Résumé

Objective To explore the application effect of prone split-leg position in percutaneous nephrolithotomy(PCNL).Methods A total of 79 patients with upper urinary tract stones who underwent surgery in our department from July 2021 to July 2022 were selected.According to the surgical position,the patients were divided into prone split-leg position group(n =35)and traditional prone position group(n =44).The two groups were compared regarding operation time,intraoperative blood loss,postoperative and preoperative hemoglobin changes,postoperative complications,one-stage stone free rate,and duration of hospitalization.Results The prone split-leg position group had significantly shorter operative time and higher one-stage stone free rate than those in the traditional prone position group[(69.6±20.7)min vs.(90.5±35.3)min,t =-3.280,P =0.002;97.1%(34/35)vs.79.5%(35/44),χ2 =3.984,P =0.037].There were 1 case of infection,3 cases of fever,and 1 case of bleeding in the prone split-leg position group,while 4 cases of postoperative fever and 2 cases of bleeding in the traditional prone position group,without significant difference in the incidence of complications between the two groups(χ2 =0.000,P =1.000).There were no statistical differences in intraoperative blood loss,number of working channel,hemoglobin changes,and duration of hospitalization(P>0.05).Conclusion Prone split-leg position PCNL can shorten the operation time,improve the one-stage stone free rate,and will not increase the risk of postoperative complications,with good safety and effectiveness.

Objective·To explore the relationship between sleep quality and carotid atherosclerosis in the population with low or moderate risk of cardiovascular and cerebrovascular diseases.Methods·Based on the population-based cohort study of chronic diseases in Xinjiang,the researchers selected residents aged 35-75 from two fixed communities in Urumqi and Korla,in Northern and Southern Xinjiang,respectively,using a two-stage random cluster sampling method from July 2019 to September 2021.In the population without a history of coronary heart disease and cerebrovascular events,the prediction model for atherosclerotic cardiovascular disease(ASCVD)risk in China(China-PAR)was used to evaluate the risk of cardiovascular and cerebrovascular diseases.Low and moderate risk population of cardiovascular and cerebrovascular diseases were included.Participants completed physical examinations,questionnaires[including the Risk Factors Assessment Scale of Cardiovascular and Cerebrovascular Diseases in Xinjiang,Pittsburgh Sleep Quality Index(PSQI),International Physical Activity Questionnaire(IPAQ),and Food Frequency Questionnaire],cardiovascular and metabolic biochemical examinations,and carotid color doppler.Carotid intima-media thickness(CIMT)and plaque formation were used to determine the carotid atherosclerosis of the study subjects.Multivariate Logistic regression model and restricted cubic spline(RCS)were used to analyze the relationship between sleep quality and carotid intima-media thickening/plaque formation in the population with low and moderate risk of cardiovascular and cerebrovascular diseases.Results·A total of 1 528 subjects were included in the study,the mean age was(49.4±8.2)years,and 685(44.8%)were male.In the included population,there were 581(38.0%)subjects with carotid intima-media thickening and 305(20.0%)subjects with carotid plaque formation.Among them,intima-media thickening and plaque formation both occurred in 154(10.1%)people.Therefore,the prevalence of carotid atherosclerosis was 47.9%(732 subjects).Compared with the group without carotid artery thickening,the group with carotid intima-media thickening/plaque formation had higher levels of general cardiovascular and cerebrovascular risk factors,including age,male ratio,blood lipid levels and obesity,and higher PSQ1 sleep score[(7.06±2.13)vs(7.43±2.51),P=0.001].The proportion of patients with poor sleep quality was higher(6.5%vs 12.1%,P=0.001).Multivariate Logistic regression analysis showed that poor sleep quality was an independent risk factor for carotid atherosclerosis[adjusted OR(aOR)=1.22,95%CI 1.004-1.492,P=0.040].RCS analysis suggested that PSQI and the risk of carotid atherosclerosis showed a positive linear correlation,that is,the worse quality of sleep,the higher risk of carotid atherosclerosis.Conclusion·Although the traditional metabolic risk factors are at a low risk level,the prevalence of carotid atherosclerosis is high and poor sleep quality is an independent risk factor for carotid atherosclerosis in the low and moderate risk population of cardiovascular and cerebrovascular diseases.

This study aims to investigate the effect and mechanism of total triterpenes of Euphorbium in treating rheumatoid arthritis(RA). The rat model of RA was established with Freund's complete adjuvant(FCA). Male rats were randomly assigned into control, model, Tripterygium glycosides(7.5 mg·kg~(-1)), and low-, medium-, and high-dose total triterpenes of Euphorbium(32, 64, and 128 mg·kg~(-1), respectively) groups, with 10 rats in each group. In other groups except the control group, 0.2 mL FCA was injected into the right hind toe. Rats in the intervention groups were administrated with corresponding drugs by gavage, and the control group and the model group with the same volume of 0.5% CMC-Na solution once a day. During the treatment period, the swelling degree of the hind paw was measured and the arthritis was scored until day 30. At the end of drug administration, the pathological changes of the joint tissue were observed by hematoxylin-eosin staining. The content of malondialdehyde(MDA), glutathione(GSH), and Fe~(2+) and the activity of superoxide dismutase(SOD) in the joint tissue were measured by biochemical colorimetry. RT-PCR was performed to determine the mRNA levels of nuclear factor erythroid 2-related factor 2(Nrf2), glutathione peroxidase 4(GPX4), and acyl-CoA synthetase long chain family member 4(ACSL4) in the joint tissue. Western blot was employed to determine the protein levels of Nrf2, Kelch-like ECH-associated protein 1(Keap1), heme oxygenase-1(HO-1), NAD(P)H quinone oxidoreductase 1(NQO1), SOD2, GPX4, and ACSL4 in the joint tissue. The results showed that the treatment with Tripterygium glycosides(7.5 mg·kg~(-1)) and total triterpenes of Euphorbium(32, 64, and 128 mg·kg~(-1)) alleviated the swelling degree of bilateral hind limbs, decreased the arthritis score, reduced joint tissue lesions and the content of MDA and Fe~(2+) in the joint tissue, and increased GSH content and SOD activity. Furthermore, the interventions up-regulated the protein and mRNA levels of Nrf2 and GPX4, down-regulated the protein and mRNA levels of ACSL4, and up-regulated the protein levels of Keap1, NQO1, HO-1, and SOD2 in the Nrf2/HO-1/GPX4. In summary, the total triterpenes of Euphorbium can treat RA by inhibiting lipid peroxidation and abnormal ferroptosis, which may involve the Nrf2/HO-1/GPX4 signaling pathway.

Objective: To explore the relationship between sleep/physical activity and metabolic syndrome (MS) in urban population of Xinjiang. Methods: This is a prospective, cross-sectional study. From July 2019 to September 2021, a two-stage random sampling method was used to randomly select residents aged 30-74 years from two communities in Urumqi of northern Xinjiang and Korla of southern Xinjiang. General situation questionnaire, Pittsburgh Sleep Quality Index Scale (PSQI) survey, International Physical Activity Questionnaire (IPAQ) survey, physical examination, physiological and biochemical indicators were obtained and analyzed. The dose-response curves of healthy sleep score and physical activity with metabolic syndrome were plotted using restricted cubic spline curves. Multivariate logistic regression model was used to analyze the independent and combined effects of sleep quality and physical activity on MS risk. Results: A total of 10 209 participants were included. The mean age of the subjects was (47.1±9.1) years, and males accounted for 51.3% (5 275/10 209). The prevalence of MS was significantly associated with the healthy sleep score and physical activity. Compared to the subjects with healthy sleep, OR (95%CI) of MS with intermediate, and poor sleep were 1.20(1.06-1.35), 1.23(1.04-1.45), respectively. Compared to the subjects with high physical activity, OR (95%CI) of MS with medium, low physical activity was 1.34(1.15-1.56), 1.42(1.19-1.70), respectively. There was a significant interaction between sleep and physical activity in MS (P for interaction=0.002). Compared to the subjects with high physical activity and healthy sleep, OR (95%CI) of MS with poor sleep and high physical activity was 2.03 (1.24-3.33, P for trend=0.016). Conclusion: Poor sleep quality and lack of physical activity are not only independent risk factors for an increased risk of MS but also have a combined effect with an increased risk of MS.

Résumé

Objective To discusse the diagnosis and treatment of kidney stones with emphysema pyelonephritis.Methods The clinical data of 8 patients with kidney stones complicated with emphysema pyelonephritis diagnosed in our hospital from January 2015 to October 2018 were retrospectively analyzed.There were 5 males and 3 females.The average age was 50 years old.The clinical manifestations including chills fever in 6 cases,low back pain in 5 cases,nausea and vomiting in 3 cases.Six patients had diabetes,one had thalassemia,and two had contralateral kidney stones.The maximum cross-sectional area of stones was 737.6 mm2.Among the 8 cases,there were 7 cases which number of white blood cells and procalcitonin were higher than normal reference value.4 cases of hemoglobin < 110 g/L,2 cases of platelet count < 125 × 109/L.The patient was cultured with urine and/or blood and drainage fluid.5 cases were Escherichia coli,2 cases were infected with Proteus mirabilis,and 1 case was infected with Pseudomonas aeruginosa.According to the CT findings of emphysematous pyelonephritis reported in the literature,it was divided into type Ⅰ-Ⅳ:There were 2 cases of type Ⅰ,2 cases of type Ⅱ,3 cases of type Ⅲ,and 1 case of type Ⅳ.Results 4 cases of type [and type Ⅱ patients,2 cases without SIRS were given positive medical treatment to control infection then performed PCNL.2 cases with SIRS,first treated with percutaneous nephrolithotomy and active medical,after control infection the PCNL was performed.None of the 4 patients were treated with ICU and recovered well after surgery.Three patients with type Ⅲ and one patient with type Ⅳ were complicated with SIRS.Two of them underwent percutaneous nephrolithotomy in the emergency department.They were transferred to the ICU after surgery.After the infection and general condition improved,PCNL was performed.The postoperative recovery was satisfied.One patient percutaneous nephrolithotomy,due to poor drainage,secondary percutaneous nephrolithotomy,large intrachannel,low pressure perfusion in the operation of partial obstruction of renal pelvis stones,dredge obstruction,after ICU control infection PCNL was performed,postoperative recovery was good.One patient with type Ⅲ also had poor peritoneal drainage for the first time.Secondary percutaneous nephrolithotomy was performed.After the infection was controlled by ICU,PCNL was performed to remove the stones.However,because the patient had contralateral kidney stones and thalassemia,an epileptic-like reaction occurred during the anti-infection with imipenem,and a serious infection occurred again after the operation,and eventually the patient died.Conclusions Patients with type Ⅰ and Ⅱ emphysematous pyelonephritis with renal calculi treated with conservative medical treatment alone or combined with percutaneous renal puncture drainage with SIRS can achieve better therapeutic effects after PCNL surgery.Type Ⅲ,Ⅳ emphysema pyelonephritis with renal calculus patients need to be actively anti-infective accompany with percutaneous renal puncture drainage.When the stone leads to multiple renal pelvic obstruction,large channels,low-pressure perfusion can be used to crush stones,dredge obstruction.PCNL was performed after infection control.

BACKGROUND: As an emerging stem cell family, urine-derived stem cells have attracted more and more attentions in the tissue engineering construction. OBJECTIVE: To study the culture method of urine-derived stem cells, and to identify the biological characteristics of urine-derived stem cells. METHODS: Cleaning urine samples were harvested and centrifuged, and urine-derived stem cells were isolated from the urine samples and extensively expanded in vitro.Cell growth curve was measured by MTT method,and cell surface markers were detected by flow cytometry. Meanwhile, the cells were subjected to osteogenic and adipogenic induction. RESULTS AND CONCLUSION: Urine-derived stem cells were successfully isolated from the urine samples, which grew and proliferated rapidly. After subculture, the cells exhibited an S-shaped growth. The isolated urine-derived stem cells expressed CD29, CD44, CD73 and CD90, indicating that the cells maintained the activity of stem cells. Moreover, the isolated cells had the ability of osteogenic and adipogenic differentiation. To conclude, urine-derived stem cells have strong proliferation and differentiation potentials, as a kind of economic, convenient, non-invasive source of cells, which can provide great convenience for urethral defect repair and organ reconstruction.

BACKGROUND: Little is reported on human urine derived stem cells, and there is still no stable and efficient culture method until now. OBJECTIVE: To compare the effects of three kinds of culture media on the growth and proliferation of human urine derived stem cells, and to optimize the culture methods. METHODS: Human urine derived stem cells were isolated and purified using centrifugation method, and then cultured using adherent method in keratinocyte serum-free medium, progenitor cell culture medium, and urinary cell culture medium (equal-proportion mixture of keratinocyte serum-free medium and progenitor cell culture medium). Cell morphology was observed, and cell proliferation was detected by MTT method to draw a cell growth curve.RESULTS AND CONCLUSION: Human urine derived stem cells could be successfully cultured in these three kinds of culture media, which grew slowly in the keratinocyte serum-free medium, grew rapidly in the progenitor cell culture medium, and grew best in the urinary cell culture medium. To conclude, the urinary cell culture medium which can rapidly and efficiently culture target cells is the best choice for the growth of human urine derived stem cells.

Objective To introduce a new trocar position in the posterior laparoscopic for the treatment of retrocaval ureter.Methods From August 2011 to October 2014,5 cases with retrocaval ureter treated with posterior laparoscopic were retrospectively analyzed,including 3 males and 2 females,aged from 15 to 46 years(mean 34 years).The history of disease ranged from 1 to 10 months,with 3 cases presented with low back pain,and 2 cases being detected uronephrosis by check-up.Results All the operations were successfully completed,with the operation duration ranged from 75-125 min (mean 90min),and blood loss ranged from 20ml to 50 ml(mean 35 ml).The average hospital stay was 6 days(5-7d).There was no wound infection or urine leakage.Ureteral double-J tubes were removed 4 weeks after surgery.Postoperative followup ranged from 2 weeks to 6 months (mean 30 months).There was no anastomotic stricture,and the hydronephrosis relieved.Conclusions Trocar position adjustment of posterior laparoscopic in treatment of retrocaval ureter is convenient to operate,which also shortened the time of operation,reduced the difficulty of operation and the surgeons' fatigue.

Objective To detect the circulating miR-106a levels in serum before and after surgery in patients with renal clear cell carcinoma,and to explore its relationship with clinical-pathological parameters.Methods 30 serum samples from patients with renal clear cell carcinoma were collected before and after surgery during February 2013 to July 2014.This study included 30 normal controls.All serum miR-106a levels were detected using the real-time PCR.Results The serum miR-l06a levels in patients with renal clear cell carcinoma pre-operatively wcre significantly greater than normal controls (Z =-4.251,P =0.0001).The serum miR-106a levels in patients post-operatively had no significant differences compared to normal controls (Z =-0.244,P =0.807).The serum miR-106a levels in post-operative samples were significantly lower than the pre-operative samples (Z =-4.229,P =0.0001).Serum miR-106a levels and other clinical-pathological parameters had no correlation in patients with renal clear cell carcinoma(tumor size:Z =-0.775,P =0.439;Fuhrman grade:Z =-1.694,P =0.090).The receiver operating characteristic curve was used to distinguish pre-operative samples and normal controls,its AUC was 0.819 (95% CI:0.710-0.929,P =0.0001) with 86.7% sensitivity and 70.0% specificity.Conclusions The serum miR-106a levels in patients with renal clear cell carcinoma pre-operatively were significantly greater than post-operatively with no correlation in tumor size and Fuhrman grade.The outcome suggested that serum miR-106a can be regarded as a potential molecular marker in renal clear cell carcinoma.

Objective To study the relationship between lipoprotein lipase (LPL) gene Hind Ⅲ and Pvu Ⅱ polymorphisms and type 2 diabetes mellitus (T2DM).Methods Polymerase chain reaction-restriction fragment length polymorphism-(PCR RFLP) technique was used to detect the genotypes of Hind Ⅲ and Pvu Ⅱ polymorphisms in the LPL gene from 110 T2DM patients and 157 healthy controls.Results The frequencies ofgenotype (H-H-,H+H-,H+H+)and allele(H,H+) of Hind Ⅲ between T2DM cases and controls showed no differences (x2 =1.998,1.262,P 0.368,0.261).There were differences in the frequency of genotype P-P-(20.9% vs.10.8%),P+P-(40.0% vs.58.6%),P+P+(39.1% vs.30.6%) of PvuⅡ between T2DM and controls (x2 =10.157,P=0.006).The individuals who carried P+P genotype had an higher morbidity of T2DM than those who carried P P genotype (OR 2.833,95%CI:1.374 5.814).Conclusions The Hind Ⅲ polymorphisms in the LPL gene is not associated with T2DM risk,but the Pvu Ⅱ polymorphisms in the LPL gene is related with T2DM risk in Chinese Han population in Xinjiang,and the P+P- genotype might be a susceptible factor of T2DM.

OBJECTIVE@#To construct a NF-kappaB siRNA expression vector and to detect the specific silencing effect of the siRNA on the expression of NF-kappaB protein.@*METHODS@#pcDNA3.1/CT-GFP-TOPO recombinant eukaryotic expression vector and pSilencer 1.0-U6-siRNA-NF-kappaB recombinant vector were constructed respectively. These 2 recombination plasmids were co-transfected into COS-7 cells, and the NF-kappaB silence induced by RNAi was detected by Western blot and the inverted fluorescence microscope.@*RESULTS@#The levels of NF-kappaB protein in COS-7 cells could be silenced effectively and specifically by pSilencer 1.0-U6-siRNA- NF-kappa recombinant vector. The expression of NF-kappaB protein was reduced gradually with the increase of pSilencer 1.0-U6-siRNA- NF-kappaB recombinant vector,which could be detected by Western blot under the inverted fluorescence microscope.@*CONCLUSION@#NF-kappaB siRNA expression vector is constructed successfully.

OBJECTIVE@#To observe the effects of xipayi mouth rinse on the DNA synthesis and change of cell cycles of human gingival fibroblast (HGF) induced by lipopolysaccharide(LPS).@*METHODS@#HGF was stimulated with LPS at 25 mg/L. Flow cytometry was used to examine the effect of xipayi mouth rinse at 25 mg/L on the DNA synthesis and change of HGF cell cycles.@*RESULTS@#The percentage of HGF in G( 1) phase increased after the cells were induced by LPS, while the percentage of HGF in S phase decreased. Xipayi mouth rinse could ameliorate this phenomenon.@*CONCLUSION@#Xipayi mouth rinse can significantly ameliorate the inhibitory effect of LPS on the proliferation of HGF, suggesting the anti-inflammatory effect of xipayi mouth rinse in the treatment and prevention of periodontal diseases.

OBJECTIVE@#To observe the effects of xipayi mouth rinse of different concentrations on the activities of interleukin-6 (IL-6) from human gingival fibroblast (HGF) induced by lipopolysaccharide (LPS).@*METHODS@#HGF was stimulated with LPS at 25 g/mL, and the radioimmunoassay (RIA) was used to examine the effect of xipayi mouth rinse at 12.5 approximately 200 g/mL on the secretion of IL-6 in the supernatant of the cell culture.@*RESULTS@#IL-6 secreted by human gingival fibroblast was significantly inhibited by xipayi mouth rinse in a dose dependent manner.@*CONCLUSION@#Xipayi mouth rinse can inhibit the secretion of IL-6 from HGF induced by LPS, suggesting the anti-inflammatory effect of xipayi mouth rinse to treat and prevent periodontal diseases.

Résumé

Objective To investigate the reverse effect of the extracted solution of uygur medicine ASMq on the liver cancer drug-resistance cell line Bel/Fu in vitro.Methods MTT assay was used to determine the IC50 of 5-Fu on Bel/Fu cell line and the reverse multiples of the drug-resistance before and after ASMq treatment.The concentration was used to determine the fold reversal of ASMq.By clone forming text to determine the reversal effect on drug resistance of Bel/Fu cell line.Results The IC50 value of ASMq crude extract was 2 000.68 ?g/mL.ASMq at 390 ?g/mL concentration was no toxic but had a coordinate effect on Bel/Fu cell line with 5-Fu.The fold reversal of ASMq combining with 5-Fu was 7.8,CDI was 0.89.The clone forming size became smaller comply with the concentration of ASMq.Conclusion ASMq has reversal effect on drug resistance of Bel/Fu cell line.

To explore the antioxidant mechanism of munziq and mushil of Abnormal Savda. Scavenging effects of munziq and mushil of Abnormal Savda on superoxide anion radical(O -? 2) and hydroxyl radical (OH ?) were measured by electron paramagnetic resonance(EPR). Munziq and mushil possessed obvious scavenging effects on O -? 2 and OH ? and the effect of mushil was superior to that of the munziq.[Conclusion] The EPR is a reliable, effective, accurate, sensitive method for measuring the antioxidant properties of herbal medicines. Munziq and mushil are two kinds of effective antioxidants, and the antioxidant effect may be one of the therapeutic mechanisms of munziq and mushil.

Object To study the hydroxyl radical scavenging and DNA damage protecting activities of 5 Uighur herbal medicines Euphorbia humifusa Willd , Borago officinalis L, Cassia angustifolia Vahl, Terminalia chebula Retz. and Fomes officinalis (Vill ex Fr ) Bres Methods The hydroxyl radical scavenging action and DNA damage protection action were determined by CuSO 4 Vit C H 2O 2 yeast and CuSO 4 Vit C H 2O 2 phen DNA chemiluminescence systems respectively Results All 5 Uighur medicinal herbs showed hydroxyl radical scavenging and DNA damage protection activities in dose dependent manners Conculsion The hydroxyl radical scavenging and DNA protective effects may possibly be the mechanisms of their antiaging and therapeutic effects against various diseases
