected by aesthetic crown lengthening. Case report: 22-year-old female patient, who was treated by aesthetic crown lengthening for the correction of type 1B altered passive eruption. Discussion: Considering the patient's age and periodontal phenotype, surgical correction of the gummy smile by aesthetic crown lengthening shows stable long-term results. Conclusion: Surgically correcting excessive gingival exposure through esthetic crown lengthening can help patients improve the appearance of their smile and regain their self-confidence.

Résumé

Mucogingival deformities are a group of conditions that affect many patients, having an impact on function and esthetics; they can be congenital, developmental, or acquired. Gingival recession is defined by the American Academy of Periodontology as the location of the gingival margin apical to the amelocemental junction. They can be localized or generalized and include one or more sides of the tooth. Among the treatments, subepithelial connective tissue grafting and acellular dermal matrix can be considered, whose objectives are root coverage and keratinized tissue width. Case report: A 54-year-old female patient diagnosed with mucogingival deformities around the tooth, was treated with subepithelial connective tissue graft to obtain root coverage in recessions RT2 and RT3. Conclusion: The purpose of using a bilaminar technique where a flap is made, and a connective tissue graft is placed to cover gingival recessions is to obtain better esthetic results.

Introducción: Hoy en día, uno de los motivos de consulta de los pacientes es la inconformidad con su apariencia estética debido a la presencia de recesiones gingivales. Dentro de las diversas técnicas para tratar este tipo de defectos se encuentra el colgajo posicionado lateral, el cual, se utiliza para la cobertura radicular de recesiones gingivales únicas en dientes mandibulares y cuyos resultados han demostrado ganancia en altura de tejido queratinizado, ganancia en inserción clínica y una cobertura radicular completa. Reporte de Caso: Paciente femenino de 32 años, diagnosticado con defecto mucogingival único en torno a pieza dental 3.1 y que fue tratado con un colgajo posicionado lateral en combinación con proteínas derivadas de la matriz del esmalte, con el objetivo de cubrir la recesión gingival. Conclusión: La técnica de colgajo posicionado lateral es un tratamiento viable y eficaz para la cobertura radicular completa de defectos mucogingivales únicos en torno a diente, donde además se obtiene ganancia en altura de encía queratinizada, así como de nivel de inserción clínica.


Introduction: Nowadays, one of the reasons for consultation of patients is the dissatisfaction with their aesthetic appearance due to the presence of gingival recessions. Among the various techniques to treat this type of defects is the laterally positioned flap, which is used for root coverage of single gingival recessions in mandibular teeth and whose results have shown gain in height of keratinized tissue, clinical attachment gain and complete root coverage. Case Report: 32-year-old female patient, diagnosed with a single mucogingival defect around tooth 3.1 and treated with a laterally positioned flap in combination with enamel matrix derivatives, in order to cover the gingival recession. Conclusion: The laterally positioned flap technique is a viable and effective treatment for complete root coverage of single mucogingival defects around the tooth. In addition, there is a gain in keratinized gingiva and in clinical attachment.

Résumé

La ausencia de un diente puede originar sobre-erupción de su antagonista debido al movimiento fisiológico. Distintos métodos se han empleado para lograr la rehabilitación en espacios interoclusales reducidos, la mayoría implica un tallado del diente antagonista o incluso la extracción dental. La intrusión dental periodontalmente asistida (corticotomía), es una alternativa terapéutica en la que el tallado del diente extruido ya no es necesario, conservando su integridad coronaria. Reporte de caso: Paciente femenino de 56 años con ausencia del 4.7, además presenta sobre-erupción del 1.7, limitando el espacio interoclusal y su rehabilitación. Se realizó intrusión del 1.7 mediante corticotomía con el Piezoeléctrico (técnica Piezocision) así como la colocación de minitornillos de ortodoncia para anclaje. Obtenido dicho espacio interoclusal, se rehabilitó con un implante dental. Conclusiones: La intrusión de molares con el apoyo de mini-implantes y corticotomías es un procedimiento que tiene como beneficio la creación de un espacio interoclusal para su restauración en un periodo corto de tiempo, sin tallar la estructura dentaria.


The absence of a tooth can cause over-eruption of its antagonist due to a physiological movement. Different methods have been used to achieve rehabilitation in reduced interocclusal spaces. Most of them involve the wear of the opposing tooth or even dental extraction. Periodontal-assisted dental intrusion is a therapeutic alternative that does not require the reduction of the extruded tooth and preserves its structure. Case report: 56-year-old female patient with absence of tooth 4.7, also presenting an over-eruption of 1.7, limiting the interocclusal space and rehabilitation. Intrusion of tooth 1.7 was performed with corticotomy using the piezocision technique and placement of mini-screws followed by the placement of a dental implant. Conclusions: The intrusion of molars aided by orthodontic mini-screws and piezocision technique is an effective treatment, whose benefit is the creation of an ideal prosthetic space in a short period of time, avoiding the reduction of dental structure.

La instrumentación mecánica durante el tratamiento periodontal trae consigo la formación de escombros microcristalinos que inhiben la adhesión tisular a la superfi cie radicular y favorece la proliferación bacteriana, lo cual perjudica los resultados del tratamiento periodontal a corto y largo plazo. Hoy en día el acondicionamiento radicular con el uso de biomodifi cadores es una opción de tratamiento adicional en el tratamiento de la periodontitis y el tratamiento de cobertura radicular. El objetivo del presente estudio es realizar una revisión de la literatura acerca de las aplicaciones y del acondicionamiento radicular con ácido cítrico, tetraciclina, EDTA y láser en el tratamiento periodontal no quirúrgico y quirúrgico (AU)


Mechanical instrumentation during periodontal treatment brings the formation of microcrystalline debris that inhibits tissue adhesion to the root surface and favors bacterial proliferation, which harms the results of the short and long term periodontal treatment. Nowadays, root conditioning with the use of biomodifi cators is an additional treatment option in the treatment of periodontitis and root coverage therapy. The aim of the present study is to conduct a literature review about the applications and the root conditioning with citric acid, tetracycline, EDTA and laser in the non surgical and surgical periodontal treatment (AU)
