OBJECTIVES: Body weight, body mass index (BMI), body fat, and blood pressure are important indicators of a person's health. In this experimental study, we evaluated the effectiveness of an integrated personalized health care system, Health Improvement and Management System (HIMS)-everyday, which instantly provides subjects with biofeedback on their measured body weight, BMI, body fat and blood pressure using a database that stores subjects-customized information. METHODS: The subjects of this study used the system once or twice a week for 8 weeks. We analyzed the changes in their body weight, BMI, body fat, and blood pressure according to their respective usage of the system, and analyzed the changes in their perceived health status and health promoting behavior accordingly. RESULTS: Subjects' body weight, BMI, and blood pressure decreased significantly with respect to their individual usage of the system. Subjects who used the system more frequently showed significant improvement in their body weight, BMI, and body fat. However, subjects' perceived health status and health promoting behavior did not improve significantly. CONCLUSIONS: The study showed that the biofeedback-based personalized health care system was effective in controlling middle-aged and elderly women' body weight, BMI, body fat, and blood pressure.

OBJECTIVE: Body weight is an important indicator of a person's health status. In this study, we evaluated the effectiveness of the body weight management system, HIMS (Health Improvement and Management System)-Diet, which instantly provides users with feedback on their measured weight using the database that stores the user.customized information. METHODS: The participants of this experimental study used the system two or three times a week for 10 weeks. We analyzed the changes in their body weight and body mass index (BMI) according to their respective usage of the system. RESULTS: Participants' body weight and BMI decreased with respect to their individual usage of the system. Participants that used the system frequently for a longer time period improved significantly in their body weight and BMI. CONCLUSION: The experimental study showed that the body weight management system, HIMS-Diet, was effective in controlling people's body weight.

OBJECTIVE: People can accomplish their objectives of exercise only when they exercise in moderation, meaning according to the personalized exercise prescriptions that are adjusted to their health status and fitness level. In this study, we evaluated the effectiveness of the personalized exercise prescriptions generated from HIMS(Health Improvement and Management System). METHODS: Subjects of experimental study exercised in their daily life according to their exercise prescriptions generated from HIMS for 10 weeks. We compared and analyzed the difference of their physical and psycho-social health status between before and after 10 weeks of exercise. RESULTS: Subjects' heart rate and blood pressure was improved meaningfully after 10 weeks of exercise. Subjects' body fat and agility was improved meaningfully after 5 weeks of exercise. Subjects' psycho-social health was improved meaningfully after 5 and 10 weeks of exercise. CONCLUSION: The experimental study showed that the personalized exercise prescriptions generated from HIMS were effective to improve and maintain the health and wellness.

OBJECTIVE: For disease prevention and health promotion, it is important that individual health information is managed continuously and can be retrieved easily when necessary. We propose a model for integrated management of diverse individual health information that is generated and managed in heterogeneous Health Information System(HIS)s at hospitals or health centers. METHODS: We use XML and relational database together to represent the health information structurally and manage it flexibly. In order to show the effectiveness of our method, we developed a prototype of Intermediate System for integration and analysed the result. RESULTS: The Intermediate System for integration provides the schema of health information, represents the information in forms of XML strings by the subjects, and stores each XML string as one field of relational database. In each heterogeneous HIS, there is an adaptor that transforms the health information into XML according to the schema. The messages for transmission between each HIS and Intermediate System are represented in XML. CONCLUSION: The experimental study shows that using relational database and XML together provides a flexible, extensible and structured way of representing complex, dynamic, structurally-variant and large-scale information, on the premise that the database tables are partitioned or distributed according to the status of server.

Résumé

The nursing process, composed of diagnosis, intervention and outcome, is practical and scientific approach. In addition, many studies on the nursing process have been performed. However, the nursing process has not applied to clinical fields. Therefore, we developed a nursing information system that supports nurses nursing diagnosis and intervention management. This system provides expected nursing diagnoses and interventions for subjective patients automatically so that nurses can make more accurate diagnoses and perform more adequate interventions. For that purpose, we have analyzed the relations of medical diagnoses and standardized nursing classifications and developed the outputs into a database system. In this study, we performed clinical tests for the system and verify the usefulness of the system with case database accumulated through the tests. We expect the system can be used in many hospitals efficiently after some upgrade based on the results of this study is completed.

This paper proposes a system for nursing diagnosis and intervention management that is using nursing outcome indicators to guide the nursing intervention. In Korea, it has been studied on computerization of nursing process, but most of the studies are on the management of nursing records and not on the databases of nursing intervention. So far, the actual nursing processes have been performed by individual nurses' judgement without any supporting programs. Therefore, we provide the system with standardized database for nursing diagnosis and interventions so that nurses can make more accurate diagnoses and perform more adequate interventions. For that purpose, we have developed an algorithm that links nursing outcome indicators to nursing diagnoses and interventions. As a result, we expect the system can be used in many hospitals efficiently in the future after pilot operations.

Résumé

The purpose of this study is to develop the computerized health instruction system to manage the Health Management Members(HMM) of the Gangbook-ku Health center in Seoul who have registered for the life-time health promotion. The HMM have been checked up and classified into 3 groups - healthy, borderline and ill group - based on their health assessment data through the computerized management system. But the computerized supporting programs for intensive management have not been sufficient enough to offer clear health instructions to the HMM and more than one user in the health center have not been able to use the system at the same time. Therefore, we have developed some new health instruction programs and upgraded the system to be used concurrently. For that purposes, we have used Delphi, Microsoft Access, ActiveX Data Objects(ADO) technology as development tools. We expect thi system to be used for health management of other public health centers, schools, and occupational settings, furthermore for evaluation of health promotion services. Additionally, this new computerized health management system supplemented with health instruction programs should be integrated to the computerized health care system at health centers in near future.

Résumé

The risk factors of chronic degenerative diseases are related to daily life style such as lack of exercise, obesity, smoking, stress, insufficient sleep, and etc. This study is conducted to develop a computerized system that check-up, guide and manage the residents' health through life-span. The morbidity of chronic degenerative diseases such as hypertension, diabetes mellitus and arthritis has been high in the survey on community health needs. These diseases are selected as the object of the study. The subjects who register as a health management member are over 40 and under 65 years old in the community. The combined approach method to individuals and groups is applied to the health management. The modeling of database for the system is constructed by entity-relationship model and relational data model. The subjects are classified into three groups of health, borderline and disease, on the basis of their clinical test result and daily life style. The intensive health care is provided to the subjects according to the result of analysis. We anticipate the community health promotion through the life style modification of the subjects by this intensive health care system.

Résumé

A number of application systems for medical information services were developed. However, each of those application systems was developed independently out of consideration for other applications. Therefore, there are many problems in operation of the systems. Users(hospitals) have to prepare communication lines and equipments for using each service redundantly, and facilities for operation of those systems are also furnished individually. Thus, it is necessary to integrate the medical information systems in standardized environment in order to reduce operating costs and upgrade the services. This paper analyzes the methodology for development of an integrated medical information system in common environment and summarizes the result of the integration and standardization. Finally, we studies the directions for expanding the integrated system and activating the services.
