Résumé

Objective:To evaluate the imaging characteristics and etiology of subarachnoid hemorrhage (SAH) by multi-slice spiral CT angiography (CTA).Methods:A total of 68 SAH patients admitted to Xinchang People′s Hospital from July 2018 to December 2019 were selected as study subjects. CTA and digital subtraction angiography (DSA) were performed on all subjects to compare the detection results of CTA and DSA.Results:Taken DSA results as the "gold standard", this study included 68 SAH patients, including 44 cases of aneurysm detected by DSA correctly, 16 cases of arteriovenous malformation and 8 cases of normal. CTA correctly detected 61 cases, including 45 cases of aneurysms, 16 cases of arteriovenous malformations and 7 cases of normal cases. One case was misdiagnosed as aneurysm, and the diagnosis coincidence rate was 98.53% (67/68). There was no significant difference between CTA and DSA in the detection results of tumor body width and neck width [(6.63 ± 0.75) mm vs. (6.58 ± 0.77) mm, (3.55 ± 0.47) mm vs. (3.49 ± 0.45) mm] ( P > 0.05). Conclusions:CTA has the advantages of non-invasive, rapid and convenient diagnosis of the etiology of SAH, which is conducive to the selection and assessment of clinical surgical treatment.

OBJECTIVE To investigate the management criteria for selecting and applying of prosthesis and implants.METHODS Through a standard system and appropriate measures,the strict management to the purchase and entire application of prosthesis and implants was encouraged.RESULTS The systemic management ensured the hospital to use prosthesis and implants safely on patients and prevent from improper or illegal use,thus to safeguard the interests of both the patients and the hospital,avoiding unnecessary harms to patients′ body and spirit,therefore to minimize the conflicts between hospitals and patients.During the implementation of the management,further improvement and investigation were required.CONCLUSIONS Maintaining the mutual inferests between patients and hospital,avoiding unnecessary harms to patients′can minmize the medical conflicts and persistent improvements.

OBJECTIVE To investigate the respiratory tract infection rate,pathogenic organism and drug resistance for the patients with tracheotomy and endotracheal incubation in ICU,and to provide first-hand information for diseases prevention and clinical treatment.METHODS Fifty eight patients admitted in our hospital ICU from Dec 2006 to May 2007 were analyzed for their pathogenic organisms of lower respiratory tract infection,drug resistance,underlying diseases and age.RESULTS Pathogenic organisms from sputum culture for 58 patients with tracheotomy and endotracheal incubation were all positive.A total of 139 strains of pathogenic organisms have been found.Among them 106 were Gram-negative bacteria,accounted for 76.3%,and 26 were Gram-positive bacteria,accounted for 18.7% and 7 were fungi,accounted for 5%.From sputum culture of 29 patients Burkholderia cepacia was positive,which was the No.1 bacterial infection,accounted for 20.7% of total.Drug sensitive test for most Gram-negative bacteria showed resistance.CONCLUSIONS Bacteria infection of respiratory tract of the patients with mechanic ventilation in ICU is mostly Gram-negative.Clinical doctors should emphasize the inspection of pathogenic infection regularly and its drug resistance evaluation,to administer appropriate antibiotics.

OBJECTIVE To analyze the traits and the related factors of the injured patients who had hospitalized in ICU in order to make the corresponding strategies and decrease the incidence of hospital infection effectively. METHODS The hospital infection and the associated factors of 384 cases in ICU from Jan 2006 to Dec 2007,including the patients age,injuried severity,interventional manipulation,emergency operation before or after admission to ICU,the length of stay in ICU and the use of tranquillizer were investigated. RESULTS Of 384 cases,156 cases occurred in patients with nosocomial infection,with the infection rate 40.6%,infection site or respiratory tract was accounted for 63.3%,followed by urinary tract,skin and soft tissue.Gram-negative bacteria,were accounted for 71.7%,followed by Gram-positive cocci(15.2%) and fungi(13.1%),the severe level of diseases,interventional manipulation,emergency operation,the use of tranquillizer,the length of stay in ICU were related to the hospital infection. CONCLUSIONS The key measures to preventing hospital infection are strengthening the care of the injured patients before admission to ICU,establishing the nursing regulations of the interventional manipulation,performing sterile manipulation strictly,and enhancing the monitoring and administration about environment to control the cross-infection.
